OBJECTIVES/GOALS: There is a need to better understand how governments develop strategies to adopt, evaluate, and implement novel health technologies in a public healthcare system. The goal of this project is to understand this strategy development process for the translation of pharmacogenomic (PGx) testing in Ontario, Canada. METHODS/STUDY POPULATION: This observational case study of the Ontario Health PGx Working Group focused on developing recommendations for a PGx testing implementation strategy in the province. The group included 9 individuals affiliated with Ontario Health and 13 healthcare experts from multiple clinical fields. Ontario Health is the government agency that oversees provincial healthcare planning and service delivery. Guided by the Translational Thinking Framework and qualitative research methods, we observed the working group’s activities for eight months. We collected meeting recordings, slideshow decks, emails, and group characteristics. We used descriptive statistics and a nine-step inductive approach to analyze the data to create process maps, a case report, and key decision summaries. RESULTS/ANTICIPATED RESULTS: There were 19 meetings conducted remotely with videoconferencing technology. Throughout the working group’s activities, we identified 15 key decisions related to either administrative processes or PGx scientific content. We further stratified these two categories into four main themes relating to decisions about 1) membership involvement, 2) logistical management, 3) discussion and recommendation scope, and 4) information dissemination. These four decision themes represent tools by which Ontario Health guided the expert group activities and achieved their goal of generating a strategic roadmap for PGx testing implementation in Ontario. DISCUSSION/SIGNIFICANCE: The Ontario government makes decisions about how expert groups function by monitoring and controlling the group’s activities to ensure efficiency, standardization, and practicality. Describing expert group decision-making increases transparency and highlights the critical role they play in the translational pathway of health technologies.
Mineralogical and Elemental Trends in Regolith on Historically Managed Sites in the southeastern United States Piedmont

The deep regolith of the southeastern United States has undergone rapid erosion in the last two centuries due to intensive agricultural practices, which has altered the landscape and its inherent fertility. Parent material, landscape position, and land use are important factors in controlling the mineral and elemental composition of soil profiles. Independent quantitative X-ray diffraction (QXRD) and whole-rock chemical analysis of eight weathering profiles agreed well and allow mineral reaction pathways to be constrained as particles are conveyed in the subsurface. QXRD analysis of saprolite, argillic, and soil A-horizons in the profiles highlights the imprint of bedrock on the regolith, which includes Neoproterozoic meta-tonalitic to meta-granodioritic and Paleozoic meta-granitic to biotite- and amphibolite-gneissic lithologies. Also, aeolian input slightly influenced A-horizon composition. The clay mineral assemblage is dominated by kaolinite, but profiles differ in the amount of interstratified clay minerals, halloysite, hematite, goethite, and gibbsite. Rare-earth element totals vary between 30 and 1048 ppm and are generally correlated positively with clay and clay mineral content. Eu and Ce anomalies reflect parent rocks and subsequent hydrolysis and redox history, with trends depending upon landscape position and clay content in the weathering profile. Weathering profiles on a high-order interfluve and those that were actively cultivated have thick argillic horizons (as defined by clay mineral abundance) and are depleted in alkali and alkaline-earth elements. Profiles proximally developed on old-field pine and never-cultivated hardwood forest land do not show large differences in mineral composition trends, whereas profiles on old-field sites with ongoing cultivation exhibit assemblages enriched in clay minerals and (oxyhydr)oxides. Old-field pine sites that were historically eroded by previous cultivation tend to have shallower and thinner argillic horizons, which may well impact critical-zone processes involving gas and water fluxes. This study highlights that mineral compositions of deep regolith, saprolite, and shallow soil horizons are dependent on local geomorphology (i.e. watershed- and hillshed-orders). Quantifying soil and regolith compositional trends across the landscape is a prerequisite for determining rates of chemical and physical erosion on human and geologic time scales.

