There are numerous challenges pertaining to epilepsy care across Ontario, including Epilepsy Monitoring Unit (EMU) bed pressures, surgical access and community supports. We sampled the current clinical, community and operational state of Ontario epilepsy centres and community epilepsy agencies post COVID-19 pandemic. A 44-item survey was distributed to all 11 district and regional adult and paediatric Ontario epilepsy centres. Qualitative responses were collected from community epilepsy agencies. Results revealed ongoing gaps in epilepsy care across Ontario, with EMU bed pressures and labour shortages being limiting factors. A clinical network advising the Ontario Ministry of Health will improve access to epilepsy care.
Association between duration of antimicrobial prophylaxis and postoperative outcomes after lumbar spine surgery

Objectives:
To describe the association between duration of antimicrobial prophylaxis (AMP) and 30-day surgical site infection (SSI), 7-day acute kidney injury (AKI), 90-day Clostridioides difficile infection (CDI), prolonged hospitalization, and 30-day reoperation after lumbar spine surgery for noninfectious indications, and to report adherence to current guidelines.
Design:Survey.
Participants and setting:The study cohort comprised 6,198 patients who underwent lumbar spine surgery for noninfectious indications across 137 Veterans’ Health Administration surgery centers between 2016 and 2020.
Methods:Used univariate and multivariate logistic regression to determine the association between type and duration of AMP with 30-day SSI, 7-day AKI, 90-day CDI, prolonged hospitalization, and 30-day reoperation.
Results:Only 1,160 participants (18.7%) received the recommended duration of AMP. On multivariate analysis, the use of multiple prophylactic antimicrobials was associated with increased odds of 90-day CDI (adjusted odds ratio [aOR], 5.5; 95% confidence interval [CI], 1.1–28.2) and 30-day reoperation (aOR, 2.3; 95% CI, 1.2–4.4). Courses of antimicrobials ≥3 days were associated with increased odds of prolonged hospitalization (aOR,1.8; 95% CI, 1.4–2.3) and 30-day reoperation (aOR, 3.5; 95% CI, 2.2–5.7). In univariate analysis, increasing days of AMP was associated with a trend toward increasing odds of 90-day CDI (cOR, 1.4; 95% CI, 1.0–1.8 per additional day; P = .056).
Conclusions:Longer courses of AMP after lumbar spine surgery were associated with higher odds of CDI, prolonged hospitalization, and reoperation, but not with lower odds of SSI. However, adherence to the recommended duration of AMP is very low, hinting at a wide evidence-to-practice gap that needs to be addressed by spine surgeons and antimicrobial stewardship programs.

Background: Surgical instruments used in patients with prion disease must be decontaminated or decommissioned to prevent iatrogenic transmission. This is only done when the diagnosis of prion disease is known. To assess the potential for iatrogenic transmission, we determined the prevalence of surgeries and use of precautions in patients with prion disease at two academic medical centers. Methods: Clinical details, results of investigations, and surgical interventions (performed within one-year of symptom onset) were extracted for patients with probable/definite prion disease at Mayo Clinic (n=107; 1-2014 to 12-2020) and Washington University School of Medicine (n=14; 2-2015 to 12-2019). Results: Twenty-six patients (21.5%) underwent 32 procedures, including 2 high-risk procedures involving the brain. Most procedures (17/32, 53%) occurred in the 1-year period preceding the onset of symptoms attributed to CJD. History of arthritis (OR: 7.4, 95%CI: 1.05-51.8), lack of behavioral symptoms (OR: 3.0, 95%CI 0.97-9.1), and greater time (months) from symptom onset to first MRI (OR: 1.1, 95%CI 1.03-1.2) were independently associated with odds of undergoing an invasive procedure. Prion disease precautions were observed in one case (diagnostic brain biopsy). Conclusions: Procedures were common in patients with diagnosis of prion disease; precautions were not. Coordinated approaches to screening and reporting are needed to prevent iatrogenic transmission.
Geographic Access to Stroke Care Services in Rural Communities in Ontario, Canada

