Healthcare worker perception of a global outbreak of novel coronavirus (COVID-19) and personal protective equipment: Survey of a pediatric tertiary-care hospital

Objective:
In this study, we aimed to capture perspectives of healthcare workers (HCWs) on coronavirus disease 2019 (COVID-19) and infection prevention and control (IPAC) measures implemented during the early phase of the COVID-19 pandemic.
Design:A cross-sectional survey of HCWs.
Participants:HCWs from the Hospital for Sick Children, Toronto, Canada.
Intervention:A self-administered survey was distributed to HCWs. We analyzed factors influencing HCW knowledge and self-reported use of personal protective equipment (PPE), concerns about contracting COVID-19 and acceptance of the recommended IPAC precautions for COVID-19.
Results:In total, 175 HCWs completed the survey between March 6 and March 10: 35 staff physicians (20%), 24 residents or fellows (14%), 72 nurses (41%), 14 respiratory therapists (8%), 14 administration staff (8%), and 14 other employees (8%). Most of the respondents were from the emergency department (n = 58, 33%) and the intensive care unit (n = 58, 33%). Only 86 respondents (50%) identified the correct donning order; only 60 (35%) identified the correct doffing order; but the majority (n = 113, 70%) indicated the need to wash their hands immediately prior to removal of their mask and eye protection. Also, 91 (54%) respondents felt comfortable with recommendations for droplet and/or contact precautions for routine care of patients with COVID-19. HCW occupation and concerns about contracting COVID-19 outside work were associated with nonacceptance of the recommendations (P = .016 and P = .036 respectively).
Conclusion:As part of their pandemic response plans, healthcare institutions should have ongoing training for HCWs that focus on appropriate PPE doffing and discussions around modes of transmission of COVID-19.

Background
Childhood maltreatment (CM) plays an important role in the development of major depressive disorder (MDD). The aim of this study was to examine whether CM severity and type are associated with MDD-related brain alterations, and how they interact with sex and age.
MethodsWithin the ENIGMA-MDD network, severity and subtypes of CM using the Childhood Trauma Questionnaire were assessed and structural magnetic resonance imaging data from patients with MDD and healthy controls were analyzed in a mega-analysis comprising a total of 3872 participants aged between 13 and 89 years. Cortical thickness and surface area were extracted at each site using FreeSurfer.
ResultsCM severity was associated with reduced cortical thickness in the banks of the superior temporal sulcus and supramarginal gyrus as well as with reduced surface area of the middle temporal lobe. Participants reporting both childhood neglect and abuse had a lower cortical thickness in the inferior parietal lobe, middle temporal lobe, and precuneus compared to participants not exposed to CM. In males only, regardless of diagnosis, CM severity was associated with higher cortical thickness of the rostral anterior cingulate cortex. Finally, a significant interaction between CM and age in predicting thickness was seen across several prefrontal, temporal, and temporo-parietal regions.
ConclusionsSeverity and type of CM may impact cortical thickness and surface area. Importantly, CM may influence age-dependent brain maturation, particularly in regions related to the default mode network, perception, and theory of mind.
Comparison of Glyphosate Salts (Isopropylamine, Diammonium, and Potassium) and Calcium and Magnesium Concentrations on the Control of Various Weeds

Greenhouse and field experiments were conducted near Knoxville, TN, during 2002 and 2003 to investigate the effects of calcium and magnesium ions on the performance of three glyphosate formulations with and without diammonium sulfate (AMS). Weed species investigated in the greenhouse were broadleaf signalgrass, pitted morningglory, Palmer amaranth, and yellow nutsedge. Three glyphosate formulations (isopropylamine salt, diammonium salt, and potassium salt) and two glyphosate application rates (0.42 and 0.84 kg ae/ha) were applied to weeds in water fortified with either calcium or magnesium at concentrations of 0, 250, 500, 750, and 1,000 ppm. In all comparisons, there were no differences in the three glyphosate formulations. Glyphosate activity was reduced only when cation concentration was >250 ppm, and this antagonism was not observed when 2% w/ w AMS was added to the spray solution. A chemical analysis of the calcium and magnesium concentrations in water collected from farmers indicated that water samples from eight different producers contained relatively low amounts of cations, with calcium at <40 ppm and magnesium at <8 ppm. In the field results using these and other waters as the herbicide carrier, broadleaf signalgrass control was greater with the 0.84 kg ae/ha than 0.42 kg ae/ha glyphosate rate regardless of water source or addition of AMS. Pitted morningglory responded similarly to glyphosate with water from all farms and with AMS added, and the addition of AMS gave similar results for both glyphosate rates. In 2003, common cocklebur was evaluated and control was >93% regardless of glyphosate rate, water source, or AMS addition. Based on these results, the addition of AMS-based adjuvants to many glyphosate applications may not be warranted.
Soybean Tolerance to Early Preplant Applications of 2,4-D Ester, 2,4-D Amine, and Dicamba

