Markers of susceptibility to acute rheumatic fever: the B-cell antigen D8/17 is not robust as a marker in South Africa

Background
Acute rheumatic fever and its chronic sequelae, rheumatic cardiac disease, and neuropsychiatric movement disorders, remain major public health problems in South Africa. Early identification and treatment of streptococcal pharyngitis in susceptible individuals would prevent rheumatic cardiac disease. The B-cell antigen D8/17 is a marker of susceptibility to rheumatic fever in some populations.
Methods and resultsWe assessed the significance of the D8/17 marker in a group of South Africans. Blood was collected from 107 individuals; 40 patients had previous confirmed rheumatic fever, 20 were first-degree relatives, and 47 were controls. The expression of D8/17 in each sample was analysed by flow cytometry. The mean proportion of B-cells that were D8/17 positive was 0.5% in the index cases, 0.47% in their relatives, and 0.27% in the controls. There was a significant difference between the index cases and the controls, p = 0.03, but the mean percentage positive in each group was very low.
ConclusionsPatients with a history of rheumatic fever had statistically increased expression of the D8/17 marker. However, the actual percentages in this observational study were markedly lower than in other populations, ranging from 0.14%–1.53% compared to 11.6%–39.3%. The D8/17 marker would be an impractical screening tool in the South African population.

A new emphasis on understanding natural systems at large spatial scales has led to an interest in deriving ecological variables from satellite reflectance images. The normalized difference vegetation index (NDVI) is a measure of canopy greenness that can be derived from reflectances at near-infrared and red wavelengths. For this study we investigated the relationships between NDVI and leaf-area index (LAI), intercepted photosynthetically active radiation (IPAR), and biomass in an Arctic tundra ecosystem. Reflectance spectra from a portable field spectrometer, LAI, IPAR, and biomass data were collected for 180 vegetation samples near Toolik Lake and Imnavait Creek, Alaska, during July and August 1993. NDVI values were calculated from red and near-infrared reflectances of the field spectrometer spectra. Strong linear relationships are seen between mean NDVI for major vegetation categories and mean LAI and biomass. The relationship between mean NDVI and mean IPAR for these categories is not significant. Average NDVI values for major vegetation categories calculated from a SPOT image of the study area were found to be highly linearly correlated to average field NDVI measurements for the same categories. This indicates that in this case it is appropriate to apply equations derived for field-based NDVI measurements to NDVI images. Using the regression equations for those relationships, biomass and LAI images were calculated from the SPOT NDVI image. The resulting images show expected trends in LAI and biomass across the landscape.

Background
Studies suggest that neuronal density in left dorsolateral prefrontal cortex is increased in schizophrenia.
AimsTo replicate these findings and extend them to both hemispheres.
MethodNeuronal density, size and shape were estimated in the prefrontal cortex (Brodmann area 9) of the left and right hemispheres of brains taken postmortem from 10 people with schizophrenia and 10 without mental illness (6 men, 4 women in both groups).
ResultsOverall neuronal density (individually corrected for shrinkage) did not differ between the groups. In the control brains, density was generally greater in the left than the right hemisphere, the reverse was seen in the schizophrenia brains; this loss or reversal of asymmetry was most significant in cortical layer 3. Pyramidal neurons in this cell layer were significantly larger on the left and more spherical in shape than on the right side in control brains, but size and shape did not differ between the two sides in schizophrenia. Non-pyramidal and glial cell densities were unchanged.
ConclusionsWe failed to find an increase in neuronal density, but found evidence at a cellular level of loss or reversal of asymmetry, consistent with the hypothesis of a primary change in the relative development of areas of heteromodal association cortex in the two hemispheres.
Schizophrenia and the frontal lobes: Post-mortem stereological study of tissue volume

It has been suggested that there is frontal lobe involvement in schizophrenia, and that it may be lateralised and gender-specific.
AimsTo clarify the structure of the frontal lobes in schizophrenia in a postmortem series.
MethodThe volume of white matter and cortical components of the frontal lobes was measured in brains of controls and patients with schizophrenia using planimetry and the Cavalieri principle. The components measured were: superior frontal gyrus, middle frontal gyrus, a composite of inferior frontal gyrus and orbito-frontal cortex, as well as total frontal lobe cortex and white matter. In addition, the anterior cingulate gyrus was measured.
ResultsNo diagnosis, gender, diagnosis × side, diagnosis × gender or diagnosis × gender × side interactions were observed in the volume of any of the components, the grey matter as a whole or the white matter. No evidence for volumetric inter-group differences was found for the anterior cingulate gyrus.
ConclusionsSuch structural abnormalities as are present in the frontal lobes are more subtle than straightforward alterations in tissue volume; they may include changes in shape and the pattern of gyral folding.

A previous report by Crow of a left-sided increase in temporal horn volume in schizophrenia implies a left-sided loss of tissue.
AimsTo elucidate the structural nature of schizophrenia.
MethodThe volume of grey matter in the temporal pole and inferior, middle and superior temporal gyri was measured, in addition to the total volume of grey and white matter, in the temporal lobes of the brains of 29 patients with schizophrenia and 27 controls.
ResultsWe found a significant left-sided reduction in the superior temporal gyrus in both males and females with schizophrenia, which was related to increasing age of onset in the males. The total volume of temporal lobe grey and white matter was also significantly reduced. Although being more marked on the left than the right, the lateralisation for these total grey and white measures (by contrast with the superior temporal gyrus alone) did not attain formal statistical significance.
ConclusionsConfirmation of a lateralised reduction in the superior temporal gyrus, which is differentially related to age of onset according to gender, adds to evidence that the changes in schizophrenia are in systems that are lateralised. The findings implicate language as the relevant function.

The working of the Mental Health (Scotland) Act 1960 is examined by analysis of compulsory admissions both nationally and to one Glasgow hospital. Comparisons are drawn with England and Wales. Despite a six-fold increase in admissions since 1945 the use of compulsory powers has remained almost constant. It is suggested that an annual number of 45 compulsory admissions per 100,000 population may represent an inevitable basic level as foreseen by the Royal Commission in 1957. Section 31 (‘Emergency Recommendation’) secures 80 per cent of Part IV admissions, and Section 24 (‘Full Recommendation’) is employed in only 25 per cent. Part V accounts for only 300 admissions per annum, mostly remands for psychiatric report. Two suggestions are made for possible future amendments to the Act.
