Recognition and treatment of patients with sleep problems in general health care

The form, frequency and burden of sleep problems in general health care in the WHO Collaborative Study on Psychological Problems are presented in this same issue. This study was conducted in 5,438 primary care patients belonging to 15 centres in 14 countries after 25,916 patients were screened with the GHQ-12. Diagnoses according to the International Classification of Diseases (ICD-10) were obtained from the Composite International Diagnostic Interview (CIDI-PHC). The presence of difficulties falling asleep, staying asleep, waking too early or sleeping too much were assessed by the CIDI. General practitioners (GPs) gave their opinion on the existence of a psychological problem and/or a physical disease and indicated what therapeutic intervention was proposed to patients they recognized as psychological cases. The existence of sleep problems increased the recognition of patients with psychiatric disorders by their GP. No specific subtype of sleep problems influences recognition. The existence of a somatic disease had little influence on the identification of sleep problems as psychological cases. Sleep problems were not frequently expressed as a main presenting complaint. In such a case the recognition rate of patients with ICD-10 diagnosis was unchanged but a sedative treatment was offered more frequently. Overall, 85.9% of patients with sleep problems and recognized as cases were offered treatment, 53.9% drug treatment. The most frequent treatment consumed was stimulants, tonics or vitamins while the most frequently prescribed were antidepressants, hypnotic and anxiolytics with rather similar proportions. Drug consumption was substantially lower than drug prescription.
Herbicide Efficacy on Four Amaranthus Species in Soybean (Glycine max)

Field and greenhouse studies were conducted in 1995 and 1996 in Kansas to determine the efficacy of 21 herbicide treatments for control of tumble pigweed, Palmer amaranth, redroot pigweed, and two biotypes of common waterhemp in soybean. In field studies, nine of eleven preemergence treatments controlled all four species 90% or more. However, pendimethalin and trifluralin controlled Palmer amaranth, redroot pigweed, and tumble pigweed less than the other preplant incorporated and preemergence treatments. With the exception of flumiclorac and NAF-75, postemergence treatments controlled 75 to 90% of all four species. A biotype of common waterhemp collected in Iowa was not controlled by acetolactate synthase-inhibiting herbicides. Greenhouse results were similar to field experiments. Results suggest at least 90% control of these Amaranthus species is possible with proper herbicide selection.
Declines in traditional marine food intake and vitamin D levels from the 1960s to present in young Alaska Native women

Objective
To measure the trends in traditional marine food intake and serum vitamin D levels in Alaska Native women of childbearing age (20–29 years old) from the 1960s to the present.
DesignWe measured a biomarker of traditional food intake, the δ15N value, and vitamin D level, as 25-hydroxycholecalciferol (25(OH)D3) concentration, in 100 serum samples from 20–29-year-old women archived in the Alaska Area Specimen Bank, selecting twenty-five per decade from the 1960s to the 1990s. We compared these with measurements of red-blood-cell δ15N values and serum 25(OH)D3 concentrations from 20–29-year-old women from the same region collected during the 2000s and 2010s in a Center for Alaska Native Health Research study.

ResultsIntake of traditional marine foods, as measured by serum δ15N values, decreased significantly each decade from the 1960s through the 1990s, then remained constant from the 1990s through the present (F5,306=77·4, P<0·0001). Serum vitamin D concentrations also decreased from the 1960s to the present (F4,162=26·1, P<0·0001).
ConclusionsConsumption of traditional marine foods by young Alaska Native women dropped significantly between the 1960s and the 1990s and was associated with a significant decline in serum vitamin D concentrations. Studies are needed to evaluate the promotion of traditional marine foods and routine vitamin D supplementation during pregnancy for this population.
Antibiotic-associated diarrhoea in emergency department observation unit patients

Clostridium difficile diarrhoea is an urgent threat to patients, but little is known about the role of antibiotic administration that starts in emergency department observation units (EDOUs). We studied risk factors for antibiotic-associated diarrhoea (AAD) and C. difficile infection (CDI) in EDOU patients. This prospective cohort study enrolled adult patients discharged after EDOU antibiotic treatment between January 2013 and 2014. We obtained medical histories, EDOU treatment and occurrence of AAD and CDI over 28 days after discharge. We enrolled and followed 275 patients treated with antibiotics in the EDOU. We found that 52 (18·6%) developed AAD and four (1·5%) had CDI. Patients treated with vancomycin [relative risk (RR) 0·52, 95% confidence interval (CI) 0·3–0·9] were less likely to develop AAD. History of developing diarrhoea with antibiotics (RR 3·11, 95% CI 1·92–5·03) and currently failing antibiotics (RR 1·90, 95% CI 1·14–3·16) were also predictors of AAD. Patients with CDI were likely to be treated with clindamycin. In conclusion, AAD occurred in almost 20% of EDOU patients with risk factors including a previous history of diarrhoea with antibiotics and prior antibiotic therapy, while the risk of AAD was lower in patients receiving treatment regimens utilizing intravenous vancomycin.

