Background
This study investigates associations of several dimensions of childhood adversities (CAs) with lifetime mental disorders, 12-month disorder persistence, and impairment among incoming college students.
MethodsData come from the World Mental Health International College Student Initiative (WMH-ICS). Web-based surveys conducted in nine countries (n = 20 427) assessed lifetime and 12-month mental disorders, 12-month role impairment, and seven types of CAs occurring before the age of 18: parental psychopathology, emotional, physical, and sexual abuse, neglect, bullying victimization, and dating violence. Poisson regressions estimated associations using three dimensions of CA exposure: type, number, and frequency.
ResultsOverall, 75.8% of students reported exposure to at least one CA. In multivariate regression models, lifetime onset and 12-month mood, anxiety, and substance use disorders were all associated with either the type, number, or frequency of CAs. In contrast, none of these associations was significant when predicting disorder persistence. Of the three CA dimensions examined, only frequency was associated with severe role impairment among students with 12-month disorders. Population-attributable risk simulations suggest that 18.7–57.5% of 12-month disorders and 16.3% of severe role impairment among those with disorders were associated with these CAs.
ConclusionCAs are associated with an elevated risk of onset and impairment among 12-month cases of diverse mental disorders but are not involved in disorder persistence. Future research on the associations of CAs with psychopathology should include fine-grained assessments of CA exposure and attempt to trace out modifiable intervention targets linked to mechanisms of associations with lifetime psychopathology and burden of 12-month mental disorders.

A consensus workshop on low-calorie sweeteners (LCS) was held in November 2018 where seventeen experts (the panel) discussed three themes identified as key to the science and policy of LCS: (1) weight management and glucose control; (2) consumption, safety and perception; (3) nutrition policy. The aims were to identify the reliable facts on LCS, suggest research gaps and propose future actions. The panel agreed that the safety of LCS is demonstrated by a substantial body of evidence reviewed by regulatory experts and current levels of consumption, even for high users, are within agreed safety margins. However, better risk communication is needed. More emphasis is required on the role of LCS in helping individuals reduce their sugar and energy intake, which is a public health priority. Based on reviews of clinical evidence to date, the panel concluded that LCS can be beneficial for weight management when they are used to replace sugar in products consumed in the diet (without energy substitution). The available evidence suggests no grounds for concerns about adverse effects of LCS on sweet preference, appetite or glucose control; indeed, LCS may improve diabetic control and dietary compliance. Regarding effects on the human gut microbiota, data are limited and do not provide adequate evidence that LCS affect gut health at doses relevant to human use. The panel identified research priorities, including collation of the totality of evidence on LCS and body weight control, monitoring and modelling of LCS intakes, impacts on sugar reduction and diet quality and developing effective communication strategies to foster informed choice. There is also a need to reconcile policy discrepancies between organisations and reduce regulatory hurdles that impede low-energy product development and reformulation.

INTRODUCTION:
The Spanish National Network (REDETS) is a group of eight agencies, units and services, depending on National and Regional Governments that coordinate their work within a common methodological framework, guided by the principles of mutual recognition and cooperation. The necessity of considering a Quality Management System has been detected and, consequently, a common tool for all the members needs to be developed. We describe in this study the process to achieve that goal.
METHODS:Based on both a review of previous literature and the proposal for a self-evaluating tool, a group of experts from each agency through consensus have developed a tool for self-evaluation in Health Technology Assessment (HTA) agencies. Through the structure described in the handbook of the Andalusian Agency for Healthcare Quality (ACSA), each standard should have a statement or proposal that needs to also include evidence or good practices, and the corresponding evaluation questions. In separate workgroups, the definition of these proposals, evidence and evaluation questions were developed. One face-to-face meeting and two meetings via teleconference were necessary to achieve a final document with all the quality standards.
RESULTS:From a proposed structure of sixty-six standards, the titles, definitions, statements and evidence as well as good practices and evaluation questions were established in workgroups with consensus among all of the members (1 - 3). The final version of the self-assessment tool was composed of sixty-eight standards, grouped in twelve quality criteria structured in four dimensions: I Responsibility, II Clients and Stakeholders, III Production Process, and IV Resources.
CONCLUSIONS:Quality management requires an evaluation tool and this version, based on a systematic review and consensus, is a useful and practical instrument for developing a handbook by each member of REDETS. An online version of the tool is in process of development.

