Background
Studying phenotypic and genetic characteristics of age at onset (AAO) and polarity at onset (PAO) in bipolar disorder can provide new insights into disease pathology and facilitate the development of screening tools.
AimsTo examine the genetic architecture of AAO and PAO and their association with bipolar disorder disease characteristics.
MethodGenome-wide association studies (GWASs) and polygenic score (PGS) analyses of AAO (n = 12 977) and PAO (n = 6773) were conducted in patients with bipolar disorder from 34 cohorts and a replication sample (n = 2237). The association of onset with disease characteristics was investigated in two of these cohorts.
ResultsEarlier AAO was associated with a higher probability of psychotic symptoms, suicidality, lower educational attainment, not living together and fewer episodes. Depressive onset correlated with suicidality and manic onset correlated with delusions and manic episodes. Systematic differences in AAO between cohorts and continents of origin were observed. This was also reflected in single-nucleotide variant-based heritability estimates, with higher heritabilities for stricter onset definitions. Increased PGS for autism spectrum disorder (β = −0.34 years, s.e. = 0.08), major depression (β = −0.34 years, s.e. = 0.08), schizophrenia (β = −0.39 years, s.e. = 0.08), and educational attainment (β = −0.31 years, s.e. = 0.08) were associated with an earlier AAO. The AAO GWAS identified one significant locus, but this finding did not replicate. Neither GWAS nor PGS analyses yielded significant associations with PAO.
ConclusionsAAO and PAO are associated with indicators of bipolar disorder severity. Individuals with an earlier onset show an increased polygenic liability for a broad spectrum of psychiatric traits. Systematic differences in AAO across cohorts, continents and phenotype definitions introduce significant heterogeneity, affecting analyses.
Sensitivity of Tobacco (Nicotiana tabacum) and Vegetable Crop Seedlings to Fluridone in Irrigation Water

The sensitivities of tobacco, tomato, pepper, and cucumber seedlings to an aquatic herbicide, fluridone, were examined in growth chamber and greenhouse studies. New leaves of tobacco seedlings became chlorotic after 6-d exposure to fluridone concentrations of 5 μg/L or greater in hydroponic float culture, and dry weights were significantly reduced at 100 μg/L or greater. When treated only once at the beginning of a 2-wk evaluation period, all crops except cucumber exhibited injury symptoms at 100 μg/L. When grown in sand, pepper and tomato were injured at 25 μg/L; tobacco injury occurred at 10 μg/L. Dry weights of pepper, tobacco, and tomato seedlings decreased significantly at 250, 50, and 250 μg/L, respectively, on potting mix; effects on dry weights were not significant for plants on sand. Cucumber was injured by treatment at 100 μg/L or greater in sand, but there were no effects on dry weights in either sand or potting mix. When treated three times weekly for 2 wk with fluridone, pepper, tobacco, and tomato grown in potting mix were injured by 50, 25, and 100 μg/L; when grown in sand, injury occurred at 10, 5, and 10 μg/L, respectively. Dry weights of pepper, tobacco, and tomato were reduced by 50,50, and 100 μg/L, respectively, in potting mix; effects on dry weights were not significant for plants in sand. Cucumber seedlings were damaged by 250 μg/L or higher on potting mix and 100 μg/L or higher on sand, but there were no effects on dry weights regardless of substrate. Threshold injury levels for plants grown in potting mix to the second true leaf stage and then treated three times weekly were 25 μg/L for pepper and tobacco and 50 μg/L for tomato; dry weights were significantly reduced in pepper at 25 μg/L and in tobacco and tomato at 50 μg/L. Cucumber seedlings were not injured by any treatment in this test.

