The proposal of improving reproducibility by lowering the significance threshold to 0.005 has been discussed, but the impact on conducting clinical trials has yet to be examined from a study design perspective. The impact on sample size and study duration was investigated using design setups from 125 phase II studies published between 2015 and 2022. The impact was assessed using percent increase in sample size and additional years of accrual with the medians being 110.97% higher and 2.65 years longer respectively. The results indicated that this proposal causes additional financial burdens that reduce the efficiency of conducting clinical trials.
‘They deserve a treat’: the nature and antecedents of parental licensing surrounding children’s physical activity

Objective:
Little is known about parents’ compensatory health beliefs (CHB) surrounding their children’s engagement in physical activity (PA). Our aim was to provide evidence regarding the nature of, and factors underpinning, parents’ PA-related compensatory beliefs for their children.
Design:A qualitative descriptive approach and thematic content analysis were employed.
Setting:Parents were recruited from community sport and PA programmes.
Participants:Eighteen parents aged 32–52 years (mean age = 40·8 (sd 5·4) years; six males; twelve females).
Results:Analyses indicated that parents compensate through ‘passive’ or ‘active’ means. Among parents who compensated, most described their provision of ‘treat’ foods/drinks and a minority described allowing extended sedentary time to their children. Parents’ reasons underpinning these beliefs related to their child’s general physical/health status and psychological characteristics, and their own motivation and mood state.
Conclusions:These findings provide the first evidence of unhealthy dietary and sedentary behaviour CHB that parents may hold regarding their children’s involvement in PA.

Background
Childhood maltreatment (CM) plays an important role in the development of major depressive disorder (MDD). The aim of this study was to examine whether CM severity and type are associated with MDD-related brain alterations, and how they interact with sex and age.
MethodsWithin the ENIGMA-MDD network, severity and subtypes of CM using the Childhood Trauma Questionnaire were assessed and structural magnetic resonance imaging data from patients with MDD and healthy controls were analyzed in a mega-analysis comprising a total of 3872 participants aged between 13 and 89 years. Cortical thickness and surface area were extracted at each site using FreeSurfer.
ResultsCM severity was associated with reduced cortical thickness in the banks of the superior temporal sulcus and supramarginal gyrus as well as with reduced surface area of the middle temporal lobe. Participants reporting both childhood neglect and abuse had a lower cortical thickness in the inferior parietal lobe, middle temporal lobe, and precuneus compared to participants not exposed to CM. In males only, regardless of diagnosis, CM severity was associated with higher cortical thickness of the rostral anterior cingulate cortex. Finally, a significant interaction between CM and age in predicting thickness was seen across several prefrontal, temporal, and temporo-parietal regions.
ConclusionsSeverity and type of CM may impact cortical thickness and surface area. Importantly, CM may influence age-dependent brain maturation, particularly in regions related to the default mode network, perception, and theory of mind.

We read with interest the recent editorial, “The Hennepin Ketamine Study,” by Dr. Samuel Stratton commenting on the research ethics, methodology, and the current public controversy surrounding this study.1 As researchers and investigators of this study, we strongly agree that prospective clinical research in the prehospital environment is necessary to advance the science of Emergency Medical Services (EMS) and emergency medicine. We also agree that accomplishing this is challenging as the prehospital environment often encounters patient populations who cannot provide meaningful informed consent due to their emergent conditions. To ensure that fellow emergency medicine researchers understand the facts of our work so they may plan future studies, and to address some of the questions and concerns in Dr. Stratton’s editorial, the lay press, and in social media,2 we would like to call attention to some inaccuracies in Dr. Stratton’s editorial, and to the lay media stories on which it appears to be based.

