EPA-1694 – Antipsychotics and Prolactin. Study of Prevalence and Associated Sexual Disfunction

Introduction
Hyperprolactinaemia is a common side effect of some APS, associated to important clinical manifestations (sexual dysfunction, breast disturbances and even increase of certain types of cancer risk).
ObjectiveTo evaluate the levels of prolactinemia associated to different APS, including the newest ones, and its association with sexual dysfunction (SD).
MethodsObservational cross-sectional study. Adult patients treated with one APS for at least 4 weeks and with no other PRL-rising treatment were included. Hyperprolactinaemia was defined as 20 microgr/L in women, 18 microgr/L in men. SD was evaluated with the specific SD questionnaire PR-Sex-DQ-SALSEX (Montejo et al, 2001).
Results288 patients were evaluated, with the following APS treatment distribution: aripiprazol (22.2%), risperidone (17.01%), olanzapine (16.67%), quetiapine (7.99%), long-acting paliperidone (6.25%), long-acting risperidone (4.51%), oral paloperidone (4.17%), oral risperidone (4.17%) and others (21.18%; APS with N<10 were not evaluated). Paliperidone was associated with the higher mean PRL levels (98.28 and 71.48 microgr/L for LAP and OP respectively), followed by oral risperidone (71.36 microgr/L). Aripiprazol, oral olanzapine and quetiapine showed the lowest PRL levels (13.25, 27.10 and 28.55 microgr/L respectively). More than 70% of the sexually active patients treated with paliperidone or risperidone presented SD, less frequent in non PRL-raising APS.
ConclusionsIn our sample paliperidone and risperidone were associated to higher mean PRL levels and sexual disfunction, while quetiapine, olanzapine and aripiprazol were the less PRL-raising APS. This might be taken in consideration when electing a long-term antipsychotic treatment for patients, given the important clinical consequences associated to sustained hyperprolactinaemia.

Objective
To demonstrate the therapeutic efficacy of aripiprazole LP by a case of difficult clinical management and that we often find in our daily practice.
MethodsDescription of a clinical case of a 21-year-old man, recently diagnosed with paranoid schizophrenia with no awareness of disease neither treatment adherence and harmful use of THC and cocaine that are identified as precipitating factors for multiple hospital admissions.
ResultsInjectable medication with objective clinical improvement is significant, cessation of readmissions, achieving improvement in all parameters measured functionality and proper adherence to treatment as well as outpatient mental health of both devices as of drug dependence.
ConclusionPsychiatry is facing the great challenge of modifying the natural history to the deterioration of schizophrenia, a disease considered one of the leading causes of years lived with disability. The objectives medium and long-term treatment of this disease are centered on the delay-avoidance disability and improving the functioning and quality of life of people with this disease.

Introduction
Patients with anxiety disorders are more vulnerable to develop other comorbid conditions. In particular, large epidemiological studies show a strong association between different anxiety disorders and substance use disorders.
ObjectivesTo show the prevalence of major anxiety disorders and the consumption of different substances. As well as the particular characteristics of this dual diagnosis and treatments that have proven more effective.
MethodsExhaustive review of all the material published on this topic in the recent years.
ConclusionsNearly 24% of patients with anxiety disorder suffer from a comorbid substance disorder use in their lifetime (17.9% diagnosis of alcohol abuse or dependence diagnosis and 11.9% of abuse or dependence on other drugs). Dual patients show a number of distinctive features, such as more frequency in males, family history of alcohol or other substances abuse and behavioral problems, early parental loss among others.

Introduction
The use of antipsychotics in children is controversial, one of the considerations to take into account is the possible effect on the values of fasting glucose, prolactin or weight gain are very important.
ObjectivesTo study the effect of these drugs on metabolic rate in children.
MethodsWe measured the weight, waist circumference, fasting glucose and basal prolactin in 6 children at Children's Hospital in the province of Huelva, diagnosed with bipolar disorder and early schizophrenia, atypical antipsychotics before starting treatment and 6 months later.
ResultsIn the provisional results it is found that a there is not a significant weight gain (less than 3%), no impairment of glucose and only in one case basal prolactin was elevated.
ConclusionsThe use of atypical antipsychotics in children should be reserved when strictly necessary. Once established, keep tight control of metabolic parameters, although the data of our study coincide with the literature, do not produce significant alterations.

