Summary
RESEARCH INTO MEDICAL collections demonstrates that numerous universities and hospitals collected in order to preserve the history of their profession and to train new generations. Unlike some of the more esoteric subjects in this book, medical memories are shared by the majority of the population. For many, these memories tend to take a particular form associated with trauma and sickness. While positive associations towards medical collections will of course also be held by individuals, often the memories are more difficult. The emotive nature of many medical collections adds a unique layer of meaning to these objects, offering enhanced potential for them to be considered as “unloved.” In this medical museum context, the term “unloved” is characterized by collections that not only have difficult associations but also lack aesthetic appeal and personalization. However, for professionals who worked with medical objects and medicalized bodies for decades, the emotional resonance may be very different.
My perspective on this subject is that of a museum practitioner who has observed many interactions between visitors and medical collections. I come to the issue of “care” for collections as someone interested in exploring the humanity within medical collections. This revaluing and re-evaluation of medical collections is becoming a common element of curatorial practice. Hence, over time, medical curation has become less about technically cataloguing specimens and soulless equipment and more about understanding how medical objects can become objects of universal enquiry and shared experience. Ultimately this helps us to understand what it means to be human. This subject follows wider shifts in the medical profession towards greater acknowledgement of emotions in medical work, both from the practitioner and patient perspectives, and it is a subject I will return to at the end of the discussion.
This chapter focuses on the medical handling collection used at The Infirmary, Worcester, a collection comprised of seemingly “unloved” objects. Two specific themes will be drawn out in the discussions: the contribution that former medical and healthcare professionals, acting as “specialist volunteers,” make to reviving and engaging others with medical collections and, second, the role of collaborative community-orientated projects as sites for the reinvigoration of these collections. In other chapters in this volume (see Smith, Hess, and Woodham and Kelleher), authors discuss how the skills and knowledge of retired professionals who are also collections enthusiasts might breathe life into stagnant objects.

Aims and method
Community treatment orders (CTOs) were introduced in Scotland in 2005, but are controversial owing to a lack of supportive randomised evidence. The non-randomised studies provide mixed results on their efficacy and utility. We aimed to examine hospital bed day usage across Scotland both before and after CTOs were initiated in a national cohort of patients, spanning 5 years.
ResultsIn total, 1558 individuals who were subject to a CTO between 2007 and 2012, of whom 63% were male, were included. After CTO initiation the number of hospital bed days fell, on average, from 66 to 39 per annum per patient. Those with a longer psychiatric history appeared to benefit more from a CTO, in terms of reduced time in hospital.
Clinical implicationsOur data offer cautious support for the use of CTOs in routine practice, in terms of reducing time spent in psychiatric hospital. This finding is balanced by the more rigorous randomised studies which do not find any benefit to CTOs.
Connecting Socially Isolated Older Rural Adults with Older Volunteers through Expressive Arts

Employing a participatory arts-based research approach, we examined an innovative program from rural Ontario, Canada, designed to address social isolation among older people. Older socially isolated adults were matched to trained volunteers, where in dyads, the eight pairs created expressive art in their home setting over the course of 10 home visits. With thematic and narrative inquiry, we analysed the experiences and perceptions of the program leader, older participants, and older volunteers via their artistic creations, weekly logs, evaluations, and field notes. The findings reveal a successful intervention that positively influenced the well-being of older adult participants and older volunteers, especially in regards to relationships, personal development, and creating meaning as well as extending the intervention’s impact beyond the program’s duration. We also discuss opportunities for similar programs to inform policy and enable positive community-based health and social service responses to rural social isolation.

Aims and method
Community compulsion via community compulsory treatment orders is used routinely in Scotland. We aimed to describe the common characteristics of individuals subject to community compulsion. We collected standardised information from a national database about individuals subject to community compulsion and compared them with people subject to hospital detention.
ResultsAnalysis of 499 cases revealed that the majority of individuals subject to community compulsion had a psychotic illness, had a history of non-adherence to services and treatment, and were more likely than not to be in receipt of a long-acting injection of antipsychotic medication. Patients subject to community compulsion were clinically similar to patients subject to hospital-based treatment orders and usually were considered to pose a risk to other people.
Clinical implicationsCommunity compulsion has been widely adopted despite a relative lack of supporting scientific evidence. Our findings are similar to those of other related studies and highlight that individuals with a psychotic illness who are ambivalent about treatment and who pose a risk to self or others are likely to be considered for community compulsion.

Recent developments in the understanding, assessment and treatment of suicidal behaviour are reviewed. The accumulated social and demographic knowledge of suicidal populations has proved to be clinically useful in shaping the general level of concern, but at the individual level an understanding of the psychological processes involved in suicidal behaviour is required. Studies examining psychological processes involved in suicidal behaviour is required. Studies examining psychological processes in parasuicidal groups have revealed a number of deficits, including poor interpersonal problem solving, hopelessness about the future, and reduced ability to regulate affect. Research has also begun to look at some of the processes underlying these deficits, such as over-general retrieval of autobiographical memories and reduced anticipation of specific positive experiences. The clinical picture is now more optimistic, with therapies, such as Dialectical Behaviour Therapy, which focus on using problem solving strategies with those clients who are most vulnerable to repeat parasuicide episodes, producing demonstrable delays in parasuicide and reduced risk of repetition. The importance of the relationship between research and clinical practice is emphasized.
