A penicillin allergy testing service (PATS) assessed penicillin allergy in patients with hematologic malignancies; 17 patients who met criteria had negative skin testing. Patients who underwent penicillin challenge passed and were delabeled. Of delabeled patients, 87% received and tolerated β-lactams during follow-up. Providers found the PATS valuable.

Background:
Bloodstream infections (BSIs) are a frequent cause of morbidity in patients with acute myeloid leukemia (AML), due in part to the presence of central venous access devices (CVADs) required to deliver therapy.
Objective:To determine the differential risk of bacterial BSI during neutropenia by CVAD type in pediatric patients with AML.
Methods:We performed a secondary analysis in a cohort of 560 pediatric patients (1,828 chemotherapy courses) receiving frontline AML chemotherapy at 17 US centers. The exposure was CVAD type at course start: tunneled externalized catheter (TEC), peripherally inserted central catheter (PICC), or totally implanted catheter (TIC). The primary outcome was course-specific incident bacterial BSI; secondary outcomes included mucosal barrier injury (MBI)-BSI and non-MBI BSI. Poisson regression was used to compute adjusted rate ratios comparing BSI occurrence during neutropenia by line type, controlling for demographic, clinical, and hospital-level characteristics.
Results:The rate of BSI did not differ by CVAD type: 11 BSIs per 1,000 neutropenic days for TECs, 13.7 for PICCs, and 10.7 for TICs. After adjustment, there was no statistically significant association between CVAD type and BSI: PICC incident rate ratio [IRR] = 1.00 (95% confidence interval [CI], 0.75–1.32) and TIC IRR = 0.83 (95% CI, 0.49–1.41) compared to TEC. When MBI and non-MBI were examined separately, results were similar.
Conclusions:In this large, multicenter cohort of pediatric AML patients, we found no difference in the rate of BSI during neutropenia by CVAD type. This may be due to a risk-profile for BSI that is unique to AML patients.
Implementation and evaluation of a community-based treatment for late-life hoarding

Objectives:
The objective of this paper was to examine the implementation and effectiveness of a community-based intervention for hoarding disorder (HD) using Cognitive Rehabilitation and Exposure/Sorting Therapy (CREST).
Design:This was a mixed-method, pre-post quasi-experimental study informed by the Practical, Robust Implementation and Sustainability Model for implementation science.
Setting:Program activities took place in San Diego County, mainly within clients’ homes or community, with some activities in-office.
Participants:Participants were aged 60 years or older, met eligibility for Medi-Cal or were uninsured, and met criteria for HD.
Intervention:A manualized, mobile protocol that incorporated CREST was utilized.
Measurements:The Clutter Image Rating and Hoarding Rating Scale were used as effectiveness outcomes. An investigator-created staff questionnaire was used to evaluate implementation.
Results:Thirty-seven clients were reached and enrolled in treatment and 15 completed treatment during the initial 2 years of the program. There were significant changes in hoarding severity and clutter volume. Based on the initial 2 years of the program, funding was provided for expansion to cover additional San Diego County regions and hire more staff clinicians in year three.
Conclusion:Preliminary data suggest that the CREST intervention can be successfully implemented in a community setting with positive results for older adults with HD.
Effects of TiO2 Blocking Layer on Photovoltaic Characteristics of TiO2/Nb2O5 Dye Sensitized Solar Cells

This study reports on the effect of introducing TiO2 compact layer on the photovoltaic characteristics of TiO2/Nb2O5 composite dye sensitized solar cell. The compact layer was deposited by spray pyrolysis technique. It was observed that introduction of 60 nm thick compact layer improved the short circuit current density Jsc ,Open circuit voltage Voc, and efficiency of the cell from 4.9 mA/cm2 to 8.2 mA/cm2, 6.8×10-1 V to 7.2×10-1 V and 1.9 % to 3.4 % respectively compared to traditional cell prepared without compact layer. Electrochemical impedance spectroscopy confirmed an increase in recombination resistance from 5.5×101 Ω.cm2 for bare DSSC to 9.0×101 Ω.cm2 for DSSC with compact layer thereby improving electron lifetime of the cells from 2.5×10-4 s to 386.9×10-4 s.

Objective:
We evaluated whether a diagnostic stewardship initiative consisting of ASP preauthorization paired with education could reduce false-positive hospital-onset (HO) Clostridioides difficile infection (CDI).
Design:Single center, quasi-experimental study.
Setting:Tertiary academic medical center in Chicago, Illinois.
Patients:Adult inpatients were included in the intervention if they were admitted between October 1, 2016, and April 30, 2018, and were eligible for C. difficile preauthorization review. Patients admitted to the stem cell transplant (SCT) unit were not included in the intervention and were therefore considered a contemporaneous noninterventional control group.
Intervention:The intervention consisted of requiring prescriber attestation that diarrhea has met CDI clinical criteria, ASP preauthorization, and verbal clinician feedback. Data were compared 33 months before and 19 months after implementation. Facility-wide HO-CDI incidence rates (IR) per 10,000 patient days (PD) and standardized infection ratios (SIR) were extracted from hospital infection prevention reports.
Results:During the entire 52 month period, the mean facility-wide HO-CDI-IR was 7.8 per 10,000 PD and the SIR was 0.9 overall. The mean ± SD HO-CDI-IR (8.5 ± 2.0 vs 6.5 ± 2.3; P < .001) and SIR (0.97 ± 0.23 vs 0.78 ± 0.26; P = .015) decreased from baseline during the intervention. Segmented regression models identified significant decreases in HO-CDI-IR (Pstep = .06; Ptrend = .008) and SIR (Pstep = .1; Ptrend = .017) trends concurrent with decreases in oral vancomycin (Pstep < .001; Ptrend < .001). HO-CDI-IR within a noninterventional control unit did not change (Pstep = .125; Ptrend = .115).
Conclusions:A multidisciplinary, multifaceted intervention leveraging clinician education and feedback reduced the HO-CDI-IR and the SIR in select populations. Institutions may consider interventions like ours to reduce false-positive C. difficile NAAT tests.

