Clinical diagnosis of Lewy body dementia

Background
Lewy body dementia, consisting of both dementia with Lewy bodies (DLB) and Parkinson's disease dementia (PDD), is considerably under-recognised clinically compared with its frequency in autopsy series.
AimsThis study investigated the clinical diagnostic pathways of patients with Lewy body dementia to assess if difficulties in diagnosis may be contributing to these differences.
MethodWe reviewed the medical notes of 74 people with DLB and 72 with non-DLB dementia matched for age, gender and cognitive performance, together with 38 people with PDD and 35 with Parkinson's disease, matched for age and gender, from two geographically distinct UK regions.
ResultsThe cases of individuals with DLB took longer to reach a final diagnosis (1.2 v. 0.6 years, P = 0.017), underwent more scans (1.7 v. 1.2, P = 0.002) and had more alternative prior diagnoses (0.8 v. 0.4, P = 0.002), than the cases of those with non-DLB dementia. Individuals diagnosed in one region of the UK had significantly more core features (2.1 v. 1.5, P = 0.007) than those in the other region, and were less likely to have dopamine transporter imaging (P < 0.001). For patients with PDD, more than 1.4 years prior to receiving a dementia diagnosis: 46% (12 of 26) had documented impaired activities of daily living because of cognitive impairment, 57% (16 of 28) had cognitive impairment in multiple domains, with 38% (6 of 16) having both, and 39% (9 of 23) already receiving anti-dementia drugs.
ConclusionsOur results show the pathway to diagnosis of DLB is longer and more complex than for non-DLB dementia. There were also marked differences between regions in the thresholds clinicians adopt for diagnosing DLB and also in the use of dopamine transporter imaging. For PDD, a diagnosis of dementia was delayed well beyond symptom onset and even treatment.
A Pediatric Cardiology Fellowship Boot Camp improves trainee confidence*

Introduction
New paediatric cardiology trainees are required to rapidly assimilate knowledge and gain clinical skills to which they have limited or no exposure during residency. The Pediatric Cardiology Fellowship Boot Camp (PCBC) at Boston Children’s Hospital was designed to provide incoming fellows with an intensive exposure to congenital cardiac pathology and a broad overview of major areas of paediatric cardiology practice.
MethodsThe PCBC curriculum was designed by core faculty in cardiac pathology, echocardiography, electrophysiology, interventional cardiology, exercise physiology, and cardiac intensive care. Individual faculty contributed learning objectives, which were refined by fellowship directors and used to build a programme of didactics, hands-on/simulation-based activities, and self-guided learning opportunities.
ResultsA total of 16 incoming fellows participated in the 4-week boot camp, with no concurrent clinical responsibilities, over 2 years. On the basis of pre- and post-PCBC surveys, 80% of trainees strongly agreed that they felt more prepared for clinical responsibilities, and a similar percentage felt that PCBC should be offered to future incoming fellows. Fellows showed significant increase in their confidence in all specific knowledge and skills related to the learning objectives. Fellows rated hands-on learning experiences and simulation-based exercises most highly.
ConclusionsWe describe a novel 4-week-long boot camp designed to expose incoming paediatric cardiology fellows to the broad spectrum of knowledge and skills required for the practice of paediatric cardiology. The experience increased trainee confidence and sense of preparedness to begin fellowship-related responsibilities. Given that highly interactive activities were rated most highly, boot camps in paediatric cardiology should strongly emphasise these elements.

Genetic diversity in the USDA Limnanthes germplasm collection assessed by simple sequence repeats

The genus Limnanthes (Limnanthaceae), also known as meadowfoam, has attracted attention for industrial use due to the unique characteristics of its seed oil. Samples from wild populations showed variability in agronomically important traits involved in seed oil yield, warranting the establishment and continued development of a germplasm collection. The level of genetic diversity within the United States Department of Agriculture, Agriculture Research Service, National Plant Germplasm System (USDA-ARS-NPGS) Limnanthes collection was evaluated using 15 simple sequence repeat (SSR) markers across 62 accessions representing 7 species. Parsimony analysis separated the accessions into two main groups consistent with the traditional taxonomic sections Inflexae and Reflexae, but there was little resolution within groups. These two groups were confirmed using neighbour-joining analysis and principal coordinate analysis. SSR marker variation suggests that the Limnanthes germplasm collection is genetically diverse and the accessions within the species likely contain novel alleles, and therefore the collection contributes to the conservation of the wild Limnanthes gene pool.

We describe an unusual heart in which double outlet from the left atrium resulted in atrioventricular connections to a dominant left ventricle and to a left-sided rudimentary right ventricle, while the right atrium also connected to the left ventricle. This produced, in effect, double inlet left ventricle in association with discordant atrioventricular connections. The diagnosis was made by echocardiography and confirmed at post mortem examination.

The dominance within the EFL world of the Jones transcription system (DJ), as in the EPD (Jones, 1967), has been seriously challenged by the adoption of the new Windsor Lewis system (WL) in the Concise Pronouncing Dictionary (Lewis, 1972a) and the new edition of the ALD (Hornby, 1974). This issue (recently discussed in, e.g., Lewis, 1972b, and Walsh, 1974) is likely to remain a lively one at least until the appearance of the simplified Gimson system in the next edition of the EPD ; meanwhile there are other systems that have some claim to use in EFL teaching, e.g. those in Gimson (1970) (G), Abercrombie (1964), and the Edinburgh Course in Applied Linguistics (Allen and Corder, 1974/1975).
