Development of a fully automated surgical site infection detection algorithm for use in cardiac and orthopedic surgery research

Objective:
To develop a fully automated algorithm using data from the Veterans’ Affairs (VA) electrical medical record (EMR) to identify deep-incisional surgical site infections (SSIs) after cardiac surgeries and total joint arthroplasties (TJAs) to be used for research studies.
Design:Retrospective cohort study.
Setting:This study was conducted in 11 VA hospitals.
Participants:Patients who underwent coronary artery bypass grafting or valve replacement between January 1, 2010, and March 31, 2018 (cardiac cohort) and patients who underwent total hip arthroplasty or total knee arthroplasty between January 1, 2007, and March 31, 2018 (TJA cohort).
Methods:Relevant clinical information and administrative code data were extracted from the EMR. The outcomes of interest were mediastinitis, endocarditis, or deep-incisional or organ-space SSI within 30 days after surgery. Multiple logistic regression analysis with a repeated regular bootstrap procedure was used to select variables and to assign points in the models. Sensitivities, specificities, positive predictive values (PPVs) and negative predictive values were calculated with comparison to outcomes collected by the Veterans’ Affairs Surgical Quality Improvement Program (VASQIP).
Results:Overall, 49 (0.5%) of the 13,341 cardiac surgeries were classified as mediastinitis or endocarditis, and 83 (0.6%) of the 12,992 TJAs were classified as deep-incisional or organ-space SSIs. With at least 60% sensitivity, the PPVs of the SSI detection algorithms after cardiac surgeries and TJAs were 52.5% and 62.0%, respectively.
Conclusions:Considering the low prevalence rate of SSIs, our algorithms were successful in identifying a majority of patients with a true SSI while simultaneously reducing false-positive cases. As a next step, validation of these algorithms in different hospital systems with EMR will be needed.
P022: A multimodal evaluation of an emergency department (ED) electronic tracking board utility designed to improve throughput by optimizing stretcher utilization

Introduction: Access block is a pervasive problem, even during times of minimal boarding in the ED, suggesting suboptimal use of ED stretchers can contribute. A tracking board utility was embedded into the electronic health record in Calgary, AB, allowing MDs and RNs to consider patients who could be relocated from a stretcher to a chair. Objectives of this study were to evaluate the feature's impact on total stretcher time (TST) and ED length of stay (LOS) for patients relocated to a chair. We also sought to identify facilitators and barriers to the tool's use amongst ED MDs and RNs. Methods: A retrospective cohort design was used to compare TST between those where the tool was used and not used amongst patients relocated to a chair between September 1 2017 and August 15 2018. Each use of the location tool was time-stamped in an administrative database. Median TST and ED LOS were compared between patients where the tool was used and not used using a Mann-Whitney U Test. A cross sectional convenience sample survey was used to determine facilitators and barriers to the tool's use amongst ED staff. Response proportions were used to report Likert scale questions; thematic analysis was used to code themes. Results: 194882 patients met inclusion criteria. The tool was used 4301 times, with “Ok for Chairs” selected 3914(2%) times and “Not Ok for Chairs” selected 384(0.2%) times; 54462(30%) patients were moved to a chair without the tool's use. Mean age, sex, mode of arrival and triage scores were similar between both groups. Median (IQR) TST amongst patients moved to a chair via the prompt was shorter than when the prompt was not used [142.7 (100.5) mins vs 152.3 (112.3) mins, p < 0.001], resulting in 37574 mins of saved stretcher time. LOS was similar between both groups (p = 0.22). 125 questionnaires were completed by 90 ED nurses and 35 ED MDs. 95% of staff were aware of the tool and 70% agreed/strongly agreed the tool could improve ED flow; however, 38% reported only “sometimes” using the tool. MDs reported the most common barrier was forgetting to use the tool and lack of perceived action in relocating patients. Commonly reported nursing barriers were lack of chair space and increased workload. Conclusion: Despite minimal use of the tracking board utility, triggering was associated with reduced TST amongst ED patients eventually relocated to a chair. To encourage increased use, future versions should prompt staff to select a location.

INTRODUCTION
Models are used increasingly to simulate hydrological processes within tropical regions. There is now a wealth of publications addressing evaporation modelling (particularly wet-canopy evaporation) of local areas of tropical forest in, for example, Niger (Gash et al., 1997), Guyana (Jetten, 1996), Puerto Rico (Schellekens et al., 1999), Columbia (Marin et al., 2000) and Indonesia (Asdak et al., 1999; van Dijk and Bruijnzeel, 2001). Elsewhere in this volume, Roberts et al. provide an overview of evaporation processes and modelling. Other modelling studies have addressed the impact of such tropical evaporation on regional climates and global circulation (e.g. Polcher and Laval, 1994; Zeng, 1999; Zeng and Neelin, 1999; Zheng et al., 2001). New studies using time-series models are highlighting the effects of cycles in the rainfall, such as the El Niño Southern Oscillation (ENSO) on tropical evaporation, riverflow and water quality (e.g. Zeng, 1999; Chappell et al., 2001; Krishnaswamy et al., 2001; Whitaker et al., 2001; Chappell, Tych et al., this volume). Similarly, models that simulate the generation of riverflow from the rainfall received by a tropical catchment are also beginning to be applied more frequently. These models include:
Metric-conceptual models of waterflow, based upon transfer functions. For example, application of the DBM modelling approach to a nested catchment system in Malaysian Borneo (Chappell et al., 1999a) and the application of IHACRES to a large Thai basin (Scoccimarro et al., 1999).
Conceptual models of waterflow based upon stores and pre-determined empirical relationships. For example, application of the Nash model to Kenyan catchments (Onyando and Sharma, 1995), the Modhydrolog model to a tropical catchment (Chiew et al., 1996), the Reservoir-Water-Balance-Simulation model to Namibian catchments (Hughes and Metzler, 1998), and the HBV-96 model (discussed in Barnes and Bonell, this volume) to catchments in Zimbabwe, Tanzania and Bolivia (Liden and Harlin, 2000).