Summary
This chapter shows how to evaluate the assumptions that underlie dynamical systems modeling often applied to dyadic outcomes, and how to communicate the results; both through graphical methods. First the chapter provides a conceptual overview of dynamical systems theory (DST) as applied to interpersonal emotion processes. This conceptual framework is then integrated into regression logic wherein the assumptions of regression are merged with the assumptions of DST. The chapter then goes through an example using second-by-second heart rate taken from a couple over a ten-minute period. Each time series are examined individually and then simultanouslysimultaneously. Special attention is given towards the role of set points, stability, coupling, and nonlinear dynamics, all of which are of particular importance within the interpersonal emotion circumstance. Syntax for all the figures are provided in R.

A tendency to extract spurious, message-like meaning from meaningless noise was assessed as a risk factor leading to shizophrenia-spectrum disorders by assessing word length of speech illusions elicited by multispeaker babble in 43 people with prodromal symptoms. These individuals were randomised to olanzapine v. placebo groups during year 1 followed by no pharmacological treatment for those with no disorder conversion during year 2. A time-dependent Cox regression analysis of conversion to schizophrenia-spectrum disorder revealed a significant interaction between condition (olanzapine v. no drug) and length of speech illusion, with the latter strongly predicting subsequent conversion during medication-free intervals but not during olanzapine treatment.
Childhood adversities and risk for suicidal ideation and attempts: a longitudinal population-based study

Background. Developmental adversities may be risk factors for adult suicidal behavior, but this relationship has rarely been studied prospectively. The present study examined the association between childhood adversities and new onset suicidal ideation and attempts in an adult population-based sample.
Method. The study used a large community mental health survey (the Netherlands Mental Health Survey and Incidence Study; n=7076, age range 18–64 years). Logistic regression analyses were used to evaluate the relationship between childhood adversities and new onset of suicidal ideation and attempts over 3 years of longitudinal follow-up.
Results. During the study period 85 new cases of suicidal ideation and 39 new onset suicide attempts were observed. The incidence rate for new suicide ideation was 0·67% per year and the incidence rate for new suicide attempts was 0·28% per year. Childhood neglect, psychological abuse and physical abuse were strongly associated with new onset suicidal ideation and suicide attempts. Odds ratios (ORs) ranged from 2·80 to 4·66 for new onset suicidal ideation and from 3·60 to 5·43 for new onset suicide attempts. The total number of adversities reported had a strong graded relationship to new onset suicidal ideation and attempts. These associations remained significant after controlling for the effects of mental disorders.
Conclusions. Childhood abuse and multiple adversities are strongly associated with future suicidal behavior and the mental disorders assessed in the present study do not fully account for this effect. A comprehensive understanding of suicidal behavior must take childhood adversities into account.
The effect of feeding frequency and meal size on amounts of total and parotid saliva secreted by sheep

Total and unilateral parotid saliva production during eating were measured in response to offering sheep a fixed amount of lucerne (Medicago sativa) hay as one, two, four or eight meals. Total saliva measurements were obtained using sham-fed oesophageal-fistulated sheep. Unilateral parotid saliva was collected from sheep fitted with reversible re-entrant cannulas. Dry matter intakes and eating times were measured for each meal but were not constrained to particular values. Total and unilateral parotid saliva production during eating increased linearly with the log of the number of meals (p = 0.0001). The amounts corresponding to one, two, four and eight meals of lucerne hay were 1553, 1737, 1851 and 2087 ml during total collections and 209, 248, 307 and 352 ml during unilateral parotid collections. The time-period spent eating and the amount of food consumed both increased as meal number increased. Total saliva collections when lucerne hay was sham-fed as one, two, four or eight meals were associated with eating times of 56.9, 57.4, 70.8 and 86.0 min and intakes of 562, 622, 629 and 638 g dry matter respectively. For unilateral parotid collections, eating times and dry matter intakes were 64.2, 71.3, 78.0, 82.1 min and 515, 579, 614 and 627 g for one, two, four and eight meals of lucerne hay respectively. The saliva production response appeared to be determined through the effects of the time-period spent eating and amounts consumed, but other undetermined effects of feeding frequency contributed to the response. The importance of meal duration on total saliva production was assessed by sham-feeding of 800 g lucerne as stem, leaf, hay, chopped hay or ground and pelleted hay. Increasing meal duration by feeding with stems resulted in the production of 1808 ml saliva, whereas the rapid consumption of pellets resulted in only 442 ml being produced.
