Psychiatric co-morbidity in children and adolescents with CHDs: a systematic review

The population of long-term survivors with CHDs is increasing due to better diagnostics and treatment. This has revealed many co-morbidities including different neurocognitive difficulties. However, the prevalence of psychiatric disorders among children and adolescents and the specific types of disorders they may experience are unclear. We systematically reviewed the existing literature, where psychiatric diagnoses or psychiatric symptoms were investigated in children and adolescents (age: 2–18 aged) with CHDs and compared them with a heart-healthy control group or normative data. The searches were done in the three databases PubMed, psychINFO, and Embase. We included 20 articles reporting on 8035 unique patients with CHDs. Fourteen articles repoted on psychological symptoms, four reported on psychiatric diagnoses, and two reported on both symptoms and diagnoses. We found that children and adolescents with a CHD had a higher prevalence of attention deficit hyperactivity disorder (ranging between 1.4 and 9 times higher) and autism (ranging between 1.8 and 5 times higher) than controls, but inconsistent results regarding depression and anxiety.
The dynamic relationship between sleep and psychotic experiences across the early stages of the psychosis continuum

Background:
Psychotic disorders develop gradually along a continuum of severity. Understanding factors associated with psychosis development, such as sleep, could aid in identification of individuals at elevated risk. This study aimed to assess (1) the dynamic relationship between psychotic experiences (PEs) and sleep quality and quantity, and (2) whether this relationship differed between different clinical stages along the psychosis continuum.
Methods:We used daily diary data (90 days) of individuals (N = 96) at early stages (i.e. before a first diagnosis of psychosis) along the psychosis continuum. Multilevel models were constructed with sleep quality and sleep quantity as predictors of PEs and vice versa. Post-hoc, we constructed a multilevel model with both sleep quality and quantity as predictors of PEs. In addition, we tested whether associations differed between clinical stages.
Results:Within persons, poorer sleep predicted next day PEs (B = −0.02, p = 0.01), but not vice versa. Between persons, shorter sleep over the 90-day period predicted more PEs (B = −0.04, p = 0.002). Experiencing more PEs over 90-days predicted poorer (B = −0.02, p = 0.02) and shorter (B = −1.06, p = 0.008) sleep. We did not find any significant moderation effects for clinical stage.
Conclusions:We found a bidirectional relationship between sleep and PEs with daily fluctuations in sleep predicting next day PEs and general patterns of more PEs predicting poorer and shorter sleep. Our results highlight the importance of assessing sleep as a risk marker in the early clinical stages for psychosis.

Metacognitive beliefs mediate the effect of emotional abuse on depressive and psychotic symptoms in severe mental disorders

Background
Early trauma is linked to higher symptom levels in bipolar and psychotic disorders, but the translating mechanisms are not well understood. This study examines whether the relationship between early emotional abuse and depressive symptoms is mediated by metacognitive beliefs about thoughts being uncontrollable/dangerous, and whether this pathway extends to influence positive symptoms.
MethodPatients (N = 261) with psychotic or bipolar disorders were assessed for early trauma experiences, metacognitive beliefs, and current depression/anxiety and positive symptoms. Mediation path analyses using ordinary least-squares regressions tested if the effect of early emotional abuse on depression/anxiety was mediated by metacognitive beliefs, and if the effect of early emotional abuse on positive symptoms was mediated by metacognitive beliefs and depression/anxiety.
ResultsMetacognitive beliefs about thoughts being uncontrollable/dangerous significantly mediated the relationship between early emotional abuse and depression/anxiety. Metacognitive beliefs and depression/anxiety significantly mediated the relationship between early emotional abuse and positive symptoms. The models explained a moderate amount of the variance in symptoms (R2 = 0.21–0.29).
ConclusionOur results indicate that early emotional abuse is relevant to depression/anxiety and positive symptoms in bipolar and psychotic disorders, and suggest that metacognitive beliefs could play a role in an affective pathway to psychosis. Metacognitive beliefs could be relevant treatment targets with regards to depression/anxiety and positive symptoms in bipolar and psychotic disorders.
The effect of farrowing environment and previous experience on the maternal behaviour of sows in indoor pens and outdoor huts