Quantification of mineral assemblages in near-surface Earth materials is a challenge because of the often abundant and highly variable crystalline and chemical nature of discrete clay minerals. Further adding to this challenge is the occurrence of mixed-layer clay minerals, which is complicated because of the numerous possible combinations of clay layer types, as defined by their relative proportions and the ordering schemes. The problem of ensuring accurate quantification is important to understanding landscape evolution because mineral abundances have a large influence on ecosystem function. X-ray diffraction analysis of the variable cation-saturated clay fraction in soil and regolith from the Calhoun Critical Zone observatory near Clinton, South Carolina, USA, was coupled with modeling using NEWMOD2 to show that mixed-layer clays are often dominant components in the mineral assemblages. Deep samples in the profile (>6.5 m) contain mixed-layer kaolinite/smectite, kaolinite/illite-like, kaolinite-vermiculite, illite-like/biotite, and illite-like/vermiculite species (with ‘illite-like’ defined herein as Fe-oxidized 2:1 layer structure with a negative layer charge of ~0.75 per unit formula, i.e. weathered biotite). The 2:1 layers in the mixed-layer structures are proposed to serve as exchange sites for K+, which is known to cycle seasonally between plant biomass and subsurface weathering horizons. Forested landscapes have a greater number of 2:1 layer types than cultivated landscapes. Of two nearby cultivated sites, the one higher in landscape position has fewer 2:1 layer types. Bulk potassium concentrations for the forested and two cultivated sites show the greatest abundances in the surface forested site and lowest abundance in the surface upland cultivated site. These observations suggest that landscape use and landscape position are factors controlling the mixed-layer mineral assemblages in Kanhapludults typical of the S.E. United States Piedmont. These mixed-layer clays are key components of the proposed mechanism for K+ uplift concepts, whereby subsurface cation storage may occur in the interlayer sites (with increased negative 2:1 layer charge) during wetter reduced conditions of the winter season and as biomass decay releases cation nutrients. Cation release from the mixed-layer clays (by decreased 2:1 layer charge) occurs under drier oxidized conditions during the growing seasons as biota utilize cation nutrients. The types and abundances of mixed layers also reflect long-term geologic factors including dissolution/alteration of primary feldspar and biotite and the subsequent transformation and dissolution/precipitation reactions that operate within the soil horizons. Thus, the resulting mixed-layer clay mineral assemblages are often complex and heterogeneous at every depth within a profile and across landscapes. X-ray diffraction (XRD) assessment, using multiple cation saturation state and modeling, is essential for quantifying the clay mineral assemblage and pools for cation nutrients, such as potassium, in the critical zone.
Modifications of 2:1 Clay Minerals in a Kaolinite-Dominated Ultisol under Changing Land-Use Regimes

Chemical denudation and chemical weathering rates vary under climatic, bedrock, biotic, and topographic conditions. Constraints for landscape evolution models must consider changes in these factors on human and geologic time scales. Changes in nutrient dynamics, related to the storage and exchange of K+ in clay minerals as a response to land use change, can affect the rates of chemical weathering and denudation. Incorporation of these changes in landscape evolution models can add insight into how land use changes affect soil thickness and erodibility. In order to assess changes in soil clay mineralogy that result from land-use differences, the present study contrasts the clay mineral assemblages in three proximal sites that were managed differently over nearly the past two centuries where contemporary vegetation was dominated by old hardwood forest, old-field pine, and cultivated biomes. X-ray diffraction (XRD) of the oriented clay fraction using K-, Mg-, and Na-saturation treatments for the air-dried, ethylene glycol (Mg-EG and K-EG) solvated, and heated (100, 350, and 550°C) states were used to characterize the clay mineral assemblages. XRD patterns of degraded biotite (oxidized Fe and expelled charge-compensating interlayer K) exhibited coherent scattering characteristics similar to illite. XRD patterns of the Mg-EG samples were, therefore, accurately modeled using NEWMOD2® software by the use of mineral structure files for discrete illite, vermiculite, kaolinite, mixed-layer kaolinite-smectite, illite-vermiculite, kaolinite-illite, and hydroxy-interlayered vermiculite. The soil and upper saprolite profiles that formed on a Neoproterozoic gneiss in the Calhoun Experimental Forest in South Carolina, USA, revealed a depth-dependence for the deeply weathered kaolinitic to the shallowly weathered illitic/vermiculitic mineral assemblages that varied in the cultivated, pine, and hardwood sites, respectively. An analysis of archived samples that were collected over a five-decade growth period from the pine site suggests that the content of illite-like layers increased at the surface within 8 y. Historical management of the sites has resulted in different states of dynamic equilibrium, whereby deep rooting at the hardwood and pine sites promotes nutrient uplift of K from the weathering of orthoclase and micas. Differences in the denudation rates at the cultivated, pine, and hardwood sites through time were reflected by changes in the soil clay mineralogy. Specifically, an increased abundance of illite-like layers in the surface soils can serve as a reservoir of K+.