The deficiency of Se, an essential micronutrient, has been implicated in adverse pregnancy outcomes. Our study was designed to determine total serum Se, selenoproteins (extracellular glutathione peroxidase (GPx-3), selenoprotein P (SeP)), selenoalbumin (SeAlb) and selenometabolites in healthy women and their newborns at delivery. This cross-sectional study included eighty-three healthy mother–baby couples. Total Se and Se species concentrations were measured in maternal and umbilical cord sera by an in-series coupling of two-dimensional size-exclusion and affinity HPLC. Additional measurements of serum SeP concentration and of serum GPx-3 enzyme activity were carried out using ELISA. Total Se concentration was significantly higher in maternal serum than in cord serum (68·9 (sd 15·2) and 56·1 (sd 14·6) µg/l, respectively; P<0·01). There were significant correlations between selenoprotein and SeAlb concentrations in mothers and newborns, although they also showed significant differences in GPx-3 (11·2 (sd 3·7) v. 10·5 (sd 3·5) µg/l; P<0·01), SeP (42·5 (sd 9·5) v. 28·1 (sd 7·7) µg/l; P<0·01) and SeAlb (11·6 (sd 3·6) v. 14·1 (sd 4·3) µg/l; P<0·01) concentrations in maternal and cord sera, respectively. Serum GPx-3 activity and concentration were positively correlated in mothers (r 0·33; P=0·038) but not in newborns. GPx-3 activity in cord serum was significantly correlated with gestational age (r 0·44; P=0·009). SeAlb concentration was significantly higher in babies, whereas SeP and GPx-3 concentrations were significantly higher in mothers. The differences cannot be explained by simple diffusion; specific transfer mechanisms are probably involved. GPx-3 concentrations in mothers, at delivery, are related to maternal Se status, whereas the GPx-3 activity in cord serum depends on gestational age.

Summary
Abstract
Future water needs and sources of supply are often highly uncertain. This is especially the case in the many areas under stress from climate change and rapid, variable urban development. Innovations in water treatment technologies, and price volatility in energy and other resources combine to aggravate the uncertainties concerning the risks and opportunities associated with planning investments in water infrastructure systems. The problem is that conventional techniques for the planning and design of water supply systems do not adequately deal with these important uncertainties. This chapter presents “Flexible Design” as an approach to deal with the situation, and demonstrates its effectiveness in a case study, inspired by Singapore, of a long- term design of the water supply system for a major city. The results show that Flexible Design can significantly increase overall performance compared to conventional design methods— a 15 percent increase in expected present monetary value, while decreasing downside risk and increasing upside opportunities. Flexible Design achieves these improvements by carefully analyzing possible futures using Monte Carlo simulation and designing the system with modules that managers can implement as and when needed. Flexible Design is shown to be an effective tool and is increasingly used in infrastructure development.
Introduction
The future is uncertain. Technologies, needs, policies, economies, and environments change frequently. Some changes are quantifiable, and we attempt to make best estimates of what will happen, but we will always encounter game- changing events and unanticipated new technologies.
When decision-makers consider needs for future water infrastructure, they often use a conventional, deterministic approach. They assume that current conditions will not change and that we can safely rely on a forecast of long- term requirements. However, the water supply and demand system is complex. Natural, societal, and political forces interact to drive changes in both supply and demand over time. Our ability to predict future conditions is low when dealing with such complexity, leading to forecasts that are rarely accurate.
For projects in transboundary watersheds or other multi- stakeholder water systems, reaching consensus among stakeholders on highly uncertain future conditions is a key challenge; there is a tendency to downplay uncertainty to make the problem more tractable and to advocate for a vision of the future convenient to dominant interests (Lempert and Kalra 2011).

Summary
Abstract
Future needs for urban water supply are often highly uncertain, especially in cities with rapid, variable population growth. Technological innovations and volatility in energy prices aggravate the risks of investments in water infrastructure systems. In such uncertain conditions, Flexible Design can significantly increase expected performance compared to conventional designs, as Chapter 6 in this book demonstrates. Flexible Design achieves this result by embedding Real Options in the design and using them to adjust the system performance over time according to need. The problem is: How should we provide the contractual incentives to insure that the designers and suppliers of the system deliver appropriate flexible designs? This chapter shows, through a detailed case study of desalination projects for Qatar, that the form of the contract between the water authority and the suppliers greatly influences the overall design and economic performance. This chapter applies a waterfall analysis to trace how contracts allocate benefits and differentially motivate designers to create or avoid flexible designs. This result means that, contrary to current practice, joint consideration of physical design and contractual arrangements can improve the design of water supply systems. Additionally, understanding the impact of contract choice on the willingness of a public– private partnership (PPP) to implement a flexible design offers insights for water infrastructure projects with other types of multi- stakeholder relationships.
Introduction
This chapter argues that, to achieve the expected outcomes, designers and negotiators need to jointly consider both the physical and contractual arrangements of a project. The reasoning is:
The future outcomes of any project are uncertain because of inescapable changes in needs, supplies, costs, and technologies.
Contractual arrangements inevitably allocate these outcomes differentially between water suppliers and end users— largely, but not only, because their costs and benefits occur unevenly over time.
Designers will shape the project in response to the risks associated with the contracts, which may not be best from the overall societal perspective.
Therefore, to get the best project designs, we need to consider jointly the physical and contractual arrangements for a project.