Background:
Optimal stroke care requires access to resources such as neuroimaging, acute revascularization, rehabilitation, and stroke prevention services, which may not be available in rural areas. We aimed to determine geographic access to stroke care for residents of rural communities in the province of Ontario, Canada.
Methods:We used the Ontario Road Network File database linked with the 2016 Ontario Acute Stroke Care Resource Inventory to estimate the proportion of people in rural communities, defined as those with a population size <10,000, who were within 30, 60, and 240 minutes of travel time by car from stroke care services, including brain imaging, thrombolysis treatment centers, stroke units, stroke prevention clinics, inpatient rehabilitation facilities, and endovascular treatment centers.
Results:Of the 1,496,262 people residing in rural communities, the majority resided within 60 minutes of driving time to a center with computed tomography (85%), thrombolysis (81%), a stroke unit (68%), a stroke prevention clinic (74%), or inpatient rehabilitation (77.0%), but a much lower proportion (32%) were within 60 minutes of driving time to a center capable of providing endovascular thrombectomy (EVT).
Conclusions:Most rural Ontario residents have appropriate geographic access to stroke services, with the exception of EVT. This information may be useful for jurisdictions seeking to optimize the regional organization of stroke care services.

Because polarization encodes geometrical information about unresolved scattering regions, it provides a unique tool for analyzing the 3-D structures of supernovae (SNe) and their surroundings. SNe of all types exhibit time-dependent spectropolarimetric signatures produced primarily by electron scattering. These signatures reveal physical phenomena such as complex velocity structures, changing illumination patterns, and asymmetric morphologies within the ejecta and surrounding material. Interpreting changes in polarization over time yields unprecedentedly detailed information about supernovae, their progenitors, and their evolution.
Begun in 2012, the SNSPOL Project continues to amass the largest database of time-dependent spectropolarimetric data on SNe. I present an overview of the project and its recent results. In the future, combining such data with interpretive radiative transfer models will further constrain explosion mechanisms and processes that shape SN ejecta, uncover new relationships among SN types, and probe the properties of progenitor winds and circumstellar material.

The number of cell types required for the construction of a metazoan body plan can serve as an index of morphological (or anatomical) complexity; living metazoans range from four (placozoans) to over 200 (hominids) somatic cell types. A plot of the times of origin of body plans against their cell type numbers suggests that the upper bound of complexity has increased more or less steadily from the earliest metazoans until today, at an average rate of about one cell type per 3 m.y. (when nerve cell types are lumped). Computer models in which increase or decrease in cell type number was random were used to investigate the behavior of the upper bound of cell type number in evolving clades. The models are Markovian; variance in cell type number increases linearly through time. Scaled to the fossil record of the upper bound of cell type numbers, the models suggest that early rates of increase in maximum complexity were relatively high. The models and the data are mutually consistent and suggest that the Metazoa originated near 600 Ma, that the metazoan “explosion” near the Precambrian/Cambrian transition was not associated with any important increase in complexity of body plans, and that important decreases in the upper bound of complexity are unlikely to have occurred.

During airway surgery, the anaesthetist may be required to manipulate or withdraw the endotracheal tube. Traditional surgical head drapes often make access to the tube difficult, therefore limiting control of the airway and risking de-sterilisation of the surgical field. We report a new method of draping for major neck operations that permits easy access to the endotracheal tube while maintaining sterility of the operative field.

An estimate of the benefits which would result from a ban on the sale of non-pasteurized milk in Scotland has been assessed by costing a recent outbreak of milk-borne salmonellosis in the Grampian Region.
The cost of such a ban would not exceed the benefits under any but the most severe assumptions about the values attached to intangible benefits
Prediction of methane emission from beef cattle using data measured in indirect open-circuit respiration calorimeters

