Patterns, predictors, and patient-reported reasons for antidepressant discontinuation in the WHO World Mental Health Surveys

Background
Despite their documented efficacy, substantial proportions of patients discontinue antidepressant medication (ADM) without a doctor's recommendation. The current report integrates data on patient-reported reasons into an investigation of patterns and predictors of ADM discontinuation.
MethodsFace-to-face interviews with community samples from 13 countries (n = 30 697) in the World Mental Health (WMH) Surveys included n = 1890 respondents who used ADMs within the past 12 months.
Results10.9% of 12-month ADM users reported discontinuation-based on recommendation of the prescriber while 15.7% discontinued in the absence of prescriber recommendation. The main patient-reported reason for discontinuation was feeling better (46.6%), which was reported by a higher proportion of patients who discontinued within the first 2 weeks of treatment than later. Perceived ineffectiveness (18.5%), predisposing factors (e.g. fear of dependence) (20.0%), and enabling factors (e.g. inability to afford treatment cost) (5.0%) were much less commonly reported reasons. Discontinuation in the absence of prescriber recommendation was associated with low country income level, being employed, and having above average personal income. Age, prior history of psychotropic medication use, and being prescribed treatment from a psychiatrist rather than from a general medical practitioner, in comparison, were associated with a lower probability of this type of discontinuation. However, these predictors varied substantially depending on patient-reported reasons for discontinuation.
ConclusionDropping out early is not necessarily negative with almost half of individuals noting they felt better. The study underscores the diverse reasons given for dropping out and the need to evaluate how and whether dropping out influences short- or long-term functioning.

Background
The most common treatment for major depressive disorder (MDD) is antidepressant medication (ADM). Results are reported on frequency of ADM use, reasons for use, and perceived effectiveness of use in general population surveys across 20 countries.
MethodsFace-to-face interviews with community samples totaling n = 49 919 respondents in the World Health Organization (WHO) World Mental Health (WMH) Surveys asked about ADM use anytime in the prior 12 months in conjunction with validated fully structured diagnostic interviews. Treatment questions were administered independently of diagnoses and asked of all respondents.
Results3.1% of respondents reported ADM use within the past 12 months. In high-income countries (HICs), depression (49.2%) and anxiety (36.4%) were the most common reasons for use. In low- and middle-income countries (LMICs), depression (38.4%) and sleep problems (31.9%) were the most common reasons for use. Prevalence of use was 2–4 times as high in HICs as LMICs across all examined diagnoses. Newer ADMs were proportionally used more often in HICs than LMICs. Across all conditions, ADMs were reported as very effective by 58.8% of users and somewhat effective by an additional 28.3% of users, with both proportions higher in LMICs than HICs. Neither ADM class nor reason for use was a significant predictor of perceived effectiveness.
ConclusionADMs are in widespread use and for a variety of conditions including but going beyond depression and anxiety. In a general population sample from multiple LMICs and HICs, ADMs were widely perceived to be either very or somewhat effective by the people who use them.