The central nervous system undertakes the homeostatic role of sensing nutrient intake and body reserves, integrating the information, and regulating energy intake and/or energy expenditure. Few tasks regulated by the brain hold greater survival value, particularly important in farmed ruminant species, where the demands of pregnancy, lactation and/or growth are not easily met by often bulky plant-based and sometimes nutrient-sparse diets. Information regarding metabolic state can be transmitted to the appetite control centres of the brain by a diverse array of signals, such as stimulation of the vagus nerve, or metabolic ‘feedback’ factors derived from the pituitary gland, adipose tissue, stomach/abomasum, intestine, pancreas and/or muscle. These signals act directly on the neurons located in the arcuate nucleus of the medio-basal hypothalamus, a key integration, and hunger (orexigenic) and satiety (anorexigenic) control centre of the brain. Interest in human obesity and associated disorders has fuelled considerable research effort in this area, resulting in increased understanding of chronic and acute factors influencing feed intake. In recent years, research has demonstrated that these results have relevance to animal production, with genetic selection for production found to affect orexigenic hormones, feeding found to reduce the concentration of acute controllers of orexigenic signals, and exogenous administration of orexigenic hormones (i.e. growth hormone or ghrelin) reportedly increasing DM intake in ruminant animals as well as single-stomached species. The current state of knowledge on factors influencing the hypothalamic orexigenic and anorexigenic control centres is reviewed, particularly as it relates to domesticated ruminant animals, and potential avenues for future research are identified.

CLASSIFICATION, CLINICOPATHOLOGICAL CORRELATIONS, AND FUNCTIONAL-ORGANIC OVERLAP In this millennial issue, I shall recall some of the issues in the psychiatry of old age that were current in England half a century ago, based on my experiences at the Maudsley Hospital, Graylingwell, Stockholm, and Newcastle. In 1946, Aubrey Lewis, then Professor of Psychiatry at the Maudsley Hospital, lamented that while the numbers of old people being admitted to mental hospitals had steadily increased, the clinical psychiatry of old age was neglected (Lewis, 1946). Classification was in a state of confusion. Senile and vascular degenerations characterized the aging brain, and though affective and paranoid states occurred, they were widely regarded as depressive and paranoid forms of senile psychosis and not clearly distinguished from purely functional states. In the presence of degenerative changes in the brains of normal old people and the apparent lack of a clear quantitative relationship between clinical severity and neuropathology, the presence or absence of symptoms was determined by the premorbid personality and the ability to resist the effects of the cerebral changes (Rothschild, 1937).

Long-term survival, place of death, and death certification in clinically diagnosed pre-senile dementia in northern England: Follow-up after 8–12 years

Background
Information on survival and cause of death in pre-senile dementia is scarce and the organisation of services controversial.
AimsTo study survival, place of death and death certification in pre-senile dementia.
MethodPatients aged 45–64 were identified from hospital and community sources in the Northern health region (1985–89) and classified as having pre-senile dementia of Alzheimer type (PDAT) or pre-senile vascular dementia (PVD) by applying an algorithm to case notes. Deaths were ascertained from the National Health Service Central Registry (NHSCR) to 31 December 1998. Survival analysis was performed using the SPSS/PC program, and expected survival calculated from life tables.
ResultsMedian survival time from diagnosis was 6.08 years and did not differ significantly in PDAT and PVD, or by age or gender; 19.3% of deaths occurred at home, 24.5% in nursing or residential homes and 56.3% in hospital; 72.4% of the death certificates mentioned dementia or Alzheimer's disease; 15.4% were still alive.
ConclusionsPre-senile dementia has a variable but usually chronic course, requiring appropriate planning and services.

Background
In a study of patients with presenile dementia of Alzheimer type (PDAT), we aimed to investigate functional dependency in relation to the time since diagnosis and place of residence. Use of community and support services and the time between diagnosis and entry to permanent residential care were also studied.
MethodInterviews with 109 cases of PDAT (diagnosed before age 65) and their carers.
ResultsThe need for assistance in activities of daily living (ADL) was progressive with time from diagnosis. Five years after diagnosis, 57% of patients remained at home, 20% of whom required some assistance in each of six areas of ADL. The need for assistance in each ADL category was significantly greater for cases in permanent residential care than for those at home. There were no significant differences between the Mini-Mental State Examination (MMSE) scores of patients at home and those of patients in permanent care. Incontinence and the relationship of the carer to the patient were the strongest predictors of permanent residential care. Respite care had been offered in 63% of cases; 71% of these offers had been accepted.
ConclusionsPatients with PDAT who have a living relative are cared for at home for a considerable period of time despite severe cognitive impairment and loss of independence measured by ADL. The reports of carers suggest there is scope for improvement in supportive services to meet this community need.