Objective
The theory of self-medication in patients with severe mental illness has been exposed for years but to date has not been confirmed or ruled out. With this study, we intend to show the latest available evidence regarding this context.
MethodsAn exhaustive literature research in Medline and the latest forth in APA 2015.
ResultsMore and more evidence refute the veracity of this theory deeply rooted among some professionals.
ConclusionThere are theoretical alternatives that relate more sustained manner the relationship between consumption and toxic psychosis.

Online psychoeducation: E-patients are looking for e-doctors

Method
A website was designed to offer psychoeducation and advice to women who demand more information about health after traditional visit to GPs. A total of 564 patients visited the website in three months. Women asked for information and interacted with different topics on the platform. Only the women who completed all the form (age, previous diseases, current treatment) were analyzed. A total of 226 e-patients were included.
ResultsThe most demanded information was about: excess weight, fibromyalgia, depression, anxiety, bipolar disorder and the menopause although other topics related to medicine such as HTA, breast cancer, and pregnancy were part of the website contents.
DiscussionEven when many topics about medicine were available on the website, women were more interested in, and showed the need for information about mental health.
Online psychoeducation could help the clinician to save time at the personal consultation and could be especially useful in following the patient for a long time.
However, there are technological barriers and at least one first face-to-face interview is required in complicated cases to determine a correct diagnosis.
ConclusionsA high demand for e-mental online health education does exist. It would be useful for professionals to be trained in information technology in order to cover that demand and avoid misinforming patients.
It would be interesting to undertake a meta-analysis with all the studies around the world and determine the profile of the patient that could be helped through online attention.

AGB stars are important contributors of processed matter to the ISM. However, the physical and chemical mechanisms involved in its ejection are still poorly known. This process is expected to have remarkable effects in the innermost envelope, where the dust grains are formed, the gas is accelerated, the chemistry is active, and the radiative excitation becomes important. A good tracer of this region in C-rich stars is SiS, an abundant refractory molecule that can display maser lines, very sensitive to changes in the physical conditions. We present high angular resolution interferometer observations (HPBW ≳0.″.25) of the v = 0 J = 14 – 13 and 15 – 14 SiS maser lines towards the archetypal AGB star IRC+10216, carried out with CARMA and ALMA to explore the inner 1” region around the central star. We also present an ambitious monitoring of these lines along one single pulsation period carried out with the IRAM 30 m telescope.

This work proposes a methodology that uses remote sensing (RS) images to obtain optimum nitrogen crop uptake (Nuptake) maps, for the all pixels in the image included in the field during the entire growing season. The Nuptake was determined from relationship between critical nitrogen concentration (Nc) and biomass where biomass was estimated by a crop growth model based on the water use efficiency. The paper proposes the use of this methodology in commercial wheat farm. The results are discussed with respect to field measurements of crop biomass and N concentration on different dates and in zones with different nitrogen treatments from 8 commercial wheat farms in Albacete, Spain during 2015 and 2016.

Outflows of Low and High Ionization Gas in Nearby AGN

We present results from a survey of nearby AGN using the adaptive optics assisted integral-field spectrometer SINFONI on the VLT. These data enable us to study at high angular resolution (~ 0.″08) the distribution and kinematics of the low-ionization gas traced by the BrΓλ2.17 μm recombination line and [Si vi] λ 1.96 μm, [Al ix] λ 2.04 μm, and [Ca viii] λ 2.32 μm, the most prominent coronal lines in the near-IR. Overall, our results show that the coronal lines extend to a radius similar to that of narrow Brγ emission, mimicking in most cases the narrow-line region (NLR). In addition, the kinematics of the highly ionized species, and most of the Brγ kinematics, is dominated by non-circular motions with higher dispersion along the ionization cones, indicating that material is being accelerated and outflowing from a region very close to the AGN.
