Device-measured sedentary behaviour and anxiety symptoms during adolescence: a 6-year prospective cohort study

Background
Sedentary behaviour is potentially a modifiable risk factor for anxiety disorders, a major source of global disability that typically starts during adolescence. This is the first prospective study of associations between repeated, device-based measures of sedentary behaviour and anxiety symptoms in adolescents.
MethodsA UK cohort with 4257 adolescents aged 12 at baseline (56% female). Main exposures were sedentary behaviour and physical activity measured using accelerometers for 7-days at ages 12, 14, and 16. Primary outcome was anxiety symptom scores at age 18 from a Clinical Interview Schedule-Revised. We used adjusted negative binomial regression and iso-temporal substitution methods to analyse the data.
ResultsWe found a positive association between sedentary behaviour at ages 12, 14, and 16, with anxiety symptoms at age 18, independent of total physical activity volume. Theoretically replacing an hour of daily sedentary behaviour for light activity at ages 12, 14, and 16, was associated with lower anxiety symptoms by age 18 by 15.9% (95% CI 8.7–22.4), 12.1% (95% CI 3.4–20.1), and 14.7% (95% CI 4–24.2), respectively. Whereas, theoretically replacing an hour of sedentary behaviour with moderate-to-vigorous physical activity was not associated with differences in anxiety symptoms. These results were robust to a series of sensitivity analyses.
ConclusionSedentary behaviour is a possible risk factor for increasing anxiety symptoms during adolescence, independent of total physical activity volume. Instead of focusing on moderate-to-vigorous activity, replacing daily sedentary behaviour with light activity during adolescence could be a more suitable method of reducing future anxiety symptoms.

Holocene glacial advances in the Banff–Jasper–Yoho area of the Canadian Rocky Mountains have been extremely limited in extent. Limiting 14C dates from two sites within 1 km of contemporary glaciers of fresh terminal moraines indicate that the late Wisconsin Ice Sheet and valley glaciers disappeared prior to 9660 yr B.P. Two subsequent glacial advances are recognized. The earlier Crowfoot Advance is represented by moraines and rock-glacier deposits overlain by Mazama ash (6600 yr B.P.) and is therefore early Holocene or possibly late Wisconsin in age. The late Neoglacial Cavell Advance of the last few centuries is dated by dendrochronology and lichenometry. In addition, there is fragmentary, undated evidence of intermediate-age advance(s), mainly from rock-glacier deposits. All these advances were of limited extent (1–2 km beyond present ice margins) and the Cavell Advance was usually the most extensive. Major exceptions to this pattern occur only where rock glaciers or extensive ice-cored moraines developed during the earlier advance(s?). These deposits were not overrun by glaciers during the Cavell Advance because of their relatively greater downvalley extent and the physical barrier they presented to subsequent glacial advances. Earlier work which postulated more extensive early Holocene advances in the Canadian Rocky Mountains is shown to have inadequate dating control: Many of the features previously attributed to older Holocene events are late Wisconsin in age.

In traditional transit timing variations (TTVs) analysis of multi-planetary systems, the individual TTVs are first derived from transit fitting and later modelled using n-body dynamic simulations to constrain planetary masses. We show that fitting simultaneously the transit light curves with the system dynamics (photo-dynamical model) increases the precision of the TTV measurements and helps constrain the system architecture. We exemplify the advantages of applying this photo-dynamical model to a multi-planetary system found in K2 data very close to 3:2 mean motion resonance, K2-19. In this case the period of the larger TTV variations (libration period) is much longer (>1.5 years) than the duration of the K2 observations (80 days). However, our method allows to detect the short period TTVs produced by the orbital conjunctions between the planets that in turn permits to uniquely characterise the system. Therefore, our method can be used to constrain the masses of near-resonant systems even when the full libration curve is not observed.

Background
Key questions regarding residential alternatives to standard acute psychiatric care, such as crisis houses and short-stay in-patient units, concern the role that they fulfil within local acute care systems, and whether they manage people with needs and illnesses of comparable severity to those admitted to standard acute wards.
AimsTo study the extent to which people admitted to residential alternatives and to standard acute services are similar, and the role within local acute care systems of admission to an alternative service.
MethodOur approach combined quantitative and qualitative methods. Consecutive cohorts of patients in six residential alternatives across England and six standard acute wards in the same areas were identified, and clinical and demographic characteristics, severity of symptoms, impairments and risks compared. Semi-structured interviews with key stakeholders in each local service system were used to explore the role and functioning of each alternative.
ResultsBeing already known to services (OR = 2.6, 95% CI 1.3–5.2), posing a lower risk to others (OR = 0.49, 95% CI 0.31–0.78) and having initiated help-seeking in the current crisis (OR = 2.2, 95% CI 1.2–4.3) were associated with being admitted to an alternative rather than a standard service. Stakeholder interviews suggested that alternatives have a role that is similar but not identical to standard hospital services. They can divert some, but not all, patients from acute admission.
ConclusionsResidential alternatives are integrated into catchment area mental health systems. They serve similar, but not identical, clinical populations to standard acute wards and provide some, but not all, of the functions of these wards.

