Rising incidence of psychiatric disorders before diagnosis of immune-mediated inflammatory disease

Aims.
After the diagnosis of immune-mediated inflammatory diseases (IMID) such as inflammatory bowel disease (IBD), multiple sclerosis (MS) and rheumatoid arthritis (RA), the incidence of psychiatric comorbidity is increased relative to the general population. We aimed to determine whether the incidence of psychiatric disorders is increased in the 5 years before the diagnosis of IMID as compared with the general population.
Methods.Using population-based administrative health data from the Canadian province of Manitoba, we identified all persons with incident IBD, MS and RA between 1989 and 2012, and cohorts from the general population matched 5 : 1 on year of birth, sex and region to each disease cohort. We identified members of these groups with at least 5 years of residency before and after the IMID diagnosis date. We applied validated algorithms for depression, anxiety disorders, bipolar disorder, schizophrenia, and any psychiatric disorder to determine the annual incidence of these conditions in the 5-year periods before and after the diagnosis year.
Results.We identified 12 141 incident cases of IMID (3766 IBD, 2190 MS, 6350 RA) and 65 424 matched individuals. As early as 5 years before diagnosis, the incidence of depression [incidence rate ratio (IRR) 1.54; 95% CI 1.30–1.84) and anxiety disorders (IRR 1.30; 95% CI 1.12–1.51) were elevated in the IMID cohort as compared with the matched cohort. Similar results were obtained for each of the IBD, MS and RA cohorts. The incidence of bipolar disorder was elevated beginning 3 years before IMID diagnosis (IRR 1.63; 95% CI 1.10–2.40).
Conclusion.The incidence of psychiatric comorbidity is elevated in the IMID population as compared with a matched population as early as 5 years before diagnosis. Future studies should elucidate whether this reflects shared risk factors for psychiatric disorders and IMID, a shared final common inflammatory pathway or other aetiology.

The European Model for Inhabited Areas (ERMIN) was developed to allow a user to explore different recovery options following the contamination of an urban environment with radioactive material and to refine an appropriate strategy for the whole region affected. The input data include a description of the environment, initial deposition of radionuclides on to a reference surface and a description of countermeasures. Output information includes the average doses to members of the public from external exposure to gamma and beta radiation from deposited radionuclides and inhalation of resuspended radioactivity, the contamination on urban surfaces, the activity concentration in air from resuspension, the doses to workers undertaking the recovery work, the quantity and activity of waste generated and the cost and work required to implement the countermeasure. ERMIN has been designed to be implemented as a tool that supports the approach of decision-makers and allows the area to be broken down into smaller regions where different conditions prevail and different countermeasure packages are enacted.

The structure of MoS2 films grown by ion beam assisted deposition is investigated using transmission electron microscopy. Films consist of stacks of S–Mo–S planes with a [001] texture; however, three-dimensional crystal symmetry is disrupted by a high density of planar defects. Selected area electron diffraction patterns show (hk0) and (00l) reflections, features similar to a random layer structure, as well as diffuse (103) reflections. It is suggested that these films do not have a true random layer structure, but rather a two-dimensional structure formed by nonrandom in-plane translations.

Microstructural aspects of wear resistance have been investigated using cross-sectional transmission electron microscopy (TEM) to analyze both as-deposited and worn films. Asdeposited Pb-Mo-S films were virtually amorphous, i.e., show no long range structure. Wear tracks revealed a two-part wear process localized at the sliding surface. First, Pb-Mo-S at the sliding surface was transformed into basal-oriented, crystalline MoS2 from I to 4 monolayers thick. Then, as sliding continued, the MoS2 layers were detached one or two layers at a time. Evidence for the nanometer scale wear process is presented.

The origin of a previously reported anomalous low 2θ x-ray diffraction peak from MoS2 thin films grown by ion beam assisted deposition was investigated. The anomalous peak, observed in a film grown on Si(100), was removed by ion irradiating the film with 180 keV Ar++ ions to a dose of 1 × 1015 ions/cm2. Microstructures of the two films were investigated using x-ray diffraction and cross-section transmission electron microscopy. Diffraction data and bright-field images indicated that the low 2θ peak was due to a local interplanar expansion of the crystal structure normal to MoS2 basal planes. This expansion was attributed to molecular defects.

Steel (AISI-304) disks, polished to a 3 μm diamond finish, were implanted to doses of about 2 × 1017/cm2 with N+, Ni+ or, Ne+, ions. Polishing wear rates, measured to a depth resolution of about 20 nm, showed that each of the implanted disks wore 20% to 40% faster than nonimplanted layers. Auger analysis showed (1) Gaussian-like profiles of the N-implanted layer, but with somewhat enhanced oxidation of the surface, and (2) a sputter-limited implantation profile in the Ni-implantcd layer, with some vacuum carburization. Transmission electron microscope analysis indicated that polishing produced an α′-martensite layer in the mainly austenitic 304 surface, but that implantation of N transformed the martensite to austenite at the outermost layers and produced iron nitrides below. In addition. Nimplantation stabilized the austenite against martensite transformation during subsequent wear. Martensite vanished after Ni implantation because of sputter removal, not phase transformation, during Ni bombardment. The Ne-implanted layer remained predominantly martensite. Changes in wear rates and microstructures were confined to depths commensurate with the range of ions in the implanted layer. Several hypotheses on the effects of ion implantation on microstructureand wear are discussed.

The efficacy of antidepressants is measured primarily by their ability to treat the acute symptoms of depression yet the underlying process may take longer to resolve. A period of emotional frailty follows resolution of the acute stage, during which the patient, although apparently symptom-free, is prone to suffer relapse of the original symptoms. Antidepressants should not be stopped immediately a response is observed, but should be given for a longer period to prevent early relapse. In those suffering from recurrent depression, long-term treatment may prevent later recurrences of new episodes of depression.
The distinction between relapse of old symptoms and recurrence of new episodes is of particular importance, both in assessing efficacy of antidepressants during the continuation phase of acute treatment and in the separate assessment of their prophylactic efficacy. Unfortunately, though, many investigations have failed to distinguish between these two distinct phenomena. Klerman & Paykel (1970) emphasised the need to distinguish between early return of depressive symptoms, which they label ‘relapses’, from the later new episodes which they term ‘recurrence’. However, there has been a general lack of systematic investigation of this question, as well as a failure to use sufficiently precise methodology to distinguish between relapse and recurrence. Some studies which purported to be of prophylaxis were in effect only examining the continuation phase of acute treatment.
Friction and Wear Behavior of a Cobalt-Based Alloy Implanted with Ti or N

Dry sliding friction tests and relative abrasive wear measurements were used to investigate the effects of ion implantation on the tribology of a centrifugally cast Co-Cr-W-C alloy. Titanium implantation was found to significantly reduce the friction and wear of the alloy. Auger spectroscopy showed that vacuum carburization of the surface occurred during Ti implantation. Similar Ti and C profiles were seen in both carbide and (Co-rich) matrix phases. The abrasive wear resistance of the alloy decreased as a result of N-implantation. Nitrogen implantation did not reduce the friction during dry sliding, however, a change in the wear mode was observed.

Martensite will form in austenitic 304 stainless steel when it is deformed. Transmission electron microscope studies show that nitrogen ion implantation causes a reversion of the martensite to austenite. Specimens containing martensite resulting from fine surface polishing and heavy rolling are examined. The transformation is shown not to occur because of temperature increases during implantation. The effect is related to recent wear results in 304 stainless steel.
