Lameness is consistently better at predicting broiler chicken performance in mobility tests than other broiler characteristics

To determine whether lame broilers are in pain it is necessary to compare measures of lameness and mobility before and after analgesic treatment. Such measures should not be unduly affected by other bird characteristics. This study assessed the performance of lame (gait score, GS 3-4) and non-lame (GS 0-1) broilers using two mobility tests: (i) a novel test to assess broiler ability to access resources when housed in groups (Group Obstacle test); and (ii) a Latency-to-Lie (LTL) test. Outcome test measures included number of obstacle crossings, latency to cross an obstacle, and time taken to sit in shallow water. Associations between outcome test measures and other bird characteristics (established lameness risk-factors), including strain, sex, age, mass, contact dermatitis and pathology, were also investigated. The performance of high-GS and low-GS broilers differed in both mobility tests and no other bird characteristics were as consistent a predictor as lameness. This demonstrates that mobility impairments are closely related to lameness assessed using GS, and that there is a component of lameness that cannot be explained by other bird characteristics (eg being male and heavy). This component may represent pain or discomfort. Both mobility tests are suitable for further application with analgesic testing to classify lameness-associated pain in broilers.

Lame broiler chickens perform poorly in standardised mobility tests and have nociceptive thresholds that differ from those of nonlame birds, even when confounding factors such as differences in bodyweight are accounted for. This study investigated whether these altered responses could be due to pain, by comparing performance in a Group Obstacle test and a Latency to Lie (LTL) test of lame (Gait Score [GS] 2.5-4) and non-lame (GS 0-1) broilers administered analgesia or a saline control. We used exploratory subcutaneous doses of the non-steroidal anti-inflammatory drugs (NSAIDs), meloxicam (5 mg kg−1) or carprofen (35 mg kg−1) or the opioid butorphanol tartrate (4 mg kg−1). We included butorphanol to explore the possibility that NSAIDs could improve mobility by reducing inflammation without necessarily invoking an analgesic effect. Lameness was a significant predictor in all analyses. Neither the number of obstacle crossings nor latency to cross an obstacle was significantly changed by either NSAID, but LTL was longer in lame birds given carprofen and meloxicam than in lame birds given saline. LTL was associated with foot-pad dermatitis and ameliorated by both NSAIDs. Butorphanol did not affect LTL but appeared soporific in the obstacle test, increasing latency to cross and, in non-lame birds, reducing the number of crossings. Combined with data from other studies, the results suggest carprofen and meloxicam had some analgesic effect on lame birds, lending further support to concerns that lameness compromises broiler welfare. Further investigation of opioid treatments and lameness types is needed to disentangle effects on mobility and on pain.

Introduction
Fecal incontinence (FI) is the involuntary loss of feces and can affect up to 17 percent of community dwelling individuals, rising to 40 percent of older people in residential care homes. There is limited up-to-date evidence which formally set research priorities addressing FI. This project aimed to identify research topics of highest importance to key FI stakeholders.
MethodsAn evidence gap map was produced incorporating three streams of evidence coded against predefined topic domains. The evidence streams included: emerging evidence identified through horizon scanning; existing evidence identified through systematic searches of bibliographic databases; and key FI stakeholder insights collected through an international survey. Findings were presented as a visual map to facilitate knowledge exchange during an online workshop with a purposeful sample of multidisciplinary stakeholders. The identified gaps in research were explored to see whether they were deemed representative of unmet needs, and as such, areas of priority to key FI stakeholders. Ideation techniques and group discussions were used to refine and rank priority areas.
ResultsOverall, there was a mismatch between the existing and emerging evidence, and the priorities of key FI stakeholders. New pharmaceutical and medical technology innovations were limited. Eight percent of early-stage trials identified were concerned with the use of repurposed drugs. Within the existing evidence base, individual bowel management strategies and treatments were examined, however, key FI stakeholders desired interventions to improve patient education and the psychological aspects of living with FI. The five priority topics identified in order of importance are as follows: psychological support; lifestyle interventions; long-term effects; education; and constipation.
ConclusionsThe robust methodology used to identify priority topics were successful in identifying broad and wide-ranging areas of importance to key stakeholders. The evidence gap map was a useful visual tool to facilitate knowledge exchange and highlight where research efforts have been focused historically, identifying a mismatch between the existing evidence base and what stakeholders consider important.
Exercise, Arterial Stiffness, and Cerebral Vascular Function: Potential Impact on Brain Health

