Factors associated with patient-to-healthcare personnel (HCP) and HCP-to-subsequent patient transmission of methicillin-resistant Staphylococcus aureus

Background:
Transient acquisition of methicillin-resistant Staphylococcus aureus (MRSA) on healthcare personnel (HCP) gloves and gowns following patient care has been examined. However, the potential for transmission to the subsequent patient has not been studied. We explored the frequency of MRSA transmission from patient to HCP, and then in separate encounters from contaminated HCP gloves and gowns to a subsequent simulated patient as well as the factors associated with these 2 transmission pathways.
Methods:We conducted a prospective cohort study with 2 parts. In objective 1, we studied MRSA transmission from random MRSA-positive patients to HCP gloves and gowns after specific routine patient care activities. In objective 2, we simulated subsequent transmission from random HCP gloves and gowns without hand hygiene to the next patient using a manikin proxy.
Results:For the first objective, among 98 MRSA-positive patients with 333 randomly selected individual patient–HCP interactions, HCP gloves or gowns were contaminated in 54 interactions (16.2%). In a multivariable analysis, performing endotracheal tube care had the greatest odds of glove or gown contamination (OR, 4.06; 95% CI, 1.3–12.6 relative to physical examination). For the second objective, after 147 simulated HCP–patient interactions, the subsequent transmission of MRSA to the manikin proxy occurred 15 times (10.2%).
Conclusion:After caring for a patient with MRSA, contamination of HCP gloves and gown and transmission to subsequent patients following HCP-patient interactions occurs frequently if contact precautions are not used. Proper infection control practices, including the use of gloves and gown, can prevent this potential subsequent transmission.

Stonehenge is a site that continues to yield surprises. Excavation in 2009 added a new and unexpected feature: a smaller, dismantled stone circle on the banks of the River Avon, connected to Stonehenge itself by the Avenue. This new structure has been labelled ‘Bluestonehenge’ from the evidence that it once held a circle of bluestones that were later removed to Stonehenge. Investigation of the Avenue closer to Stonehenge revealed deep periglacial fissures within it. Their alignment on Stonehenge's solstitial axis (midwinter sunset–midsummer sunrise) raises questions about the early origins of this ritual landscape.

We present an update of the ‘key points’ from the Antarctic Climate Change and the Environment (ACCE) report that was published by the Scientific Committee on Antarctic Research (SCAR) in 2009. We summarise subsequent advances in knowledge concerning how the climates of the Antarctic and Southern Ocean have changed in the past, how they might change in the future, and examine the associated impacts on the marine and terrestrial biota. We also incorporate relevant material presented by SCAR to the Antarctic Treaty Consultative Meetings, and make use of emerging results that will form part of the Intergovernmental Panel on Climate Change (IPCC) Fifth Assessment Report.

The antimalarial drug artemisinin (ART) is commercially extracted from the medicinal plant Artemisia annua L. Here, we report the screening of 70 A. annua plants representing 14 diverse germplasm accessions sourced from around the world, and identify lines containing >2% ART. These extremely high-yielding individuals have been maintained as vegetative clones, and they represent promising germplasm resources for future A. annua breeding programmes.

On 25 March 2008, a stranded Cuvier's beaked whale was discovered on the southern coast of the Moray Firth in north-east Scotland. Whilst strandings of the species are not uncommon in Scotland, the vast majority occur along the Atlantic coasts to the west of Scotland and Ireland. By comparison, records from the North Sea coast are notably absent, and this event is believed to be the first documented stranding of a Ziphius cavirostris from this mainland coastal region until now. Due to the severe state of decomposition of the specimen, no necropsy was made and therefore the cause of death could not be determined. However, this event may have been linked to a number of other beaked whale strandings in Scotland at this time. Whilst the North Sea region is considered to be an entrapment area for several species of oceanic whale, the Cuvier's beaked whale is rarely thought to travel far enough north to enter this trap. However, the phenology of these and other recent Cuvier's beaked whale strandings would suggest a possible northern range expansion for this species in UK waters, which may explain the increasing number of incidental events now being observed.

