Flow pulsatility is ubiquitous in biological and biomedical fluid dynamics. Cardiovascular flow, the most well-studied pulsatile flow, has benefited from decades of fundamental studies of the temporal influences of basic parameter changes in simplified models. A similar approach is employed herein on a relatively unstudied, canonical flow configuration. Using experiments and simulations, we examine highly pulsatile flow over a surface-mounted bluff body. The pulsatile (i.e. with no flow reversal) inflow waveform is sinusoidal, and the inflow pulsation frequency is varied from low-frequency, quasi-steady pulsation to high-frequency pulsatility. A wake regime map encompassing the range of pulsation frequency is created, and a mechanistic explanation of the regimes observed is put forth. Finally, we introduce a non-dimensional parameter applicable to pulsatile flows and point out remarkable similarity to the formation time parameter associated with vortex ring generation, including a similar critical value, despite substantive differences in the flow configurations.

Objectives
A panel of emergency medicine (EM) leaders endeavoured to define the key elements of leadership and its models, as well as to formulate consensus recommendations to build and strengthen academic leadership in the Canadian EM community in the areas of mentorship, education, and resources.
MethodsThe expert panel comprised EM leaders from across Canada and met regularly by teleconference over the course of 9 months. From the breadth of backgrounds and experience, as well as a literature review and the development of a leadership video series, broad themes for recommendations around the building and strengthening of EM leadership were presented at the CAEP 2015 Academic Symposium held in Edmonton, Alberta. Feedback from the attendees (about 80 emergency physicians interested in leadership) was sought. Subsequently, draft recommendations were developed by the panel through attendee feedback, further review of the leadership video series, and expert opinion. The recommendations were distributed to the CAEP Academic Section for further feedback and updated by consensus of the expert panel.
ResultsThe methods informed the panel who framed recommendations around four themes: 1) leadership preparation and training, 2) self-reflection/emotional intelligence, 3) academic leadership skills, and 4) gender balance in academic EM leadership. The recommendations aimed to support and nurture the next generation of academic EM leaders in Canada and included leadership mentors, availability of formal educational courses/programs in leadership, self-directed education of aspiring leaders, creation of a Canadian subgroup with the AACEM/SAEM Chair Development Program, and gender balance in leadership roles.
ConclusionsThese recommendations serve as a roadmap for all EM leaders (and aspiring leaders) to build on their success, inspire their colleagues, and foster the next generation of Canadian EM academic leaders.
Geographic Distribution of Disaster-Specific Emergency Department Use After Hurricane Sandy in New York City

Objective
We aimed to characterize the geographic distribution of post-Hurricane Sandy emergency department use in administrative flood evacuation zones of New York City.
MethodsUsing emergency claims data, we identified significant deviations in emergency department use after Hurricane Sandy. Using time-series analysis, we analyzed the frequency of visits for specific conditions and comorbidities to identify medically vulnerable populations who developed acute postdisaster medical needs.
ResultsWe found statistically significant decreases in overall post-Sandy emergency department use in New York City but increased utilization in the most vulnerable evacuation zone. In addition to dialysis- and ventilator-dependent patients, we identified that patients who were elderly or homeless or who had diabetes, dementia, cardiac conditions, limitations in mobility, or drug dependence were more likely to visit emergency departments after Hurricane Sandy. Furthermore, patients were more likely to develop drug-resistant infections, require isolation, and present for hypothermia, environmental exposures, or administrative reasons.
ConclusionsOur study identified high-risk populations who developed acute medical and social needs in specific geographic areas after Hurricane Sandy. Our findings can inform coherent and targeted responses to disasters. Early identification of medically vulnerable populations can help to map “hot spots” requiring additional medical and social attention and prioritize resources for areas most impacted by disasters. We describe eight cases of primary cerebral lymphoma seen in Manitoba from 1980 to 1985.
The clinical presentation is similar to other primary brain tumors. The diagnosis should be considered when single or multiple, often deep lesions, show dense enhancement on computerized tomographic (CT) scan, but are avascular at angiography. These tumors are histologically indistinguishable from non-Hodgkins lymphomas arising outside the CNS.
The prognosis is poor. However, radiotherapy with or without surgery may offer significant palliation. Although there is no consensus on the value of chemotherapy, corticosteroids alone or multiagent chemotherapy have shown promise in a few cases. For these reasons, histologic diagnosis should be sought in all cases and surgery, radiotherapy, and chemotherapy should be considered, as prolonged survival is possible.

The evidence for variations in winter phosphate concentration as a component of the ‘Russell Cycle’ has been re-examined. The change from high phosphate concentrations prior to 1930 is the largest apparent variation in concentration through the period 1923–1987. However, these values are the result of a retrospective application in 1938, of a salt correction factor of 1–35. The justification for this factor is examined. It is concluded that there is sufficient uncertainty in the accuracy of the pre-1948 measurements of phosphate concentration to preclude their use in long time-series analysis. A statistical analysis of the post-1948 data shows that a simple first order autoregression model provides a perfectly adequate description of the data; the data can be considered as essentially random, with a small component of carry over from one year to the next. The evidence is weak that variations in winter concentrations of phosphate support the Russell Cycle hypothesis.

The pyroelectric coefficients of a range of polymers with a methacrylate backbone and nitrostilbene or nitrobipheny mesogens attached by a 3 or 6 carbon spacer have been measured in order to assess the efficiency of the poling process. It is found that the response is much lower than the guest-host system MNA in PMMA, suggesting that the polar mesogens pack anti-parallel and that this alignment is not broken by the applied field.
These strong interactions also provide an explanation for the absence of the β relaxation process in the experimental polymers and therefore the brittle nature of the films. They may also explain the stability of the polarisation achieved.
