If We Open the Door–An Analysis of Prehospital Emergency Cases with the Need to Force the Door Open from Dresden, Germany

Introduction:
Germany is a highly developed country. Nevertheless, there are reports that people have supply problems or die lonely and alone in their homes. Despite a differentiated social system, there seem to be significant gaps for individuals or old people. The rescue service is often involved in situations with unclear emergency calls or calls from third parties regarding social and supply problems. In some emergency cases the rescue service is alerted to someone's home but there is no response. Depending on the available information, a timely decision must be made to have the opportunity for life-saving. The door opening procedure is executed by the fire department. Afterward, the medical emergency teams take over. The goal of the study is to analyze findings discovered after opening the door.
Method:Data of all emergencies from the dispatch center of Dresden operated by the fire and rescue department between January 2021 and December 2021 were recorded and transferred to a central database. All cases with the need to force the door open were extracted and analyzed.
Results:There was a total number of 157.522 cases of emergency. In 847 cases the door was opened by the fire department. After door opening there was no emergency reported in 265 cases. 100 patients were found dead, six patients had cardiac arrest and received cardiopulmonary resuscitation, and 310 patients were transported to a hospital. The causes for emergencies were social problems, downfall and injuries, hypoglycemia, convulsion, stroke, psychiatric emergencies, dementia and suicide attempt. The mean age was 71 [18-103], 54,15% of patients were female.
Conclusion:There is a high number of emergencies with the need to open the door. The number of patients transported to a hospital is also high and justified the procedure. Since numerous patients were found dead, this underlines a gap in the German social system.

A consensus conference on the reasons for the undertreatment of depression was organized by the National Depressive and Manic Depressive Association (NDMDA) on January 17–18,1996. The target audience included health policymakers, clinicians, patients and their families, and the public at large. Six key questions were addressed: (1) Is depression undertreated in the community and in the clinic? (2) What is the economic cost to society of depression? (3) What have been the efforts in the past to redress undertreatment and how successful have they been? (4) What are the reasons for the gap between our knowledge of the diagnosis and treatment of depression and actual treatment received in this country? (5) What can we do to narrow this gap? (6) What can we do immediately to narrow this gap?

The main purpose of this paper is to provide experimental evidence that two syntactic reflexes of split intransitivity in German – the selection of perfective auxiliaries and the impersonal passive construction – are sensitive to an aspectual/thematic hierarchy of verb classes. We show that there is a split between ‘core’ verbs that elicit categorical intuitions from native speakers, and ‘intermediate’ verbs that exhibit gradience. Furthermore, crossdialectal differences between northern and southern German with respect to auxiliary selection tend to occur only with intermediate verbs. We argue that these findings lend support to the view that the unaccusative/unergative distinction is considerably more unstable than often assumed, and suggest that projectionist theories of the lexicon-syntax interface such as those directly derived from the Unaccusative Hypothesis may not be able to account for the systematic variation exhibited by the data.
Polymer Crystallization Studied in Confined Dimensions using Nanocomposites from Polymers and Layered Minerals

Polymer-layered silicate nanocomposites from polyamide-11 (PA-11) were prepared and the morphology and properties have been investigated in order to link the fundamental research field of polymer crystallization with the technical important field of composite materials. Semi-crystalline polymers are known to crystallize in different phases (e. g. monoclinic, hexagonal) forming chain-folded lamellar crystals. Depending on experimental conditions (e. g. temperature, pressure) the crystal size (lamellar thickness) affects the stability of these phases. The incorporation of layered silicates acting as hard walls into semi-crystalline polymers opens new possibilities to: i) study polymer crystallization in confined dimensions, and ii) provide materials from commodity or engineering polymers with enhanced properties. The effect of an external confinement introduced by highly anisotropic silicate layers of organically modified clay minerals on crystal growth and nanocomposite properties has been studied. The composites are prepared by in situ polycondensation of polyamides and/or blending via melt extrusion. The nanocomposites exhibit a homogeneous distribution of individual silicate layers at low clay contents. The lamellar thickening growth is reduced in polyamide crystallization due to the external constrained of the silicate layers in the host polymer. Furthermore these nanocomposites show a slightly enhanced thermal stability, tensile modulus and an increased elastic behavior over a broader temperature range. No distinct glass transition has been observed at highest clay contents.

In order to link the fundamental research field of polymer crystallization with the technical important field of composite materials polymer-layered silicate nanocomposites from polyethylene (PE) are prepared and their morphology and properties are investigated. The effect of an external confinement introduced by highly anisotropic silicate layers of organically modified clay minerals on crystal growth and nanocomposite properties has been studied. The prepared nanocomposites of organically modified clay minerals and PE exhibit not only a homogeneous distribution of individual silicate layers but also of tactoids. The isothermal crystallization rate of PE in the corresponding nanocomposite at 120 °C is increased in the composite material due to the action of the silicate layers. A significant effect on crystal phase has not been observed so far but from X-ray experiments, however the crystal size seems to be influenced. The thermal stability of the nanocomposites is significantly enhanced and the decomposition mechanism has been changed due to the presence of the layered silicates within the host polymer acting as a barrier for the diffusion of small molecules like oxygen. Due to the none permanent attachment of the polymer chains to the silicate surface the thermal-mechanical properties are only moderately enhanced at low temperature while the glass transition temperature remains unaffected.

The integration process can be intensified and accelerated not only by specialization resulting from broadening of markets through liberalization of trade but also through use of such instruments as agreements for complementary production within economic sectors … Title III, Declaration of Punta del Este, Montevideo, 1961.
The European pattern of economic integration has found expression in the Western Hemisphere in three politically divergent, widelyseparated regional groupings. One such group is the now-defunct West Indies Federation, a rearrangement of the remaining bits and pieces of British colonialism in the Caribbean region. Another is the Latin American Free Trade Association, straddling gaps from Mexico to Argentina, which appears to be a superficial bid for economic affinity among the more developed Latin American nations. But the most vigorous approach to regional economic integration is being pursued by the Organization for the Economic Development of Central America, which boldly proposes to weld at least five individually inefficient, insufferably nationalistic entities into one productive effort capable of initiating and sustaining economic take-off. Thus, in Central America, regional integration is being enforced even before national identity has fully emerged.
