Coronavirus disease 2019 (COVID-19) outbreak on an in-patient medical unit associated with unrecognized exposures in common areas—Epidemiological and whole-genome sequencing investigation

Objective:
Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) hospital outbreaks have been common and devastating during the coronavirus disease 2019 (COVID-19) pandemic. Understanding SARS-CoV-2 transmission in these environments is critical for preventing and managing outbreaks.
Design:Outbreak investigation through epidemiological mapping and whole-genome sequencing phylogeny.
Setting:Hospital in-patient medical unit outbreak in Toronto, Canada, from November 2020 to January 2021.
Participants:The outbreak involved 8 patients and 10 staff and was associated with 3 patient deaths.
Results:Patients being cared for in geriatric chairs at the nursing station were at high risk for both acquiring and transmitting SARS-CoV-2 to other patients and staff. Furthermore, given the informal nature of these transmissions, they were not initially recognized, which led to further transmission and missing the opportunity for preventative COVID-19 therapies.
Conclusions:During outbreak prevention and management, the risk of informal patient care settings, such as geriatric chairs, should be considered. During high-risk periods or during outbreaks, efforts should be made to care for patients in their rooms when possible.

Background:
Spinal muscular atrophy (SMA) is a devastating rare disease that affects individuals regardless of ethnicity, gender, and age. The first-approved disease-modifying therapy for SMA, nusinursen, was approved by Health Canada, as well as by American and European regulatory agencies following positive clinical trial outcomes. The trials were conducted in a narrow pediatric population defined by age, severity, and genotype. Broad approval of therapy necessitates close follow-up of potential rare adverse events and effectiveness in the larger real-world population.
Methods:The Canadian Neuromuscular Disease Registry (CNDR) undertook an iterative multi-stakeholder process to expand the existing SMA dataset to capture items relevant to patient outcomes in a post-marketing environment. The CNDR SMA expanded registry is a longitudinal, prospective, observational study of patients with SMA in Canada designed to evaluate the safety and effectiveness of novel therapies and provide practical information unattainable in trials.
Results:The consensus expanded dataset includes items that address therapy effectiveness and safety and is collected in a multicenter, prospective, observational study, including SMA patients regardless of therapeutic status. The expanded dataset is aligned with global datasets to facilitate collaboration. Additionally, consensus dataset development aimed to standardize appropriate outcome measures across the network and broader Canadian community. Prospective outcome studies, data use, and analyses are independent of the funding partner.
Conclusion:Prospective outcome data collected will provide results on safety and effectiveness in a post-therapy approval era. These data are essential to inform improvements in care and access to therapy for all SMA patients.

Space Infrared Telescope for Cosmology and Astrophysics (SPICA), the cryogenic infrared space telescope recently pre-selected for a ‘Phase A’ concept study as one of the three remaining candidates for European Space Agency (ESA's) fifth medium class (M5) mission, is foreseen to include a far-infrared polarimetric imager [SPICA-POL, now called B-fields with BOlometers and Polarizers (B-BOP)], which would offer a unique opportunity to resolve major issues in our understanding of the nearby, cold magnetised Universe. This paper presents an overview of the main science drivers for B-BOP, including high dynamic range polarimetric imaging of the cold interstellar medium (ISM) in both our Milky Way and nearby galaxies. Thanks to a cooled telescope, B-BOP will deliver wide-field 100–350 $\mu$m images of linearly polarised dust emission in Stokes Q and U with a resolution, signal-to-noise ratio, and both intensity and spatial dynamic ranges comparable to those achieved by Herschel images of the cold ISM in total intensity (Stokes I). The B-BOP 200 $\mu$m images will also have a factor $\sim $30 higher resolution than Planck polarisation data. This will make B-BOP a unique tool for characterising the statistical properties of the magnetised ISM and probing the role of magnetic fields in the formation and evolution of the interstellar web of dusty molecular filaments giving birth to most stars in our Galaxy. B-BOP will also be a powerful instrument for studying the magnetism of nearby galaxies and testing Galactic dynamo models, constraining the physics of dust grain alignment, informing the problem of the interaction of cosmic rays with molecular clouds, tracing magnetic fields in the inner layers of protoplanetary disks, and monitoring accretion bursts in embedded protostars.

The SPICA mid- and far-infrared telescope will address fundamental issues in our understanding of star formation and ISM physics in galaxies. A particular hallmark of SPICA is the outstanding sensitivity enabled by the cold telescope, optimised detectors, and wide instantaneous bandwidth throughout the mid- and far-infrared. The spectroscopic, imaging, and polarimetric observations that SPICA will be able to collect will help in clarifying the complex physical mechanisms which underlie the baryon cycle of galaxies. In particular, (i) the access to a large suite of atomic and ionic fine-structure lines for large samples of galaxies will shed light on the origin of the observed spread in star-formation rates within and between galaxies, (ii) observations of HD rotational lines (out to ~10 Mpc) and fine structure lines such as [C ii] 158 μm (out to ~100 Mpc) will clarify the main reservoirs of interstellar matter in galaxies, including phases where CO does not emit, (iii) far-infrared spectroscopy of dust and ice features will address uncertainties in the mass and composition of dust in galaxies, and the contributions of supernovae to the interstellar dust budget will be quantified by photometry and monitoring of supernova remnants in nearby galaxies, (iv) observations of far-infrared cooling lines such as [O i] 63 μm from star-forming molecular clouds in our Galaxy will evaluate the importance of shocks to dissipate turbulent energy. The paper concludes with requirements for the telescope and instruments, and recommendations for the observing strategy.

