Intrusion history of the Portrush Sill, County Antrim, Northern Ireland: evidence for rapid emplacement and high-temperature contact metamorphism

The gabbroic Portrush Sill in Northern Ireland, part of the North Atlantic Igneous Province, intruded Lower Jurassic mudstones and siltstones about 55 Ma ago. We used petrologic observations and geochemical analyses to study how the sill interacted with the sedimentary rocks. Field relationships show that an Upper Sill and numerous associated Minor Intrusions were emplaced in the sedimentary host rocks before intrusion of the Main Sill, some 10 m above its upper contact. Geochemical analyses reveal two magma contamination processes: Nb and Ta anomalies, coupled with incompatible element enrichment, record contamination by deep crustal rocks, whereas Li, Pb and Ba anomalies reveal a superficial contamination through fluid circulation at the contact between magmatic and sedimentary rocks. Analysis of mineral assemblages and geochemical data from the contact aureole demonstrate uniform metamorphic conditions between the two main intrusions and an absence of a thermal gradient. The identification of pyrrhotite by magnetization analyses and of orthopyroxene by microprobe analyses indicates very high temperatures, up to 660°C. Thermal modelling explains these temperatures as the coupled effects of the Main Sill and the earlier intruded Upper Sill and Minor Intrusions. Even though the chemical composition of the Main Sill suggests another type of parental liquid, all three units were emplaced in a very short time, certainly less than five years.

This paper describes the fluid mechanics within a box containing a vertical plane distributed source of buoyancy. A theoretical analysis is presented that models the development of plumes from such sources in an unconfined ambient of uniform density. Two extensions are considered. The first concerns a sealed box and the second involves the more general situation where the box is ventilated by openings at top and bottom. In the sealed box the stratification develops in much the same way as for a ‘filling box’ containing a single-point source of buoyancy on the floor. An initial front descends from the ceiling of the box and an asymptotic stratification eventually develops which is continuous in the vertical direction. In the case of the ventilated box it is found that a complex stratification develops where one or more horizontal intrusions are formed by detachment of the plume/boundary layer from the vertically distributed source where the buoyancy of the plume is less than, or equal to, that of the stratified ambient at a given height. Experimental results are presented to demonstrate the validity of the theory. The findings are relevant to both forced and naturally ventilated buildings containing non-adiabatic vertical surfaces.

Introduction:
Recent events have heightened awareness of disaster health issues and the need to prepare the health workforce to plan for and respond to major incidents. This has been reinforced at an international level by the World Association for Disaster and Emergency Medicine, which has proposed an international educational framework.
Objective:The aim of this paper is to outline the development of a national educational framework for disaster health in Australia.
Methods:The framework was developed on the basis of the literature and the previous experience of members of a National Collaborative for Disaster Health Education and Research. The Collaborative was brought together in a series of workshops and teleconferences, utilizing a modified Delphi technique to finalize the content at each level of the framework and to assign a value to the inclusion of that content at the various levels.
Framework:The framework identifies seven educational levels along with educational outcomes for each level. The framework also identifies the recommended contents at each level and assigns a rating of depth for each component. The framework is not intended as a detailed curriculum, but rather as a guide for educationalists to develop specific programs at each level.
Conclusions:This educational framework will provide an infrastructure around which future educational programs in Disaster Health in Australia may be designed and delivered. It will permit improved articulation for students between the various levels and greater consistency between programs so that operational responders may have a consistent language and operational approach to the management of major events.

The completion of the SCAR Antarctic digital database (ADD) has provided a new basis for statistical calculations for Antarctica: data-sets are available at the scale of the original source material, and generalised to 1:1,000,000, 1:3,000,000, 1:10,000,000, and 1:30,000,000. The new descriptive statistics presented are based on the ADD 1:1,000,000 data-set since this is the largest scale at which source maps provided complete cover of the coastline and ice-free areas. The statistics include the total length and proportions of coastline types and the total area of Antarctica with the proportions of its constituent feature types. The areas of the Ross and Filchner-Ronne ice shelves have also been computed. Whilst the total area of Antarctica has remained static compared with previous studies, the relative proportions of coastline types and constituent feature types within the total area show significant changes. In particular the calculated area of ice-free ground is only approximately one-seventh of that often quoted from previous studies. The changes reported result from improved mapping, reinterpretation of data, and actual changes of coastline.

