Effectiveness of a peer-led adolescent mental health intervention on HIV virological suppression and mental health in Zimbabwe: protocol of a cluster-randomised trial

Background
Adolescents living with HIV (ALHIV) experience a high burden of mental health disorder which is a barrier to antiretroviral therapy adherence. In Zimbabwe, trained, mentored peer supporters living with HIV (Community Adolescent Treatment Supporters – CATS) have been found to improve adherence, viral suppression and psychosocial well-being among ALHIV. The Friendship Bench is the largest integrated mental health programme in Africa. We hypothesise that combining the CATS programme and Friendship Bench will improve mental health and virological suppression among ALHIV compared with the CATS programme alone.
MethodsWe will conduct a cluster-randomised controlled trial in 60 clinics randomised 1:1 in five provinces. ALHIV attending the control arm clinics will receive standard CATS support and clinic support following the Ministry of Health guidelines. Those attending the intervention arm clinics will receive Friendship Bench problem-solving therapy, delivered by trained CATS. Participants with the signs of psychological distress will be referred to the clinic for further assessment and management. The primary outcome is HIV virological failure (≥1000 copies/ml) or death at 48 weeks. Secondary outcomes include the proportion of adolescents with common mental disorder symptoms (defined as Shona Symptom Questionnaire (SSQ-14) score ≥8), proportion with depression symptoms (defined as Patient Health Questionnaire (PHQ-9) score ≥11), symptom severity (mean SSQ-14 and PHQ-9 scores) and EQ-5D score for health-related quality of life.
ConclusionsThis trial evaluates the effectiveness of peer-delivery of mental health care on mental health and HIV viral load among ALHIV. If effective this intervention has the potential to be scaled-up to improve these outcomes.

The literature on neuropsychiatric, neuropsychological, and neuroimaging findings in autism and Asperger's disorder is briefly reviewed. Neurological soft sign, neuropsychological, and positron-emission tomography findings in one patient with Asperger's disorder and one patient with autism are highlighted, discussed as examples of heterogeneity, and integrated with the relevant literature.

A numerical investigation of the mean wall shear stress properties on a spatially developing turbulent boundary layer over a smooth flat plate was carried out by means of a zonal detached eddy simulation (ZDES) technique for the Reynolds number range
$3060\leq Re_{\theta }\leq 13\, 650$. Some asymptotic trends of global parameters are suggested. Consistently with previous findings, the calculation confirms the occurrence of very large-scale motions approximately
$5\delta $ to
$6 \delta $ long which are meandering with a lateral amplitude of
$0.3 \delta $ and which maintain a footprint in the near-wall region. It is shown that these large scales carry a significant amount of Reynolds shear stress and their influence on the skin friction, denoted
$C_{f,2}$, is revisited through the FIK identity by Fukagata, Iwamoto & Kasagi (Phys. Fluids, vol. 14, 2002, p. L73). It is argued that
$C_{f,2}$ is the relevant parameter to characterize the high-Reynolds-number turbulent skin friction since the term describing the spatial heterogeneity of the boundary layer also characterizes the total shear stress variations across the boundary layer. The behaviour of the latter term seems to follow some remarkable self-similarity trends towards high Reynolds numbers. A spectral analysis of the weighted Reynolds stress with respect to the distance to the wall and to the wavelength is provided for the first time to our knowledge and allows us to analyse the influence of the largest scales on the skin friction. It is shown that structures with a streamwise wavelength
$\lambda _x >\delta $ contribute to more than
$60\, \%$ of
$C_{f,2}$, and that those larger than
$\lambda _x >2\delta $ still represent approximately
$45\, \%$ of
$C_{f,2}$.

Cardiopulmonary bypass in neonates is markedly different from the procedure used in children above 10 kg and in adults. There are two major reasons for this—first, the mismatch between the volume of the heart-lung machine circuit and that of the blood in the patient and second, the immaturity of tissues in the first three months of life.

Ethylene glycol modified precursors, such as tetrakis(2-hydroxyethyl)orthosilicate (EGMS) or bis(2-hydroxyethyl)titanate (EGMT), have distinct advantages in the synthesis of mesoporous materials by sol-gel processing compared to the commercially available tetraalkoxide precursors. The glycols released upon hydrolysis have proven to be compatible with lyotropic surfactant mesophases and in addition, these precursors allow for processing in purely aqueous conditions. Besides the standard characterization of the resulting titania and silica-based materials by XRD, electron microscopy, and nitrogen sorption, the potential of the titania-based materials for catalytic applications was tested using Au/TiO2 catalysts in low temperature CO oxidation reactions.
