Summary
Reward Deficiency Syndrome (RDS) is an umbrella term for all drug and nondrug addictive behaviors, due to a dopamine deficiency, “hypodopaminergia.” There is an opioid-overdose epidemic in the USA, which may result in or worsen RDS. A paradigm shift is needed to combat a system that is not working. This shift involves the recognition of dopamine homeostasis as the ultimate treatment of RDS via precision, genetically guided KB220 variants, called Precision Behavioral Management (PBM). Recognition of RDS as an endophenotype and an umbrella term in the future DSM 6, following the Research Domain Criteria (RDoC), would assist in shifting this paradigm.

We implemented universal severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) testing of patients undergoing surgical procedures as a means to conserve personal protective equipment (PPE). The rate of asymptomatic coronavirus disease 2019 (COVID-19) was <0.5%, which suggests that early local public health interventions were successful. Although our protocol was resource intensive, it prevented exposures to healthcare team members.

The role that vitamin D plays in pulmonary function remains uncertain. Epidemiological studies reported mixed findings for serum 25-hydroxyvitamin D (25(OH)D)–pulmonary function association. We conducted the largest cross-sectional meta-analysis of the 25(OH)D–pulmonary function association to date, based on nine European ancestry (EA) cohorts (n 22 838) and five African ancestry (AA) cohorts (n 4290) in the Cohorts for Heart and Aging Research in Genomic Epidemiology Consortium. Data were analysed using linear models by cohort and ancestry. Effect modification by smoking status (current/former/never) was tested. Results were combined using fixed-effects meta-analysis. Mean serum 25(OH)D was 68 (sd 29) nmol/l for EA and 49 (sd 21) nmol/l for AA. For each 1 nmol/l higher 25(OH)D, forced expiratory volume in the 1st second (FEV1) was higher by 1·1 ml in EA (95 % CI 0·9, 1·3; P<0·0001) and 1·8 ml (95 % CI 1·1, 2·5; P<0·0001) in AA (Prace difference=0·06), and forced vital capacity (FVC) was higher by 1·3 ml in EA (95 % CI 1·0, 1·6; P<0·0001) and 1·5 ml (95 % CI 0·8, 2·3; P=0·0001) in AA (Prace difference=0·56). Among EA, the 25(OH)D–FVC association was stronger in smokers: per 1 nmol/l higher 25(OH)D, FVC was higher by 1·7 ml (95 % CI 1·1, 2·3) for current smokers and 1·7 ml (95 % CI 1·2, 2·1) for former smokers, compared with 0·8 ml (95 % CI 0·4, 1·2) for never smokers. In summary, the 25(OH)D associations with FEV1 and FVC were positive in both ancestries. In EA, a stronger association was observed for smokers compared with never smokers, which supports the importance of vitamin D in vulnerable populations.

Between November 1988 and January 1989, a total of 155 people in 36 reported outbreaks suffered gastrointestinal symptoms associated with eating Stilton cheese, produced from unpasteurized cow's milk in the English midlands. Symptoms were suggestive of a staphylococcal illness but extensive laboratory testing of cheeses implicated in several of the outbreaks failed to detect any pathogen, toxin or chemical. Control measures were implemented, and included a voluntary withdrawal of the implicated Stilton cheese from sale on 23 January 1989 and a subsequent decision to use pasteurized milk in production of the cheese.
Cmos Polysilicon Thin Film Transistors with Simultaneously Deposited Layers for Source-Drain and Gate

A new process architecture for fabricating CMOS thin film transistors (TFTs) using in-situ-doped polysilicon source-drain layers is proposed. In the new architecture, a top-gate n-channel TFT and a bottom-gate p-channel TFT, or vice versa, form a CMOS pair. This allows an n+ - doped polysilicon bottom (or top) layer to serve simultaneously as the source-drain layer of the n-channel TFTs and the gate layer of the p-channel TFTs; while a p+ - doped polysilicon top (or bottom) layer serves as the source-drain layer of the p-channel TFT and the gate layer of the n-channel TFT. It thus eliminates the deposition of a separate doped gate layer normally required in the conventional process flow. In addition, a thin tri-layer stack, consisting of undoped-poly / gate dielectric / undoped-poly, separates the two doped polysilicon layers, thus allowing the use of a single island mask to define the channel regions for both the n- and p-channel TFTs. As a result, the photolithographic steps are also reduced by one mask. Working n- and p-channel TFTs with both top- and bottomgate structures, obtained by reversing the dopant types of the top and the bottom layers, have been successfully demonstrated using low-temperature (< 600 °C) polysilicon technology.

