Mortality gap for people with bipolar disorder and schizophrenia: UK-based cohort study 2000–2014

Background
Bipolar disorder and schizophrenia are associated with increased mortality relative to the general population. There is an international emphasis on decreasing this excess mortality.
AimsTo determine whether the mortality gap between individuals with bipolar disorder and schizophrenia and the general population has decreased.
MethodA nationally representative cohort study using primary care electronic health records from 2000 to 2014, comparing all patients diagnosed with bipolar disorder or schizophrenia and the general population. The primary outcome was all-cause mortality.
ResultsIndividuals with bipolar disorder and schizophrenia had elevated mortality (adjusted hazard ratio (HR) = 1.79, 95% CI 1.67–1.88 and 2.08, 95% CI 1.98–2.19 respectively). Adjusted HRs for bipolar disorder increased by 0.14/year (95% CI 0.10–0.19) from 2006 to 2014. The adjusted HRs for schizophrenia increased gradually from 2004 to 2010 (0.11/year, 95% CI 0.04–0.17) and rapidly after 2010 (0.34/year, 95% CI 0.18–0.49).
ConclusionsThe mortality gap between individuals with bipolar disorder and schizophrenia, and the general population is widening.
Emergency Department Visits after Diagnosed Chronic Obstructive Pulmonary Disease in Aboriginal People in Alberta, Canada

Objectives
This retrospective cohort study compared rates of emergency department (ED) visits after a diagnosis of chronic obstructive pulmonary disease (COPD) in the three Aboriginal groups (Registered First Nations, Métis and Inuit) relative to a non-Aboriginal cohort.
MethodsWe linked eight years of administrative health data from Alberta and calculated age- and sex-standardized ED visit rates in cohorts of Aboriginal and non-Aboriginal individuals diagnosed with COPD. Rate ratios (RR) with 95% confidence intervals (CIs) were calculated in a Poisson regression model that adjusted for important sociodemographic factors and comorbidities. Differences in ED length of stay (LOS) and disposition status were also evaluated.
ResultsA total of 2,274 Aboriginal people and 1,611 non-Aboriginals were newly diagnosed with COPD during the study period. After adjusting for important sociodemographic and clinical factors, the rate of all-cause ED visits in all Aboriginal people (RR=1.72, 95% CI: 1.67, 1.77), particularly among Registered First Nations people (RR=2.02; 95% CI: 1.97, 2.08) and Inuit (RR=1.28; 95% CI: 1.22, 1.35), were significantly higher than that in non-Aboriginals, while ED visit rates were significantly lower in the Métis (RR=0.94; 95% CI: 0.90, 0.98). The ED LOS in all Aboriginal groups were significantly lower than that of the non-Aboriginal group.
ConclusionsAboriginal people with COPD use almost twice the amount of ED services compared to their non-Aboriginal counterparts. There are also important variations in patterns of ED services use among different Aboriginal groups with COPD in Alberta.

This article summarises internet-mediated approaches to conducting quantitative and qualitative cross-sectional mental health research, and describes aspects of research design to consider for optimising scientific rigour and validity as well as response. Rapid adoption of internet-mediated approaches risks compromising the quality of the methods used. Not only can it cause distress to participants, but methodological problems may lead to inappropriate inferences being made from research findings. In this article the advantages of using internet communication for research purposes are balanced against the disadvantages, using examples of recent internet-mediated research (IMR) studies to illustrate good practice.

Contemporary research on prehispanic Maya landscapes has focused on caves as core features of the cultural geography. Investigations within a number of large caves have suggested that they served as the loci for important rituals, legitimized inhabitants’ claims to their territory, and helped establish the authority of a site’s ruling elite. The ubiquity and centrality of caves in the Maya worldview raises questions about what happened in regions where large caves did not naturally form. Recent investigations at the site of Maax Na in northern Belize suggest that small caves, despite their diminutive size, still functioned to establish legitimacy and uphold power. The results serve to demonstrate the pervasive power of key ideological concepts in shaping the cultural landscape and indicate the need to take these into account in documenting landmarks at Maya sites, as even the less imposing ones may have been important to their inhabitants.

Five hundred and eighty schoolgirls living in a small North Indian industrial town were screened using the EAT-26. Although twenty-nine per cent scored above the threshold for the questionnaire (≥ 20), closer scrutiny of the pattern of responses revealed misinterpretations of a large number of questions possibly on a conceptual basis. Linguistic and conceptual pitfalls inherent to cross-cultural epidemiological research are discussed with reference to previous work.

There is an increasing need for early detection strategies to take a part in the daily routine of general practice. A cost—benefit analysis is performed for the use of the CAGE questionnaire in case finding for at risk drinkers among general practice attenders. The questionnaire is shown to be of advantage to the general practitioner over and above his usual diagnostic methods.

Jockeys based in racing stables in the Newmarket and Epsom areas of England were screened using the 26-item Eating Attitudes Test. The mean score of the EAT was 14·9, significantly above that reported for males in other studies. The response rate was poor, but 10 jockeys agreed to a full psychiatric and eating interview. The majority of jockeys interviewed reported food avoidance, the use of saunas, and the abuse of laxatives. Diuretics and appetite suppressants were also used. Bingeing was common, but self-induced vomiting was unusual. Current weights were 13% below, and the lowest reported weights 21% below, matched population mean weights. The relationship between these forms of eating behaviour and clinical eating disorders is discussed.

