Colliding respiratory jets as a mechanism of air exchange and pathogen transport during conversations

Air exchange between people has emerged in the COVID-19 pandemic as the important vector for transmission of the SARS-CoV-2 virus. We study the airflow and exchange between two unmasked individuals conversing face-to-face at short range, which can potentially transfer a high dose of a pathogen, because the dilution is small when compared to long-range airborne transmission. We conduct flow visualization experiments and direct numerical simulations of colliding respiratory jets mimicking the initial phase of a conversation. The evolution and dynamics of the jets are affected by the vertical offset between the mouths of the speakers. At low offsets the head-on collision of jets results in a `blocking effect', temporarily shielding the susceptible speaker from the pathogen carrying jet, although, the lateral spread of the jets is enhanced. Sufficiently large offsets prevent the interaction of the jets. At intermediate offsets (8-10 cm for 1 m separation), jet entrainment and the inhaled breath assist the transport of the pathogen-loaded saliva droplets towards the susceptible speaker's mouth. Air exchange is expected, in spite of the blocking effect arising from the interaction of the respiratory jets from the two speakers.

Summary
Introduction
In this chapter, we discuss RAiISE (Raising Awareness of invisible Illnesses in Schools and Education), a charity founded in the United Kingdom that is led by young people, for young people. From the perspective of the Chief Executive Officer (CEO) and the Board of Trustees, all of whom have lived experience of invisible illnesses themselves or as a parent/carer, we will provide insight into day-to-day life for the charity and its unique journey from an initial idea of one young person diagnosed with a chronic health condition to a national organisation with a global presence. RAiISE is a positive example of young people leading health and wellbeing initiatives with the support of healthcare and education professionals, rather than simple instances of young people's engagement or involvement. It also highlights the importance of collaboration across sectors, including schools and charities, to ensure that young people with health conditions are adequately supported, acknowledging the wider impact of health on a young person's life – away from the hospital. In this chapter, we will refer to ‘invisible illnesses’, which for context, is an umbrella term for any health condition that isn't easily visible to others. This includes chronic conditions such as cystic fibrosis, diabetes, inflammatory bowel diseases and musculoskeletal diseases, among others – but also mental health illnesses such as anxiety, depression and schizophrenia. RAiISE attempts to be as inclusive as possible in reference to invisible illnesses and equally helps to place the spotlight on visible illnesses affecting young people too.
What matters to us: young people's perspective on participation in healthcare
The importance of young people's involvement in healthcare is undeniable, and recent years have shown a positive increase in the inclusion of young people's voices in decision-making processes in healthcare (Weil et al, 2015). While there are discussions in both child and adult healthcare, it is important to acknowledge that there is no distinct line or ‘cut-off’ between childhood and adulthood, and the ways in which these groups engage with healthcare, despite organisational and legal definitions of who is defined as a child or adult. Therefore, the ‘young person’ demographic must have a place to voice their own unique experiences and concerns of healthcare, which indeed differs from younger children and older adults with health conditions (Betz et al, 2013).

