The utility of a non-verbal prospective memory measure as a sensitive marker for early-stage Alzheimer's disease in Hong Kong

Background:
With the proportion of older adults in Hong Kong projected to double in size in the next 30 years, it is important to develop measures for detecting individuals in the earliest stage of Alzheimer's disease (AD, 0.5 in Clinical Dementia Rating, CDR). We tested the utility of a non-verbal prospective memory task (PM, ability to remember what one has to do when a specific event occurs in the future) as an early marker for AD in Hong Kong Chinese.
Methods:A large community dwelling sample of older adults who are healthy controls (CDR 0, N = 125), in the earliest stage of AD (CDR 0.5, N = 125), or with mild AD (CDR 1, N = 30) participated in this study. Their reaction time/accuracy data were analyzed by mixed-factor analyses of variance to compare the performance of the three CDR groups. Logistic regression analyses were performed to test the discriminative power of these measures for CDR 0 versus 0.5 participants.
Results:Prospective memory performance declined as a function of AD severity: CDR 0 > CDR 0.5 > CDR 1, suggesting the effects of early-stage AD and AD progression on PM. After partialling out the variance explained by psychometric measures (e.g., ADAS-Cog), reaction time/accuracy measures that reflected the PM still significantly discriminated between CDR 0 versus 0.5 participants in most of the cases.
Conclusion:The effectiveness of PM measures in discriminating individuals in the earliest stage of AD from healthy older adults suggests that these measures should be further developed as tools for early-stage AD discrimination.
Cognitive predictors for five-year conversion to dementia in community-dwelling Chinese older adults

Background: This study evaluates which cognitive measure is best for predicting incident dementia in a population-based random sample of Chinese older adults without dementia over a five-year period.
Methods: A total of 787 community-dwelling Chinese older adults without dementia in Hong Kong were assessed at baseline (T0), at two years (T1), and at five years (T2).
Results: The annual conversion rate to dementia was 1.6% and 6.3% for baseline normal participants and baseline mild cognitive impairment (MCI) participants, respectively. The Cantonese version of Mini-mental State Examination (CMMSE) scores declined significantly over time. In participants who progressed to dementia, Category Verbal Fluency Test (CVFT) scores dropped significantly from T0 to T1. A 1-SD drop of either CMMSE or CVFT in two years predicted dementia at five years with 91.5% sensitivity and 62.0% specificity. A stable CMMSE and CVFT at two years predicted a 91% chance of not progressing to clinical dementia at five years.
Conclusion: In this community sample of Chinese older adults, a decline in cognitive screening tests in short term (two years) offered useful information in predicting dementia conversion over a longer period.
Prototype testing of a composite-type active grating bender with Si substrate for surface profile high-order term elimination

The previous monolithic active grating bender design met some basic design requirements. However, after a real grating (BM-AGM) had been fabricated and installed for testing, the results showed that the usable length is a mere 60 mm because of the higher-order term error in the surface profile. A method was thus derived to eliminate the higher-order term error by modifying the width of the bender substrate through finite-element method simulation, reducing the residual error from about 100 nm to 6 nm. Owing to the closure of the grating department of Zeiss, ruling the monolithic bender is no longer available and the design has to be modified to a composite-type bender with Si substrate. A prototype was fabricated and assembled to examine all the design situations. The surface roughness of the width-modified Si substrate is around 30 nm before assembly. The residual error after assembly and bending is less than 10 nm. It proves that the design is feasible. However, due to the manufacturing capacity of the vendor, a short-length substrate is required and the design has to be modified. The detailed design modification and testing results are presented in this paper.

