Summary
This chapter addresses the major cardiovascular conditions faced by clinicians caring for older adults: ischemic heart disease, heart failure, valvular disease, arrhythmias and conduction system disorders, and pericardial diseases. After a brief review of the global burden of heart disease and the effects of aging on the cardiovascular system, each subsequent section of the chapter provides a consistent approach to the description of these disease processes. Beginning with a summary of the epidemiology and clinical presentations relevant to older adults, pharmacologic and procedural recommendations are then presented based on clinical trial data and practice guidelines specific to this patient population. Each section also highlights the importance of individualized care for patients with advanced age or substantial medical comorbidities, including commentary about prognosis and palliative care, when appropriate. With clinically relevant treatment recommendations, comprehensive trial review and data tables, and several illustrative figures, this chapter provides evidence-based guidance to assist with the daily clinical care of our aging population with cardiovascular diseases.

Summary
The incidence and prevalence of cardiovascular disease (CVD) increase progressively with age, and CVD is the leading cause of death and major disability in older adults. Aging affects cardiovascular structure and function. Common acute and chronic cardiovascular disorders affecting older adults include coronary artery disease, heart failure, valvular heart disease, and heart rhythm disorders. Because older adults with CVD comprise a markedly heterogeneous population, management must be individualized, with due consideration given to comorbidities, physical and cognitive function, and personal preferences regarding short-term and long-term goals of care.

We use the underlying data of the IMPLAN Pro 3.0 regional economic simulation model to estimate the current economic contribution of Michigan's local food system and explore the chain of transactions giving rise to consumption of locally sourced goods from producer to processor to consumption. The proposed methodology includes both unprocessed and processed foods in the estimation of the local food system's economic value. The model also provides a replicable and consistent approach to estimating the value of local food systems within regional and state economies.

The following study assesses cold-season hoop house lettuce production in the context of local food systems’ relative environmental effects. For this purpose, we compare the carbon footprints of leaf lettuce production in two climatic zones, one close to the consumer market and one distant, via environmental impact modeling in SimaPro 7.3. A site-specific scenario is first detailed with organic leaf lettuce locally grown in an East Lansing, Michigan hoop house. This is compared to a hypothetical scenario, modeled using average industry data, with leaf lettuce conventionally grown in California then shipped to East Lansing. The system boundaries used in this analysis extend from manufacturing of farm production inputs to a hypothetical retail gate. We assumed that the consumer drove the same distance to the retailer in each case. The functional unit used is 1 kg of leaf lettuce. Results demonstrate that the distant system exhibits 4.3 times the CO2 ‘footprint’ per kg of lettuce. This nonlocal system also resulted in higher resource depletion, health impact and ecological damage potential as demonstrated via the SimaPro simulation. This study concludes that unheated, hoop house lettuce production, given the assumption on within-area travel, has a smaller carbon footprint than outdoor, distant production, and speaks to both the potential value of more localized food systems and the need for a more diverse set of scenario modeling to understand the boundaries of this value.

SIGNIFICANCE OF GERIATRIC CARDIOLOGY
As the leading cause of death and major disability in the United States, cardiovascular disease (CVD) has become a modern epidemic resulting in part from the aging of our population. Nearly two-thirds of all cardiovascular hospitalizations occur in patients aged 65 years and older, more than 83% of cardiovascular deaths occur in geriatric individuals, and an estimated 70% of Americans older than the age of 70 years have clinically recognized cardiovascular diagnoses. For these reasons, both geriatric and nongeriatric practitioners should become familiar with strategies for CVD prevention and management in older individuals.
EFFECTS OF AGING ON THE CARDIOVASCULAR SYSTEM
Aging is associated with many alterations of cardiovascular structure and function (Table 11.1). Some of the most clinically relevant changes include increasing vascular stiffness, impaired endothelial function, impaired left ventricular relaxation and compliance, diminished responsiveness to neurohormonal signals such as β-adrenergic stimulation, and degeneration of the sinus node and electrical conduction system. These factors contribute to the development of medical conditions such as hypertension, coronary artery disease (CAD), and heart failure (HF), and many components of these aging processes also modulate clinical presentations and responses to cardiovascular therapies in the geriatric population. Similarly, aging affects other organ systems (Table 11.2), which frequently interact with CVDs and therapeutics. Other influences related to medical comorbidities, absorptive and metabolic alterations, behavioral or neuropsychiatric changes, and financial issues frequently play a role in modulating cardiovascular prognosis in older individuals as well.
The NICMOS View of M31's Metal Rich Globular Clusters

We have obtained HST-NICMOS observations of five of M31's most metal-rich globular clusters: G1, G170, G174, G177 & G280. For the two clusters farthest from the nucleus, G1 and G280, we statistically subtract the field population and estimate metallicities using K-(J - K) color-magnitude diagrams (CMDs). Based on the slopes of their infrared giant branches we estimate [Fe/H] = −1.22 ± 0.43 dex for G1 and −0.15 ± 0.37 dex for G280. We combine our infrared observations of G1 with two epochs of optical HST-WFPC2 F-band data and identify at least one LPV based on color and variability. The location of G1's giant branch in the K, (V - K) CMD is very similar to that of M107, indicating a somewhat higher metallicity than our purely infrared CMD; [Fe/H]= −0.9 ± 0.2 dex.
