Summary
Selection of the anesthetic technique to be employed during a procedure begins during the preoperative evaluation with consideration of factors such as the patient’s comorbidities and preferences and the type of procedure to be performed. Oftentimes, general anesthesia is not necessary and the procedure can be performed under a lesser depth of sedation. Procedural sedation is a technique that allows the patient to tolerate the discomfort of a procedure while still maintaining cardiorespiratory function. In order to accomplish this, the anesthesia provider administers sedative, dissociative, and/or analgesic agents alone or in combination [1].

Summary
Preoperatively, the patient will transition from different depths of anesthesia, including the levels of sedation, to general anesthesia (GA). Sedation is a continuum of symptoms that range from minimal symptoms of anxiolysis to symptoms of moderate and deep sedation. Moderate sedation is defined by the patient remaining asleep, but being easily arousable. Deep sedation is achieved when the patient is only arousable to painful stimulation. GA refers to medically induced loss of consciousness with concurrent loss of protective reflexes and skeletal muscle relaxation. GA is most commonly achieved via induction with intravenous sedatives and analgesics, followed by maintenance of volatile anesthetics [1]. Table 9.1 lists the depths of anesthesia and associated characteristics.

Mcalpineite, ideally Cu3TeO6·H2O, occurs as isolated 0.5 mm-sized emerald green cryptocrystalline crusts on white quartz at the long-abandoned McAlpine mine, Tuolumne County, California, U.S.A. Associated nonmetallic phases are muscovite (mariposite), calcite, goethite, hematite, chlorargyrite, choloalite, keystoneite, mimetite, malachite, azurite, annabergite and a host of unidentified crusts, both crystalline and amorphous. Associated metallic minerals include pyrite, acanthite, hessite, electrum, altaite, native silver, galena, pyrargyrite, sphalerite and owyheeite. The mineral has also been identified at the Centennial Eureka mine, Juab County, Utah, U.S.A., where it occurs as interstitial olive-green coatings and as millimetre-sized dark green-black cryptocrystalline nodules lining drusy quartz vugs. Associated minerals are xocomecatlite, hinsdalite-svanbergite, goethite and several new species including two hydrated copper tellurates, a hydrated copper-zinc tellurate/tellurite, and a hydrated copper-zinc tellurate/tellurite-arsenate-chloride. Mcalpineite is cubic, P-lattice (space group unknown), a = 9.555(2) Å, V = 872.4(4) Å. The strongest six lines in the X-ray powder-diffraction pattern [d in Å (I) (hkl)] are: 4.26(40)(210), 2.763(100)(222), 2.384(70)(400), 1.873(40)(431,510), 1.689(80)(440) and 1.440(60)(622). The average of four electron-microprobe analyses (McAlpine mine) is CuO 50.84, NiO 0.17, PbO 4.68, SiO2 0.65, TeO3 39.05, H2O (calc.) [4.51], total [100.00] wt. %. With O = 7, the empirical formula is (Cu2.79Pb0.09Ni0.01)∑2.89(Te0.97Si0.05)∑1.02O5.90·1.10H2O. This gives a calculated density of 6.65. g/cm3 for Z = 8. The average of two electron-microprobe analyses (Centennial Eureka mine) is CuO 51.2, ZnO 3.1, TeO3 39.0, SiO2 0.2, As2O5 0.8, H2O (by CHN elemental analyser) 7, total 101.3 wt. %, leading to the empirical formula (Cu2.56Zn0.15)∑2.71 (Te0.88Si0.02As0.02)∑0.92O5.47·1.53H2O. The infrared absorption spectrum shows definite bands for structural H2O with an O-H stretching frequency centred at 3320 cm−1 and a H-O-H flexing frequency centred at 1600 cm−1. In reflected light Mcalpineite is isotropic, nondescript grey, with ubiquitous brilliant apple to lime green internal reflections. The refractive index calculated from Fresnel equations is 2.01. Measured reflectance values in air and in oil are tabulated. Reflectance study also shows that cryptocrystalline aggregates are composed of micron-sized sheaves of fibrous or prismatic crystals. Other physical properties include: adamantine lustre; light green streak; brittle; uneven fracture; translucent to transparent and nonfluorescent under both long- and short-wave ultraviolet light. The name is for the first known locality, the McAlpine mine.

Summary
Synopsis
Beaches and dunes of the open coast form one of the globe’s longest ecological interfaces, linking the oceans with the land. These systems are of great importance to society as prime sites for housing and recreation, buffers against storms, and providers of fisheries and mineral resources. By contrast, their unique ecological attributes and biodiversity are much less recognized. In this chapter, we provide a synthesis of the key ecological features and functions of beaches and dunes, outline the main elements of their faunal biodiversity, examine human threats and their biological consequences, and sketch some salient issues in management to achieve conservation of these unique ecosystems. It is apparent that the range of ecosystem goods and services is broad, but nutrient cycling, water filtration, and the provision of habitat and prey for a diverse range of animals are often the key ecological traits. Contrary to common perceptions, beaches and dunes contain a diverse and unique set of species, many of which are found nowhere else. In addition to the complement of highly adapted invertebrates, many wildlife species (e.g. birds, turtles, fishes) are dependent on beaches and dunes for nesting and feeding, and they use these habitats extensively. Human pressures on sandy shorelines and their biodiversity are numerous. Coastal squeeze is, however, the most pervasive, trapping beaches and their biota between the pressures of development from the terrestrial side and the consequences of climate change from the marine side. Beaches are also naturally malleable habitats whose interlinkages, including the exchange of organisms, with the abutting dunes and surf zones are essential to their functioning. Unfortunately, human actions intended to arrest the dynamics of beach habitats, such as seawalls and dune stabilizations, run counter to these natural dynamics and generally produce negative environmental outcomes. These present a set of formidable management challenges when the primary goal is to conserve intact ecosystems and biodiversity, calling for more systematic approaches in conservation design and implementation for beach and dune ecosystems.
Are diet and feeding behaviours associated with the onset of and recovery from slow weight gain in early infancy?

