Infection prevention program leaders report frequent use of criteria to distinguish recently recovered coronavirus disease 2019 (COVID-19) cases from actively infectious cases when incidentally positive asymptomatic patients were identified on routine severe acute respiratory coronavirus virus 2 (SARS-CoV-2) polymerase chain reaction (PCR) testing. Guidance on appropriate interpretation of high-sensitivity molecular tests can prevent harm from unnecessary precautions that delay admission and impede medical care.

Spotted fever group rickettsiae (SFG) are a neglected group of bacteria, belonging to the genus Rickettsia, that represent a large number of new and emerging infectious diseases with a worldwide distribution. The diseases are zoonotic and are transmitted by arthropod vectors, mainly ticks, fleas and mites, to hosts such as wild animals. Domesticated animals and humans are accidental hosts. In Asia, local people in endemic areas as well as travellers to these regions are at high risk of infection. In this review we compare SFG molecular and serological diagnostic methods and discuss their limitations. While there is a large range of molecular diagnostics and serological assays, both approaches have limitations and a positive result is dependent on the timing of sample collection. There is an increasing need for less expensive and easy-to-use diagnostic tests. However, despite many tests being available, their lack of suitability for use in resource-limited regions is of concern, as many require technical expertise, expensive equipment and reagents. In addition, many existing diagnostic tests still require rigorous validation in the regions and populations where these tests may be used, in particular to establish coherent and worthwhile cut-offs. It is likely that the best strategy is to use a real-time quantitative polymerase chain reaction (qPCR) and immunofluorescence assay in tandem. If the specimen is collected early enough in the infection there will be no antibodies but there will be a greater chance of a PCR positive result. Conversely, when there are detectable antibodies it is less likely that there will be a positive PCR result. It is therefore extremely important that a complete medical history is provided especially the number of days of fever prior to sample collection. More effort is required to develop and validate SFG diagnostics and those of other rickettsial infections.

A controversy at the 2016 IUCN World Conservation Congress on the topic of closing domestic ivory markets (the 007, or so-called James Bond, motion) has given rise to a debate on IUCN's value proposition. A cross-section of authors who are engaged in IUCN but not employed by the organization, and with diverse perspectives and opinions, here argue for the importance of safeguarding and strengthening the unique technical and convening roles of IUCN, providing examples of what has and has not worked. Recommendations for protecting and enhancing IUCN's contribution to global conservation debates and policy formulation are given.

Electrical and Optical measurements were carried out on permalloy oxide (PyO) thin films made by reactive dual ion beam sputtering at room temperature. VSM measurements at room temperature and 15 Kelvin did not reveal any magnetic moment in 120 nm thick films. The optical refraction and extinction spectra from 200-1000 nm were determined from ellipsometry measurements using a Cody-Lorentz model and provided in a reproducible method to determine the film thickness of PyO films on different substrate materials. PyO is transparent above 700 nm and is strongly absorbing below 500 nm. The resistivity values of PyO samples sputtered at room temperature depend on the oxygen flow rate and is approximately 4E3 Ohm cm for films prepared at 10 sccm. The resistivity of PyO decreases as a function of temperature. The dielectric constant is strongly frequency dependent, decreasing from 500 at 500 Hz to 10 at 1 MHz.

Summary
ABSTRACT
Lake Lisan existed between 75 and 14 ka BP and provides a unique opportunity for investigating climate change for the southern Levant region. The exposures of Lake Lisan deposits on the eastern shores of the current Dead Sea have received limited study in comparison to the western shores. Here we present new U-series and elevation data for a sequence of Lisan deposits ranging between 25 and 8 ka BP. The data show an elevation–age record consistent with previously published data while providing new information about the extent of decrease in lake levels in the Late Pleistocene and early Holocene. All dates cited in this chapter are calibrated; those that were uncalibrated in their original publications were calibrated using the online OxCAL program.
INTRODUCTION
Lake Lisan existed from approximately 70 to 15 cal ka BP (see Kaufman, 1971, Kaufman et al., 1992, Schramm et al., 2000; Hasse-Schramm et al., 2004 and references therein) and extended up to 200 km along the Dead Sea Transform at its highest levels (Hazan et al., 2005; Chapter 6, this volume; Figures 9.1, 9.2). During its history, the lake fluctuated between highstands of 165–180 mbsl (metres below sea level) and lowstands of perhaps as much as 700 mbsl (Chapter 6, this volume; Begin et al., 1985, Bartov et al., 2002).

ABSTRACT
The southern Levant has a long history of human habitation, and it has been previously suggested that climatic changes during the Late Pleistocene to Holocene stimulated changes in human behaviour and society. In order to evaluate such linkages it is necessary to have a detailed understanding of the climate record. We have conducted an extensive and up-to-date review of terrestrial and marine climatic conditions in the Levant and eastern Mediterranean during the past 25,000 years. We firstly present data from general circulation models (GCMs) simulating the climate for the Last Glacial Maximum (LGM), and evaluate the output of the model by reference to geological climate proxy data. We consider the types of climate data available from different environments and proxies and then present the spatial climatic ‘picture’ for key climatic events. This exercise suggests that the major Northern Hemisphere climatic fluctuations of the past 25,000 years are recorded in the eastern Mediterranean and Levantine region. However, this review also highlights problems and inadequacies with the existing data.
INTRODUCTION
The effects of global climate change on human society, on the environments we inhabit and on the sustainable development of our planet's poorest people are of prime concern to all. Predicting changes in water availability, local environments and climates will be the key to determining which areas of the world will require greatest assistance in dealing with increased global warmth and climate change in the coming century, and beyond. However, the detailed linkages between the development of human civilisation, anthropogenic activities, climate and environmental change remain poorly understood.