Background
Childhood adversity and cannabis use are considered independent risk factors for psychosis, but whether different patterns of cannabis use may be acting as mediator between adversity and psychotic disorders has not yet been explored. The aim of this study is to examine whether cannabis use mediates the relationship between childhood adversity and psychosis.
MethodsData were utilised on 881 first-episode psychosis patients and 1231 controls from the European network of national schizophrenia networks studying Gene–Environment Interactions (EU-GEI) study. Detailed history of cannabis use was collected with the Cannabis Experience Questionnaire. The Childhood Experience of Care and Abuse Questionnaire was used to assess exposure to household discord, sexual, physical or emotional abuse and bullying in two periods: early (0–11 years), and late (12–17 years). A path decomposition method was used to analyse whether the association between childhood adversity and psychosis was mediated by (1) lifetime cannabis use, (2) cannabis potency and (3) frequency of use.
ResultsThe association between household discord and psychosis was partially mediated by lifetime use of cannabis (indirect effect coef. 0.078, s.e. 0.022, 17%), its potency (indirect effect coef. 0.059, s.e. 0.018, 14%) and by frequency (indirect effect coef. 0.117, s.e. 0.038, 29%). Similar findings were obtained when analyses were restricted to early exposure to household discord.
ConclusionsHarmful patterns of cannabis use mediated the association between specific childhood adversities, like household discord, with later psychosis. Children exposed to particularly challenging environments in their household could benefit from psychosocial interventions aimed at preventing cannabis misuse.

Background
While cannabis use is a well-established risk factor for psychosis, little is known about any association between reasons for first using cannabis (RFUC) and later patterns of use and risk of psychosis.
MethodsWe used data from 11 sites of the multicentre European Gene-Environment Interaction (EU-GEI) case–control study. 558 first-episode psychosis patients (FEPp) and 567 population controls who had used cannabis and reported their RFUC.
We ran logistic regressions to examine whether RFUC were associated with first-episode psychosis (FEP) case–control status. Path analysis then examined the relationship between RFUC, subsequent patterns of cannabis use, and case–control status.
ResultsControls (86.1%) and FEPp (75.63%) were most likely to report ‘because of friends’ as their most common RFUC. However, 20.1% of FEPp compared to 5.8% of controls reported: ‘to feel better’ as their RFUC (χ2 = 50.97; p < 0.001). RFUC ‘to feel better’ was associated with being a FEPp (OR 1.74; 95% CI 1.03–2.95) while RFUC ‘with friends’ was associated with being a control (OR 0.56; 95% CI 0.37–0.83). The path model indicated an association between RFUC ‘to feel better’ with heavy cannabis use and with FEPp-control status.
ConclusionsBoth FEPp and controls usually started using cannabis with their friends, but more patients than controls had begun to use ‘to feel better’. People who reported their reason for first using cannabis to ‘feel better’ were more likely to progress to heavy use and develop a psychotic disorder than those reporting ‘because of friends’.
The Jarigole mortuary tradition reconsidered