Exercise is associated with higher cognitive function and is a promising intervention to reduce the risk of dementia. With advancing age, there are changes in the vasculature that have important clinical implications for brain health and cognition. Primary aging and vascular risk factors are associated with increases in arterial stiffness and pulse pressure, and reductions in peripheral vascular function.
Objective:The purpose is to discuss the epidemiological, observational, and mechanistic evidence regarding the link between age-related changes in vascular health and brain health.
Methods:We performed a literature review and integrated with our published data.
Results:Epidemiological evidence suggests a link between age-related increases in arterial stiffness and lower cognitive function, which may be mediated by cerebral vascular function, including cerebral vasoreactivity and cerebral pulsatility. Age-associated impairments in central arterial stiffness and peripheral vascular function have been attenuated or reversed through lifestyle behaviors such as exercise. Greater volumes of habitual exercise and higher cardiorespiratory fitness are associated with beneficial effects on both peripheral vascular health and cognition. Yet, the extent to which exercise directly influences cerebral vascular function and brain health, as well as the associated mechanisms remains unclear.
Conclusion:Although there is evidence that exercise positively impacts cerebral vascular function, more research is necessary in humans to optimize experimental protocols and address methodological limitations and physiological considerations. Understanding the impact of exercise on cerebral vascular function is important for understanding the association between exercise and brain health and may inform future intervention studies that seek to improve cognition.
Changing landscape of nutrition and dietetics research? A bibliographic analysis of top-tier published research in 1998 and 2018

Objective:
The current study sought to describe and compare study type, research design and translation phase of published research in nutrition and dietetic journals in 1998 and 2018.
Design:This was a repeat cross-sectional bibliographic analysis of Nutrition and Dietetics research. All eligible studies in the top eight Nutrition and Dietetics indexed journals in 1998 and 2018 were included. Two independent reviewers coded each study for research design (study type and study design) and translation phase (T0-T4) of the research using seminal texts in the field.
Setting:Not relevant.
Participants:Not relevant.
Results:The number of publications (1998, n 1030; 2018, n 1016) has not changed over time, but the research type, design and translation phases have. The proportion of intervention studies in 1998 (43·8 %) was significantly higher than 2018 (19·4 %). In 2018, more reviews (46·9 % v. 15·6 % in 1998) and less randomised trials (14·3 % v. 37·8 % in 1998) were published. In regard to translation phase, there was a higher proportion of T2–T4 research in 2018 (18·3 % v. 3·8 % in 1998); however, the proportion of T3/T4 (dissemination, implementation and population-level research) research was still low (<3 %). Our sensitivity analysis with the four journals that remained in the top eight journal across the two time periods found no differences in the research type, design and translation phases across time.
Conclusions:There was a reduction in intervention and T0 publications, alongside higher publication of clinical study designs over time; however, published T3/T4 research in Nutrition and Dietetics is low. A greater focus on publishing interventions and dissemination and implementation may be needed.

The East Mediterranean Radiocarbon (inter-)Comparison Project (EMRCP) has measured the 14C ages of a number of sets of tree rings from the Gordion Area dendrochronology from central Anatolia at the Heidelberg Radiocarbon Laboratory. In several cases, multiple measurements were made over a period from the 1980s to 2009. This paper presents the final data set from this work (128 high-precision measurements), and considers (i) the relationship of these data against the standard Northern Hemisphere 14C calibration data set (IntCal09), and (ii) the optimum calendar dating of this floating tree-ring record on the basis of the final set of high-precision 14C data. It finds good agreement between the Anatolian data and IntCal09 in some important intervals (e.g. ∼1729 to 1350 cal BC) and observes one period (9th–8th centuries BC) where there appears to be some indication of a regional/growing season signal, and another period (later 14th–13th centuries BC) where IntCal09 may not best reflect the real 14C record. The scale of the typical growing-season-related regional 14C offset (ΔR) between the Aegean/Anatolian region and IntCal09 is also assessed (for the mid-2nd millennium BC and mid-2nd millennium AD), and found to be usually minor (at times where there are no major additional forcing factors and/or issues with the IntCal09 data set): of the order of 2–4 ± 2–4 yr.

In 1997 the Association for Professionals in Infection Control and Epidemiology and the Society for Healthcare Epidemiology of America established a consensus panel to develop recommendations for optimal infrastructure and essential activities of infection control and epidemiology programs in out-of-hospital settings. The following report represents the Consensus Panel's best assessment of requirements for a healthy and effective out-of-hospital-based infection control and epidemiology program. The recommendations fall into 5 categories: managing critical data and information; developing and recommending policies and procedures; intervening directly to prevent infections; educating and training of health care workers, patients, and nonmedical caregivers; and resources. The Consensus Panel used an evidence-based approach and categorized recommendations according to modifications of the scheme developed by the Clinical Affairs Committee of the Infectious Diseases Society of America and the Centers for Disease Control and Prevention's Healthcare Infection Control Practices Advisory Committee.
