Efficacy of the Zero Suicide framework in reducing recurrent suicide attempts: cross-sectional and time-to-recurrent-event analyses

Background
The Zero Suicide framework is a system-wide approach to prevent suicides in health services. It has been implemented worldwide but has a poor evidence-base of effectiveness.
AimsTo evaluate the effectiveness of the Zero Suicide framework, implemented in a clinical suicide prevention pathway (SPP) by a large public mental health service in Australia, in reducing repeated suicide attempts after an index attempt.
MethodA total of 604 persons with 737 suicide attempt presentations were identified between 1 July and 31 December 2017. Relative risk for a subsequent suicide attempt within various time periods was calculated using cross-sectional analysis. Subsequently, a 10-year suicide attempt history (2009–2018) for the cohort was used in time-to-recurrent-event analyses.
ResultsPlacement on the SPP reduced risk for a repeated suicide attempt within 7 days (RR = 0.29; 95% CI 0.11–0.75), 14 days (RR = 0.38; 95% CI 0.18–0.78), 30 days (RR = 0.55; 95% CI 0.33–0.94) and 90 days (RR = 0.62; 95% CI 0.41–0.95). Time-to-recurrent event analysis showed that SPP placement extended time to re-presentation (HR = 0.65; 95% CI 0.57–0.67). A diagnosis of personality disorder (HR = 2.70; 95% CI 2.03–3.58), previous suicide attempt (HR = 1.78; 95% CI 1.46–2.17) and Indigenous status (HR = 1.46; 95% CI 0.98–2.25) increased the hazard for re-presentation, whereas older age decreased it (HR = 0.92; 95% CI 0.86–0.98). The effect of the SPP was similar across all groups, reducing the risk of re-presentation to about 65% of that seen in those not placed on the SPP.
ConclusionsThis paper demonstrates a reduction in repeated suicide attempts after an index attempt and a longer time to a subsequent attempt for those receiving multilevel care based on the Zero Suicide framework.
Improvised Field Expedient Method for Renal Replacement Therapy in a Porcine Model of Acute Kidney Injury

Objective:
Dialysis patients may not have access to conventional renal replacement therapy (RRT) following disasters. We hypothesized that improvised renal replacement therapy (ImpRRT) would be comparable to continuous renal replacement therapy (CRRT) in a porcine acute kidney injury model.
Methods:Following bilateral nephrectomies and 2 hours of caudal aortic occlusion, 12 pigs were randomized to 4 hours of ImpRRT or CRRT. In the ImpRRT group, blood was circulated through a dialysis filter using a rapid infuser to collect the ultrafiltrate. Improvised replacement fluid, made with stock solutions, was infused pre-pump. In the CRRT group, commercial replacement fluid was used. During RRT, animals received isotonic crystalloids and norepinephrine.
Results:There were no differences in serum creatinine, calcium, magnesium, or phosphorus concentrations. While there was a difference between groups in serum potassium concentration over time (P < 0.001), significance was lost in pairwise comparison at specific time points. Replacement fluids or ultrafiltrate flows did not differ between groups. There were no differences in lactate concentration, isotonic crystalloid requirement, or norepinephrine doses. No difference was found in electrolyte concentrations between the commercial and improvised replacement solutions.
Conclusion:The ImpRRT system achieved similar performance to CRRT and may represent a potential option for temporary RRT following disasters.

Descriptions of the Cordilleran Ice Sheet retreat after the last glacial maximum have included short-lived readvances occurring during the Older and Younger Dryas stadial periods and into the Holocene, but identification of these events has been largely limited to southwest and central British Columbia and northwest Washington State. We present evidence of a late Pleistocene readvance of Cordilleran ice occurring on the central coast of British Columbia on Calvert Island, between northern Vancouver Island and Haida Gwaii. Evidence is provided by sedimentological and paleoecological information contained in a sedimentary sequence combined with geomorphic mapping of glacial features in the region. Results indicate that a cold climate existed between 15.1 and 14.3 cal ka BP and that ice advanced to, and then retreated from, the western edge of the island between 14.2 and 13.8 cal ka BP. These data provide the first evidence of a major fluctuation in the retreating ice sheet margin in this region and suggest that a cold climate was a major factor in ice readvance. These data contribute to the understanding of past temperature, ice loading and crustal response, the nature of ice margin retreat, and the paleoenvironment of an understudied area of the Pacific Northwest.

