The Letters
Here is a jolting moment in a letter written during the Conrads’ honeymoon in Brittany:
Our captivity within the incomprehensible logic of accident is the only fact of the universe. From that reality flows deception and inspiration, error and faith, egoism and sacrifice, love and hate. That truth fearlessly faced becomes an austere and trusted friend, a companion of victory or a giver of peace.
(to T. Fisher Unwin, 22 August 1896, CL 1, 303)Not every author with only two well received but not widely purchased novels in print would be so firm with his publisher. These sentences come from what Joseph Conrad himself calls an “impertinent rigmarole” (CL 1, 304). He has just been reading a new Unwin title, A First Fleet Family, by two Australians, Louis Becke and Walter Jeffery. This historical romance, published in the Colonial Library series, is presented as the journal of a sergeant in the Royal Marines who sailed in the flotilla that reached Botany Bay, New South Wales, in January 1788. Conrad had told Thomas Fisher Unwin two weeks previously how much he admired Becke's By Reef and Palm, a collection of South Sea island stories, and how sorry he was to have missed a chance of meeting him. Conrad, a prodigiously fast reader, liked to judge by second, third, and even fourth impressions; going back to the collection had deepened his admiration for Becke's “perfect unselfishness” as a storyteller. Thus he had a “great curiosity” about the new novel—Becke's first (CL 1, 298). Unwin sent him a copy straight away.
Conrad's response to Unwin's gift is a discourse on point of view, detail, the inevitable, and the unpredictable. He complains that Becke has made a bad choice in giving the narration to Sergeant Dew, who is (to borrow a term from Henry James) an inadequate reflector with no eye for “the merciless vividness of detail” (CL 1, 303). Conrad argues both for a world view that must come to terms with contingency, accident, and chance, as well as an artistic view that the author's task is to write fiction that seems not accidental but bound to be the way it is. “Everything” in A First Fleet Family “is quite true and even obtrusively possible—but not a single episode, event, thought, word; not a single pang of joy or sorrow is inevitable” (CL 1, 302).
Variability in ice motion and dynamic discharge from Devon Ice Cap, Nunavut, Canada

Feature tracking of approximately annually separated Landsat-7 ETM+ imagery acquired from 1999 to 2010 and speckle tracking of 24-day separated RADARSAT-2 imagery acquired from 2009 to 2015 reveal that motion of the major tidewater glaciers of Devon Ice Cap is more variable than previously described. The flow of almost half (six of 14) of the outlet glaciers slowed over the observation period, while that of the terminus regions of three of 14 of the glaciers sped up in the most recent years of observation. The North Croker Bay Glacier of southern Devon Ice Cap showed the greatest variability in motion, oscillating between multi-year (three or more) periods of slower and faster flow and exhibited a pattern of velocity variability that is different from that of the rest of the ice cap's outlet glaciers. Comparisons between areas of dynamic variability and glacier bed topography indicate that velocity variability is largely restricted to regions where the glacier bed is grounded below sea level. Derived velocities are combined with measurements of ice thickness at the fronts of tidewater glacier to determine a mean annual (2009; 2011–15) dynamic ice discharge of 0.41 ± 0.11 Gt a−1 for Devon Ice Cap. The Belcher Glacier is becoming a larger source of mass loss via ice discharge.

OBJECTIVE
To determine the impact of total household decolonization with intranasal mupirocin and chlorhexidine gluconate body wash on recurrent methicillin-resistant Staphylococcus aureus (MRSA) infection among subjects with MRSA skin and soft-tissue infection.
DESIGNThree-arm nonmasked randomized controlled trial.
SETTINGFive academic medical centers in Southeastern Pennsylvania.
PARTICIPANTSAdults and children presenting to ambulatory care settings with community-onset MRSA skin and soft-tissue infection (ie, index cases) and their household members.
INTERVENTIONEnrolled households were randomized to 1 of 3 intervention groups: (1) education on routine hygiene measures, (2) education plus decolonization without reminders (intranasal mupirocin ointment twice daily for 7 days and chlorhexidine gluconate on the first and last day), or (3) education plus decolonization with reminders, where subjects received daily telephone call or text message reminders.
MAIN OUTCOME MEASURESOwing to small numbers of recurrent infections, this analysis focused on time to clearance of colonization in the index case.
RESULTSOf 223 households, 73 were randomized to education-only, 76 to decolonization without reminders, 74 to decolonization with reminders. There was no significant difference in time to clearance of colonization between the education-only and decolonization groups (log-rank P=.768). In secondary analyses, compliance with decolonization was associated with decreased time to clearance (P=.018).
CONCLUSIONSTotal household decolonization did not result in decreased time to clearance of MRSA colonization among adults and children with MRSA skin and soft-tissue infection. However, subjects who were compliant with the protocol had more rapid clearance