Background
Despite concern about the incidence of coronary heart disease (CHD) in people with severe mental illness (SMI), there is little systematic research on CHD risk factors in this population.
AimsTo compare the main risk factors for CHD in people with and without SMI in primary care, to investigate the role of socio-economic variables, and to examine any association between antipsychotic medication and CHD risk.
MethodCross-sectional screening.
ResultsIn total, 75 of 182 general practice patients with SMI and 150 of 313 such patients without SMI attended the interview. SMI was associated with: raised 10-year CHD risk scores (OR= 1.8, 95% CI 1.0–3.1); high-density-lipoprotein (HDL)-cholesterol levels <l.0 mmol/l (OR=4.0, 95% CI 1.5–10.7); raised cholesterol/HDL-cholesterol ratios (OR=1.8, 95% CI 1.0–3.2); diabetes mellitus (OR=3.8, 95% CI 1.1–13.3) and smoking (OR=3.0, 95% CI 1.7–3.4). These associations varied significantly with age. Adjustment for unemployment did not fully explain the associations.
ConclusionsExcess risk factors for CHD are not wholly accounted for by medication or socio-economic deprivation. There is an urgent need for CHD screening and for relevant interventions for smoking cessation and diabetes, as well as advice on diet and exercise, in patients with SMI.

Foolish as the theory of Durkeim may be in confusing what is religious with what is social, it yet contains an element of truth; that is to say that the social feeling is so much like the religious as to be mistaken for it. (Simone Weil, 1951).
Psychiatrists concern themselves with human mental suffering. Behind the consulting room door they reflect with their patients on questions of meaning and existence, issues that concern philosophy and religion as much as psychiatry. It is striking, therefore, that psychiatrists regard spirituality and religion as, at best, cultural noise to be respected but not addressed directly, or at worst pathological thinking that requires modification (Larson et al. 1993).
Despite two millennia of debate we are little nearer a consensus on the meanings of spirituality and religion. The word ‘religion’ has as many definitions as writers. Spirituality and religion are often used interchangeably. Spilka (1985) doubts that a single definition is even possible. Dittes (1969) argues that religion contains so many unrelated variables that it cannot be considered as a unidimensional concept in research. We would argue that religion is the outward practice of a spiritual system of beliefs, values, codes of conduct and rituals (Speck, 1988). Religious groups may function like any other with codes of behaviour, political alliances and ‘in’ and ‘out’ group member ideology (Sherif et al. 1966).
Unfortunately, a concentration on the religious variable has led to a failure to appreciate the broader concept of spiritual and the presumption that if someone does not profess a recognized, religious faith, they have no spiritual discernment or need (Speck, 1988). We propose a definition of ‘spiritual’ as a person's experience of, or a belief in, a power apart from their own existence. It may exist within them but is ultimately apart. It is the sense of relationship or connection with a power or force. It is more specific than a search for meaning or a feeling of unity with others. People may use the word ‘spiritual’ to describe intense emotional pleasure when moved by natural beauty or by an important relationship. Spiritual belief is more specific than that. Some people may use the word ‘God’ to describe this power; others may be less specific. Spirituality differs from belief in other powers, such as nuclear power or magnetism, in its ‘set apart’ quality and the degree to which it is revered and ritualized, the quality which Durkheim (1915) refers to as the sacred.
The Hampstead Schizophrenia Survey 1991: II: Incidence and migration in inner London

Background
The previous paper reports a high prevalence of schizophrenia (broad definition) in an inner London area. In this paper we test hypotheses for this finding and examine the characteristics of people with schizophrenia who move frequently.
MethodPeople with schizophrenia in the Hampstead area were identified by key informant methodology, at two censuses five years apart. This allowed identification of incident cases during these five years and identification of people who had moved into and out of the area.
ResultsThe incidence of DSM-III-R schizophrenia in Hampstead between 1986 and 1991 was at least 0.21 per 1000 of the population aged 15 to 54. There was a significant movement of people with schizophrenia to this inner London area from outer London between 1986 and 1991. People with schizophrenia who were relatively mobile were significantly more likely to be male, to suffer with prominent hallucinations, and to have no contact with a GP.
ConclusionsThe high prevalence of broad schizophrenia in this inner London area is, in part, due to geographical drift. A significant excess of the people with schizophrenia who move frequently are men with positive symptoms.

Background
A point prevalence study al schizophrenia was carried out in 1986 in the former Hampstead Health District using a key informant method to identify cases. The point prevalence al broadly defined schizophrenia was 4.7 per 1000.
MethodA repeat census al people with schizophrenia, using the same method, was carried out in 1991 and the point prevalence calculated. The accuracy al the census method was estimated. Contact with services, social and occupational activity, and medication usage in the 1986 and 1991 samples were compared.
ResultsThe point prevalence rate al broadly defined schizophrenia in 1991 was 5.1 per 1000. The results al both censuses showed we underestimated the number of individuals with DSM-III-R positive schizophrenia by about 14%. The level of contact with specialist services was greater in 1991 than 1986. Patients in 1991 were discharged on higher doses of medication, and their most recent admission was more likely to be due to non-compliance, than the 1986 group.
ConclusionThe point prevalence confirmed the high rate reported in 1986. The accuracy of the census was within acceptable limits for service planning. Contact with specialist services increased between 1986 and 1991.
