The epidemiology of cephalosporin allergy labels in pediatric primary care

Background:
Recent studies have sought to understand the epidemiology and impact of beta-lactam allergy labels on children; however, most of these studies have focused on penicillin allergy labels. Fewer studies assess cephalosporin antibiotic allergy labels in children. The objective of this study was to determine the prevalence, factors associated with, and impact of cephalosporin allergy labels in children cared for in the primary care setting.
Methods:Cephalosporin allergy labels were reviewed among children in a dual center, retrospective, birth cohort who were born between 2010 and 2020 and followed in 90 pediatric primary care practices. Antibiotic prescriptions for acute otitis media were compared in children with and without cephalosporin allergies.
Results:334,465 children comprised the birth cohort and 2,877 (0.9%) were labeled as cephalosporin allergic during the study period at a median age of 1.6 years. Third-generation cephalosporins were the most common class of cephalosporin allergy (83.0%). Cephalosporin allergy labels were more common in children with penicillin allergy labels than those without (5.8% vs. 0.6%). Other factors associated with a cephalosporin allergy label included white race, private insurance, presence of a chronic condition, and increased health care utilization. Children with third-generation cephalosporin allergy labels received more amoxicillin/clavulanate (28.8% vs. 10.2%) and macrolides (10.4% vs. 1.9%) and less amoxicillin (55.8% vs. 70.9%) for treatment of acute otitis media than non-allergic peers p < 0.001.
Conclusions:One in 100 children is labeled as cephalosporin allergic, and these children receive different antibiotics for the treatment of acute otitis media compared to non-allergic peers.

Key theoretical frameworks have proposed that examining the impact of exposure to specific dimensions of stress at specific developmental periods is likely to yield important insight into processes of risk and resilience. Utilizing a sample of N = 549 young adults who provided a detailed retrospective history of their lifetime exposure to numerous dimensions of traumatic stress and ratings of their current trauma-related symptomatology via completion of an online survey, here we test whether an individual’s perception of their lifetime stress as either controllable or predictable buffered the impact of exposure on trauma-related symptomatology assessed in adulthood. Further, we tested whether this moderation effect differed when evaluated in the context of early childhood, middle childhood, adolescence, and young adulthood stress. Consistent with hypotheses, results highlight both stressor controllability and stressor predictability as buffering the impact of traumatic stress exposure on trauma-related symptomatology and suggest that the potency of this buffering effect varies across unique developmental periods. Leveraging dimensional ratings of lifetime stress exposure to probe heterogeneity in outcomes following stress – and, critically, considering interactions between dimensions of exposure and the developmental period when stress occurred – is likely to yield increased understanding of risk and resilience following traumatic stress.
Estimates of emerald ash borer (Coleoptera: Buprestidae) larval galleries in branch samples from asymptomatic urban ash trees (Oleaceae)

Agrilus planipennis (Coleoptera: Buprestidae), is causing extensive mortality of ash (Fraxinus Linnaeus; Oleaceae) in North America. Once detected in an area, resource managers require methods to obtain estimates that could improve management decisions. We studied the within-crown and within-branch distribution and abundance of A. planipennis feeding galleries by sampling 3-m-long branches from asymptomatic urban ash trees and subdividing each branch into 12 sections of 25 cm each. We found galleries in all 12 sections of some, but not all, branches. Section was a significant source of variation in A. planipennis gallery density/m2 of branch surface area. A comparison of predictive power and efficiency of estimates for samples of increasing length, and for samples of the same length but consisting of different combinations of sections, revealed that those based on the two basal 25-cm sections of a branch from the lower-crown or mid-crown of an asymptomatic tree were less accurate and precise than those based on more sections, but were the most cost effective. Whittling more sections per branch, irrespective of the combinations of branch sections per length, improved predictive power but reduced cost effectiveness. We also observed that crown level was not important, and aspect was only marginally so, when estimating gallery abundance per sampled branch.

Objective
The purpose of this study was to measure the reported use of nutrition information on food labels by a population of university students and to determine if label users differed from non-users in terms of gender and specific beliefs related to label information and diet–disease relationships, specifically fat and heart disease and fibre and cancer.
DesignA single-stage cluster sampling technique was used. Data was obtained using a self-administered, validated questionnaire.
SettingThe present investigation took place at the University of Saskatchewan, Canada in the autumn of 1997.
SubjectsA total of 553 students in randomly selected classes in the College of Arts and Science took part in the survey (92% response rate). The sample consisted of roughly equal numbers of males and females, most between the ages of 18 and 24.
ResultsThere were approximately equal numbers of label users and non-users among males, while label users outnumbered non-users by almost four to one among females. The importance of nutrition information on food labels was the only belief that differed significantly between label users and non-users for both sexes. For females, no other beliefs distinguished label users from non-users. However, for males, significant differences were found between label users and non-users on the beliefs that nutrition information is truthful and that a relationship between fibre and cancer exists.
ConclusionsFemales appear to use food labels more often than do males. The only consistently observed difference between label users and non-users (male and female) was that users believed in the importance of nutrition information on food labels while non-users did not.

Between 1870 and 1913 economic convergence among present OECD members (or an even wider sample of countries) was dramatic, about as dramatic as it has been over the past century and a half. What were the sources of the convergence? One prime candidate is mass migration. This paper offers some estimates which suggest that migration could account for very large shares of the convergence in labour productivity and real wages, though a much smaller share in GDP per capita. One might conclude, therefore, that virtual cessation of convergence in the interwar period could be partially explained by the imposition of quotas and other barriers to migration.
