Peritoneal catheters in neonates undergoing complex cardiac surgery: a multi-centre descriptive study

Background:
The use of peritoneal catheters for prophylactic dialysis or drainage to prevent fluid overload after neonatal cardiac surgery is common in some centres; however, the multi-centre variability and details of peritoneal catheter use are not well described.
Methods:Twenty-two-centre NEonatal and Pediatric Heart Renal Outcomes Network (NEPHRON) study to describe multi-centre peritoneal catheter use after STAT category 3–5 neonatal cardiac surgery using cardiopulmonary bypass. Patient characteristics and acute kidney injury/fluid outcomes for six post-operative days are described among three cohorts: peritoneal catheter with dialysis, peritoneal catheter with passive drainage, and no peritoneal catheter.
Results:Of 1490 neonates, 471 (32%) had an intraoperative peritoneal catheter placed; 177 (12%) received prophylactic dialysis and 294 (20%) received passive drainage. Sixteen (73%) centres used peritoneal catheter at some frequency, including six centres in >50% of neonates. Four centres utilised prophylactic peritoneal dialysis. Time to post-operative dialysis initiation was 3 hours [1, 5] with the duration of 56 hours [37, 90]; passive drainage cohort drained for 92 hours [64, 163]. Peritoneal catheter were more common among patients receiving pre-operative mechanical ventilation, single ventricle physiology, and higher complexity surgery. There was no association with adverse events. Serum creatinine and daily fluid balance were not clinically different on any post-operative day. Mortality was similar.
Conclusions:In neonates undergoing complex cardiac surgery, peritoneal catheter use is not rare, with substantial variability among centres. Peritoneal catheters are used more commonly with higher surgical complexity. Adverse event rates, including mortality, are not different with peritoneal catheter use. Fluid overload and creatinine-based acute kidney injury rates are not different in peritoneal catheter cohorts.

We describe an adolescent with Streptococcus pneumoniae meningitis and symptomatic high-grade, second-degree atrioventricular block requiring permanent pacemaker placement. It is difficult to ascertain if these two diagnoses were independent or had a causal relationship though ongoing symptoms were not present prior to the infection. Because of this uncertainty, awareness that rhythm disturbances can be cardiac in origin but also secondary to other aetiologies, such as infection, is warranted.
Standardized Antimicrobial Administration Ratio (SAAR) Clinical Outcomes Assessment in a Large Community Healthcare System

Background: Research on the association between the standardized antimicrobial administration ratio (SAAR) and clinical outcomes is lacking. Objective: We compared SAAR and patient outcomes in 97 acute-care facilities affiliated with a large healthcare system. Methods: Facilities were classified using the broad-spectrum hospital-onset (BSHO) SAAR for medical, surgical, and medical-surgical wards as low, moderate, or high antimicrobial use: low use SAAR, <0.8; moderate use SAAR, 0.95–1.05; and high-use SAAR, >1.2. Data were included from patients aged ≥18 years who were discharged between the first quarter of 2018 and the second quarter of 2019, had nonmissing matching criteria, BMI between 10 and 90, and at least 1 BSHO medication administered in a medical, surgical, or medical-surgical ward. Patients were matched for gender, age group, BMI category, year and quarter of discharge, ICU stay, and diagnosis-related group (DRG). Eligible drugs included all routes for cefepime, ceftazidime, doripenem, imipenem/cilastatin, meropenem, and piperacillin/tazobactam and IV only for amikacin, aztreonam, gentamicin, and tobramycin. Outcomes were evaluated in a pairwise manner using t tests or χ2 tests. Results: Each of the 3 study groups consisted of 6,327 patients, 51% of whom were men; average age, 63 years; 70% of whom were obese or overweight, and 19% of whom had an ICU stay. The most common DRG code was infectious and parasitic diseases (57%) followed by digestive system (9%), respiratory system (7%), and kidney and urinary tract (6%). High antibiotic use was associated with longer length of stay and a higher estimated cost per visit. Low antibiotic use was associated with higher rate of mortality and a lower rate of readmissions compared to moderate use. The low-usage group did not exhibit a statistically significant difference in mortality, readmissions, or rate of C. difficile compared to the high-usage group. Conclusions: The optimal antibiotic utilization group varied among outcomes. Further evaluation of outcomes is needed for the SAAR to understand the ranges and the relationship between the measure and clinical outcomes.

