Predicting relapse or recurrence of depression: systematic review of prognostic models

Background
Relapse and recurrence of depression are common, contributing to the overall burden of depression globally. Accurate prediction of relapse or recurrence while patients are well would allow the identification of high-risk individuals and may effectively guide the allocation of interventions to prevent relapse and recurrence.
AimsTo review prognostic models developed to predict the risk of relapse, recurrence, sustained remission, or recovery in adults with remitted major depressive disorder.
MethodWe searched the Cochrane Library (current issue); Ovid MEDLINE (1946 onwards); Ovid Embase (1980 onwards); Ovid PsycINFO (1806 onwards); and Web of Science (1900 onwards) up to May 2021. We included development and external validation studies of multivariable prognostic models. We assessed risk of bias of included studies using the Prediction model risk of bias assessment tool (PROBAST).
ResultsWe identified 12 eligible prognostic model studies (11 unique prognostic models): 8 model development-only studies, 3 model development and external validation studies and 1 external validation-only study. Multiple estimates of performance measures were not available and meta-analysis was therefore not necessary. Eleven out of the 12 included studies were assessed as being at high overall risk of bias and none examined clinical utility.
ConclusionsDue to high risk of bias of the included studies, poor predictive performance and limited external validation of the models identified, presently available clinical prediction models for relapse and recurrence of depression are not yet sufficiently developed for deploying in clinical settings. There is a need for improved prognosis research in this clinical area and future studies should conform to best practice methodological and reporting guidelines.

We welcome Kline's attempt to develop an overarching framework to allow much needed collaboration between fields in the study of teaching. While we see much utility in this enterprise, we are concerned that there is too much focus on the behavior of the teacher, without examining results or costs, and the categories within the framework are not sufficiently distinct.

We agree with Ramsey et al. regarding the need for new methods and concepts in the study of innovation, and welcome their initiative, but are concerned that their operationalization is over-reliant on subjective judgements.

John W. Lederle, age 94, born in Royal Oak, Michigan in 1912, president of the University of Massachusetts, 1960–1970, died February 13, 2007. His wife of almost 69 years, Angie, died March 4th, both in Naples, Florida, where they lived in retirement. They leave a daughter, Pamela Marro, and a son, Thomas.
John Lederle's papers, some 32.5 linear feet of shelf space, are in the Special Collections and University Archives of the Du Bois Library at the University of Massachusetts, Amherst. They cover chiefly his time as University president (1960–1970), but the collection spans the years 1947–1983. The include professional correspondence, administrative records, subject files, committee notes, reports, and clippings. Administrative records document the operation of the Amherst campus as well as the development of two new campuses (Boston and the Medical School in Worcester). Extra-university records document other Lederle activities, including work with various boards and agencies. Personal correspondence and a transcript of an oral history are included.

A continuing study of ship's bridge design and ship control conducted by the Defence Research Board of Canada has recognized that the conventional method of issuing and executing verbal orders contains inherent delay and is a possible source of error in ship control. As a partial solution to this problem, it was proposed that ‘the officer originating decisions to change the course and speed should implement these decisions himself through the direct control of helm and engines on the bridge’ (p. 21). It was further proposed that the direct controls be located on a console positioned at the front of the bridge on the centre-line of the ship.
The conventional location and arrangement of the bridge restricts visibility, however, in areas close to the ship, and this limited visibility from the bridge often causes manœuvring difficulties. In some instances, distances as great as 800 ft. ahead of the bow and 50 ft. abeam are ‘blind areas’. In most cases, areas aft on the centre-line cannot be seen at all. In an attempt to alleviate these problems, bridges have been extended to the sides of ships, or bridge wings provided so that much of the ship's side can be seen by the conning officer. With direct control from the centre-line and the use of a television system, bridge wings could possibly be eliminated, while still providing better close-in vision.