Outdoor farrowing huts facilitate a less restricted maternal behaviour in sows compared with sows kept indoors in farrowing pens. The aim of our study was to investigate whether there are behavioural differences between primiparous sows kept outdoors in farrowing huts and indoors in pens, and whether the maternal behaviour during the second parity, when all sows were kept outdoors in farrowing huts, would differ between sows that have experienced the indoor or the outdoor environment, respectively, during their first parturition. A total of 26 Yorkshire×Swedish Landrace sows were studied. Of these, 11 sows were housed outdoors in farrowing huts during both parturitions (group=OUTOUT). The other 15 sows were kept indoors in a barn with single farrowing pens during their first parturition. During their second parturition, sows were kept outdoors in farrowing huts (group=INOUT). The behaviour was video recorded from 2 h prepartum to 48 h postpartum. The sows’ responsiveness to playbacks of a piglet’s screams was tested on days 2 to 3 postpartum. Parity 1: during the last 2 h prepartum, OUTOUT sows had a higher proportion of observations in the sternal lying position (P<0.01). During parturition, OUTOUT sows changed posture more often (P<0.05) and were lying less (P<0.05) than INOUT sows. All sows in both groups responded with ‘lifting head’ towards the playback of piglet scream, whereas 100% of OUTOUT sows and only 43% of INOUT sows thereafter were ‘getting up’ (P <0.01). Parity 2: There were no behavioural differences between INOUT and OUTOUT sows. In conclusion, it is not problematic for a second parity sow with initial maternal experience from an indoor farrowing pen to be kept outdoors in farrowing huts during its following farrowing.

Inter-rater reliability of expressed emotion ratings based on the Camberwell Family Interview

High expressed emotion in relatives has been shown to predict a poorer outcome in a range of disorders and cultures. However, variation in predictive power is considerable, and this could partly be due to low inter-rater reliability. Sixty-nine relatives of 47 acutely admitted schizophrenic patients were interviewed using the Camberwell Family Interview. Audiotapes were assessed by pairs of raters drawn from a group of seven approved raters. Inter-rater reliability was good for criticism, hostility, emotional over-involvement (as a six level scale) and expressed emotion index (EOI cut-off score 2–3); and it was fair, but unsatisfactory, for emotional over-involvement (binary scales), warmth, positive remarks and expressed emotion (EOI cut-off score 3–4). These findings suggest that the warmth scale and the dichotomized versions of the EOI scale should be used with caution. The influence of warmth on the outcome in schizophrenia may have been underestimated because of low reliability.

The Swedish Adoption/Twin Study of Aging (SATSA) is a longitudinal program of research in gerontological genetics which is currently in its fifth year. The base population is comprised of 351 pairs of twins reared apart and 407 matched control pairs of twins reared together who responded to a questionnaire (Q1) in 1984. Two additional stages of SATSA have recently been completed: a longitudinal follow-up questionnaire mailed out in 1987 (Q2) and extensive in-person testing (IPT1) which included a health examination and cognitive battery. A second wave of IPT was started in January 1989. A summary of some of the major findings from Q1 and a description of IPT1 are reported.
Influence of Chlorhexidine in Ethanol and in Isopropanol on the Bacterial Colonization of the Umbilicus of Newborns

The effect of daily treatment with 0.5% Chlorhexidine in 70% ethanol and in 70% isopropanol, respectively, on navel colonization and on rates of infection in newborns has been studied in 438 infants in two maternity wards during a 3-month period. In spite of isopropanol being reported as a more efficient skin disinfectant than ethanol in several experimental models, no significant differences were seen in the frequency of navel colonization or in infection rates between the two treatment groups. The colonization rate with S. aureus was lower in this than in an earlier investigation on navel disinfection with Chlorhexidine in ethanol performed in the same wards. This may reflect a progressive effectiveness of the treatment due to fewer S. aureus sources in the nursery. For practical reasons we continue to recommend daily navel disinfection with 0.5% Chlorhexidine in 70% ethanol on healthy newborns in hospital nurseries.