Summary
MESSENGER’s exploration of Mercury has led to many important discoveries and a global perspective on its magnetosphere, exosphere, and interior as a coupled system. Mercury’s proximity to the Sun, weak planetary magnetic field, electrically conducting core, and sodium-dominated exosphere give rise to a highly dynamic magnetosphere unlike that of any other planet. The strong interplanetary magnetic fields so close to the Sun result in a high rate of energy transfer from the solar wind into Mercury’s magnetosphere. Surprisingly, direct solar wind impact on the surface during coronal mass ejection impact has been found to be infrequent. Electric currents induced in Mercury’s highly conducting interior buttress the weak planetary magnetic field against direct impact for all but the strongest solar events. Kinetic effects associated with the large orbits of planetary ions about the magnetic field and the small dimensions of the magnetosphere are observed to significantly affect some fluid instabilities such as Kelvin-Helmholtz waves along the magnetopause. As at Earth, magnetic reconnection, dipolarization fronts, and plasmoid ejection are closely associated with substorms in Mercury’s magnetosphere, and MESSENGER frequently observed energetic electrons with energies of tens of to several hundred thousand electron volts. However, no “Van Allen” radiation belts with durable trapping are present.

Critically significant parental effects in behavioral genetics may be partly understood as a consequence of maternal brain structure and function of caregiving systems recently studied in humans as well as rodents. Key parental brain areas regulate emotions, motivation/reward, and decision making, as well as more complex social-cognitive circuits. Additional key environmental factors must include socioeconomic status and paternal brain physiology. These have implications for developmental and evolutionary biology as well as public policy.

Congenital cardiac malformations which include isomerism of the atrial appendages are amongst the most challenging of problems for diagnosis and also for medical and surgical management. The nomenclature for pathological description is controversial, but difficulties can be overcome by the use of a segmental approach. Such an approach sets out the morphology and the topology of the chambers of the heart, together with the types and modes of the atrioventricular, ventriculo-arterial, and venous connections. We have applied this method to a study of 35 hearts known to have isomerism of the atrial appendages. We have already published accounts of 27 of these cases, but these were reviewed for this study in the light of our increased awareness of the implications of isomerism, and 8 new cases were added. After examining, or re-examining, the morphology of every heart in detail, we grouped them together according to their ventricular topology and modes of atrioventricular connection. Then we studied the course of the specialised conduction system, by the use of the light microscope, first in each individual case, and then together in their groups. We conclude that the pathways for atrioventricular conduction in hearts with isomerism of the atrial appendages are conditioned both by ventricular topology, and by the atrioventricular connections. Based on our experience, we have been able to establish guidelines that direct the clinician to the likely location of the conduction tissues.

Objectives: To establish the prevalence of coronary arterial abnormalities in mid-trimester fetuses with pulmonary atresia with intact ventricular septum, and whether their presence correlates with right ventricular morphology. Background: The presence of coronary arterial fistulas significantly alters the surgical options and prognosis for patients with pulmonary atresia with intact ventricular septum. The lesion can reliably be diagnosed using fetal echocardiography, and further definition of the prognosis is important for counselling parents. Methods: We examined the hearts of 39 pathological specimens diagnosed during fetal life, 3 of whom died postnatally. Coronary arterial abnormalities were defined as non-connection of the left or right coronary arteries to the aorta, ostial stenosis, marked tortuosity, dilation, thickening or abnormal myocardial branching. Mild tortuosity, or myocardial bridging, were considered normal. We measured the dimensions of the tricuspid valve along with the inlet and outlet portions of the ventricles. Ebstein's malformation, tricuspid valvar dysplasia, and the presence or absence of the infundibulum, were especially noted. We examined also 12 normal hearts as controls. Results: Coronary arterial abnormalities were found in 14/39 (36%). The dimensions of the right ventricle and tricuspid valves, and the gestational ages of the fetuses, were compared for these 14 with the 25 having no abnormalities using independent t-tests. The gestational ages were similar, 21.9 vs 21.1 weeks. The mean dimensions of the tricuspid valve, median z-scores, and right ventricle were smaller, 2.9 vs 7.2 mm; p < 0.002; −4.46 vs 0.23; p < 0.03; and 6.9 vs 13.7 mm; p < 0.002, for those with coronary arterial abnormalities. Ebstein's malformation, or dysplasia of the tricuspid valve, was present in 4 of 14 with, vs 15 of 25 without, coronary arterial abnormalities. A patent infundibulum was noted in 34 of 39 specimens. Conclusions: Mid-trimester fetuses with pulmonary atresia with intact ventricular septum already exhibit coronary arterial abnormalities, with a prevalence of 36%. The presence of a patent infundibulum confirms that atresia of the pulmonary valve is an acquired process. Coronary arterial abnormalities are seen in 50% of those with hypoplastic right ventricles, but less frequently in the presence of well developed ventricles. This is important information for those involved in counselling parents.

