Background
Most people with bipolar disorder spend a significant percentage of their lifetime experiencing either subsyndromal depressive symptoms or major depressive episodes, which contribute greatly to the high levels of disability and mortality associated with the disorder. Despite the importance of bipolar depression, there are only a small number of recognised treatment options available. Consecutive treatment failures can quickly exhaust these options leading to treatment-resistant bipolar depression (TRBD). Remarkably few studies have evaluated TRBD and those available lack a comprehensive definition of multi-therapy-resistant bipolar depression (MTRBD).
AimsTo reach consensus regarding threshold definitions criteria for TRBD and MTRBD.
MethodBased on the evidence of standard treatments available in the latest bipolar disorder treatment guidelines, TRBD and MTRBD criteria were agreed by a representative panel of bipolar disorder experts using a modified Delphi method.
ResultsTRBD criteria in bipolar depression was defined as failure to reach sustained symptomatic remission for 8 consecutive weeks after two different treatment trials, at adequate therapeutic doses, with at least two recommended monotherapy treatments or at least one monotherapy treatment and another combination treatment. MTRBD included the same initial definition as TRBD, with the addition of failure of at least one trial with an antidepressant, a psychological treatment and a course of electroconvulsive therapy.
ConclusionsThe proposed TRBD and MTRBD criteria may provide an important signpost to help clinicians, researchers and stakeholders in judging how and when to consider new non-standard treatments. However, some challenging diagnostic and therapeutic issues were identified in the consensus process that need further evaluation and research.

The use of channel pressurisation in drawing microstructured optical fibres (MOFs) potentially allows for fine control of the internal structure of the fibre. By applying extra pressure inside the channels it is possible to counteract the effect of surface tension which would otherwise act to close the channels in the fibre as it is drawn. This paper extends the modelling approach of Stokes et al. (J. Fluid Mech., vol. 755, 2014, pp. 176–203) to include channel pressurisation. This approach treats the problem as two submodels for the flow, one in the axial direction along the fibre and another in the plane perpendicular to that direction. In the absence of channel pressurisation these models decoupled and were solved independently; we show that they become fully coupled when the internal channels are pressurised. The fundamental case of a fibre with an annular cross-section (containing one central channel) will be examined in detail. In doing this we consider both a forward problem to determine the shape of fibre from a known preform and an inverse problem to design a preform such that when drawn it will give a desired fibre geometry. Criteria on the pressure corresponding to fibre explosion and closure of the channel will be given that represent an improvement over similar criteria in the literature. A comparison between our model and a recent experiment is presented to demonstrate the effectiveness of the modelling approach. We make use of some recent work by Buchak et al. (J. Fluid Mech., vol. 778, 2015, pp. 5–38) to examine more complicated fibre geometries, where the cross-sectional shape of the internal channels is assumed to be elliptical and multiple channels are present. The examples presented here demonstrate the versatility of our modelling approach, where the subtleties of the interaction between surface tension and pressurisation can be revealed even for complex patterns of cross-sectional channels.
The glacial geomorphology of the Antarctic ice sheet bed

In 1976, David Sugden and Brian John developed a classification for Antarctic landscapes of glacial erosion based upon exposed and eroded coastal topography, providing insight into the past glacial dynamics of the Antarctic ice sheets. We extend this classification to cover the continental interior of Antarctica by analysing the hypsometry of the subglacial landscape using a recently released dataset of bed topography (BEDMAP2). We used the existing classification as a basis for first developing a low-resolution description of landscape evolution under the ice sheet before building a more detailed classification of patterns of glacial erosion. Our key finding is that a more widespread distribution of ancient, preserved alpine landscapes may survive beneath the Antarctic ice sheets than has been previously recognized. Furthermore, the findings suggest that landscapes of selective erosion exist further inland than might be expected, and may reflect the presence of thinner, less extensive ice in the past. Much of the selective nature of erosion may be controlled by pre-glacial topography, and especially by the large-scale tectonic structure and fluvial valley network. The hypotheses of landscape evolution presented here can be tested by future surveys of the Antarctic ice sheet bed.

The chemical composition (body and glaze) of over thirty Yaozhou porcelain fragments are determined by X-ray fluorescence (XRF) analysis. Statistical analysis of these results combined with data already published indicate that the full evolution process of Yaozhou porcelain can be divided into four stages. Each stage exhibits a characteristic composition determined by the raw materials and batch composition used to produce the porcelain bodies and glazes. Almost all of Yaozhou celadon and white ware glazes belong predominantly to the CaO-type glaze (or lime glaze), but the glazes of the Song Dynasty (A.D. 960–1280) and later periods have higher K2O content and less CaO than the earlier periods.