OBJECTIVES/SPECIFIC AIMS: Using national Medicaid claims, this retrospective cohort study aims to compare the outcomes of levetiracetam (LEV) versus phenobarbital (PHB) as initial monotherapy in infants with epilepsy aged one month to one year. We primarily analyzed health services outcomes, as follows: (1) Emergency Department (ED) visits: proportion of those with at least 1 ED visit, time to first ED visit, total number of ED visits, proportion leading to inpatient admission (2) Inpatient Admissions: proportion of those with at least 1 admission, time to first admission, total number of inpatient admissions, total length of stay (3) Treatment discontinuation: proportion of those who discontinued medication, time to discontinuation, proportion of those with prescription of second antiepileptic drug (AED) (4) Cost: total cost at 1 year from first prescription. METHODS/STUDY POPULATION: The project is a retrospective cohort study utilizing Medicaid claims from 2009-2012 from all states and DC. Inclusion criteria were: (1) Epilepsy diagnosis (ICD9 345.x) at age 1 month to 1 year (2) 1 year of continuous Medicaid enrollment Exclusion criteria were: (1) Neither LEV nor PHB monotherapy as initial treatment (2) Neither LEV nor PHB prescription within 45 days of epilepsy diagnosis (3) Less than 1 year of follow-up after first LEV or PHB prescription (4) Infantile Spasms diagnosis (5) Brain Surgery (6) Death within 1 year of follow-up Using R for statistical analysis, we analyzed outcomes including ED visits, inpatient admissions, treatment discontinuation, and total cost. In this abstract, we present our preliminary bivariate analysis. RESULTS/ANTICIPATED RESULTS: Demographics and Etiology: Compared to infants prescribed PHB (n = 1954), infants prescribed LEV (n = 1248) were older (median 6 months [IQR 3-9] vs 3[2-6]; p < 0.001), more likely to be white (44.2% vs 38.9%; p < 0.05) and not Hispanic (63.5% vs 58.9%; p<0.05). There were also important differences in epilepsy etiologies (p < 0.05). For example, infants prescribed LEV were more likely to have a diagnosis of tuberous sclerosis (1% vs 0.15%) or traumatic brain injury (12.8% vs 0.56%). Health Services Outcomes: After 1 year, infants prescribed LEV had more ED visits (2 [0-4] vs 1 [0-3]; p < 0.001) but shorter inpatient length of stay when admitted (3 days [2-5] vs 3 [2-6]; p < 0.001). They were less likely to discontinue the medication (46.6% vs 64.3%; p<0.001) but more likely to have a second AED prescription (53.3% vs 43.4%; p < 0.001). Other outcomes, including total cost, were similar. DISCUSSION/SIGNIFICANCE OF IMPACT: This preliminary analysis suggests that the healthcare trajectory of infants treated with LEV and PHB differ in complex ways. In ongoing work, we are conducting a multivariable comparative effectiveness analysis of LEV versus PHB using propensity score weighting to account for observable selection bias and multiple imputation to account for missing data.

Background: Endovascular thrombectomy (EVT) has shown efficacy in acute ischemic stroke (AIS) patients with infective endocarditis (IE). The possibility to undertake advanced histopathological clot analysis following EVT offers a new avenue to establish the etiological basis of the stroke – which is often labelled “cryptogenic.” In this paper, we present our findings from four consecutive patients with IE who underwent EVT following an AIS at our tertiary referral comprehensive stroke centre. Methods: Comprehensive histopathological analysis of clot retrieved after EVT, including morphology, was undertaken. Results: The consistent observation was the presence of dense paucicellular fibrinoid material mixed/interspersed with clusters of bacterial cocci. This clot morphology may be specific to septic embolus due to IE unlike incidental bacteraemia and could possibly explain the refractoriness of such clots to systemic thrombolysis. Conclusion: Detailed morphological and histopathological analysis of EVT-retrieved clots including Gram staining can assist in etiological classification of the clot. Understanding the composition of the clot may be of clinical value in early diagnostics and mapping treatment planning in IE.
Revisiting the Revised Hierarchical Model: Evidence for concept mediation in backward translation

A claim fundamental to the revised hierarchical model (Kroll & Stewart, 1994) was that concepts did not mediate backward translation, based on their findings of a category interference effect in forward translation in relatively fluent bilinguals but no category effect in backward translation. This study hypothesized that there was a category facilitation effect in L2-to-concept, which counterbalanced the category interference effect in concept-to-L1, resulting in an overall L2-to-L1 null category effect.
In a novel English word-pair semantic comparison task, participants were presented with a sequence of English word-pairs, and judged which word's real-world referent was bigger in size. Results found a significant category facilitation effect in both L2-to-concept for young Chinese adults and L1-to-concept for young English adults when increasing the number of trials. The findings help explain why Kroll and Stewart's finding of an overall L2-to-L1 null category effect cannot be evidence against concept mediation in backward translation.

The Magellan Adaptive Optics (MagAO) system saw first light in November 2012 at Las Campanas Observatory (LCO) on the 6.5m Clay telescope. Here we present an introduction to MagAO's visible wavelength diffraction limited imager, VisAO. VisAO delivers Strehl ratios greater than 30% from 0.62 microns (r') through 1 micron, where Strehl is even higher, and achieved resolutions as small as 20 milli-arcseconds. We took advantage of the excellent performance of MagAO/VisAO to conduct high contrast observations of an exoplanet in the optical. With VisAO, we are, for the first time, able to begin characterizing exoplanet atmospheres in the optical from the ground.