Here we describe the evolution of a silicon, MEMS-based chip design developed for infrared gas and chemical detection. The “Sensor-Chip,” with integrated photonic crystal and reflective optics, employs narrow-band optical emission/absorption for selective identification of gas and chemical species. Gas concentration is derived from attenuated optical power, which results in a change in device set point. This change in temperature results in a change in device resistance, via the TCR of the Si. Thermal non-uniformity across the device results in optical “noise” and accelerates localized thermal and electrical failures. This paper reports the influence of processing and design, on achieving uniformly heated, high reliability devices. Specifically, we examine the role of contacts, drive scheme, and device thermal distribution on chip design. Experimentally the temperature uniformity was characterized using an infrared camera. Experimental results indicate that the design of the contact areas in combination with the device design is essential for the reliable performance of the Sensor-Chip. Redesigned devices were fabricated and demonstrated as highly-selective gas and chemical sensors.

We have developed a thermally stimulated narrow-band infrared source for sensing, spectroscopy and thermophotovoltaic applications by combining the unique advantages of two different structures: a photonic crystal that consists of an array of holes etched into a dielectric substrate and a periodically perforated metallic thin film. The dielectric photonic crystal structure is passive and exhibits a strong absorption at resonance. This acts as a radiation reservoir for the conductive array, which plays an active role through plasmon interactions and is opaque at all wavelengths except those at which coupling occurs. We have fabricated the arrays on silicon, silicon dioxide and silicon nitride substrates using MEMS-based processing methods. Infrared spectroscopic studies were used to characterize reflection, absorption and emission in the 2 to 14 micron range showing narrow band resonance. Spectral tuning was accomplished by controlling symmetry and lattice spacing of the arrays. The effects of the etch depth, metal and dielectric properties have been studied experimentally and theoretically. These structures have been used as an emitter/detector sensor chip to selectively detect industrial pollutants like carbon dioxide.

A sensor chip has been designed and tested that uses a MEMS strip heater as both source and detector of infrared radiation. An optical cavity reflects infrared radiation back onto the source filament. Changes in reflected light intensity modify heater temperature, and the measured signal is a change in resistance. The effects of processing on electrical and thermal isolation were characterized and used to evaluate device performance. Thermally isolated, uniformly heated emitters are achieved using a backside release etch process. The fully released devices demonstrated superior electric to thermal-optical conversion, with the requisite narrow band emission for CO2 detection. Using these sensor-chips, CO2 detection was demonstrated, with projected sensitivities ≤0.1%.

Summary
Interstitial cystitis (IC) is characterized by pain in the region of the bladder and pelvic musculature, and variable motor and sensory dysfunctions of the bladder. The most prevalent symptoms of IC are urinary frequency, urgency and suprapubic, perineal pain. IC can be classified into the two categories of Hunner's ulcer and nonulcer, based on cystoscopic findings. This chapter highlights research contradictions regarding IC, along with research findings relating to the therapeutic effectiveness of various treatment modalities in IC. The lack of specific pathognomonic markers for the diagnosis of IC has resulted in a host of plausible hypotheses for its etiology. To be diagnosed with IC, patients must undergo cystoscopic examination under general anesthesia with hydrodistention of the bladder and bladder biopsy. Antihistamines are critical to managing IC in people with hay fever, sinusitis or food allergies. One major breakthrough in therapy is the use of heparin-like drugs.