The megalithic pillar sites found around Lake Turkana, Kenya, are monumental cemeteries built approximately 5000 years ago. Their construction coincides with the spread of pastoralism into the region during a period of profound climate change. Early work at the Jarigole pillar site suggested that these places were secondary burial grounds. Subsequent excavations at other pillar sites, however, have revealed planned mortuary cavities for predominantly primary burials, challenging the idea that all pillar sites belonged to a single ‘Jarigole mortuary tradition’. Here, the authors report new findings from the Jarigole site that resolve long-standing questions about eastern Africa's earliest monuments and provide insight into the social lives, and deaths, of the region's first pastoralists.
Interdisciplinary analysis of drugs: Structural features and clinical data

Background:
Chemical structure is a vital consideration early in the drug development process. Its role in analysis of safety and efficacy is relatively diminished after drugs are approved for clinical use. This interdisciplinary study explores a strategy by which readily available clinical data may be used along with structural features of drugs to identify associations with potential utility for both clinical decision-making and drug development.
Methods:Chemical functional groups and structural groups (SGs) of 261 drugs were manually classified in tiers, and their incidence of gastrointestinal (GI) and central nervous system (CNS) adverse drug reactions (ADRs) were obtained from a clinical database. Drugs with an GI or CNS ADR incidence of at least 10% were analyzed for correlations with their functional and SGs.
Results:Eight statistically significant associations were detected by preliminary analysis: piperazine and methylene groups were associated with higher rate of CNS ADRs; while amides, secondary alcohols, and di-substituted phenyl groups were associated with lower rates of GI or CNS ADRs or both.
Conclusions:Although further study is necessary to understand these associations and build upon this strategy, this exploratory analysis establishes a methodology by which chemical properties of drugs may be used to aid in clinical decision-making when choosing between otherwise equivalent drug therapy options, as the presence of specific groups on drugs may be associated with increased or decreased risks of specific ADRs.
Assessing implementation fidelity in the First Episode Rapid Early Intervention for Eating Disorders service model

Background
The First Episode Rapid Early Intervention for Eating Disorders (FREED) service model is associated with significant reductions in wait times and improved clinical outcomes for emerging adults with recent-onset eating disorders. An understanding of how FREED is implemented is a necessary precondition to enable an attribution of these findings to key components of the model, namely the wait-time targets and care package.
AimsThis study evaluated fidelity to the FREED service model during the multicentre FREED-Up study.
MethodParticipants were 259 emerging adults (aged 16–25 years) with an eating disorder of <3 years duration, offered treatment through the FREED care pathway. Patient journey records documented patient care from screening to end of treatment. Adherence to wait-time targets (engagement call within 48 h, assessment within 2 weeks, treatment within 4 weeks) and care package, and differences in adherence across diagnosis and treatment group were examined.
ResultsThere were significant increases (16–40%) in adherence to the wait-time targets following the introduction of FREED, irrespective of diagnosis. Receiving FREED under optimal conditions also increased adherence to the targets. Care package use differed by component and diagnosis. The most used care package activities were psychoeducation and dietary change. Attention to transitions was less well used.
ConclusionsThis study provides an indication of adherence levels to key components of the FREED model. These adherence rates can tentatively be considered as clinically meaningful thresholds. Results highlight aspects of the model and its implementation that warrant future examination.