MagAO is the newly-commissioned adaptive optics (AO) instrument on the Magellan Clay telescope at Las Companas Observatory, Chile. MagAO has two co-mounted science cameras: VisAO for visible-light direct and spectral-differential imaging; and Clio for near to thermal IR direct imaging, non-redundant-mask interference, and prism spectroscopy. We demonstrate MagAO's simultaneous visible and infrared AO performance via direct images of exoplanet Beta Pictoris b. The planet was detected in 5 passbands from 0.9–5μm. Here we show the infrared images; the visible observations are presented in Males et al. 2013. MagAO is the first AO system to offer good performance with extensive coverage across the O/IR spectrum and thus offers an unprecedented opportunity to study the spectral energy distributions of directly-imaged extrasolar planetary atmospheres.

Ganglion cell activity in response to drifting or alternating stimulus patterns was recorded in an eyecup preparation of the turtle. The stimuli were sinusoidal gratings with various spatial frequencies and spatial phases. Spike histograms of the activity were Fourier-analyzed. The results indicated that the retinal circuitry always functioned in a nonlinear manner. The average, first, and second harmonics of the activity were all a function of stimulus conditions, including spatial phase. Similar outcomes were obtained with full-field (retina-wide) and a small-field (1 mm diameter) stimulation. In the middle of the spatial-frequency range, the ratio of second to first harmonic activity was higher in response to static, contrast-reversing gratings than for drifting, constant-contrast ones. The findings suggest that, as distinct from the usual X, Y, and W segregation of ganglion cell properties in other retinas, there is a single class of cells whose responses seem to have attributes of one or another of the usual categories depending on stimulus characteristics. This may be the result of a degree of stimulus-dependent functional modification in the retinal input circuitry to the turtle ganglion cells.
Purinergic receptors activating rapid intracellular Ca2+ increases in microglia

We provide both molecular and pharmacological evidence that the metabotropic, purinergic, P2Y6, P2Y12 and P2Y13 receptors and the ionotropic P2X4 receptor contribute strongly to the rapid calcium response caused by ATP and its analogues in mouse microglia. Real-time PCR demonstrates that the most prevalent P2 receptor in microglia is P2Y6 followed, in order, by P2X4, P2Y12, and P2X7 = P2Y13. Only very small quantities of mRNA for P2Y1, P2Y2, P2Y4, P2Y14, P2X3 and P2X5 were found. Dose-response curves of the rapid calcium response gave a potency order of: 2MeSADP>ADP=UDP=IDP=UTP>ATP>BzATP, whereas A2P4 had little effect. Pertussis toxin partially blocked responses to 2MeSADP, ADP and UDP. The P2X4 antagonist suramin, but not PPADS, significantly blocked responses to ATP. These data indicate that P2Y6, P2Y12, P2Y13 and P2X receptors mediate much of the rapid calcium responses and shape changes in microglia to low concentrations of ATP, presumably at least partly because ATP is rapidly hydrolyzed to ADP. Expression of P2Y6, P2Y12 and P2Y13 receptors appears to be largely glial in the brain, so that peripheral immune cells and CNS microglia share these receptors. Thus, purinergic, metabotropic, P2Y6, P2Y12, P2Y13 and P2X4 receptors might share a role in the activation and recruitment of microglia in the brain and spinal cord by widely varying stimuli that cause the release of ATP, including infection, injury and degeneration in the CNS, and peripheral tissue injury and inflammation which is signaled via nerve signaling to the spinal cord.