To advance our restricted knowledge on mosquito biodiversity and distribution in Belgium, a national inventory started in 2007 (MODIRISK) based on a random selection of 936 collection points in three main environmental types: urban, rural and natural areas. Additionally, 64 sites were selected because of the risk of importing a vector or pathogen in these sites. Each site was sampled once between May and October 2007 and once in 2008 using Mosquito Magnet Liberty Plus traps. Diversity in pre-defined habitat types was calculated using three indices. The association between species and environmental types was assessed using a correspondence analysis. Twenty-three mosquito species belonging to traditionally recognized genera were found, including 21 indigenous and two exotic species. Highest species diversity (Simpson 0.765) and species richness (20 species) was observed in natural areas, although urban sites scored also well (Simpson 0.476, 16 species). Four clusters could be distinguished based on the correspondence analysis. The first one is related to human modified landscapes (such as urban, rural and industrial sites). A second is composed of species not associated with a specific habitat type, including the now widely distributed Anopheles plumbeus. A third group includes species commonly found in restored natural or bird migration areas, and a fourth cluster is composed of forest species. Outcomes of this study demonstrate the effectiveness of the designed sampling scheme and support the choice of the trap type. Obtained results of this first country-wide inventory of the Culicidae in Belgium may serve as a basis for risk assessment of emerging mosquito-borne diseases.

The solubilities of the common propellant ingredients acetyl triethyl citrate (ATEC) and cellulose acetate butyrate (CAB) and their effects on RDX (cyclotrimethylenetrinitramine) unit cell dimensions were investigated. If there is appreciable solid solubility, solutions will form and will have enthalpies of fusion, melting temperatures, and other characteristics different from those of pure RDX. It is desirable to establish the properties of such mixtures when designing new propellant formulations. Samples were aged at an elevated temperature to speed the formation of solid solutions. A least-squares analysis of X-ray diffraction data was used to obtain the lattice parameters from which unit cell volume and solubility was deduced. Both ATEC and CAB caused an expansion of the unit cell, indicating the formation of a solid solution. The limit of solubility in the ATEC/RDX mixture appeared to be approximately 13 wt% ATEC, while the CAB/RDX limit is above 16 wt% CAB. In both mixes, the cell volume expanded linearly with increasing proportion of binder.
Lattice Dynamics and Ionicity in the High-Temperature Superconductors

Using a non-empirical ionic model, we find harmonically unstable, double well potential surfaces for both La2CuO4 and YBa2Cu3O7 which may be related to the high temperature superconductivity. We have applied the Potential Induced Breathing (PIB) Model to La2CuO4 and YBa2Cu3O7 and find that the ionic description gives semiquantitative agreement with structural and vibrational observations. Furthermore, comparison with self-consistent linearized augmented plane wave [1] (LAPW) calculations shows that ionic contributions to the bonding and lattice dynamics of oxide superconductors are large. For La2CuO4 we find that an ionic description predicts the observed tetragonal to orthorhombic distortion, predicts a lower symmetry ground state, and predicts a stable oxygen breathing mode. We find harmonically unstable phonon branches that we relate to phase transitions. Unstable (anharmonic) modes will couple via ionic forces to the charge density in the metallic planes. This ionic coupling has not been included in previous lattice dynamics calculations. For YBa2Cu3O7 we find similar unstable phonon branches that involve oxygen motions perpendicular to the Cu-O bonds. We find the crystalline charge density is best approximated by overlapping ions if Cu and O ions of charge +1.6 and -1.8, respectively, are used.

This paper is one of a series relating the symmetry groups of the principal linear partial differential equations of mathematical physics and the coordinate systems in which variables separate for these equations. In particular, we mention [1] and paper [2] which is a survey of and introduction to the series. Here we apply group-theoretic methods to study the separable coordinate systems for the Helmholtz equation.