Infants with slow weight gain cause concern in parents and professionals, but it is difficult to be certain whether such infants are genetically small or whether their energy intake is insufficient. The aim of the present study was to assess the impact of diet and feeding behaviours on slow weight gain early in infancy. The sample was 11 499 term infants from the Avon Longitudinal Study of Parents and Children (ALSPAC). A total of 507 cases of slow weight gain from birth to 8 weeks were identified and the remaining 10 992 infants were used as controls. It was found that infants who gained weight slowly between birth and 8 weeks were more likely to exhibit feeding problems such as weak sucking and slow feeding during this period. Feeding problems were substantially reduced during the recovery phase (8 weeks to 2 years) when these infants exhibited enhanced catch-up in weight. The proportion of mothers breast-feeding in the 4th week after birth was higher for slow weight gainers, but they were more likely to switch to formula at the start of recovery. During recovery, slow-weight gain infants had a slightly higher energy intake from formula and solids than controls. In conclusion, feeding problems seem to be the most important factors associated with the onset of early slow weight gain. Subsequently, a reduction of feeding problems and an increase in overall energy intake may contribute to their weight recovery. Health professionals should look for feeding problems in the first few weeks after birth and help mothers establish adequate feeding practices.

A discrete spatial model of a multi-species environment is formulated, and the behaviour of the system is studied. The model is used to explore stability and resilience of biological systems and discuss how they are dependent on spatial scale chosen.

The emerging statistical properties from the first 50 extrasolar planets are startlingly different from the picture that was imagined prior to 1995. About 0.75% of nearby solar type stars harbor jovian planets in 3 to 5 day circular orbits. Another ∽7% of stars have jupiter–mass companions orbiting in eccentric orbits within 3.5 AU. The mass distribution of substellar companions rises abruptly near 5 MJup and continues increasing down to the detection limit near 1 MJup-Orbital eccentricities correlate positively with semimajor axes, even for planets beyond the tidal circularization zone within 0.1 AU, distinguishing planets from binary stars. The planet bearing stars are metal–rich relative to both nearby stars and to the Sun. Analogs of Solar System planets have not been detected to date as they require precision of 3 m s−1 maintained for more than a decade.

In July 1986, an AMS radiocarbon target preparation laboratory was established at the Woods Hole Oceanographic Institution to produce graphite to be analyzed at the NSF-Accelerator Facility for Radioisotope Analysis at the University of Arizona (Tucson). By June 1991, 923 graphite targets had been prepared and 847 analyzed. Our lab procedures during this time included the careful documentation of weights of all starting samples, catalysts and final graphite yields, as well as the volume of CO2 gas evolved during CaCO3 hydrolysis or closed-tube organic carbon combustions. From these data, we evaluate the methods used in general and in our lab.
Nosocomial Urinary Tract Infection: A Prospective Evaluation of 108 Catheterized Patients

This study describes the evaluation of 108 patients who had indwelling urethral catheters for acute medical and surgical indications. Patients were evaluated daily, and cultures from bladders and drainage bags were obtained. Appropriateness for continuing catheterization was assessed using preset criteria. Twenty-five patients developed urinary tract infections. Exposure to antibiotics and a shorter duration of catheterization were the only factors that correlated significantly with a delayed onset or decreased prevalence of infection. Factors found to have insignificant effects included age, sex, maintenance of the closed system, underlying host disease status, catheter type, and reason for catheterization. No collection systems with one way valves were used, but significant colony counts in drainage bag urine preceded urinary tract infection in only two patients. Thirty-six percent of the total 562 catheter days were judged unnecessary. A major emphasis must be placed on prompt catheter removal if the prevalence of nosocomial urinary tract infections is to be reduced substantially in a cost-effective manner [Infect Control 1981; 2(5):380-386.]

I Discuss here some features of the type of storage amphora dubbed ‘SOS’, a large semi-decorated container in use from the later eighth to the first half of the sixth century B.C., and found at a large number of sites around the Mediterranean and beyond. In particular, the evidence of clay analyses carried out at the British School by Richard Jones will be adduced to confirm the Attic origin of the majority of these vases, while other centres of production will be reviewed. I also treat briefly the shape and decoration of the type and the inscriptions which the vases often carry. Other scholars are working on different aspects of the SOS amphora and I have therefore restricted my comments here; similarly, I do not treat at length material which is in course of publication, leaving closer discussion of dating especially to the excavators concerned.
