Social Vulnerability and Access of Local Medical Care During Hurricane Harvey: A Spatial Analysis

Objectives:
When Hurricane Harvey struck the coastline of Texas in 2017, it caused 88 fatalities and over US $125 billion in damage, along with increased emergency department visits in Houston and in cities receiving hurricane evacuees, such as the Dallas-Fort Worth metroplex (DFW).
This study explored demographic indicators of vulnerability for patients from the Hurricane Harvey impact area who sought medical care in Houston and in DFW. The objectives were to characterize the vulnerability of affected populations presenting locally, as well as those presenting away from home, and to determine whether more vulnerable communities were more likely to seek medical care locally or elsewhere.
Methods:We used syndromic surveillance data alongside the Centers for Disease Control and Prevention Social Vulnerability Index to calculate the percentage of patients seeking care locally by zip code tabulation area. We used this variable to fit a spatial lag regression model, controlling for population density and flood extent.
Results:Communities with more patients presenting for medical care locally were significantly clustered and tended to have greater socioeconomic vulnerability, lower household composition vulnerability, and more extensive flooding.
Conclusions:These findings suggest that populations remaining in place during a natural disaster event may have needs related to income, education, and employment, while evacuees may have more needs related to age, disability, and single-parent household status.

Objectives:
The aim of this study was to analyze pharmacy functionality, or the volume of operational pharmacies, among areas in North Carolina and South Carolina affected by Hurricane Florence.
Methods:Using geographic information system software and data from the Federal Emergency Management Agency and Healthcare Ready, we computed, mapped, and analyzed pharmacy functionality measures for the period of September 12, 2018, through September 20, 2018, among counties in North Carolina and South Carolina to examine health-care–related disaster readiness for and response to Hurricane Florence.
Results:In the Hurricane Florence-impacted region, counties located along the coast had the most suboptimal pharmacy functionality, whereas counties located more centrally within North Carolina and South Carolina had more optimal pharmacy functionality throughout the disaster. Generally, functionality was high at Hurricane Florence’s landfall on September 14, 2018, for which operating pharmacy capacity was reported at 85% in North Carolina and 88% in South Carolina. Both states had the lowest functionality on September 16, 2018, at 71% for North Carolina and 62% for South Carolina.
Conclusions:During the Hurricane Florence event, suboptimal pharmacy functionality was detected for coastal areas and during the disaster response period. Hurricane readiness plans and infrastructure strengthening should be emphasized for community pharmacies in hurricane-prone areas.

In North America, terrestrial records of biodiversity and climate change that span Marine Oxygen Isotope Stage (MIS) 5 are rare. Where found, they provide insight into how the coupling of the ocean–atmosphere system is manifested in biotic and environmental records and how the biosphere responds to climate change. In 2010–2011, construction at Ziegler Reservoir near Snowmass Village, Colorado (USA) revealed a nearly continuous, lacustrine/wetland sedimentary sequence that preserved evidence of past plant communities between ~140 and 55 ka, including all of MIS 5. At an elevation of 2705 m, the Ziegler Reservoir fossil site also contained thousands of well-preserved bones of late Pleistocene megafauna, including mastodons, mammoths, ground sloths, horses, camels, deer, bison, black bear, coyotes, and bighorn sheep. In addition, the site contained more than 26,000 bones from at least 30 species of small animals including salamanders, otters, muskrats, minks, rabbits, beavers, frogs, lizards, snakes, fish, and birds. The combination of macro- and micro-vertebrates, invertebrates, terrestrial and aquatic plant macrofossils, a detailed pollen record, and a robust, directly dated stratigraphic framework shows that high-elevation ecosystems in the Rocky Mountains of Colorado are climatically sensitive and varied dramatically throughout MIS 5.

The potential role of wild rabbits Oryctolagus cuniculus in the epidemiology of paratuberculosis in domestic ruminants

Mycobacterium avium subspecies paratuberculosis, the organism responsible for paratuberculosis in cattle and sheep has been found in wild rabbits (Oryctolagus cuniculus) in the east of Scotland. Few studies have investigated either the level of faecal contamination by rabbits on farms, or the potential infectivity of rabbit excreta. The rate of rabbit faecal contamination deposited and the numbers encountered were estimated for 21 fields on 4 farms with a paratuberculosis problem. 7357±2571 S.E.M. rabbit faecal pellets were deposited per hectare per day and up to 81000 pellets/ha (‘standing crop’) were encountered in October/November 1998. Where access to rabbits was restricted, the standing crop of faeces encountered fell to 22000 pellets/ha.
The prevalence of infection with M. a. paratuberculosis was assessed for 83 rabbits from the four farms. M. a. paratuberculosis was isolated from rabbits on all farms with an overall prevalence of 17%. Out of 17 rabbits from which urine was available, M. a. paratuberculosis was isolated from two – the first reported isolation from urine in wild rabbits. The mean number of colony-forming units per gram of infected rabbit faeces was 7·6×105±5·2×105.
A relative estimate of the input of M. a. paratuberculosis onto pasture, at the stocking levels found on the four farms, showed that sheep and cattle potentially contributed 4 and 125 times more organisms/ha per day respectively than rabbits. However, rabbits could still contribute millions of M. a. paratuberculosis organisms per ha per day. Existing rabbit control measures on farms may be inadequate in reducing the risk of transmission to livestock.

Paratuberculosis (Johne’s disease) is a chronic invariably fatal enteritis of cattle caused by the bacterium Mycobacterium avium subspecies paratuberculosis and has recently been isolated from wild rabbits. One potential route of transmission of M.a.paratuberculosis from rabbits to cattle is the ingestion of rabbit excreta contaminating pasture. Here we (1) determine the prevalence and level of infection in rabbits and their excreta (2) quantify the level of rabbit faeces contaminating cattle pastures and (3) determine the impact of rabbit faeces on cattle grazing behaviour.