AlN/GaN multiple quantum wells (MQWs) were grown on sapphire substrates by plasmaassisted molecular beam epitaxy. Growth temperature, III/V ratio, growth rate, and other growth parameters were optimized for the buffer layer and the MQWs, separately. The growth of AlN buffer was kept as Al-rich as possible while the formation of Al droplets was avoided. A GaN buffer layer was also tried but proved to be inferior to AlN buffer probably due to its larger surface roughness, higher dislocation density, and larger lattice mismatch with the AlN barrier layers in the MQWs. Very flat surfaces with a RMS roughness of 0.7nm were observed by atomic force microscopy (AFM) on the samples with both AlN buffer layer and 20 MQWs deposited under the optimized growth conditions. Abrupt interfaces and excellent periodicities of the MQWs were confirmed by X-ray diffraction (XRD) and reflectivity measurements with MQWs' satellite peaks clearly visible up to the 10th order. Room-temperature intense ultraviolet (UV) photoluminescence (PL) emission with wavelength in the range of 320–350nm was also observed from the MQWs with well width ranging from 1.0 to 1.5nm. These MQW structures can potentially be used for UV light emitters and quantum cascade lasers.

Within the implementation of the European Geo-stationary Navigation Overlay System (EGNOS), a significant residual error in positioning is due to tropospheric delay effects. The EGNOS guidelines recommend that tropospheric delay is modelled using an empirical correction algorithm based on a receiver's height and estimates of meteorological parameters developed from average and seasonal variation data. However, such a simple average and seasonal variation model is unlikely to emulate temporal weather changes exactly. The potential errors involved in the application of the recommended algorithm and the consequent effects on the positioning errors, under typical UK weather conditions, are detailed in this paper. This was achieved by comparing tropospheric delays produced by the EGNOS model, with tropospheric delays estimated from high precision carrier phase GPS, over a one-year period for five UK stations. The RMS EGNOS model zenith tropospheric delay errors ranged from 4·0 to 4·7 cm, with maximum errors ranging from 13·2 to 17·8 cm. The errors were also shown to be spatially correlated. The subsequent effect on position error is shown to be dependent on the satellite elevation cut-off angle adopted and on whether or not the observations are weighted according to the satellite elevation angle.

A coelomycetous genus, Pseudophragmotrichum gen. nov., with the type species P. cubense sp. nov., is described from dead culms of Oncidium lucidum from Cuba. It differs from other coelomycete genera in having intercalary conidiogenous cells, thallic conidiogenesis and flattened conidia.

Three fungicolous hyphomycetes are described and illustrated. Anastomyces gen. nov., with the type species A. microsporus sp. nov., is described on ascomata of Apiospora sp. on dead culms of a grass from China. It differs from comparable genera by its fungicolous habitat, sporodochial conidiomata and polylocal conidiogenous cells mostly with 2 loci. Each locus produces one conidium but two conidia formed from the same conidiogenous cells join together to form
H -shaped conidia in the earliest stages of their development. Idriella rhododendri sp. nov. grows over other microfungi and is described from leaf litter of Rhododendron ponticum from England. Infundibura adhaerens was found to be commonly associated with ascomata and conidiomata on dead leaves of R. ponticum in England.
Cell-cycle-specific transcription termination within the human histone H3.3 gene is correlated with specific protein–DNA interactions

In vitro studies using highly purified calf thymus RNA polymerase II and a fragment spanning the first intron of H3.3 as template DNA have demonstrated the existence of a strong transcription termination site consisting of thymidine stretches. In this study, nuclear run-on experiments have been performed to assess the extent to which transcription elongation is blocked in vivo using DNA probes corresponding to regions 5′ and 3′ of the in vitro termination sites. These studies suggest that H3.3 expression is stimulated following the inhibition of DNA synthesis through the elimination of the transcription elongation block. Interestingly, both the in vivo and in vitro experiments have revealed that the transcriptional block/termination sites are positioned immediately downstream of a 73 bp region that has been over 90% conserved between the chicken and human H3.3 genes. The extreme conservation of this intronic region suggests a possible role in maintaining cis-acting function. Electrophoretic mobility shift experiments show that HeLa cell nuclear extracts contain protein factors that bind specifically to the region of transcription elongation block. Furthermore, we demonstrate a correlation between the protein binding activity and the transcriptional block in cells that have been either arrested at the initiation of S phase or were replication-interrupted by hydroxyurea. DNA footprinting experiments indicate that the region of protein binding is at the 3′ end of the conserved region and overlaps with one of the three in-vitro-mapped termination sites.