Background
Outcomes following admission to residential alternatives to standard in-patient mental health services are underresearched.
AimsTo explore short-term outcomes and costs of admission to alternative and standard services.
MethodHealth of the Nation Outcome Scales (HoNOS), Threshold Assessment Grid (TAG), Global Assessment of Functioning (GAF) and admission cost data were collected for six alternative services and six standard services.
ResultsAll outcomes improved during admission for both types of service (n = 433). Adjusted improvement was greater for standard services in scores on HoNOS (difference 1.99, 95% CI 1.12–2.86), TAG (difference 1.40, 95% CI 0.39–2.51) and GAF functioning (difference 4.15, 95% CI 1.08–7.22) but not GAF symptoms. Admissions to alternatives were 20.6 days shorter, and hence cheaper (UK£3832 v. £9850). Standard services cost an additional £2939 per unit HoNOS improvement.
ConclusionsThe absence of clear-cut advantage for either type of service highlights the importance of the subjective experience and longer-term costs.

Background
Differences in the content of care provided by acute in-patient mental health wards and residential crisis services such as crisis houses have not been researched.
AimsTo compare planned and actual care provided at alternative and standard acute wards and to investigate the relationship between care received and patient satisfaction.
MethodPerspectives of stakeholders, including local service managers, clinicians and commissioners, were obtained from 23 qualitative interviews. Quantitative investigation of the care provided at four alternative and four standard services was undertaken using three instruments developed for this study. The relationship of care received to patient satisfaction was explored.
ResultsNo significant difference was found in intensity of staff– patient contact between alternative and standard services. Alternative services provided more psychological and less physical and pharmacological care than standard wards. Care provision may be more collaborative and informal in alternative services. All measured types of care were positively associated with patient satisfaction. Measured differences in the care provided did not explain the greater acceptability of community alternatives.
ConclusionsSimilarities in care may be more marked than differences at alternative and standard services. Staff–patient contact is an important determinant of patient satisfaction, so increasing it should be a priority for all acute in-patient services.

Ergots are the sclerotia of Claviceps purpurea (Fr.) Tul., which periodically infects certain cereals especially wheat and rye, and frequently infects numerous grasses (Mantle, 1977a). The ergots are produced in the seeding heads of the plant and are harvested with, and thus contaminate, the grain, causing the mycotoxicosis known as ‘ergotism’ in livestock fed contaminated cereals, or in humans eating bread made from contaminated flour (Mantle, 19776). The disease is caused by certain alkaloid constituents of the ergots which are derived from lysergic acid. Symptoms in humans are hallucinations, convulsions, gangrene of the limbs and other extremities, and eventually death. Seven isomeric pairs of indole alkaloids have been isolated from ergot, ergocristine and ergocristinine, ergotamine and ergotaminine, ergocryptine and ergocryptinine, ergocornine and ergocorninine, ergosine and ergosinine, ergostine and ergostinine, and ergometrine and ergometrinine. The first named of each pair is derived from lysergic acid and is physiologically active, while the second is derived from isolysergic acid and has little physiological activity.

Summary
SUMMARY
The influence of water status on photosynthesis and productivity of Gunnera tinctoria (Molina) Mirbel. was examined under natural conditions in an experimental garden in Dublin under two contrasting watering regimes. Application of water in amounts that simulated the rainfall conditions of the west of Ireland produced large increases in productivity although there were only small effects on instantaneous rates of net photosynthesis. The lack of a correlation between photosynthetic rate and biomass accumulation is considered. Stomatal conductance was often highest in the morning and subsequently declined throughout the day, irrespective of watering regimes, indicating possible limitations in water transport from the root to the shoot even when plants were supplied with additional water. The significance of these results for a plant restricted to wet, humid environments is discussed.
INTRODUCTION
The recent habitats occupied by the genus Gunnera L. are characterised by high rainfall, high humidity conditions in which low temperatures are rare (Osborne et al., 1991; Palkovic, 1974). The global distribution of the genus is also thought to have been more extensive in the past but has decreased dramatically due to a reduction in annual levels of precipitation and an increase in seasonal temperature variation (Osborne et al., 1991; Jarzen, 1980).
Gunnera tinctoria (Molina) Mirbel was introduced into Ireland about 100 years ago as an ornamental shrub and is now naturalised in westernmost regions of the country (Campbell & Osborne, 1990) in areas of high annual rainfall (> 1600 mm) and high humidity not unlike its tropical and sub-tropical habitats where rainfall approaches 2000 mm per year (Müller, 1982; Osborne, 1989).

The response of patients with major depressive illness to citalopram of amitriptyline was compared in a double-blind multi-centre trial. No differences in efficacy were observed, but citalopram had less hypnotic effect and a remarkably lower profile of side-effects.