We have considered multiple subpial transection (MST) as a treatment option for Landau–Kleffner syndrome (LKS) for the past 6 years. The effect of this technique on language and cognitive ability, behaviour, seizures, and EEG abnormalities is analysed here. Five children (4 males, 1 female; aged 5.5 to 10 years) underwent MST with sufficiently detailed pre- and postoperative data for analysis. Behaviour and seizure frequency improved dramatically after surgery in all children. Improvement in language also occurred in all children, although none improved to an age-appropriate level. All five had electrical status epilepticus in sleep (ESES) before surgery, which was eliminated by the procedure. One child has had an extension of his MST due to the recurrence of ESES and accompanying clinical deterioration with good effect. An attempt is made to set the effect of MST against the natural history of the condition. MST is an important treatment modality in LKS, although the timing of this intervention and its effect on final language outcome remains to be defined.

Summary
All cells transport proteins across membranes, but the complexity of protein traffic in plant cells is especially striking because of the variety of organelle types involved. Many proteins are inserted, during translation, into the lumen of the endoplasmic reticulum, after which they are transported via the endomembrane system to the Golgi apparatus, vacuole, protein bodies or plasma membrane. Other proteins are transported posttranslationally into glyoxysomes, mitochondria and plastids. In each case, the protein is synthesised with an appropriate signal which ensures targeting to the correct organelle, and a number of studies have attempted to define the characteristics of these targeting signals (reviewed by Bennett & Osteryoung, 1991; Robinson, 1991).
In terms of protein transport events, the biogenesis of the chloroplast is particularly complex, primarily owing to the architecture of the organelle. The chloroplast is bounded by a double-membrane envelope, between whose membranes is a soluble phase, the functions of which are presently obscure. Within the organelle is the soluble stromal phase (site of CO2 fixation, amino acid synthesis and many other key reactions) and the extensive internal thylakoid membrane. The thylakoid network also encloses a further soluble phase, usually termed the thylakoid lumen. Thus, the chloroplast comprises in total three distinct membranes and three discrete soluble phases. Most of the proteins located in each of these organellar compartments are encoded by nuclear genes, synthesised in the cytosol, and transported into the organelle. Clearly, therefore, chloroplast biogenesis requires both the specific, efficient targeting of a large number proteins into the organelle, and the operation of intraorganellar ‘sorting’ mechanisms to distribute imported proteins to their correct destinations.

The paper by Ian Kinnes et al. in the March number published the gold objects from Bush Barrow, Wiltshire, examined the evidence for the original form of the large lozenge, and described its recent restoration into a domed profile. In this paper a different view is taken of evidence for the original form of the lozenge and of the wisdom of its recent transformation.

Summary
This is a survey of results on arithmetic groups. Only a minimal acquaintance with algebraic geometry is assumed. Theorems are mostly quoted without proof: sometimes an indication of the method is given. There is a substantial bibliography, with a guide to the subjects it covers. The reader is referred to the sources therein for the proofs omitted here.
DEFINITIONS AND GENERAL PROPERTIES ([9], [26])
Let G be an algebraic subgroup of GLn, defined over the field Q of rational numbers. Thus there exists a set of polynomials (with rational coefficients) in the n2 matrix entries and the inverse of the determinant, whose set of solutions in any extension E of Q is a subgroup GE, of GLn (E). We call GE the group of E-points of G. The groups GR and Gc are respectively real and complex Lie groups.
We write GZ for GQ ∩ GLn (Z).
Definition. A subgroup Г of GQ is arithmetic if it is commensurable with GZ: that is, if Г ∩GZ has finite index both in Г and in Gz.
A group Г is arithmetic if it can be embedded as an arithmetic subgroup in GQ for some Q-algebraic subgroup G of GLn. Then any subgroup of finite index in Г is also an arithmetic group.
Remarks. (1) We admit all subgroups commensurable with Gz, rather than Gz alone, in order to make the definition independent of the chosen Q-embedding of G in a general linear group.