Mechanical circulatory support is an invaluable tool in the care of children with severe refractory cardiac and or pulmonary failure. Two forms of mechanical circulatory support are currently available to neonates, infants, and smaller children, namely extracorporeal membrane oxygenation and use of a ventricular assist device, with each technique having unique advantages and disadvantages. The intra-aortic balloon pump is a third form of mechanical support that has been successfully used in larger children, adolescents, and adults, but has limited applicability in smaller children. In this review, we discuss the current experiences with extracorporeal membrane oxygenation and ventricular assist devices in children with cardiac disease.
A variety of forms of mechanical circulatory support are available for children with cardiopulmonary dysfunction refractory to conventional management. These devices require extensive resources, both human and economic. Extracorporeal membrane oxygenation can be effectively used in a variety of settings to provide support to critically-ill patients with cardiac disease. Careful selection of patients and timing of intervention remains challenging. Special consideration should be given to children with cardiac disease with regard to anatomy, physiology, cannulation, and circuit management. Even though exciting progress is being made in the development of ventricular assist devices for long-term mechanical support in children, extracorporeal membrane oxygenation remains the mainstay of mechanical circulatory support in children with complex anatomy, particularly those needing rapid resuscitation and those with a functionally univentricular circulation.
As the familiarity and experience with extracorporeal membrane oxygenation has grown, new indications have evolved, including emergent resuscitation. This utilization has been termed extracorporeal cardiopulmonary resuscitation. The literature supporting emergent cardiopulmonary support is mounting. Reasonable survival rates have been achieved after initiation of support during active compressions of the chest following in-hospital cardiac arrest. Due to the limitations of conventional circuits for extracorporeal membrane oxygenation, some centres have developed novel systems for rapid cardiopulmonary support.
Many centres previously considered a functionally univentricular circulation to be a contraindication to extracorporeal membrane oxygenation, but improved results have been achieved recently with this complex subset of patients. The registry of the Extracorporeal Life Support Organization recently reported the outcome of extracorporeal life support used in neonates for cardiac indications from 1996 to 2000. Of the 740 neonates who were placed on extracorporeal life support for cardiac indications, 118 had hypoplastic left heart syndrome. There was no significant difference in survival between these patients and those with other defects. It is now common to use extracorporeal membrane oxygenation to support patients with a functionally univentricular circulation, and reasonable survival rates are to be expected.
Although extracorporeal membrane oxygenation has become a standard of care for many paediatric centres, its use is limited to those patients who require only short-term cardiopulmonary support. Mechanical ventricular assist devices have become standard therapy for adults with cardiac failure refractory to maximal medical management. Several devices are readily available in the United States of America for adults, but there are fewer options available to children. Over the last few years, substantial progress has been made in paediatric mechanical support. Ventricular assist devices are being used with increasing frequency in children with cardiac failure refractory to medical therapy for primary treatment as a long-term bridge to recovery or transplantation. The paracorporeal, pneumatic, pulsatile “Berlin Heart” ventricular assist device is being used with increasing frequency in Europe and North America to provide univentricular and biventricular support. With this device, a patient can be maintained on mechanical circulatory support while extubated, being mobilized, and feeding by mouth.
Mechanical circulatory support should be anticipated, and every attempt must be made to initiate support “urgently” rather than “emergently”, before the presence of dysfunction of end organs or circulatory collapse. In an emergency, these patients can be resuscitated with extracorporeal membrane oxygenation and subsequently transitioned to a long-term ventricular assist device after a period of stability.

Enhanced protein fold recognition using secondary structure information from NMR

NMR offers the possibility of accurate secondary structure for proteins that would be too large for structure determination. In the absence of an X-ray crystal structure, this information should be useful as an adjunct to protein fold recognition methods based on low resolution force fields. The value of this information has been tested by adding varying amounts of artificial secondary structure data and threading a sequence through a library of candidate folds. Using a literature test set, the threading method alone has only a one-third chance of producing a correct answer among the top ten guesses. With realistic secondary structure information, one can expect a 60–80% chance of finding a homologous structure. The method has then been applied to examples with published estimates of secondary structure. This implementation is completely independent of sequence homology, and sequences are optimally aligned to candidate structures with gaps and insertions allowed. Unlike work using predicted secondary structure, we test the effect of differing amounts of relatively reliable data.

Summary
Mechanical disorders of the spine are defined as degenerative spinal conditions, such as disc degeneration and spondylosis, which are frequent causes of radiculopathy and myelopathy, and which are not associated with trauma or neoplasm. These conditions are among the most commonly treated diseases in neurosurgical practice. Lumbar discectomy represents the most common neurosurgical procedure performed in the United States. Lumbar stenosis is the most commonly diagnosed condition in adults, older than 65 years, undergoing lumbar spine surgery (Turner et al. 1992). Similarly, cervical spondylotic myelopathy is said to be ‘the most common spinal cord disorder in persons over 55’ (Rengachary & Redford 1991). The operative treatment of mechanical disorders of the spine has undergone unparalleled technical advancement during the past 10 years. However, the reported outcome of surgery for these conditions is highly variable. The majority of outcome studies are retrospective analyses which are methodologically imperfect. A critical review of the literature reveals several consistent flaws in the study design as well as in the analysis and reporting of outcome data (Turner et al. 1992).
This chapter is divided into four sections. We have reviewed the outcome of surgical treatment of four common mechanical disorders of the spine: cervical spondylotic myelopathy, herniated lumbar disc, lumbar stenosis and degenerative lumbar instability. A critical analysis of the results reported in the literature is undertaken.
Cervical spondylotic myelopathy
Cervical spondylosis is extremely common in the adult population (Rengachary & Redford 1991). Radiographic spondylosis is present in 25–50% of the population by age 50 years and increases to 75–85% by age 65 years (Montgomery & Brower 1992).