The objectives of the present study were to examine relationships between methane (CH4) output and animal and dietary factors, and to use these relationships to develop prediction equations for CH4 emission from beef cattle. The dataset was obtained from 108 growing-to-finishing beef steers in five studies and CH4 production and energy metabolism data were measured in indirect respiration calorimeter chambers. Dietary forage proportion ranged from 29.5% to 100% (dry matter (DM) basis) and forages included grass silage, fresh grass, dried grass and fodder beet. Linear and multiple regression techniques were used to examine relationships between CH4 emission and animal and dietary variables, with the effects of experiment or forage type removed. Total CH4 emission was positively related to live weight (LW), feeding level and intake of feed (DM and organic matter) and energy (gross energy (GE), digestible energy (DE) and metabolisable energy (ME)) (P < 0.001), while CH4/DM intake (DMI) was negatively related to energy digestibility and ME/GE (P < 0.05 or less). Using LW alone to predict CH4 emission produced a poor relationship when compared to DMI and GE intake (GEI) (R2 = 0.26 v. 0.68 and 0.70 respectively). Adding feeding level, dietary NDF concentration and CP/ME or feeding level, energy digestibility and ME/GE to support LW resulted in a R2 of 0.66 or 0.84. The high R2 (0.84) was similar to that obtained using DMI or GEI together with energy digestibility and ME/GE as predictors. Further inclusion of dietary forage proportion and ADF and NDF concentration to the multiple relationships using GEI as the primary predictor resulted in a R2 of 0.87. These equations were evaluated through internal validation, by developing a range of similar new equations from two-thirds of the present data and then validating these new equations with the remaining one-third of data. The validation indicated that addition of energy digestibility and ME/GE to support LW with feeding level, DMI and GEI considerably increased the prediction accuracy. It is concluded that CH4 emission of beef steers can be accurately predicted from LW plus feeding level, DMI or GEI together with energy digestibility and ME/GE. The dataset was also used to validate a range of prediction equations for CH4 production of cattle published elsewhere.

A collaborative exercise, supervised by the World Health Organisation, was set up to compare ELISAs used for the serological detection of Salmonella enteritica serotype Enteritidis in chickens. The aim was to ascertain how far agreement could be reached on the interpretation of optical density readings for high titre, intermediate titre and low titre sera. Two sets of sera were sent to 14 participants. The first set compared high, medium and low titre sera raised in specified-pathogen-free and commercial broiler breeder chickens. The second set comprised 20 sera of different antibody titres raised in commercial birds reared under laboratory conditions and sent blind. Both indirect and double-antibody sandwich blocking ELISAs were used with a number of different detecting antigens. With a few exceptions good agreement was reached on the interpretation of results obtained from high and low titre sera from the optical density obtained with a single serum dilution. Differences were observed in the interpretation of medium titre sera. The results suggested that most ELISAs produce reasonably comparable results and that practical problems may arise from interpretation of the results mainly as a result of the choice of the criteria used for differentiating sera obtained from infected and uninfected chickens. These problems are discussed.
Prediction of empty body weight and composition from live weight and other live animal measurements in lactating dairy cows

Empty body (EB) composition data from 146 lactating Holstein–Friesian dairy cows were analysed, with cows selected from a large herd to represent a wide range of animal factors including parity, live weight (LW), body condition score (BCS), milk yield and stage of lactation. The objectives of the study were to examine possible relationships between EB composition and live animal characteristics and to develop prediction equations for EB weight (EBW) and EB composition. EBW was strongly related to LW with a R2 of 0·90, and addition of BCS and stage of lactation increased the R2 to 0·93 and reduced the s.e. value from 19·8 to 16·8. There was a strong relationship between LW and EB mass of crude protein (CP) or water (R2=0·89 or 0·85), and addition of BCS, milk yield and stage of lactation as supporting predictors gave little improvement in the relationship between LW and EB mass of CP or water. Similar results were also obtained for the prediction of EB ash mass, but the relationship between EB ash mass and LW or a combination of LW and parity was relatively poor (R2=0·59 or 0·63). However, including BCS and milk yield as supporting predictors in the linear relationship between LW and EB masses of lipid (0·51 v. 0·79) and dry matter (DM) (0·79 v. 0·91) and total gross energy (GE) content (0·66 v. 0·85) considerably increased the R2 values. Similar linear and multiple relationships were also developed to predict EB component proportions of lipid, CP, GE, DM and water, and the R2 values were much smaller than those for EB component masses. The equations for prediction of EB component masses were evaluated through internal validation, by developing a range of similar new equations from two-thirds of the present data and then validating these new equations with the remaining one-third of data. The validation indicated that addition of other live animal factors to support LW to predict EBW and EB masses of lipid, GE and DM considerably increased the prediction accuracy, but had little effect on the prediction accuracy for EB masses of CP, water and ash. It is concluded that in lactating dairy cows, LW is a very good predictor of EB masses of CP and water. Accurate prediction of EBW and EB masses of lipid, GE and DM can be obtained using LW together with BCS, milk yield and stage of lactation.