Noroviruses are an important aetiological agent of acute gastroenteritis. They are responsible for large outbreaks of disease in the community, hospitals and long-term-care facilities. The clinical manifestations of norovirus outbreaks in psychiatric units are rarely described. The disease burden and impact highlight the importance of timely notification and investigation of these outbreaks. We analysed the characteristics of four norovirus outbreaks which occurred during a 3-year period in an in-patient psychiatric care unit. A total of 184 patients were affected which included 172 hospitalized patients, seven healthcare workers (HCWs) and five psychiatric nursing-home residents. The mean incidence rate of norovirus gastroenteritis (NVG) in hospitalized patients during these outbreaks was 12·7%. These outbreaks were characterized by higher incidence in middle-aged male patients, predominant sickness of diarrhoea, short duration of illness, peaks in late winter and early spring, and higher susceptibility in acute psychiatric patients. HCWs had longer duration of illness than psychiatric patients. More than 10% of affected patients experienced ⩾2 infections. Infection control measures were instituted and a comprehensive, responsive standard operating procedure for NVG and outbreak management was developed. After implementation of these measures, no further outbreaks of NVG occurred during the study period.

Background: Growing evidence suggests that participation in late-life leisure activity may have beneficial effects on cognitive function. The objective of the study was to evaluate the association between leisure activity participation and cognitive function in an elderly population of community-dwelling Hong Kong Chinese.
Methods: 512 participants were assessed in the follow-up study of a population-based community survey of the prevalence of cognitive impairment among Hong Kong Chinese aged 60 years and over. Leisure activities were classified into four categories (physical, intellectual, social and recreational). Information regarding leisure activity participation, cognitive function and other variables was collected. Multivariate linear regression analyses were performed to examine the association between leisure activity participation and cognitive function.
Results: A higher level of late-life leisure activity participation, particularly in intellectual activities, was significantly associated with better cognitive function in the elderly, as reflected by the results of the Cantonese Mini-mental State Examination (p = 0.007, 0.029 and 0.005), the Category Verbal Fluency Test (p = 0.027, 0.003 and 0.005) and digit backward span (p = 0.031, 0.002 and 0.009), as measured by the total frequency, total hours per week and total number of subtypes, respectively; the Chinese Alzheimer's Disease Assessment Scale-Cognitive Subscale (p = 0.045) and word list learning (p = 0.003), as measured by the total number of subtypes; and digit forward span (p = 0.007 and 0.015), as measured by the total hours per week and total number of subtypes, respectively.
Conclusion: Late-life intellectual activity participation was associated with better cognitive function among community-dwelling Hong Kong elderly Chinese.

A mechanism previously proposed for the possible interaction between a rotating sheet of fluid and a rotating environment is re-investigated using the normal-mode method. Only azimuthal modes are considered. Stability of such a flow, if any, will appear as the neutral condition, i.e. the stability domain will be present as a stability boundary. The stability boundary for the present flow exists only for one azimuthal mode with no density inhomogeneity. For general heterogeneous rotating flows subject to non-axisymmetric disturbances, a semi-ellipse theorem is derived with a restriction. In contrast with the semicircle theorem in two-dimensional stratified flows, the restriction suggests that the semicircle in the complex phase-velocity plane does not in general provide an upper bound on all unstable waves. For rigidly rotating flows all unstable waves lie on a semicircle in the complex phase-velocity plane regardless of the density distributions.

Interfacial conditions for a cylindrical vortex sheet or a cylindrical fluid layer with radius-dependent density, velocity and magnetic fields are derived for isentropic compressible swirling flows subjected to arbitrary disturbances. Surface tension is included for possible immiscible fluids. These conditions are valid for both spatially and temporally growing waves and for flow profiles with or without discontinuities. The deformation of the sheet or the layer affects the flow in two ways: perturbing the total pressure field and disturbing the centrifugal force field created by the azimuthal components of the velocity and the magnetic flux. The latter seems to be straightforward, but is easily overlooked as in some of the previous analyses. We will show that failure to consider such a perturbation to a stable centrifugal force field will lead to the improper destabilization of certain modes with smaller axial and azimuthal wavenumbers.
The interfacial conditions and their corresponding stability characteristics are further examined for a general class of incompressible flows subject to temporal perturbations. Unlike the single role of destabilization played by the velocity in two-dimensional stratified flows or axisymmetric jet flows, the rotating velocity in vortex motions plays a dual role in flow stability: the angular-velocity gradient generates tangential shear, and the angular velocity itself creates a centrifugal force field. While the former always destabilizes the flow, the latter can either stabilize or destabilize the flow depending on whether the resultant force is centrifugally stable or unstable. These characteristics are demonstrated by examining three general types of perturbations.