Hydrilla is an invasive aquatic plant that has rapidly spread through many inland water bodies across the globe by outcompeting native aquatic plants. The negative impacts of hydrilla invasion have become a concern for water resource management authorities, power companies, and environmental scientists. The early detection of hydrilla infestation is very important to reduce the costs associated with control and removal efforts of this invasive species. Therefore, in this study, we aimed to develop a tool for rapid, frequent, and large-scale monitoring and predicting spatial extent of hydrilla habitat. This was achieved by integrating in situ and Landsat 8 Operational Land Imager satellite data for Lake J. Strom Thurmond, the largest US Army Corps of Engineers lake east of the Mississippi River, located on the border of Georgia and South Carolina border. The predictive model for presence of hydrilla incorporated radiometric and physical measurements, including remote-sensing reflectance, Secchi disk depth (SDD), light-attenuation coefficient (Kd), maximum depth of colonization (Zc), and percentage of light available through the water column (PLW). The model-predicted ideal habitat for hydrilla featured high SDD, Zc, and PLW values, low values of Kd. Monthly analyses based on satellite images showed that hydrilla starts growing in April, reaches peak coverage around October, begins retreating in the following months, and disappears in February. Analysis of physical and meteorological factors (i.e., water temperature, surface runoff, net inflow, precipitation) revealed that these parameters are closely associated with hydrilla extent. Management agencies can use these results not only to plan removal efforts but also to evaluate and adapt their current mitigation efforts.

Preamble: Executive Order (EO) 13112—defines an invasive species as “an alien species whose introduction does or is likely to cause economic or environmental harm or harm to human health.” In the Executive Summary of the National Invasive Species Management Plan (NISMP) the term invasive species is further clarified and defined as “a species that is non-native to the ecosystem under consideration and whose introduction causes or is likely to cause economic or environmental harm or harm to human health.” To provide guidance for the development and implementation of the NISMP, the National Invasive Species Council (NISC) and the Invasive Species Advisory Committee (ISAC) adopted a set of principles outlined in Appendix 6 of the NISMP. Guiding Principle #1 provides additional context for defining the term invasive species and states “many alien species are non-invasive and support human livelihoods or a preferred quality of life.”

The ability to noninvasively image the cone photoreceptor mosaic holds significant potential as a diagnostic for retinal disease. Central to the realization of this potential is the development of sensitive metrics for characterizing the organization of the mosaic. Here we evaluated previously-described and newly-developed (Fourier- and Radon-based) methods of measuring cone orientation in simulated and real images of the parafoveal cone mosaic. The proposed algorithms correlated well across both simulated and real mosaics, suggesting that each algorithm provides an accurate description of photoreceptor orientation. Despite high agreement between algorithms, each performed differently in response to image intensity variation and cone coordinate jitter. The integration property of the Fourier transform allowed the Fourier-based method to be resistant to cone coordinate jitter and perform the most robustly of all three algorithms. Conversely, when there is good image quality but unreliable cone identification, the Radon algorithm performed best. Finally, in cases where the cone coordinate reliability was excellent, the method previously described by Pum and colleagues performed best. These descriptors are complementary to conventional descriptive metrics of the cone mosaic, such as cell density and spacing, and have the potential to aid in the detection of photoreceptor pathology.

The effect of the beam scanning speed on part microstructures in the powder-bed electron beam additive manufacturing (EBAM) process was investigated in this research. Four levels of the beam speed were tested in building EBAM Ti–6Al–4V samples. The samples were subsequently used to prepare metallographic specimens for observations by optical microscopy and scanning electron microscopy. During the experiment, a near-infrared thermal imager was also used to acquire build surface temperatures for melt tool size estimates. It was found that the X-plane (side surface) shows columnar prior β grains, with the width in the range of about 40–110 µm, and martensitic structures. The width of columnar grains decreases with the increase of the scanning speed. In addition, the Z-plane (scanning surface) shows equiaxed grains, in the range of 50–85 µm. The grain size from the lowest beam speed (214 mm/s) is much larger compared to other samples of higher beam speeds (e.g., 376–689 mm/s). In addition, increasing the beam scanning speed will also result in finer α-lath. However, the porosity defect on the build surface also becomes severe at the highest scanning speed (689 mm/s).

Effective preparedness, response, and recovery from disasters require a well-planned, integrated effort with experienced professionals who can apply specialized knowledge and skills in critical situations. While some professionals are trained for this, others may lack the critical knowledge and experience needed to effectively perform under stressful disaster conditions. A set of clear, concise, and precise training standards that may be used to ensure workforce competency in such situations has been developed. The competency set has been defined by a broad and diverse set of leaders in the field and like-minded professionals through a series of Web-based surveys and expert working group meetings. The results may provide a useful starting point for delineating expected competency levels of health professionals in disaster medicine and public health.

