Psychiatric co-morbidity in children and adolescents with CHDs: a systematic review

The population of long-term survivors with CHDs is increasing due to better diagnostics and treatment. This has revealed many co-morbidities including different neurocognitive difficulties. However, the prevalence of psychiatric disorders among children and adolescents and the specific types of disorders they may experience are unclear. We systematically reviewed the existing literature, where psychiatric diagnoses or psychiatric symptoms were investigated in children and adolescents (age: 2–18 aged) with CHDs and compared them with a heart-healthy control group or normative data. The searches were done in the three databases PubMed, psychINFO, and Embase. We included 20 articles reporting on 8035 unique patients with CHDs. Fourteen articles repoted on psychological symptoms, four reported on psychiatric diagnoses, and two reported on both symptoms and diagnoses. We found that children and adolescents with a CHD had a higher prevalence of attention deficit hyperactivity disorder (ranging between 1.4 and 9 times higher) and autism (ranging between 1.8 and 5 times higher) than controls, but inconsistent results regarding depression and anxiety.
The Triage of Older Adults with Physiologic Markers of Serious Injury Using a State-Wide Prehospital Plan

Introduction:
In January of 2010, North Carolina (NC) USA implemented state-wide Trauma Triage Destination Plans (TTDPs) to provide standardized guidelines for Emergency Medical Services (EMS) decision making. No study exists to evaluate whether triage behavior has changed for geriatric trauma patients.
Hypothesis/Problem:The impact of the NC TTDPs was investigated on EMS triage of geriatric trauma patients meeting physiologic criteria of serious injury, primarily based on whether these patients were transported to a trauma center.
Methods:This is a retrospective cohort study of geriatric trauma patients transported by EMS from March 1, 2009 through September 30, 2009 (pre-TTDP) and March 1, 2010 through September 30, 2010 (post-TTDP) meeting the following inclusion criteria: (1) age 50 years or older; (2) transported to a hospital by NC EMS; (3) experienced an injury; and (4) meeting one or more of the NC TTDP’s physiologic criteria for trauma (n = 5,345). Data were obtained from the Prehospital Medical Information System (PreMIS). Data collected included proportions of patients transported to a trauma center categorized by specific physiologic criteria, age category, and distance from a trauma center.
Results:The proportion of patients transported to a trauma center pre-TTDP (24.4% [95% CI 22.7%-26.1%]; n = 604) was similar to the proportion post-TTDP (24.4% [95% CI 22.9%-26.0%]; n = 700). For patients meeting specific physiologic triage criteria, the proportions of patients transported to a trauma center were also similar pre- and post-TTDP: systolic blood pressure <90 mmHg (22.5% versus 23.5%); respiratory rate <10 or >29 (23.2% versus 22.6%); and Glascow Coma Scale (GCS) score <13 (26.0% versus 26.4%). Patients aged 80 years or older were less likely to be transported to a trauma center than younger patients in both the pre- and post-TTDP periods.
Conclusions:State-wide implementation of a TTDP had no discernible effect on the proportion of patients 50 years and older transported to a trauma center. Under-triage remained common and became increasingly prevalent among the oldest adults. Research to understand the uptake of guidelines and protocols into EMS practice is critical to improving care for older adults in the prehospital environment.

Acritarchs from the Hanford Brook Formation, New Brunswick, Canada: new biochronological constraints on the Protolenus elegans Zone and the Cambrian Series 2–3 transition

Diverse and well-preserved acritarchs are reported from the type section of the Cambrian Hanford Brook Formation at Hanford Brook, southern New Brunswick. This section fills an important gap in acritarch studies by providing the first detailed picture of changing acritarch associations close to the traditional lower–middle Cambrian boundary in Avalonia. Acritarchs from the St Martins Member, at the base of the succession, include Skiagia ciliosa, Heliosphaeridium notatum, H. longum and Liepaina plana and suggest attribution to Cambrian Stage 4. Acritarchs from the Somerset Street Member, in the middle of the formation, include Eliasum llaniscum and Comasphaeridium silesiense. This information adds new biochronological context to an ash bed in the Somerset Street Member previously dated as c. 510 Ma or 508 Ma, and to the endemic trilobites from the same member, including Protolenus elegans. It also places absolute ages on the basal range of stratigraphically important acritarchs. Both the acritarch assemblage and the radiometric age are consistent with a position very close to the traditional lower–middle Cambrian transition and likely within Cambrian Stage 5. Acritarchs from the Long Island Member, at the top of the succession, include additional taxa demonstrating assignment to Cambrian Stage 5. Both the Somerset Street and Long Island members probably correlate with the Morocconus notabilis Zone. The new acritarch species Retisphaeridium striatum Palacios is described. New data are presented on acritarchs from the upper part of the Hell's Mouth Formation, Wales, and correlation proposed with the Long Island Member.