Pollen, plant macrofossil, chironomid, and lithological analyses of late-glacial sediment from Pine Ridge Pond all indicate that the warming following deglaciation was punctuated on two separate occasions by a return to colder climes; initially by a brief and moderate cold episode (the Killarney Oscillation), followed shortly by the Younger Dryas. The Killarney Oscillation, AMS 14C dated at between 11,290 and 10,960 yr B.P., is characterized by abrupt and simultaneous increases in Betula (birch) pollen and Betula glandulosa (dwarf birch) macrofossils and a decline in tree pollen. The Younger Dryas is distinguished by the reversion of a short-lived closed Picea (spruce) forest to a prolonged forest-tundra stage (ca. 10, 820 to 9500(?) yr B.P.), during which macrofossils of arctic/alpine plants such as Salix herbaces, Cassiope hypnoides, Armeria maritima, and Thalictrum alpinum make their sole late-glacial appearance. Chironomid-inferred summer surface-water temperatures indicate a cooling of approximately 8 and 12°C during the Killarney Oscillation and Younger Dryas, respectively, and coarse-sand analysis indicates increased erosion on the landscape during these same intervals.

Background
Few randomised controlled trials (RCTs) have examined potential preventive agents in high-risk community populations.
AimsTo determine whether a mental health literacy intervention, the promotion of physical activity, or folic acid plus vitamin B12 reduce depression symptoms in community-dwelling older adults with elevated psychological distress.
MethodAn RCT with a completely crossed 2 × 2 × 2 factorial design: (400 mcg/d folic acid + 100 mcg/d vitamin B12v. placebo)×(physical activity v. nutrition promotion control) × (mental health literacy v. pain information control). The initial target sample size was 2000; however, only 909 adults (60–74 years) met the study criteria. Interventions were delivered by mail with telephone calls. The main outcome was depressive symptoms on the Patient Health Questionnaire (PHQ–9) at 6 weeks, 6, 12 and 24 months. The Clinicaltrials.gov registration number is NCT00214682.
ResultsThe drop-out rate was low (13.5%) from randomisation to 24-month assessment. Neither folic acid + B12 (F(3,856) = 0.83, P = 0.476) nor physical activity (F(3,856) = 1.65, P = 0.177) reduced depressive symptoms at any time point. At 6 weeks, depressive symptoms were lower for the mental health literacy intervention compared with its control condition (t(895) = 2.04, P = 0.042).
ConclusionsMental health literacy had a transient effect on depressive symptoms. Other than this, none of the interventions significantly reduced symptoms relative to their comparator at 6 weeks or subsequently. Neither folic acid plus B12 nor physical activity were effective in reducing depressive symptoms.
Simulation-based training in critical resuscitation procedures improves residents' competence

Objective:
Residents must become proficient in a variety of procedures. The practice of learning procedural skills on patients has come under ethical scrutiny, giving rise to the concept of simulation-based medical education. Resident training in a simulated environment allows skill acquisition without compromising patient safety. We assessed the impact of a simulation-based procedural skills training course on residents' competence in the performance of critical resuscitation procedures.
Methods:We solicited self-assessments of the knowledge and clinical skills required to perform resuscitation procedures from a cross-sectional multidisciplinary sample of 28 resident study participants. Participants were then exposed to an intensive 8-hour simulation-based training program, and asked to repeat the self-assessment questionnaires on completion of the course, and again 3 months later. We assessed the validity of the self-assessment questionnaire by evaluating participants' skills acquisition through an Objective Structured Clinical Examination station.
Results:We found statistically significant improvements in participants' ratings of both knowledge and clinical skills during the 3 self-assessment periods (p < 0.001). The participants' year of postgraduate training influenced their self-assessment of knowledge (F2,25 = 4.91, p < 0.01) and clinical skills (F2,25 = 10.89, p < 0.001). At the 3-month follow-up, junior-level residents showed consistent improvement from their baseline scores, but had regressed from their posttraining measures. Senior-level residents continued to show further increases in their assessments of both clinical skills and knowledge beyond the simulation-based training course.
Conclusion:Significant improvement in self-assessed theoretical knowledge and procedural skill competence for residents can be achieved through participation in a simulation-based resuscitation course. Gains in perceived competence appear to be stable over time, with senior learners gaining further confidence at the 3-month follow-up. Our findings support the benefits of simulation-based training for residents.

Deviations from ellipticity in the bulges of edge-on disk galaxies, seen as ”boxy“, ”peanut-shaped“, or “X-shaped“ isophotes, have been known for some time (Jarvis 1986, Whitmore & Bell 1988). Contrary to earlier proposals that these features represent accreted material, recent numerical work (Combes et al. 1990; Raha et al. 1991) suggests that they form when the bars in barred galaxies experience a vertical bending instability and deform out of the disk plane. We have found this latter mechanism at work in simulations in which a bar is induced when a large disk galaxy accretes a small companion galaxy, thus incorporating the mechanism into the evolutionary framework of galaxy interactions. In support of this picture we present observations of Hickson 87a, an edge-on S0 galaxy whose morphological peculiarities exactly match those seen in the simulation.