ABSTRACT
The Beidha archaeological site in Southern Jordan was occupied during the Natufian (two discrete occupation phases, c. 15,200–14,200 cal. BP and c. 13,600–13,200 cal. years BP) and Pre-Pottery B Neolithic periods (c. 10,300–8,600 cal. years BP). This chapter reconstructs the palaeoenvironments at Beidha during these periods, using sedimentological observations and the stable isotopic composition (oxygen and carbon) of carbonate deposits. Age control is provided by uranium-series and radiocarbon dating. Detailed analysis of a carbonate stratigraphic section related to a fossil spring close to the site, and a sequence of carbonate nodules from a section on the western edge of the archaeological site, permits a reconstruction of climatic variations between c. 18,000 and c. 8,500 years BP. The results of the palaeoenvironmental study are compared with the archaeological evidence, to explore the relationship between human occupation and climatic variability at Beidha. The results indicate a marked correspondence between more favourable (wetter) environmental conditions and phases of occupation at Beidha, and provide clues to the likely sources of water that sustained the settlement during the Late Pleistocene and early Holocene.
INTRODUCTION
Climate change during the Late Pleistocene–early Holocene is often seen as a key factor in the transition to sedentism and stable, agricultural societies in the Middle East, given the background of major events such as the start of the Younger Dryas and the Holocene (e.g. Moore and Hillman, 1992; Mithen, 2003; Cordova, 2007, see also Feynman and Ruzmaikin, 2007).

Between 1 October 1986 and 31 March 1987, 55 cases of meningococcal disease were identified in the South-West of England, an attack rate of 1·54 per 100000 during the study period. Antibiotics used in the treatment of the disease successfully eliminated nasopharyngeal carriage of meningococci in 13 out of 14 cases without use of rifampicin. The overall meningococcal carriage rate in 384 close contacts was 18·2% and the carriage rate of strains indistinguishable from the associated case strain was 11·1%. The carriage rate of indistinguishable strains in household contacts (16·0%) was higher than the carriage rate in contacts living at other addresses (7·0%, P > 0·05). A 2·day course of rifampicin successfully eradicated meningococci from 46 (98%) of 47 colonized contacts.
In one third of cases groupable meningococci were isolated from at least one household contact; 92% of these isolates were of the same serogroup as the associated case strain. When a meningococcus is not isolated from a deep site in a clinical case of meningococcal disease, culture of serogroup A or C strains from nasopharyngeal swabs of the case or of household contacts is an indication that the close contact group should be offered meningococcal A + C vaccine in addition to chemoprophylaxis. The failure in this and other studies to isolate meningococci from any household contact in the majority of cases may be due either to the relative insensitivity of nasopharyngeal swabbing in detecting meningococcal carriage or to the acquisition of meningococci by most index cases from sources outside the household.
Smoking, the environment and meningococcal disease: a case control study

This case control study investigated environmental factors in 74 confirmed cases of meningococcal disease (MD). In children aged under 5, passive smoking in the home (30 or more cigarettes daily) was associated with an odds ratio (OR) of 7.5 (95% confidence interval (CI) 1.46–38.66). ORs increased both with the numbers of cigarettes smoked and with the number of smokers in the household, suggesting a dose–response relationship. MD in this age group was also significantly associated with household overcrowding (more than 1.5 persons per room) (OR 6.0, 95% CI 1.10–32.8), with kisses on the mouth with 4 or more contacts in the previous 2 weeks (OR 2.46, 95% CI 1.09–5.56), with exposure to dust from plaster, brick or stone in the previous 2 weeks (OR 2.24, 95% CI 1.07–4.65); and with changes in residence (OR 3.0, 95% CI 1.0–8.99), marital arguments (OR 3.0, 95 % CI 1.26–7.17) and legal disputes in the previous 6 months (OR 3.10, 95% CI 1.24–7.78). These associations were independent of social class. Public health measures to lower the prevalence of cigarette smoking by parents of young children may reduce the incidence of MD. The influence of building dust and stressful life events merits further investigation.

During a prolonged outbreak of meningococcal disease caused by serogroup B serotype 15 sulphonamide-resistant strains in one British health district, there was considerable variation in attack rates by town. General practitioner (GP) antibiotic prescribing rates were compared in high and low incidence towns. The only significant difference found was that erythromycin prescribing was more frequent in the high incidence towns (rate ratio 4·0, 95% CI 3·2–4·8, in March 1987 and 3·0, 95% CI 2·4–3·7, in November 1987). This was probably due to increased GP consultation rates for upper respiratory tract infection (URTI), but higher erythromycin usage may have increased meningococcal acquisition rates or susceptibility to meningococcal disease. Antibiotic prescribing rate should be further investigated in defined areas of high and low incidence of meningococcal disease.