Preplant applications of two 2,4-D formulations and dicamba were evaluated for injury to no-till soybean. Herbicides applied 28 or 21 d before planting (DBP) at 0.56 kg ae/ha for the 2,4-D formulations and 0.28 kg ae/ha for dicamba did not injure soybean. At 14 DBP soybean injury was lower (< 6%) with applications of 2,4-D than with dicamba (13% to 17%). Injury increased when herbicides were applied at 7 DBP with soybean injury from both 2,4-D (< 11%) and dicamba (< 38%). When the herbicides were applied at planting soybean injury was < 18% with the 2,4-D formulations and < 73% with dicamba. Soybean yields were unaffected by the injury from herbicides applied 21, 14, and 7 DBP. However, soybean yield was decreased to 870 kg/ha in 2005 when dicamba was applied at planting compared to the nontreated check (1,950 kg/ha)

Postpericardiotomy syndrome: no evidence for a viral etiology

Background: Postpericardiotomy syndrome has been considered a disorder induced by viral infection. This conclusion is based on serologic criterions, but these may be unreliable following either cardiopulmonary bypass or transfusion therapy. Previous studies have not verified the proposed etiology either by isolation of viruses, or by detection of their genome. We sought, therefore, to clarify the role, if any, of viruses in this syndrome. Methods and Results: We studied prospectively 149 children aged from 6 months to 16 years who were undergoing open heart surgery. Blood samples were collected from all prior to operation, and again 7 to 10 days post-operatively, and 47 were sampled at the time of development of symptoms of pericardial involvement. Serums were analyzed for the presence of IgM and IgG antibodies to cytomegalovirus, herpes simplex virus, and Epstein-Barr virus. The polymerase chain reaction was used for amplification when assessing the genome of the enteroviruses. Cultures for viruses were established on samples of stool, urine, and throat swabs collected 7 days post-operatively, and at the time of postpericardial symptoms. Pericardial fluid obtained from 5 patients with the syndrome was cultured for viruses, and tested for enterovirus genome. On the basis of clinical and echocardiographic findings, 34 children were determined to have definite evidence of the syndrome, 13 were considered to have possible evidence, and the results from these patients were compared to those from patients with no pericardial symptoms, the latter being matched for age and transfusion status. We isolated viruses from one or more sites in five patients with definite evidence (16%), from one (9%) of those with possible evidence, and from seven (19%) of the controls. All serums and pericardial samples were negative for enterovirus genome. IgM antibodies were found in only 5 patients, three with symptoms of pericardial involvement and two without. Rates of seroconversion to IgG for the viruses were lower in the patients with symptoms of pericardial involvement compared to controls, but were strongly influenced by transfusion status. Conclusion: Our study has provided no evidence to support a viral etiology for the postpericardiotomy syndrome.

Summary
Introduction
Defects in hemoglobin synthesis and erythroid nuclear maturation both give rise to disorders of erythrocyte production. Defects of hemoglobin synthesis can result from impaired heme synthesis, as seen in iron deficiency and sideroblastic anemia, or from impaired globin chain synthesis, which occurs in the thalassemia syndromes. Defects in erythroid nuclear maturation are seen in cobalamin (vitamin B12) and folate deficiency and also in the rare hereditary abnormalities of cobalamin or folate metabolism, and result in defective erythrocyte production. Abnormal nuclear maturation is also present in other metabolic defects associated with megaloblastic anemia, such as Lesch–Nyhan syndrome, orotic aciduria, and thiamine-responsive megaloblastic anemia. In the anemia of chronic disorder, iron regulation is maladjusted so that iron is “locked away” and unavailable for erythropoiesis. Markedly ineffective erythropoiesis is seen where there are defects of nuclear maturation, in sideroblastic anemia, and in the thalassemia syndromes; iron deficiency and the anemia of chronic disorder are associated with decreased erythropoiesis. Of the nutritional deficiencies causing anemia, the commonest is iron deficiency. This is so for both children and adults in all parts of the world. The deficiency may be caused by insufficient iron in the diet, lack of absorption of iron, or increased losses primarily due to bleeding. Deficiency of vitamin B12 is rare in children; in neonates it is usually secondary to occult vitamin B12 deficiency in the mother, but when diagnosed in infancy or childhood is more often due to an inborn error of metabolism.

We investigated linkage between schizophrenia and the loci DXYS14, DXYS17, and MIC2 within the pseudoautosomal region in 85 families with two or more siblings suffering from schizophrenia or schizoaffective disorder. A maximum lod score of 2.44 was reached at MIC2, with a dominant model of inheritance at a recombination fraction of 0.367 in females and 0.046 in males (a F: M sex ratio > 1, i.e. opposite to that expected with a pseudoautosomal locus). Evidence consistent with linkage (P = 0.01) was also obtained with a sibling pair analysis at the MIC2 locus. These data do not support (although they do not definitively exclude) a locus within the pseudoautosomal region; they are consistent with the presence of a gene that predisposes to schizophrenia in the sex-specific regions of the X and Y chromosomes.
