Cardiac magnetic resonance predictors for successful primary biventricular repair of unbalanced complete common atrioventricular canal

Background:
Patients with unbalanced common atrioventricular canal can be difficult to manage. Surgical planning often depends on pre-operative echocardiographic measurements. We aimed to determine the added utility of cardiac MRI in predicting successful biventricular repair in common atrioventricular canal.
Methods:We conducted a retrospective cohort study of children with common atrioventricular canal who underwent MRI prior to repair. Associations between MRI and echocardiographic measures and surgical outcome were tested using logistic regression, and models were compared using area under the receiver operator characteristic curve.
Results:We included 28 patients (median age at MRI: 5.2 months). The optimal MRI model included the novel end-diastolic volume index (using the ratio of left ventricular end-diastolic volume to total end-diastolic volume) and the left ventricle–right ventricle angle in diastole (area under the curve 0.83, p = 0.041). End-diastolic volume index ≤ 0.18 and left ventricle–right ventricle angle in diastole ≤ 72° yield a sensitivity of 83% and specificity of 81% for successful biventricular repair. The optimal multimodality model included the end-diastolic volume index and the echocardiographic atrioventricular valve index with an area under the curve of 0.87 (p = 0.026).
Conclusions:Cardiac MRI can successfully predict successful biventricular repair in patients with unbalanced common atrioventricular canal utilising the end-diastolic volume index alone or in combination with the MRI left ventricle–right ventricle angle in diastole or the echocardiographic atrioventricular valve index. A prospective cardiac MRI study is warranted to better define the multimodality characteristic predictive of successful biventricular surgery.
Palliative and end-of-life care education in Canadian emergency medicine residency programs: A national cross-sectional survey

Objectives
Emergency physicians play an important role in providing care at the end-of-life as well as identifying patients who may benefit from a palliative approach. Several studies have shown that emergency medicine (EM) residents desire further training in palliative care. We performed a national cross-sectional survey of EM program directors. Our primary objective was to describe the number of Canadian postgraduate EM training programs with palliative and end-of-life care curricula.
MethodsA 15-question survey in English and French was sent by email to all program directors of both the Canadian College of Family Physicians emergency medicine (CCFP(EM)) and the Royal College of Physicians and Surgeons of Canada emergency medicine (RCPSC-EM) postgraduate training programs countrywide using FluidSurveys™ with a modified Dillman approach.
ResultsWe received a total of 26 responses from the 36 (response rate = 72.2%) EM postgraduate programs in Canada. Ten out of 26 (38.5%) programs had a structured educational program pertaining to palliative and end-of-life care. Lectures or seminars were the exclusive choice to teach content. Clinical palliative medicine rotations were mandatory in one out of 26 (3.8%) programs. The top two barriers to implementation of palliative and end-of-life care curricula were lack of time (84.6%) and curriculum development concerns (80.8%).
ConclusionsPalliative and end-of-life care training within EM has been identified as an area of need. This cross-sectional survey demonstrates that a minority of Canadian EM programs have palliative and end-of-life care curricula. It will be important for all EM training programs, RCPSC-EM and CCFP(EM), in Canada, to develop an agreed upon set of competencies and to structure their curricula around them.

The tegumentary ultrastructure of Leucochloridiomorpha constantiae metacercariae and adults has been described. A filamentous glycocalyx invests the tegument of metacercariae and large numbers of biconcave, disk-shaped secretion vesicles are found in its outer zone, cytoplasmic bridges and cytons. Mitochondria within the outer tegumentary zone of metacercariae are restricted to its basal half. The transformation of metacercariae to adults involves the loss of the glycocalyx, a decrease in the number of disk-shaped secretion vesicles, a more uniform distribution of mitochondria through the outer tegumentary zone, an apparent degeneration of cytons, a thinning of the fibrous basal lamina and a decrease in the number of visible cytoplasmic bridges traversing it. The significance of these events for a metacercaria which does not encyst within the molluscan host is discussed. In addition, crystalline, spine-like inclusions in the outer tegumentary zone of adult L. constantiae are described and their possible function discussed.

Plants that are used for taxonomic research are generally preserved by drying them out while they are being pressed flat for easy storage. This pressed plant material, is placed in a herbarium, in phylogenetic order, based on the species, genus, family and order designations.
Herbaria are essential for the study and verification of plant classification, the study of geographic distributions, and the standardizing of taxonomic nomenclature. Lately, herbarium materials have been utilized in biochemical and genetic studies. The key to these studies is the state of preservation of the plant material. The preservation of the reproductive and vegetative structures (hairs, stomata, cuticle, etc.), internal anatomy (vascular tissue size and arrangement), biochemistry and genetic material is essential to make accurate observations, to collect representative data and answer pertinent research questions.

The use of DSM-IV based questionnaires in child psychopathology is on the increase. The internal construct validity of a DSM-IV based model of ADHD, CD, ODD, Generalised Anxiety, and Depression was investigated in 11 samples by confirmatory factor analysis. The factorial structure of these syndrome dimensions was supported by the data. However, the model did not meet absolute standards of good model fit. Two sources of error are discussed in detail: multidimensionality of syndrome scales, and the presence of many symptoms that are diagnostically ambiguous with regard to the targeted syndrome dimension. It is argued that measurement precision may be increased by more careful operationalisation of the symptoms in the questionnaire. Additional approaches towards improved conceptualisation of DSM-IV are briefly discussed. A sharper DSM-IV model may improve the accuracy of inferences based on scale scores and provide more precise research findings with regard to relations with variables external to the taxonomy.
Shellfish tainting – a means of monitoring petroleum-contaminated effluents

Water from the separation plant at Sullom Voe is discharged into the sea under agreed conditions from a diffuser situated to the north of Calback Ness, close to natural scallop (Pecten maximus) beds.
As part of the arrangements to monitor potential effects of the discharge, Torry Research Station was asked to assess, for petroleum taint, shellfish which would be sampled at intervals from Sullom Voe. To avoid depletion of the natural shellfish beds, queen scallops (Chlamys opercularis) or scallops, whichever were available from other areas, were set out in cages suspended (or on the bottom) 100–1250 m eastwards of the outfall for a period, and then recovered, frozen, and assessed subsequently at the laboratory by a trained taste panel, and compared with the same species unexposed.
The cages were laid on 10 occasions from March 1980 to February 1987. Initially, no petroleum taint was detected. In August 1980 and May 1981, a different flavour, but not recognisable as a petroleum taint, was detected at 800–1000 m from the outfall after three days exposure. In May 1982, 33% of the queen scallops in the cage on the bottom, 100 m south east of the diffuser, were dead after six days exposure, and taint was detected at the threshold level in shellfish in the cage at 300 m. In May 1983, taint and petroleum derived hydrocarbons were detected in the sample from the cage on the bottom, 100 m south east of the diffuser, after seven days exposure. In May 1984, taint was again detected at the bottom, 200 m south east of the outfall, after 10 days exposure.
Enquiries established that the diffuser was not operating with maximum efficiency. Subsequently, monitoring revealed an improvement. Taint assessment proved to be a sensitive detector of petroleum contamination.
