Summary
As archaeologists, we seek to understand variation and change in past human societies. This goal necessitates a comparative approach, and comparisons justify the broad cross-cultural and diachronic scope of our work. Without comparisons we sink into the culture-bound theorizing against which anthropology and archaeology have long sought to broaden social science research. By undertaking comparisons that incorporate long-term social variability, archaeologists not only improve our understanding of the past, but also open the door to meaningful transdisciplinary research. Archaeologists have unique and comprehensive data sets whose analysis can contribute to dialogues surrounding contemporary issues and the myriad challenges of our era.
In the past two decades, the pendulum seems to have swung away from comparative research in archaeology. Many archaeologists focus on detailed contextual descriptions of individual cases, and only a few have dedicated themselves to explicit comparative work. Yet in that same time span, fieldwork has expanded tremendously throughout the world, leading to an explosion of well-documented diachronic data on sites and regions. We now have substantial detail on the variation inherent in phenomena such as cultural assemblages, settlement patterns, and economic activity. New methods, from dating techniques to digital data processing, promote comparative analysis and greatly advance our understanding of human societies and change. The time is ripe for a renewed commitment to comparative research in archaeology.

Quality of life after free-flap tongue reconstruction

Objective:
To analyse correlations between quality of life measures, aspiration and extent of surgical resection in patients who have undergone free-flap tongue reconstruction.
Patients and methods:Nine consecutive patients (seven men and two women; average age 51 years) who had been diagnosed with T4a carcinoma of the mobile tongue and/or tongue base and treated by glossectomy, free-flap reconstruction, and either radiation therapy or chemoradiation responded to the European Organization for Research and Treatment of Cancer Head and Neck-35 questionnaire, the performance status scale questionnaire and the hospital anxiety–depression scale questionnaire, an average of 43 months after treatment (range 18–83 months). Aspiration was evaluated by fibre-optic laryngoscopy. Correlations between quality of life domain scores, extent of surgery and the presence of aspiration were evaluated using non-parametric statistical analysis.
Results:Scores for the swallowing and aspiration domains of the European Organization for Research and Treatment of Cancer Head and Neck-35 questionnaire were significantly correlated with the extent of tongue base resection (Spearman's correlation, p = 0.037 and 0.042, respectively). Despite a strong correlation between the European Organization for Research and Treatment of Cancer Head and Neck-35 questionnaire results and the performance status scale global scores (correlation coefficient = 0.89, p = 0.048), the performance status scale domain scores were not correlated with the extent of tongue resection. Clinically apparent aspiration was not correlated with the extent of tongue resection, nor were the anxiety or depression scores. However, clinically apparent aspiration was significantly related to the swallowing and aspiration domain scores of the European Organization for Research and Treatment of Cancer Head and Neck-35 questionnaire (p = 0.017 in both cases).
Conclusions:Our results imply that the volume of tongue base resection is a major factor in swallowing- and aspiration-related quality of life following tongue resection and free-flap reconstruction. Free-flap reconstruction does not seem to palliate the effect of the loss of functional tongue base volume, as regards swallowing-related quality of life.

Aims and Method
National guidelines for the assessment and treatment of obsessive–compulsive disorder (OCD) and body dysmorphic disorder were published in 2005 by the National Institute for Health and Clinical Excellence (NICE). Local services are unable to treat a small but significant number of the most severely ill patients successfully, and the guidelines recommend that such patients should have access to highly specialised care. From 1 April 2007, the Department of Health decided to centrally fund treatment services for severe, chronic, refractory OCD and BDD. We describe a new National Service for Refractory OCD; its rationale, treatments offered, referral criteria and expected clinical outcomes.
ResultsInitial results from one centre show an average 42% reduction in OCD symptoms at the end of treatment.
Clinical ImplicationsThe operational challenges and potential generalisability of this model of healthcare delivery are discussed. We present a summary of the progress made so far in establishing a new, coherent National Service for Refractory OCD, 18 months after the NICE guideline was published. the aim of the paper is to educate clinicians about the service and describe its rationale, treatments offered, referral criteria and expected clinical outcomes.

The aim of this analysis was to evaluate the effects of the presence of the IL-1RA gene polymorphism and H. pylori infection on markers of a systemic inflammatory response taking into account virulence markers of this infection. Serum concentrations of interleukin (IL)-6, IL-8, and tumour-necrosis factor (TNF)-α of 479 occasional blood donors were not statistically significantly higher in subjects having antibodies against H. pylori, or more specifically against CagA and VacA, and being homozygous for the pro-inflammatory IL-1RN*2 allele compared to others after adjustment for covariates. The findings suggest that the possible pro-inflammatory effect of the IL-1RN*2 allele in combination with H. pylori infection is limited to the mucosal level.

In this paper, in this edition of the Journal commemorating the life and work of David Wynn-Williams, we consider approaches to the astrobiological investigation of Mars. We provide a brief account of the scientific rationale behind the approach presented here. In particular, we outline the capabilities of the Raman spectrometer for the detection of biomarkers. David Wynn-Williams was an active champion of this instrument who was keen to field-qualify a version in Antarctica with a view to flying a Raman instrument onboard a Mars-bound space mission. We examine a scenario for the deployment of such an instrument in conjunction with other instrumentation and argue that subsurface deployment of scientific instruments is essential if we are to succeed in detecting any evidence that may exist for former life on Mars. We outline a mission scenario – Vanguard – which represents a novel but low-risk, low-cost approach to Mars exploration that was conceived and developed jointly by one of the authors (Ellery) and the late David Wynn-Williams.

Electromagnetic levitation and drop tube processing were used to investigate the solidification behaviour of Co50Cu50 alloys. Deep undercooling was achieved down to 241 K, the alloy melts separated into Co-rich L1-phase and Cu-richL2-phase before solidification. In drop tube processing, the Cu-rich phases disperse morehomogeneously than in EML. This is due to the reduced gravity and high cooling rate during free fall and solidification.

We have investigated transport processes on a stepped Ag(111) surface in an aqueous electrolyte by analyzing the equilibrium step fluctuations as a function of the electrode potential. No influence of the electrolyte and the potential was found in the regime of negative potentials with respect to the saturated calomel electrode. As the potential becomes positive and approaches the dissolution limit, the increasing magnitude of the step fluctuations and the change in the time dependence indicate a rapid exchange of silver atoms with the electrolyte long before the silver begins to dissolve.

The deposition of Cu onto Au(100) and of Pb onto n- Si(111) electrodes from aqueous solutions has been monitored in situ by scanning tunneling microscopy. In the first case, a bcc structure of the Cu overlayer is observed, which allows pseudomorphic growth on Au(100) up to the 10th layer. Then a slow structural transition to fee begins. In contrast, Pb on Si(111) is shown to deposit as 3D crystallites with flat (111) terminated surfaces.

We sketch a new method for the accurate flux calibration and normalization of stellar spectra. This is of particular importance for the analysis of rapidly rotating early–type stars. Some preliminary log g determinations by profile fitting of H γ are presented.