Each year in the UK there are approximately 250,000 miscarriages, 3,000 stillbirths and 3,000 terminations following a diagnosis of fetal-abnormality. This paper draws from original empirical research into the experience of pregnancy loss and the accompanying decisionmaking processes. A key finding is that there is considerable variation across England in the range of options that are offered for disposal of pregnancy remains and the ways in which information around disposal are communicated. This analysis seeks to outline the key features of what constitutes effective communication in this context, where effective communication is taken to mean that patients are provided with the key information necessary, in an appropriate manner, so that they are fully able to make a decision. A primary source of evidence includes interviews with the bereaved and pregnancy-loss support workers, in order to understand how the options available, and associated necessary procedures, are communicated. In addition, patient information leaflets are also analyzed as they offer an important tool for information delivery at a difficult and emotionally charged time. Following this, an overview is provided of the information that these leaflets should contain, along with guidance on effective presentation of this information.
4308 DEEP-PRIMED IL-15 SUPERAGONIST IMPROVES ANTIVIRAL EFFICACY OF HIV-SPECIFIC CD8+ T-CELLS IN HUMANIZED MOUSE MODEL

OBJECTIVES/GOALS: HIV-specific CD8+ T-cells play a critical role in partially controlling viral replication in infected-individuals, but ultimately fail to eliminate infection. Enhancing these T-cell responses through lymphocyte engineering approaches has the potential as a novel therapy capable of achieving durable control or eradication of infection. METHODS/STUDY POPULATION: IL-15 Superagonist (IL-15SA) potently supports the in vivo persistence and antiviral activity of adoptively transferred CD8+ T-cells. The Deep-PrimingTM technology platform, developed by Torque, allows for loading of immunomodulators onto the surface of T-cells via electrostatic ‘nanogels’, which slowly release to deliver sustained autocrine immune stimulation without the harmful effects of systemic exposure. Here, we investigate the impact of IL-15SA Deep-Priming on HIV-specific CD8+ T-cells in a humanized mouse model of HIV infection. Humanized mice were generated by engrafting NOD-scid-IL2Rgnull mice with memory CD4+ T-cells isolated from an ARV-suppressed HIV+ donor. An autologous HIV-specific Cytotoxic T-Lymphocyte (CTL) clone was isolated, and killing potential confirmed. Four weeks post humanization, mice were infected with HIV and received an infusion of unmodified HIV-Specific CTLs, or IL-15SA Deep-Primed HIV-specific CTLs (CTL-DP). T-cell numbers and plasma viral loads were quantified weekly by flow cytometry and qRT-PCR. RESULTS/ANTICIPATED RESULTS: Mice receiving unmodified CTLs trended toward reduced viral loads compared to the No Treatment condition, while mice receiving CTL-DP saw significant, 2-Log10 reductions in VL (p < 0.01). At 41 days post-infection 100% (5/5) of the No Treatment, 66.7% (4/6) of the CTL treatment, and 16.7% (1/6) of CTL-DP treatment mice had detectable viremia. IL-15SA Deep-Priming increased CTL expansion and persistence in peripheral blood which correlated with improved CD4+T-cell preservation. DISCUSSION/SIGNIFICANCE OF IMPACT: Here we demonstrate the first in vivo analysis of IL-15SA Deep-Priming of HIV-Specific CTLs. What is progress and what is not progress? We can talk about progress in lots of different arenas; we will focus primarily on economic and scientific progress, but also make brief reference to cultural and moral progress. In our discussion, we want to distinguish, especially, between overall, long-term progress and narrower, shorter-term progress or regress. We will refer to these as “global” and “local” progress, respectively. Of course, one can also regress; therefore, we will also look at instances where progress, along some dimension, slows or even moves backwards. Generally, such regress is local, and often still in a context of broader, global progress. In scientific progress, for example, there are many instances of short-term progress which, if not completely discarded or disproved, are at least substantially modified or fundamentally challenged. And yet, those research paths, even when later abandoned, still contributed to the overall progress of the field. In that sense, the regress (that is, rejection or modification of previous theories) is corrected by, but not in conflict with, the overall progress. In the case of economic progress, the concept of regress usually takes on a different form in which things that aren’t advancing progress don’t necessarily stop it, but are simply retarding progress — that is, making the rate of progress less efficient. The consequence, we suggest, is that when talking about economic progress, objections to certain consequences of economic progress (for instance, income inequality — a type of regress, in our terminology) should not be cordoned off and dealt with independently, but should be incorporated into the way we think about economic progress itself — as instances of local regress within a context of global progress. We explore the effects of these different relations between progress and regress to suggest some of the challenges those seeking to broaden the standard measure, GDP, to incorporate other social values of well-being will face moving forward.