The species here described somewhat superficially resembles Staphylinus cinnamopterus Grav., yet appears distinct and to be hitherto uncharacterized. In admiration of Professor J. H. McGregor of Columbia University, and in appreciation of kindnesses tendered me, I propose to name this new form:
Staphylinus macgregori n. sp.
Habitus nearly as in cinnamopterus but larger in size and rather more robust; piceous, the elytra fusco-ferrugineus, the legs fulvo-ferrugineus ; pubescence inclined, on the head, thorax, and elytra of a rusty hue and with few erect, brown setae, on abdomen fusco-piceous pubescence intermixing with fewer rusty-colored hairs and an abundance of dark setae; anterior tarsi broadly dilated in both sexes ; head slightly narrower than prothorax, widest at basal fourth thence gradually narrowing anteriorly, hind angles obtusely prominent, punctures round and ocellate throughout, close-set but clearly separated by shining interspaces, sparser anteriorly; mandibles longer and more slender than in cinnamopterus, the two inner teeth much less prominent and more poorly developed; antennae piceous to fuscopiceous, a third longer than head; prothorax but slightly wider than head, truncate at apex, rounded at base, punctures ocellate, more elongate at the sides than those of head and somewhat larger, smaller and more closely set than in cinnamopterus but separated by shining interspaces, the median line evident, polished, but very narrow throughout, in places ill-defined or obliterated; scutellum velvety black; elytra fusco-ferrugineus, longer than wide, very finely but strongly and densely punctate, clothed with a decurved rusty-colored pubescence similar to that of head and thorax but more dense; abdomen obscurely fulvous at tip, piceous, coriaceo-reticulate and very sparsely and finely punctate at sides, with median velvety fuliginous spots suffering intermixture of short cinereous hairs at the sides; under surface of abdomen clothed with sparse, long, depressed, fuscs-piceous pubescence, coriaceo-reticulate, punctures very fine and sparse, slightly less sparsely distributed at sides of segments, the male fifth ventral with a very broad circular and feeble sinuation medially, the male sixth ventral obscurely fulvous at tip and with a broad, rounded, median emargination about one half as deep as long, fifth ventral of female truncate, sixth female ventral rounded evenly and fulvous. Length ( ) 10.0-12.7 mm.; width 3.0-3.3 mm.

On page 81, of the Manual, it will be noticed that the key to the genera of Tochyporini is somewhat ccnfused. The alternate to the fitst series of characters refers us to number seven of the key. As there is no number seven, I have thought it advisable to publish the key in corrected form. It seems obvious that Bradley in re-arranging the key desired to follow Leconte and Horn's sequence of genera, viz. Tachinus, Tachyporus, Leucoparyphus (Cilea), Physetoporus, Erchomus, and Conosoma.

The International Consortium for Evidence-Based Perfusion (www.bestpracticeperfusion.org) is a collaborative partnership of societies of perfusionists, professional medical societies, and interested clinicians, whose aim is to promote the continuous improvement of the delivery of care and outcomes for patients undergoing extracorporeal circulation. Despite the many advances made throughout the history of cardiopulmonary bypass, significant variation in practice and potential for complication remains. To help address this issue, the International Consortium for Evidence-Based Perfusion has joined the Multi-Societal Database Committee for Pediatric and Congenital Heart Disease to develop a list of complications in congenital cardiac surgery related to extracorporeal circulation conducted via cardiopulmonary bypass, extracorporeal membrane oxygenation, or mechanical circulatory support devices, which include ventricular assist devices and intra-aortic balloon pumps. Understanding and defining the complications that may occur related to extracorporeal circulation in congenital patients is requisite for assessing and subsequently improving the care provided to the patients we serve. The aim of this manuscript is to identify and define the myriad of complications directly related to the extracorporeal circulation of congenital patients.

As part of an effort to develop a spectroscopic structure-property relationship in platinum acetylide oligomers, we have prepared a series of bidentate Pt(PBu3)2L2 compounds. The ligand was the series o-syd-C6H4-CΞC-(C6H4-CΞC)n-H, n = 0,1,2. The terminal oligomer unit consisted of a sydnone group ortho to the acetylene carbon. The compounds were characterized by various methods, including 13C-NMR, ground state absorption, fluorescence, phosphorescence and laser flash photolysis. The acetylenic 13C-NMR resonances showed sydnone influences that decreased with increasing number of monomer units. The ground state absorption spectra were slightly red shifted from those of the baseline oligomers not having a sydnone group. The low temperature emission and excitation spectra showed complex dependence on excitation and emission wavelengths, suggesting the chromphores resided in a distribution of solvent environments and conformations. Finally, broad triplet state absorption spectra were observed, with absorption throughout the visible and near infrared regions.

Summary
Yellow fever is an acute group B virus disease of short duration transmitted to humans by different genera of mosquitoes, but especially by the Aedes aegypti (known previously as the Stegomyia fasciata). It remains endemic in the tropical regions of Africa and the Americas in a sylvan or jungle form, but historically its greatest impact on humans has been in an epidemic or urban form. The disease can appear with symptoms ranging from extremely mild to malignant; in classic cases it is characterized by fever, headache, jaundice, albuminuria (high-protein content in the urine), and hemorrhage into the stomach and intestinal tract. High mortality rates were frequently recorded during epidemics (20 to 70 percent), although today we know that yellow fever mortality is actually relatively low, suggesting of course that the majority of the cases were mild and went undiagnosed. The jaundice has prompted the appellation yellow fever, and other designations such as the mal de Siam, fièvre jaune, gelbfieber, and virus amaril, whereas the hemorrhaging of black blood led to the name “black vomit” or vomito negro.
Known early in the New World as the “Barbados distemper,” “bleeding fever,” the “maladie de Siam,” “el peste,” vomito negro, and later yellow jack (because of the yellow quarantine flag flown by ships), the disease has been called by some 150 names. It was first termed “yellow fever” apparently by Griffin Hughes in his Natural History of Barbados (1750).