Summary
The chapters in this timely volume consider many junctures, many differences – not so much polarities as swirls of literary energy between and among spoken and unspoken, spoken and written, articulate and inarticulate, multinational and domestic; terms of art and terms of common discourse, heteroglossia and polyglossia, frankness and evasion, ontology and epistemology, the locutionary or illocutionary and the perlocutionary, Jawi and Rumi, Rumi and Cyrillic, manuscript and print. At the risk of sounding like Polonius heralding the players, one could say that these studies of Conrad and language mix in various proportions the aesthetic, philosophical, ethnographic, historical, ethical, cultural and geographical with the linguistic. The volume is timely because it speaks to current – and urgent – debates about transnationalism, cosmopolitanism, modernity, migrant cultures, translation, borders, cultural privilege and the rights to being seen and heard and read. It also speaks at a time of remapping Anglophone literatures in the spirit of these debates, epitomised in the title of Wai Chee Dimock's Through Other Continents: American Literature across Deep Time, exemplified in the tables of contents of English-language journals of modernist studies such as Modernism/modernity and Modernist Cultures, and demonstrated at length in a major study by Christopher GoGwilt, a contributor to this volume, who reads Conrad side by side with Jean Rhys and the Indonesian novelist (and political prisoner under both colonial and postcolonial regimes) Pramoedya Ananta Toer.
Why, in these various realignments and debates, is language so important? And why is Conrad so worth framing in these contexts? To begin with, there is the phenomenon of his linguistic formation. As other contributors note, he passed the first ten years of his life in the Russian Empire. Part of that time was spent in exile to Russia proper, but the rest was in, to give them their present-day names, Berdychiv, Zhytomyr and Chernihiv.

OBJECTIVE
To identify risk factors for recurrent methicillin-resistant Staphylococcus aureus (MRSA) colonization.
DESIGNProspective cohort study conducted from January 1, 2010, through December 31, 2012.
SETTINGFive adult and pediatric academic medical centers.
PARTICIPANTSSubjects (ie, index cases) who presented with acute community-onset MRSA skin and soft-tissue infection.
METHODSIndex cases and all household members performed self-sampling for MRSA colonization every 2 weeks for 6 months. Clearance of colonization was defined as 2 consecutive sampling periods with negative surveillance cultures. Recurrent colonization was defined as any positive MRSA surveillance culture after clearance. Index cases with recurrent MRSA colonization were compared with those without recurrence on the basis of antibiotic exposure, household demographic characteristics, and presence of MRSA colonization in household members.
RESULTSThe study cohort comprised 195 index cases; recurrent MRSA colonization occurred in 85 (43.6%). Median time to recurrence was 53 days (interquartile range, 36–84 days). Treatment with clindamycin was associated with lower risk of recurrence (odds ratio, 0.52; 95% CI, 0.29–0.93). Higher percentage of household members younger than 18 was associated with increased risk of recurrence (odds ratio, 1.01; 95% CI, 1.00–1.02). The association between MRSA colonization in household members and recurrent colonization in index cases did not reach statistical significance in primary analyses.
CONCLUSIONA large proportion of patients initially presenting with MRSA skin and soft-tissue infection will have recurrent colonization after clearance. The reduced rate of recurrent colonization associated with clindamycin may indicate a unique role for this antibiotic in the treatment of such infection.

Summary
Despite the contemporary enthusiasm for sanitary reform, few Victorians wrote poems about drains. One exception was Kipling, who gave his poem the daringly unpromising title 'Municipal'. The speaker is a district commissioner who, when menaced by a stampeding elephant, took refuge in a blocked-up outfall wearing regulation 'snowy garments':
You may hold with surface-drainage, and the sun-for-garbage cure,
Till you've been a periwinkle shrinking coyly up a sewer.
The experience has made him 'believe in well-flushed culverts' and, as a result, the death-rate in his district has gone down. Kipling's vision of empire extended to the infrastructure of railways, riverboats, bridges and sewage systems. In his prose and poetry, his fascination with technology and its consequences shows up in unexpected places, foreign, imperial and domestic. On his visit to Brazil in 1927, he sent rapturous dispatches to the ultra-conservative (and often xenophobic) Morning Post praising the mountain railways, the giant hydroelectric installations, and the laboratory where snakes and spiders were milked for their venom. ‘“They” ’ heads towards the mystical and redemptive, but it starts by evoking the joys of motoring through rural England, and does not stint on technical details: ‘I was on the point of reversing and working my way back on the second speed.’

Reports of low-concentration nerve gas exposures during the Persian Gulf War have spurred concern about possible health consequences and refocused interest on the symptoms reported by many returning military veterans. The Portland Environmental Hazards Research Center is studying veterans from the Northwest USA who report persistent, unexplained “Persian Gulf” symptoms (cases) or who do not report those symptoms (controls). Of the first 101 veterans studied, cases differed substantially from controls on a broad range of psychological tests indicative of increased distress. A subgroup of cases was identified with objective deficits on neurobehavioral tests of memory, attention, and response speed. (JINS, 1999, 5, 203–212.)
A randomised controlled trial and cost analysis of problem-solving treatment for emotional disorders given by community nurses in primary care

Background
We set out to investigate whether community nurses could be trained in problem-solving therapy and, once trained, how effective they would be in treating emotional disorders in primary care.
MethodSeventy patients with an emotional disorder in primary care were randomly allocated to receive either problem-solving therapy from a trained community nurse or treatment as usual from their general practitioner. Interview and self-rated assessments of clinical and economic outcome were made pre-treatment, at eight weeks and at 26 weeks after treatment.
ResultsThere was no difference in clinical outcome between patients who received problem-solving treatment and patients who received the general practitioner's usual treatment. However, patients who received problem-solving treatment had fewer disability days and fewer days off work. The health care cost of problem-solving was greater than that of the general practitioner's usual treatment but this was more than offset by savings in the cost of days off work.
ConclusionsProblem-solving treatment can be given by trained community nurses. The clinical effectiveness and cost-benefit of the treatment will depend on the selection of appropriate patients.