Gene expression and nitrogen loss in senescing root systems of red clover (Trifolium pratense)

Root system senescence and nitrogen (N) release from red clover (Trifolium pratense L.) plants, grown under semi-sterile conditions and a controlled environment, were studied for 28 days following temporary or prolonged abiotic stress. Plants stressed temporarily, to simulate grazing, recovered with no additional N lost in leachate. In contrast, plants subjected to prolonged stress that simulated overwintering conditions and inhibited shoot re-growth survived stress lasting 7 days, but plant viability was reduced to 50% by 14 days and 0% at 21 days. There were no significant differences in root protein, catalase activity, root death index or total N loss in leachate over 21 days, but by 28 days total N loss in leachate increased to 214% above control levels, with a 433% increase in total oxidized N. This increase in N loss between 21 and 28 days indicated the start of cellular breakdown of the root system, coinciding with the failure of plants to recover.
Key enzyme activities and protein concentrations in nodules decreased rapidly over 10 days' prolonged stress. cDNA–amplified fragment length polymorphism (AFLP) analysis identified contaminating bacterial and fungal genes, along with plant gene sequences with consistent or altered expression profiles. Four plant sequences, glyceraldehyde-3-phosphate dehydrogenase (Tp-gapdh1), nodule senescence reduced (Tp-nsr1), nodule senescence enhanced (Tp-nse1) and a cysteine protease gene (Tp-cp8) were differentially expressed throughout the plant: Tp-nsr1 and Tp-nse1 have potential as molecular markers for nodule senescence.
Root and nodule death in agricultural legumes, such as red clover, are implicated in N release into watercourses and the wider environment. Differences in the ability of these plants to survive prolonged stress lasting 14 days, and the delayed release of root N into leachate until 28 days after the stress, highlight the potential for the development of new red clover varieties with different rates of root system senescence.

Major affective disorder clinically similar to the disorder found in conditions other than Huntington's Disease (HD) was found in 41% of patients with HD in a consecutive case series ascertained through multiple sources in a defined geographical area. The association appears to be confined to certain families, and affective disorder may appear as long as 20 years before the onset of chorea and dementia. The association may represent genetic heterogeneity in HD.

The rate of occurrence of conduct disorder and affective illness was studied for a sample of 112 offspring of 34 Huntington's Disease (HD) patients. Psychiatric disorder in the offspring was assessed as a function of: (1) age of the parent at the onset of symptoms of HD; (2) family disorganization; and (3) psychiatric disorder in either parent. The findings indicated an increased frequency of conduct disorder in disrupted families, most especially in those where the HD parent had an early onset of symptoms and the non-HD parent showed psychiatric disorder. Affective disorder in the offspring was most strongly associated with the presence of similar symptoms in the HD parent. Affective disorder, but not conduct disorder, may be an early manifestation of the HD gene. The implication of these findings for genetic counselling is discussed.

Restriction fragment length polymorphism of the gene encoding rRNA (ribotyping) was used in combination with conventional epidemiological markers to study phenotypic variations among Streptococcus agalactiae of bovine origin and the possible epidemiological interrelationship between the bovine and human reservoirs of Streptococcus agalactiae.
The bovine material constituted 53 strains (9 antigen combinations) isolated from 11 herds. Herds with a uniform as well as heterogenic antigenic pattern were included. Furthermore, strains isolated in the course of time from the same persistently infected quarters were examined. The human material constituted 16 strains, 4 each of 4 serotypes, isolated from healthy carriers. Finally, nine serotype- and the group reference strains were examined. All strains were serotyped by double diffusion in agarose gel, biotyped (lactose ±), and ribotyped using two restriction enzymes, Hind III and Hha I.
All isolates could be typed by ribotyping and seven ribotypes were identified among the reference strains. The restriction enzymes used alone or in combination gave typing results that allowed discrimination between and within serotype. Combined use of serotype, HindIII and HhaI ribotypes produced 11 types among the 16 human strains. Ribotype analysis discriminated between herds infected with the same serotype. Strains of varying antigenic patterns from the same herd had the same ribotype. Phenotypic variations in serotype observed in persistent intramammary infection were not related to genetic changes as monitored by ribotype. Two ribotypes were represented among both bovine and human strains. The discriminating capability of lactose fermentation was of limited value.

