Spillover benefit of pre-exposure prophylaxis for HIV prevention: evaluating the importance of effect modification using an agent-based model

We developed an agent-based model using a trial emulation approach to quantify effect measure modification of spillover effects of pre-exposure prophylaxis (PrEP) for HIV among men who have sex with men (MSM) in the Atlanta-Sandy Springs-Roswell metropolitan area, Georgia. PrEP may impact not only the individual prescribed, but also their partners and beyond, known as spillover. We simulated a two-stage randomised trial with eligible components (≥3 agents with ≥1 HIV+ agent) first randomised to intervention or control (no PrEP). Within intervention components, agents were randomised to PrEP with coverage of 70%, providing insight into a high PrEP coverage strategy. We evaluated effect modification by component-level characteristics and estimated spillover effects on HIV incidence using an extension of randomisation-based estimators. We observed an attenuation of the spillover effect when agents were in components with a higher prevalence of either drug use or bridging potential (if an agent acts as a mediator between ≥2 connected groups of agents). The estimated spillover effects were larger in magnitude among components with either higher HIV prevalence or greater density (number of existing partnerships compared to all possible partnerships). Consideration of effect modification is important when evaluating the spillover of PrEP among MSM.

Declines in populations of the Critically Endangered Spoon-billed Sandpiper Calidris pygmaeus have been rapid, with the breeding population now perhaps numbering fewer than 120 pairs. The reasons for this decline remain unresolved. Whilst there is evidence that hunting in wintering areas is an important factor, loss of suitable habitat on passage and wintering areas is also of concern. While some key sites for the species are already documented, many of their wintering locations are described here for the first time. Their wintering range primarily stretches from Bangladesh to China. Comprehensive surveys of potential Spoon-billed Sandpiper wintering sites from 2005 to 2013 showed a wide distribution with three key concentrations in Myanmar and Bangladesh, but also regular sites in China, Vietnam and Thailand. The identification of all important non-breeding sites remains of high priority for the conservation of the species. Here, we present the results of field surveys of wintering Spoon-billed Sandpipers that took place in six countries between 2005 and 2013 and present species distribution models which map the potential wintering areas. These include known and currently unrecognised wintering locations. Our maximum entropy model did not identify any new extensive candidate areas within the winter distribution, suggesting that most key sites are already known, but it did identify small sites on the coast of eastern Bangladesh, western Myanmar, and the Guangxi and Guangdong regions of China that may merit further investigation. As no extensive areas of new potential habitat were identified, we suggest that the priorities for the conservation of this species are habitat protection in important wintering and passage areas and reducing hunting pressure on birds at these sites.

BOUT++ is a 3D nonlinear finite-difference plasma simulation code, capable of solving quite general systems of Partial Differential Equations (PDEs), but targeted particularly on studies of the edge region of tokamak plasmas. BOUT++ is publicly available, and has been adopted by a growing number of researchers worldwide. Here we present improvements which have been made to the code since its original release, both in terms of structure and its capabilities. Some recent applications of these methods are reviewed, and areas of active development are discussed. We also present algorithms and tools which have been developed to enable creation of inputs from analytic expressions and experimental data, and for processing and visualisation of output results. This includes a new tool Hypnotoad for the creation of meshes from experimental equilibria. Algorithms have been implemented in BOUT++ to solve a range of linear algebraic problems encountered in the simulation of reduced Magnetohydrodynamics (MHD) and gyro-fluid models: A preconditioning scheme is presented which enables the plasma potential to be calculated efficiently using iterative methods supplied by the PETSc library (the Portable, Extensible Toolkit for Scientific Computation) (Balay et al. 2014), without invoking the Boussinesq approximation. Scaling studies are also performed of a linear solver used as part of physics-based preconditioning to accelerate the convergence of implicit time-integration schemes.
Is there a change in the epidemiology of nasal fractures in females in the UK?

