The Richtmyer–Meshkov instability (Richtmyer, Commun. Pure Appl. Maths, vol. 13, issue 2, 1960, pp. 297–319; Meshkov, Fluid Dyn., vol. 4, issue 5, 1972, pp. 101–104) of a twice-shocked gas interface is studied using both high spatial resolution single-shot (SS) and lower spatial resolution, time-resolved, high-speed (HS) simultaneous planar laser-induced fluorescence and particle image velocimetry in the Wisconsin Shock Tube Laboratory's vertical shock tube. The initial condition (IC) is a shear layer with broadband diffuse perturbations at the interface between a helium–acetone mixture and argon. This IC is accelerated by a shock of nominal strength Mach number $M = 1.75$, and then accelerated again by the transmitted shock that reflects off the end wall of the tube. An ensemble of experiments is analysed after reshock while the interface mixing width grows linearly with time. The kinetic and scalar energy spectra and the terms of their evolution equation are calculated and compared between SS and HS experiments. The inertial range scaling of the scalar power spectrum is found to follow Gibson's relation (Gibson, Phys. Fluids, vol. 11, issue 11, 1968, pp. 2316–2327) as a function of Schmidt number when the effective turbulent Schmidt number is used in place of the material Schmidt number that controls equilibrium scaling. Further, the spatially integrated scalar flux follows similar behaviour observed for the kinetic energy in large eddy simulation studies by Zeng et al. (Phys. Fluids, vol. 30, issue 6, 2018, 064106) while the spatially varying scalar flux exhibits back scatter along the centre of the mixing layer and forward energy transfer in the spike and bubble regions.

Objective:
To evaluate the National Health Safety Network (NHSN) hospital-onset Clostridioides difficile infection (HO-CDI) standardized infection ratio (SIR) risk adjustment for general acute-care hospitals with large numbers of intensive care unit (ICU), oncology unit, and hematopoietic cell transplant (HCT) patients.
Design:Retrospective cohort study.
Setting:Eight tertiary-care referral general hospitals in California.
Methods:We used FY 2016 data and the published 2015 rebaseline NHSN HO-CDI SIR. We compared facility-wide inpatient HO-CDI events and SIRs, with and without ICU data, oncology and/or HCT unit data, and ICU bed adjustment.
Results:For these hospitals, the median unmodified HO-CDI SIR was 1.24 (interquartile range [IQR], 1.15–1.34); 7 hospitals qualified for the highest ICU bed adjustment; 1 hospital received the second highest ICU bed adjustment; and all had oncology-HCT units with no additional adjustment per the NHSN. Removal of ICU data and the ICU bed adjustment decreased HO-CDI events (median, −25%; IQR, −20% to −29%) but increased the SIR at all hospitals (median, 104%; IQR, 90%–105%). Removal of oncology-HCT unit data decreased HO-CDI events (median, −15%; IQR, −14% to −21%) and decreased the SIR at all hospitals (median, −8%; IQR, −4% to −11%).
Conclusions:For tertiary-care referral hospitals with specialized ICUs and a large number of ICU beds, the ICU bed adjustor functions as a global adjustment in the SIR calculation, accounting for the increased complexity of patients in ICUs and non-ICUs at these facilities. However, the SIR decrease with removal of oncology and HCT unit data, even with the ICU bed adjustment, suggests that an additional adjustment should be considered for oncology and HCT units within general hospitals, perhaps similar to what is done for ICU beds in the current SIR.

Previous research examining selection to legislative committees has assumed that the impact of constituency preferences on committee assignments is due to the incentives for individual legislators to use their committee seats to increase their personal chances of re-election. Examining the case of the Mexican Chamber of Deputies (where legislators were, until recently, barred from re-election), this study argues that the impact of constituency preferences on selection to committees also occurs because parties have incentives for their members to use committee assignments to increase the party’s chances of being re-elected. Analysis of assignments to 11 committees over 4 legislative terms provides support for the argument. These findings reinforce previous research arguing that concerns with constituency representation and its impact on re-election also apply to political parties and not solely to individual legislators.