Being personally satisfied by clinical observations that certain of the acute insanities are toxic in their ætiology, it occurred to me that it might be possible to demonstrate the presence of the toxæmia by means of the delicate test of complement-deviation. I argued that if a toxin existed in the blood, it would probably be excreted in the urine, and that at certain stages of the disease process a corresponding immune body would be found in the serum. In a series of observations carried out on these lines, I used urines as the antigen, and serum as the immune body.

One of the most obvious impediments to the advance of our knowledge of mental diseases is the lack of a physical basis of classification. This deficiency is constantly presenting itself, and even in such a comparatively simple disease as mania one is often in doubt as to whether the diagnosis should be manic-depressive insanity or confusional mania.

The clinical study of the physical symptoms of mental diseases has of late years been pushed with energy and originality, not only in this country but also abroad. Advances in the general knowledge of physiological and pathological chemistry and the great doctrine of immunity have had their effect upon the views held regarding mental diseases, and particularly with regard to their causation.

While making observations upon the opsonic indices of the insane I noticed that the serum in many of the cases under observation when mixed with the red blood corpuscles of a healthy person agglutinated them.

The following observations are the result of three years' work in examining the leucocytes in the various patients admitted to the Perth District Asylum. During this period the blood of 150 patients has been systematically examined, not on one or two occasions only, but often for weeks and months at a time. We have observed in medical literature observations recorded on the leucocytosis occurring in various forms of insanity in which the observer had made many observations on different patients, but as a rule limited the observations made on each individual patient to one, two, or at most three occasions. Such records are of little or no value, and would be on a par with the observations of, let us say, an astronomer who, having examined the heavens on two nights out of the 365, and having found both nights cloudy, reported that no such things as stars existed. With regard to the technique employed for the estimation of the number of leucocytes per c.mm. of blood, we have throughout employed Coles' system of counting “fields” with the counting slide of a Thoma Zeiss' hæmocytometer. Never less than thirty fields were counted for each enumeration, and frequently sixty and eighty fields were counted when extra accuracy was deemed necessary.

During the past year we have frequently used antiserums experimentally in cases of acute insanity because we have been led to believe from our observations that many of these cases, either primarily or secondarily, are suffering from bacterial infection. The grounds upon which we base this belief are that we frequently find hyperleucocytosis, and in forty-eight out of seventy-six cases examined we have found bacterial agglutinines in the blood which do not exist in the blood of healthy people.

The following observations were made by my assistant (Dr. Peebles) and myself with the object of observing the physical symptoms of katatonia and hebephrenia. As the result of these observations we were led to make some experiments in the way of treatment, and we combined with this work some experimental observations on rabbits.

Following up my observations made upon the blood of patients suffering from acute continuous mania read before this Association at the autumn meeting, I have been able to observe three cases of acute continuous mania in adults which relapsed while in the asylum. The results of the first series of observations were that in every case of acute continuous mania there existed a leucocytosis which persisted after recovery indefinitely. I advanced the theory that this leucocytosis was a protective leucocytosis. In the three patients who relapsed the leucocytosis was found to have fallen to below 13,000 per c.mm. of blood, instead of being nearer 20,000 per c.mm. of blood, which is characteristic of the recovered cases of mania. The polymorphonuclear leucocytes averaged 60 per cent, in two of these patients, and 47 per cent, in the third. In one of these patients the attack passed off in two days, and the leucocytosis at once rose to 25,000 per c.mm. of blood. The other two patients passed into a definite second attack, and their leucocytes averaged 15,000 to 16,000 per c.mm. of blood, with a polymorphonuclear percentage of 60 or below 60. The fact that the leucocytosis fell in each patient at the commencement of the attack, and rose at once in the patient who recovered from the relapse, strengthens the hypothesis that acute continuous mania is an infective disorder, and that immunity from maniacal attacks rests upon the resistive power of the individual patient. This hypothesis receives further support from the fact that there exists in the blood of patients suffering from acute mania a specific agglutinin. During the month of November a patient suffering from acute mania was admitted to Murthly. The patient was so ill that I did not think she would live many days. I isolated from the blood a very small coccus, which was a pure growth, but, as the patient was exhausted, I regarded the organism as a terminal infection. The patient improved, however, and three weeks later I tested the agglutinative power of her serum upon this organism in a dilution of 1 in 30. Agglutination was complete in three hours, while the serum of a member of the staff in a dilution of 1 in 20 produced no action in twenty hours. Since then I have made fifty agglutination tests with this organism. Only ten of these cases, however, have been pure cases of continuous mania. Eight gave a decided definite agglutination, one was doubtful, and the tenth—one of the patients above noted, who relapsed—gave no reaction. No “control” serum ever gave a reaction, nor did the serum of these patients suffering from mania agglutinate other organisms. The agglutinin in the blood was therefore a specific agglutinin.