Europeans began introducing plants into southeastern Florida during the 16th century, and by the middle of the 18th century several of these were already naturalized. Few of these events have been recorded, but we know that the Guava, Papaya, and oranges, were among the early immigrants. American settlement in the middle 1800s began another phase of plant movement which continues to the present. H. M. Flagler, Henry Nehrling, John C. Gifford, and others, brought plants in for landscaping or other uses. Among those brought in by the United States Department of Agriculture were the Sisal, introduced by Henry Perrine, and the many food and ornamental plants sent by David Fairchild to the Miami Introduction Station. Many of the plants brought to Florida have become important as ornamentals or foods; however, some of the exotics have escaped and now present problems to the native ecosystems and to Man.
Over 170 species of ferns and flowering plants are naturalized in southeastern Florida. Hundreds of other exotic plants have been introduced into the region and the potential exists for others to escape. Species such as Melaleuca quinquenervia are invading wet habitats and decreasing diversity; the Australian Pine (Casuarina equisetifolia) has become so abundant on some beaches that it interferes with nesting seaturtles and American Crocodiles, as well as contributing to dune erosion. Water-hyacinth (Eichhornia crassipes) and Florida Elodea (Hydrilla verticillata) clog natural and artificial lakes, ponds, and canals, blocking boat traffic and slowing water movement. Several grasses that were introduced as pasture plants are now considered pests. Even human health is being threatened by exotic plants such as the Melaleuca and Pepper-tree (Schinus terebinthifolius), which cause respiratory allergies and contact dermatitis.

Objective:
The purpose of the study was to determine the incidence and risk factors for the acquisition of methicillin-resistant Staphylococcus aureus (MRSA) in our community.
Design:This study used a cross-sectional design to assess patients colonized or infected with MRSA.
Patients:The study population consisted of residents of London, Ontario, Canada, who were identified as MRSA-positive for the first time in 1997.
Setting:All acute- and chronic-care hospitals, long-term healthcare facilities, and community physicians' offices in the city of London participated in the study.
Main Outcome Measure:Incidence of MRSA in the community, risk factors for acquisition, especially previous hospitalization over a defined period, and strain type were evaluated.
Results:In 1997, 331 residents of London were newly identified as MRSA-positive, representing an annual incidence of 100/100,000 persons (95% confidence interval, 88.8-110.7). Thirty-one (9.4%) individuals were not healthcare-facility patients in the previous month, and 11 (3.3%), 10 (3.0%), and 6 (1.8%) individuals had no such contact in the previous 3, 6, and 12 months, respectively. One hundred seventy-seven strains, including five of the isolates from patients with no healthcare-facility contact in the previous year, were typed. One hundred sixty (90.3%) of these isolates, including all typed strains from patients with no healthcare facility contact, belonged to a single clone.
Conclusion:These findings demonstrate that the incidence of MRSA is higher than previously reported and that hospital contact is the single most important risk factor for the acquisition of MRSA in our community. Screening for MRSA in previously hospitalized patients at the time of hospitalization may reduce nosocomial spread and indirectly reduce the incidence of MRSA in the community.

Since the beginning of the Reconciliation and Social Justice Project in 1996, AustLII has sought to contribute to the process of reconciliation through the provision of relevant legal and cultural information relating to indigenous people. Two important components of the project have been collecting material as well as making it as accessible as possible to as many people as possible. Another significant part of the Reconciliation and Social Justice Project has been connecting remote Aboriginal communities to the Internet and providing training to them in its use. The history and achievements of the project are set out here and some consideration is given to future directions for the various aspects of the project.