Objective:
To determine whether there is a change in the epidemiology of nasal fractures in females in the UK, and the potential contribution of the ‘ladette’ culture.
Methods:This paper reports a multi-centre retrospective study. Operating theatre data for all females who underwent manipulation of nasal fractures under anaesthesia between 2002 and 2009 were analysed. In addition, the case notes of all females presenting with nasal fractures over a five-year period (2004–2009) were retrospectively reviewed and the cited cause of the fracture was noted.
Results:From 2002 to 2009, there was an 825 per cent increase in nasal fractures in women aged 13–20 years. Almost one-quarter of all nasal fractures in one centre was attributed to non-domestic violence. The highest incidence of nasal fractures (67 per cent) was amongst white British females.
Conclusion:There is an increasing trend in the number of women sustaining nasal fractures in the UK. The cause may be multi-factorial, but could be partially attributed to a rise in ladette culture. Further research on the role of alcohol consumption in this phenomenon is needed.

The following article is based on the plenary presentation given by Mildred S. Dresselhaus of the Massachusetts Institute of Technology on November 29, 2004, at the Materials Research Society Fall Meeting in Boston. Advanced materials offer new promise for addressing some of the grand societal challenges of our future, including that of global energy. This article will review opportunities that have opened up at the nanoscale, with materials of reduced dimensionality and enhanced surface-to-volume ratio. Some examples of research accomplishments and opportunities at the nanoscale will be described, with special attention given to the potential for advanced materials and nanoscience to have an impact on the grand challenges related to a sustainable energy supply for the 21st century and beyond.

The in situ dry matter disappearance technique (Ørskov and McDonald, 1979) evaluates forages for their rate and extent of degradation in the rumen. However, this method does not allow the evaluation of a large number of samples at one and the same time and therefore which limits screening of treatments applied to forages. The in vitro gas production method is faster and allows handling of many samples per batch; therefore, gas production could be an alternative to the use of nylon bags if the response to treatments between the two methods is similar among treated forages. The objective of this experiment was to compare results obtained with both the gas production and the nylon bag techniques for forages treated with four levels of maceration and conserved as hay or silage.

Summary
Amerindian and genetically related populations in North America currently are affected by a pandemic of obesity, diabetes, and gallbladder disease. This has arisen in just the past 40–50 years; the conditions were previously rare in Amerindian populations. The rapid development of the pandemic thus implicates changes in lifestyle, very likely involving dietary patterns such as excess calories or newly adopted foodstuffs, which have occurred on a continental scale. ‘Urbanisation’, even in small settlements, seems to be associated with dramatically increased risk. The evidence also suggests that the conditions have a genetic basis; that is, there is an interaction between genetic susceptibility and the environmental risk factors. In 1984 Weiss, Ferrell, and Hanis synthesised a variety of indirect pieces of evidence to hypothesise that these conditions constitute a single genetic entity, or ‘syndrome’, that is, are biologically interconnected conditions sharing genetic risk factors. Subsequent research reports have been consistent with that idea. In this paper we review some of that evidence, and present new supportive results from studies in Mexican Mayan and Mvskoke Creek Amerindians. However, no gene or genes that are responsible and would prove the syndrome hypothesis have yet been identified.
Introduction
After World War II, numerous reports began to appear documenting a rapidly increasing prevalence of non-insulin-dependent diabetes mellitus (NIDDM) in many groups of Amerindians. Tribal groups from essentially every part of North America were affected, and there were suggestions that the same problems existed in Central and South America.

Use of dielectric films is integral to most microelectronic packaging, as passive components for device insulation (interlayer dielectric, passivation, diffusion layer), as thin layer capacitive structures or as insulating coatings for thin film and thick film hybrid circuits. Depending on deposition technique, film composition may include SiO2, Al2O3, Si3N4, modified silicate or phosphate glasses as insulators. Compositions as well as methodologies for film preparation on Si, glass or metal substrate by spin coating (or spraying) are discussed, including film/substrate interface characteristics and properties related to dielectric film coatings.