OBJECTIVES/SPECIFIC AIMS: Background: Children with autism spectrum disorder (ASD) show a broad range of unusual responses to sensory stimuli and experiences. It has been hypothesized that early differences in sensory responsiveness arise from atypical neural function and produce “cascading effects” on development across a number of domains, impacting social and communication skill, as well as broader development in children affected by ASD. A primary challenge to confirming these hypotheses is that ASD cannot be definitely diagnosed in the earliest stages of development (i.e., infancy). A potential solution is to prospectively follow infants at heightened risk for ASD based on their status as infant siblings of children who are diagnosed. We examined the developmental sequelae and possible neurophysiological substrates of three different patterns of sensory responsiveness—hyporesponsiveness (reduced or absent responding to sensory stimuli) and hyperresponsiveness (exaggerated responding to sensory stimuli), as well as sensory seeking (craving of or fascination with certain sensory experiences). Infants at high risk (HR) for ASD were compared with a control group of infants at relatively lower risk for ASD (LR; siblings of children with typical developmental histories). Objectives: Research questions included: (a) Do HR infants differ from LR infants in early sensory responsiveness?, (b) Does sensory responsiveness predict future ASD and related symptomatology? and (c) Is sensory responsiveness predicted by resting brain states? METHODS/STUDY POPULATION: Methods: To answer these questions, we carried out a longitudinal correlational investigation in which 20 HR infants and 20 LR controls matched on sex and chronological age were followed over 18 months. At entry to the study, when infants were 18 months old, sensory responsiveness was measured using the Sensory Processing Assessment and the Sensory Experiences Questionnaire, and a number of putative neural signatures of early sensory differences were measured via resting state EEG. When infants were 24 and 36 months of age, ASD and related symptomatology was evaluated in a comprehensive diagnostic evaluation. RESULTS/ANTICIPATED RESULTS: Results: HR infants trended towards increased hyporesponsiveness and hyperresponsiveness and showed significantly elevated levels of sensory seeking relative to LR controls at 18 months of age. Both groups, furthermore, displayed a high degree of heterogeneity in sensory responsiveness. Atypical sensory responsiveness (increased hyperresponsiveness and/or hyporesponsiveness, as well as sensory seeking behavior) predicted several aspects of ASD and related symptomatology, including social, communication, and play skill, and was associated with differences in resting brain state, including metrics of oscillatory power, complexity, and connectivity, as well as hemispheric asymmetry. Moderation analyses revealed that several relations varied according to risk group, such that associations were stronger in magnitude in the HR Versus LR group. DISCUSSION/SIGNIFICANCE OF IMPACT: Conclusion: Findings provide empirical support for the notion that early sensory responsiveness may produce cascading effects on development in infants at heightened risk for ASD. Differences in resting brain states may underlie atypical behavioral patterns of sensory responsiveness. From a clinical standpoint, results suggest that early sensory differences may be useful for predicting developmental trajectories, and be potentially important targets for early preventive intervention, in infants at risk for autism.
Rasch Modeling and Confirmatory Factor Analysis of the Systemizing Quotient-Revised (SQ-R) Scale

This study assessed the dimensionality of the Systemizing Quotient-Revised (SQ-R), a measure of how strong a person's interest is in systems, using two statistical approaches: Rasch modeling and Confirmatory Factor Analysis (CFA). Participants included N = 675 with an autism spectrum condition (ASC), N = 1369 family members of people with ASC, and N = 2014 typical controls. Data were applied to the Rasch model (Rating Scale) using WINSTEPS. The data fit the Rasch model quite well lending support to the idea that systemizing could be seen as unidimensional. Reliability estimates were .99 for items and .92 for persons. A CFA parceling approach confirmed that a unidimensional model fit the data. There was, however, differential functioning by sex in some of these items. An abbreviated 44-item version of the scale, consisting of items without differential item functioning by sex was developed. This shorter scale also was tested from a Rasch perspective and confirmed through CFA. All measures showed differences on total scale scores between those participants with and without ASC (d = 0.71, p < .005), and between sexes (d = 0.53, p < .005). We conclude that the SQ-R is an appropriate measure of systemizing which can be measured along a single dimension.

The study of manuscripts is one of the most active areas of current research in medieval studies: manuscripts are the basic primary material evidence for literary scholars, historians and art-historians alike, and there has been an explosion of interest over the past twenty years. Manuscript study has developed enormously: codices are no longer treated as inert witnesses to a culture whose character has already been determined by the modern scholar, but are active participants in a process of exploration and discovery. The articles collected here discuss the future of this process and vital questions about manuscript study for tomorrow's explorers. They deal with codicology and book production, with textual criticism, with the material structure of the medieval book, with the relation of manuscripts to literary culture, to social history and to the medieval theatre, and with the importance to manuscript study of the emerging technology of computerised digitisation and hypertext display.