BACKGROUND
Whether contact precautions (CP) are required to control the endemic transmission of methicillin-resistant Staphylococcus aureus (MRSA) or vancomycin-resistant Enterococcus (VRE) in acute care hospitals is controversial in light of improvements in hand hygiene, MRSA decolonization, environmental cleaning and disinfection, fomite elimination, and chlorhexidine bathing.
OBJECTIVETo provide a framework for decision making around use of CP for endemic MRSA and VRE based on a summary of evidence related to use of CP, including impact on patients and patient care processes, and current practices in use of CP for MRSA and VRE in US hospitals.
DESIGNA literature review, a survey of Society for Healthcare Epidemiology of America Research Network members on use of CP, and a detailed examination of the experience of a convenience sample of hospitals not using CP for MRSA or VRE.
PARTICIPANTSHospital epidemiologists and infection prevention experts.
RESULTSNo high quality data support or reject use of CP for endemic MRSA or VRE. Our survey found more than 90% of responding hospitals currently use CP for MRSA and VRE, but approximately 60% are interested in using CP in a different manner. More than 30 US hospitals do not use CP for control of endemic MRSA or VRE.
CONCLUSIONSHigher quality research on the benefits and harms of CP in the control of endemic MRSA and VRE is needed. Until more definitive data are available, the use of CP for endemic MRSA or VRE in acute care hospitals should be guided by local needs and resources.

There are standard modifications of certain compactly supported wavelets that yield orthonormal bases on a bounded interval. We extend one such construction to those wavelets, such as ‘coiflets', that may have fewer vanishing moments than had to be assumed previously. Our motivation lies in function estimation in statistics. We use these boundary-modified coiflets to show that the discrete wavelet transform of finite data from sampled regression models asymptotically provides a close approximation to the wavelet transform of the continuous Gaussian white noise model. In particular, estimation errors in the discrete setting of computational practice need not be essentially larger than those expected in the continuous setting of statistical theory.

Wide area sub-millimeter mapping of nearby molecular clouds allows for the study of large scale structures such as the Integral Shaped Filament in the Orion A cloud. Examination of these regions suggests that they are not equilibrium isothermal structures but rather require significant, and radially dependent, non-thermal support such as produced by helical magnetic fields Also observed in the large area maps are dense condensations with masses typical for stars. The mass distribution of these clumps is similar to the stellar initial mass function; however, the clumps appear stable against collapse. The clumps are clustered within the cores of molecular clouds and restricted to those locations where the molecular cloud column density is high (AV > 4). As well, the typical sub-millimeter clump reveals little or no emission from isotopes of CO, likely indicating that the combination of high density and low temperatures within the clumps provides an environment in which these molecules freeze-out onto dust grain surfaces.
The bat fauna of Lamanai, Belize: roosts and trophic roles

Thirty-six of the 70 species of bats known from Belize were recorded from the area around Lamanai, Orange Walk County: two in roosts and 34 in about 680 mist net hours that produced 560 captures. Day roosts used by 35 of the species were located using radio-tracking (Sturnira lilium, Platyrrhinus helleri, Centurio senex and Bauerus dubiaquercus) or general searching for roosts (Rhynchonycteris naso, Saccopteryx bilineata, Saccopteryx leptura, Dicli durus albus, Mimon bennettii, Micronycteris schmidtorum, Carollia brevicauda, Carollia perspicillata and Eptesicus furinalis). Data on the day roosts of 23 other species were determined from the literature. Most species reported from Lamanai (19) roosted in hollows, while others used foliage (6), tents (3), sheltered sites (2), crevices (2), open sites (1), and a few species used more than one type of day roost (hollows and crevices (1); hollows and foliage (1); hollows, foliage and tents (1)). The fauna consisted of 13 aerial foragers, 9 gleaners, 11 fruit/leaf eaters, one trawler, one flower-visitor and one blood-feeder. In day roost use and foraging behaviour, the Lamanai fauna did not differ significantly from that of Paracou, French Guiana, but both these locations differed from the bat fauna of Kruger National Park, South Africa, in foraging behaviour.

We prove that the relation type of all high powers of an ideal in a Noetherian ring is either one or two. It is one exactly when some power of the ideal is locally principal.

Let (R, m) be a 2-dimensional regular local ring and I an m-primary ideal. The aim of this paper is to find conditions on I so that the associated graded ring of I,
and the Rees ring of I,
where t is an indeterminate, are Cohen–Macaulay (resp. Gorenstein). To this end, we use the results and techniques from Zariski's theory of complete ideals ([14], appendix 5) and its later generalizations and refinements due to Huneke [7] and Lipman[8]. The main result is an application of three deep theorems: (i) a generalization of Macaulay's classical theorem on Hilbert series of Gorenstein graded rings [13], (ii) a generalization of the Briançon–Skoda theorem due to Lipman and Sathaye [9], and (iii) a formula for the length of R/I, where I is a complete m-primary ideal, due to Hoskin[4] and Deligne[1].
