Integration of genomic and clinical data augments surveillance of healthcare-acquired infections

Background:
Determining infectious cross-transmission events in healthcare settings involves manual surveillance of case clusters by infection control personnel, followed by strain typing of clinical/environmental isolates suspected in said clusters. Recent advances in genomic sequencing and cloud computing now allow for the rapid molecular typing of infecting isolates.
Objective:To facilitate rapid recognition of transmission clusters, we aimed to assess infection control surveillance using whole-genome sequencing (WGS) of microbial pathogens to identify cross-transmission events for epidemiologic review.
Methods:Clinical isolates of Staphylococcus aureus, Enterococcus faecium, Pseudomonas aeruginosa, and Klebsiella pneumoniae were obtained prospectively at an academic medical center, from September 1, 2016, to September 30, 2017. Isolate genomes were sequenced, followed by single-nucleotide variant analysis; a cloud-computing platform was used for whole-genome sequence analysis and cluster identification.
Results:Most strains of the 4 studied pathogens were unrelated, and 34 potential transmission clusters were present. The characteristics of the potential clusters were complex and likely not identifiable by traditional surveillance alone. Notably, only 1 cluster had been suspected by routine manual surveillance.
Conclusions:Our work supports the assertion that integration of genomic and clinical epidemiologic data can augment infection control surveillance for both the identification of cross-transmission events and the inclusion of missed and exclusion of misidentified outbreaks (ie, false alarms). The integration of clinical data is essential to prioritize suspect clusters for investigation, and for existing infections, a timely review of both the clinical and WGS results can hold promise to reduce HAIs. A richer understanding of cross-transmission events within healthcare settings will require the expansion of current surveillance approaches.

The effect of the dietary n-3 long-chain PUFA, DHA (22 : 6n-3), on the growth of pre-term infants is controversial. We tested the effect of higher-dose DHA (approximately 1 % dietary fatty acids) on the growth of pre-term infants to 18 months corrected age compared with standard feeding practice (0·2–0·3 % DHA) in a randomised controlled trial. Infants born < 33 weeks gestation (n 657) were randomly allocated to receive breast milk and/or formula with higher DHA or standard DHA according to a concealed schedule stratified for sex and birth-weight ( < 1250 and ≥ 1250 g). The dietary arachidonic acid content of both diets was constant at approximately 0·4 % total fatty acids. The intervention was from day 2 to 5 of life until the infant's expected date of delivery (EDD). Growth was assessed at EDD, and at 4, 12 and 18 months corrected age. There was no effect of higher DHA on weight or head circumference at any age, but infants fed higher DHA were 0·7 cm (95 % CI 0·1, 1·4 cm; P = 0·02) longer at 18 months corrected age. There was an interaction effect between treatment and birth weight strata for weight (P = 0·01) and length (P = 0·04). Higher DHA resulted in increased length in infants born weighing ≥ 1250 g at 4 months corrected age and in both weight and length at 12 and 18 months corrected age. Our data show that DHA up to 1 % total dietary fatty acids does not adversely affect growth.

The Oxford Dictionary of National Biography (ODNB), originally designated the New Dictionary of National Biography, was published in 2004 as a successor to the renowned Dictionary of National Biography (DNB), edited by Sir Leslie Stephen (1832–1904) and itself published in sixty-three volumes between 1885 and 1900, with supplements. All of the subjects in the old DNB were retained for the ODNB; new entries were commissioned for a significant proportion of the existing subjects (including almost all of those falling within the early medieval period); and of course the opportunity was taken to add entries for a large number of new subjects. In its print version, the ODNB occupies sixty volumes, though it is unlikely to be found in that form outside reference libraries; most importantly, it is also available online, to subscribing institutions, with search facilities and other useful features. Like its most distinguished predecessor, the ODNB is already well established as an invaluable academic resource.
Influence of pasture and concentrates in the diet of grazing dairy cows on the fatty acid composition of milk

In five short-term experiments conducted in Victoria in 1997 and 1998, grazing dairy cows were given either pasture alone or pasture supplemented with high-energy concentrates, and the fatty acid profiles of milk fat were measured. We established the effects of these feeds on some aspects of milk fat of importance for human nutrition, but we specifically focused on the hypothesis that conjugated linoleic acid (CLA) concentrations in milk fat increase as pasture intake increases, and decrease as more concentrates are fed. In agreement with previous research, feeding fresh pasture alone resulted in high concentrations (1·0–1·8 g/100 g milk fat) of CLA. When the effect of level of pasture consumption on CLA content was examined, a significant positive relationship (r2=0·35; P<0·05) was obtained. When cereal grain concentrates were used to supplement pasture intake, the CLA content of milk fat generally declined (P<0·05), except where the amount of concentrates given led to a marked reduction in total milk fat concentration. The use of cereal grain concentrates also generally resulted in significant (P<0·05) increases in medium-chain saturated fatty acids, but always reduced the contribution of butyric acid to milk fat, from 4·5 to 3·9 g/100 g milk fat, on average.
The shorebird ichnofacies: an example from the Miocene of southern Spain

The Late Miocene Sorbas Member of the Sorbas Basin, Almería Province, southeast Spain contains an extensive avian ichnofauna preserved in lagoonal marls. Three distinctive avian ichnotaxa can be identified: Antarctichnus fuenzalidae Covacevich and Lamperein, 1970; Iranipeda millumi n. ichnosp.; and Roepichnus grahami n. ichnogen, n. ichnosp. In common with many other Cenozoic avian ichnofaunas, these traces are associated with shorebirds, including plovers, storks, ducks and/or gulls, respectively. Associated mammalian tracks include possible cat and artiodactyl footprints. The avian tracks are abundant and show a range of behavioural aspects in common with other recorded examples of Cretaceous–Recent shorebird tracks. These include both solitary and group activities consistent with their postulated avian tracemakers.