The inviscid instability of heterogeneous swirling flows with radius-dependent density is investigated and secular relations for the instability growth rates for several different flow configurations are obtained from explicit solutions of the governing equations. It is found, in agreement with a sufficiency condition for the stability of such flows obtained earlier, that they are stable to both axisymmetric and non-axisymmetric infinitesimal modes whenever the density is a monotonic increasing function of radius and at the same time the radial variations in both the angular and axial velocity components remain small. The instability mechanisms present in these flows are both of centrifugal and of shear origin, the classical Rayleigh–Synge criterion being a condition for centrifugal stability. It is shown, via several counter examples, that the Rayleigh–Synge criterion for the stability of swirling flows is generally neither a necessary nor a sufficient condition when non-axisymmetric disturbances are considered or large shears exist in the flow. Very stable flows occur when the angular and axial velocity components have no radial variation and simultaneously the density increases with radius as is the case in a typical centrifuge.

An organic light-emitting device (OLED) having a double-heterostructure of ITO/NPB/DCMdoped Alq3/BCP/Alq3/MgAg was constructed to form a narrow recombination zone where both charge carriers and excitons are confined, thus resulting in efficient electron-hole recombination and energy transfer. It was found that though luminance efficiency was enhanced, the low electron mobility of BCP resulted in higher driving voltages and limited the improvement of power efficiency. Significant improvements on both quantum efficiency and power efficiency were achieved by replace 1,3,5-tri(phenyl-2-benzimidazoly)-benzene (TPBI) for BCP, and were correlated with its high electron mobility. Device performance was further improved in the structure ITO/NPB/DCM-doped Alq3/TPBI/LiF/MgAg with a maximum luminance efficiency 6.03cd/A and a power efficiency of 5.94 lm/W.
Epidemiology of respiratory syncytial virus infection among paediatric patients in Hong Kong: seasonality and disease impact

In a 5-year retrospective survey of respiratory syncytial virus (RSV) infections among hospitalized children, 1340 cases were identified of which, 98·4% were children <5 years old with a male:female ratio of 1·5:1. Most cases occurred from April to September showing a significant positive correlation with temperature and relative humidity. Community-acquired infections accounted for 92·5% of the cases with a mean hospital stay of 5 days. The estimated annual incidence of RSV infection requiring hospitalization was 2·5/1000 children <5 years old with a mortality of 0·15% among hospitalized cases. On average, 248 children were admitted each year to the 1400-bed acute regional hospital accounting for an expenditure of HK $1.94 (∼US $0.25) million for hospitalization costs which equates to an annual cost in excess of HK $6.67 (∼US $0.86) million for the whole of Hong Kong. An RSV vaccine should be a priority.

We measure the hydrostatic stress, uniaxial stress, and photo induced dependence of the channel conductance of two-dimensional electron gas AlGaN/GaN heterostructures grown on c-axis sapphire. The structures examined are grown by nitrogen-plasma molecular beam epitaxy and metal organic chemical vapor deposition. Electrical conductance measurements are made with four point probes on Hall bar samples. Both, hydrostatic stress and uniaxial stress result in changes in the conductance. Moreover, these changes in conductance have long settling times after the stress is applied and may be due to deep level defects, the energy levels of which change with stress. Stress coefficients extracted from the samples are partially attributed to deep level defects and to the piezoelectric effect resulting from different piezoelectric coefficients of GaN and AlN. Photo induced changes of the two-dimensional electron gas are also observed. We find that pulsed illumination produces long transient times in the conductance. These transients are reduced by thermal heating in some samples. However, they can still be present at 153°C.

Structural and electrical properties of beryllium implanted silicon carbide have been investigated by secondary ion mass spectrometry, Rutherford backscattering as well as deep level transient spectroscopy, resistivity and Hall measurements. Strong redistributions of the beryllium profiles have been found after a short post-implantation anneal cycle at temperatures between 1500 °C and 1700 °C. In particular, diffusion towards the surface has been observed which caused severe depletion of beryllium in the surface region. The crystalline state of the implanted material is well recovered already after annealing at 1450 °C. However, four deep levels induced by the implantation process have been detected by deep level transient spectroscopy.
