Care-giving dynamics and futures planning among ageing parents of adult offspring with intellectual disability

The number of older parents ageing in tandem with their adult children with intellectual disability (ID) is increasing. This unique situation calls for greater research that investigates how older parents experience this extended care-giving role, including the extent to which they are engaging in futures planning. Participants were recruited via disability service providers in South Australia. Using the theoretical perspective of hermeneutic phenomenology to understand lived experiences, semi-structured in-depth interviews were carried out with older parents (N = 17, mean age 70 years). Six offspring were living in the family home while the remainder were in supported accommodation. Main themes to emerge from the data were: (a) perpetual parenting, (b) costs and rewards and (c) planning to plan. Parents were providing care across a range of areas, regardless of whether their offspring lived at home or in supported accommodation. While aware of the need to plan for the future, most did not have a firm plan in place. Parents are providing a high level of support to their adult children with ID regardless of whether they live in supported accommodation or the family home. While some have started to think about future care arrangements, most appear unclear over what the future holds.

Summary
This chapter focuses on the differences between the West and the Third World, such as poverty, age of the population, disease burden, lack of skilled staff and equipment. The Third World has a predominantly young population. Birth rate is higher than in the West, but there is high mortality amongst the under 5 years old, and since the HIV/AIDS epidemic a high mortality in early middle life. Evidence-based medicine starts with a clinical problem, and with the formation of a question regarding optimal care. The mother to child transmission (MTCT) studies for Sub-Saharan Africa can only be done in the Third World situation. The teaching of non-physician anaesthetists, who give the majority of anaesthetics in many countries, is often hampered by the absence of any studies as to what is the best in the conditions in which they are working.

The Poor Law Amendment Act of 1834 signaled a revolution that was more apparent than real. Studies of the operation of the Act have shown that enforcement was neither complete nor uniform; local authorities failed to build the workhouses required by the Act and were therefore unable to make the invidious but (according to the Act) essential distinction between paupers (people destitute enough to qualify to enter these institutions), and the merely poor, who were supposed to fend for themselves. Instead, Guardians continued to give allowances from the poor rates without requiring that the recipients become paupers and enter the workhouse. While the English Poor Law between 1834 and 1860 was characterized by a situation of administrative variety and confusion as unions failed to comply with the 1834 Act, the economic crises of the 1860s produced important reforms: the Union Chargeability Act (1865) improved financing; many unions built modern workhouses; at the same time, collateral care for the needy such as the aged began to emerge within the Poor Law. Then in 1870 the central Poor Law authority announced a policy of strict enforcement, recommending the suspension of outdoor relief to the able-bodied, a far-reaching change in Poor Law practice.