Objective:
We collected dietary records over the course of nine months to comprehensively characterize the consumption patterns of Malagasy people living in remote rainforest areas of north-eastern Madagascar.
Design:The present study was a prospective longitudinal cohort study to estimate dietary diversity and nutrient intake for a suite of macronutrients, micronutrients and vitamins for 152 randomly selected households in two communities.
Setting:Madagascar, with over 25 million people living in an area the size of France, faces a multitude of nutritional challenges. Micronutrient-poor staples, especially rice, roots and tubers, comprise nearly 80 % of the Malagasy diet by weight. The remaining dietary components (including wild foods and animal-source foods) are critical for nutrition. We focus our study in north-eastern Madagascar, characterized by access to rainforest, rice paddies and local agriculture.
Participants:We enrolled men, women and children of both sexes and all ages in a randomized sample of households in two communities.
Results:Although the Household Dietary Diversity Score and Food Consumption Score reflect high dietary diversity, the Minimum Dietary Diversity–Women indicator suggests poor micronutrient adequacy. The food intake data confirm a mixed nutritional picture. We found that the median individual consumed less than 50 % of his/her age/sex-specific Estimated Average Requirement (EAR) for vitamins A, B12, D and E, and Ca, and less than 100 % of his/her EAR for energy, riboflavin, folate and Na.
Conclusions:Malnutrition in remote communities of north-eastern Madagascar is pervasive and multidimensional, indicating an urgent need for comprehensive public health and development interventions focused on providing nutritional security.

Camera trap surveys are a non-invasive way to monitor wildlife populations. Although most often used to study medium- and large-sized mammals, camera traps also detect non-target species. These detections provide useful ecological information on little-known species, but such data usually remain unanalysed. We used detections from camera-trapping surveys of native carnivores and small mammals to examine distribution patterns and occupancy trends of little-known ground-dwelling rainforest birds at seven sites across the Masoala-Makira protected area complex in north-eastern Madagascar. We obtained 4,083 detections of 28 bird species over 18,056 trap nights from 200 to 2013. We estimated occupancy across the Masoala-Makira protected area complex (hereafter, landscape occupancy) and annual trends in occupancy at three resurveyed sites for five commonly observed species. Landscape occupancy across Masoala-Makira ranged from 0.75 (SE 0.09; Madagascar Magpie-robin Copsychus albospecularis) to 0.25 (SE 0.06; Scaly Ground-roller Geobiastes squamiger). Ground-dwelling forest bird occupancy was similar at forest sites that ranged from intact to fully degraded; however, three species were detected less often at sites with high feral cat trap success. Nearly half of all focal species showed declines in annual occupancy probability at one resurveyed site (S02) from 2008 to 2013. The declines in ground-dwelling bird occupancy could have community-wide consequences as birds provide ecosystem services such as seed dispersal and pest regulation. We suggest immediate conservation measures—such as feral cat removal—be implemented to protect ground-dwelling forest birds and other threatened taxa across this landscape.

Most studies examining the relationship between social cleavages and party system fragmentation maintain that higher levels of social diversity lead to greater party system fragmentation. However, most aggregate-level studies focus on one type of social cleavage: ethnic diversity. In order to develop a better understanding of how different cleavages impact electoral competition, this article considers another type of social cleavage: religious diversity. Contrary to previous literature, higher levels of religious diversity provide incentives for cross-religious cooperation, which in turn reduces party system fragmentation. Using a cross-national data set of elections from 1946–2011, the results show that, in contrast to most studies examining the effects of social cleavage diversity on the number of parties, higher religious diversity is associated with lower levels of party system fragmentation.