Dietary carbohydrates constitute a major fraction of the diets for pigs. The carbohydrate fraction consists of mono-, di- and oligosaccharides and two broad classes of polysaccharides – starch and non-starch polysaccharides (NSP). The carbohydrate fraction has a diverse composition in terms of constituent sugars (pentoses, hexoses, deoxysugars, etc.), glycosidic linkages (alfa or beta), size (degree of polymerisation from one to several thousand), and physical form (soluble in water, insoluble, cation and adsorbing properties). It is now evidential clear that the composition of the carbohydrate fraction influences the digestion and absorption processes of carbohydrates and other nutrients in the various parts of the gastrointestinal tract, it has a profound influence on the secretory response of the gut to feed intake, the volume flow, the mucosal architecture, the composition of the gut flora and the development of the gastrointestinal tract.

The impact of diets varying in type and level of carbohydrates resistant to endogenous enzymes and lignin on the establishment and location of Ascaris suum and Oesophagostomum dentatum was investigated experimentally. Fifty worm-free pigs, from a specific pathogen-free farm were used. The animals were assigned randomly to 5 diets and infected with 600 infective A. suum eggs and 6000 infective larvae of O. dentatum per pig. The diets consisted of a traditional ground barley plus protein feed (diet A), commercial full-constituent pelleted feed (diet B), barley flour plus protein (diet C), barley flour, inulin (Raftiline® ST, ORAFTI, Tienen, Belgium), sugar beet fibre plus protein (diet D), and barley flour, wheat bran, and protein (diet E). The faecal egg excretion was followed and the pigs were slaughtered at 8 weeks p.i. and samples taken from the small and large intestine. Intestinal contents were analysed for worm burdens, worm location and female worm fecundity along with the concentration of insoluble (chromic oxide) and soluble (polyethylene glycol-4000) markers, lignin, non-starch polysaccharides (NSP) and organic acids. In all diet groups A. suum worm burdens were low and comparable, whereas the O. dentatum worm burdens were significantly higher in pigs fed the diets with high levels of NSP and lignin (diets A and E) than in pigs fed diets B, C, and D. The present study suggests that a diet rich in lignin and insoluble NSP's provides favourable conditions for the establishment of O. dentatum in the large intestine of pigs while it is unlikely that the concentration of short-chain fatty acids and pH plays any major role.
Stability of PPV Derivatives: The Effect of Side Group Functionalities

We have identified excited-state singlet oxygen as a reactive intermediate in the solid state photo-oxidation of two poly(p-phenylene vinylene) (PPV) derivatives, poly(2,5-bis(5,6- cholestanoxy)-1,4-phenylene vinylene) (BCHA-PPV) and poly(2-methoxy,5-(2'-ethyl-hexoxy)- 1,4-phenylene vinylene) (MEH-PPV). Singlet oxygen is photosensitized via energy transfer from the polymer and undergoes 1,2-cycloaddition across the electron-rich vinyl double bond in the backbone of the polymer resulting in the formation of highly oxidized species such as esters. Volatile carbonyl species are also formed, and, consequently, the film thins. This degradation pathway occurs when the polymer film is exposed in air to light having energy above the bandgap of the polymer, from either a low-pressure UV mercury lamp or an Ar+ laser operating at 514 or 457 nm. Interestingly, the singlet oxygen reaction does not take place in model compounds with similar structures such as trans-stilbene. In an effort to understand which properties of the polymer make it susceptible to singlet oxygen attack, we have studied the effect of conjugation length and side groups on the reactivity of thin polymer films. Specifically, the role of electron withdrawing groups, steric protection of the vinyl double bond, and conjugation length effects have been investigated using both FrIR spectroscopy and semi-empirical computational chemistry calculations.

Over the past 10 years, heterojunction bipolar transistors (HBTs) have progressed to where integrated circuit (IC) products are being sold and foundry services are being commercially offered utilizing gallium arsenide (GaAs) based technology. We will discuss, here, an alternative HBT technology based on indium phosphide (InP). While this technology is less mature than its GaAs counterpart, it offers several attractive benefits in comparison with GaAs. These benefits are provided through several key material properties of InP and ternary compound semiconductors, eg. gallium indium arsenide (GaInAs), grown on the InP. We review the status of this npn HBT technology and present performance results which illustrate the benefits of the technology with respect to electronic applications. Finally, we present measured reliability data for this technology which shows outstanding projected lifetimes.
