Infant and young child feeding patterns in Kuwait: results of a cross-sectional survey

Objective
The beneficial role of breast-feeding for maternal and child health is now well established. Its possible role in helping to prevent diabetes and obesity in children in later life means that more attention must be given to understanding how patterns of infant feeding are changing. The present study describes breast-feeding profiles and associated factors in Kuwait.
Design/Setting/SubjectsInterviews with 1484 recent mothers were undertaken at immunisation clinics across Kuwait. Descriptive analysis and binary logistic regression of results were performed.
ResultsRates of breast-feeding initiation in Kuwait were high (98·1 %) but by the time of discharge from hospital, only 36·5 % of mothers were fully breast-feeding, 37·0 % were partially breast-feeding and 26·5 % were already fully formula-feeding. Multiple social and health reasons were given for weaning the child, with 87·6 % of mothers who had stopped breast-feeding completely doing so within 3 months postpartum. Nationality (P<0·001), employment status 6 months prior to delivery (P<0·001), mode of delivery (P=0·01), sex of the child (P=0·026) and breast-feeding information given by nurses (P=0·026) were all found to be significantly associated with breast-feeding. Few women (5·6 %) got information on infant nutrition and feeding from nursing staff, but those who did were 2·54 times more likely to be still breast-feeding at discharge from hospital. Over 70 % of mothers had enjoyed breast-feeding and 74 % said they would be very likely to breast-feed again.
ConclusionsIn Kuwait where the prevalence of both obesity and type 2 diabetes is growing rapidly, the public health role of breast-feeding must be recognised and acted upon more than it has in the past.

Fish oil supplementation during pregnancy has been associated with lower levels of cord blood IL-13, suggesting that the administration of n-3 fatty acids may attenuate the development of allergic disease. The present study aimed to investigate the mechanism by which n-3 fatty acid administration influences the production of IL-13. Pregnant BALB/c mice were fed nutritionally complete high-fat diets (15 %, w/w) with an n-3 fatty acid-enriched (DHA 1 %, w/w) or control diet (0 % DHA) immediately following delivery. Pups were exposed during suckling and weaned to the maternal diet for the remainder of the study. The production of IL-13, IL-4, IL-10 and interferon-γ from the splenocytes of ovalbumin (ova)-sensitised animals was assessed following in vitro ova stimulation or unstimulated conditions. Human T helper type 2 (Th2) cells were mitogen-stimulated in the presence or absence of DHA (10 μm) and assessed for IL-13 and IL-4 expression using intracellular flow cytometry. The influence on transcriptional activation was studied using a human IL-13 promoter reporter construct and electromobility shift assay. Ova-activated splenocytes from DHA-fed mice produced less IL-13 (57·2 (se 21·7) pg/ml) and IL-4 (7·33 (se 3·4) pg/ml) compared with cells from the animals fed the control diet (161·5 (se 45·0), P< 0·05; 33·2 (se 11·8), P< 0·05). In vitro, DHA inhibited the expression of IL-13 protein from human Th2 cells as well as transcriptional activation and binding of the transcription factors cyclic AMP response element binding and activating transcription factor 2 to the human IL-13 promoter. These data indicate the potential of n-3 fatty acids to attenuate IL-13 expression, and suggest that they may subsequently reduce allergic sensitisation and the development of allergic disease.

Getting Out and Getting On: Scottish Highland Migration in the First Half of the Twentieth Century1

This paper draws on data collected for the author's doctoral thesis (Maclean, 1997), which examined migration and social change in remote rural areas. The research took place mainly in 1995–6, and consisted of tape-recorded interviews and 18 months' ethnographic fieldwork in a parish in the Scottish Highlands, and the use of data including the Census Small Area Statistics and Register of Sasines. Although informed by the research as a whole, the central focus of this paper is data drawn from twenty interviews with members of fifteen families, most of whom were born in the first three decades of the twentieth century. A further ten people provided supplementary data in interviews in which family migration patterns were not the main focus (see table 1 for details). Places and people involved in the research have been given pseudonyms to protect anonymity, privacy and confidentiality, in accordance with the British Sociological Association's Ethical Guidelines.

The paper explores whether key oil and gas companies are expressing awareness of the work–family debate and reports findings from an ongoing three-year study, funded by the ESRC, which provides an account of the work-family interface in relation to the oil and gas industry. Although this paper is informed by our research as a whole, it draws on a specific subset of our data, namely in-depth interviews with human resource personnel in eighteen oil and gas companies (see Table 1). Companies determined who should be interviewed and this varied depending on size, internal functional design (e.g. geographical location of senior staff), and the extent to which the research was seen to be of value. In eight companies, the interviews were conducted with the most senior human resources manager, while in others the personnel interviewed had titles such as ‘employee relations manager’ or ‘support services manager’. American, Canadian, Italian, French, Norwegian and British-owned firms are included. Interviews lasted between one and one-and-a-half hours.