Increasingly ‘Irrational’ Party Systems: Evidence that There Are Increasingly More Than Two Parties at the District Level

Recent work has noted an increase in the number of parties at the national level in both proportional and majoritarian electoral systems. While the conventional wisdom maintains that the incentives provided by the electoral system will prevent the number of parties at the district level from exceeding two in majoritarian systems, the evidence presented here demonstrates otherwise. I argue that this has occurred because the number of cleavages articulated by parties has increased as several third parties have begun articulating cleavages that are not well represented by the two larger parties.

Aims: Bosentan is efficacious in idiopathic pulmonary arterial hypertension, and the variants associated with connective tissue disease, but not currently approved for treatment of pulmonary arterial hypertension due to Eisenmenger's syndrome. We sought to evaluate its effect in adults with Eisenmenger's syndrome. Methods: We administered bosentan on the basis of compassionate use in 23 patients with Eisenmenger's syndrome, aged 37 plus or minus 14 years. Of the patients, 17 had never received specific treatment for pulmonary arterial hypertension, five were transitioned from treprostinil, and one from beraprost to bosentan. We measured functional class, saturation of oxygen, haemoglobin levels and six-minute walk distance at baseline, one, six months and at most recent follow-up. Results: Baseline functional class was IV in three, III in fifteen, and II in five patients. At follow-up, with a mean of 15 plus or minus 10 months, 13 of the 23 patients (57%) had improved by at least one functional class, from a median baseline of III to II (p equal to 0.016), mean saturation of oxygen at rest had increased from 81% to 84% (p equal to 0.001), and levels of haemoglobin had decreased from 178 plus or minus 26 grams per litre to 167 plus or minus 19 grams per litre (p equal to 0.001). Overall, the six-minute walk distance did not change from baseline of 335 metres. The distance walked by those not previously receiving specific therapy, however, improved from 318 plus or minus 129 to 345 plus or minus 123 metres (p equal to 0.03). Conclusion: Treatment of adults with Eisenmenger's syndrome using bosentan significantly improved functional class, saturation of oxygen at rest, and decreased levels of haemoglobin. Treatment with bosentan was associated with improvement in six-minute walk distance in those not previously receiving specific therapy. In patients already in receipt of specific therapy, transition to bosentan resulted in no clinical deterioration.

Summary
Currently, positron emission tomography (PET) and single photon emission computed tomography (SPECT) are the only noninvasive imaging modalities that can be used to image specific receptor molecules and to quantify their kinetics. By formally comparing the output of the model to the experimentally obtained PET or SPECT data, one can estimate values for the kinetic parameters and thus extract information on binding, or any hypothesized process. The simplest of compartmental models applied to receptor-ligand studies postulates two tissue compartments. These two tissue compartments along with a plasma compartment are arranged in series. The partial volume effect is widely recognized as a limiting factor in the ability to quantify the amount of radioactivity in tissue accurately. The pharmacokinetic approach requires the PET or SPECT data to be analyzed using a mathematical model of radioactivity uptake. PET and SPECT brain imaging studies are divided into two groups: receptor-ligand studies and tracer studies.

This paper reviews recent advances at LaRC in quantitative measurement science applied to characterizing materials in a nondestructive environment. Recent demands on NDE have resulted in new thrusts to achieve measurements that represent material properties rather than indications or anomalies in a background measurement. Good physical models must be developed of the geometry, material properties, and the interaction of the probing energy with the material to interpret the results quantitatively.
In this paper are presented NDE models that were used to develop measurement technologies for characterizing the curing of a polymer system for composite materials. The procedure uses the changes in ultrasonic properties of the material to determine the glass transition temperature, the degree of cure, and the cure rate. A practical application of this technology is a closed feedback system for controlling autoclave processing of composite materials.
An additional example is in the area of thermal NDE. Thermal diffusion models combined with controlled thermal input/measurement have been used to determine the thermal diffusivity of materials. These measurements are remote, require no contact with the material under test and thus have interesting promise for NDE applications.
