Background
Studying phenotypic and genetic characteristics of age at onset (AAO) and polarity at onset (PAO) in bipolar disorder can provide new insights into disease pathology and facilitate the development of screening tools.
AimsTo examine the genetic architecture of AAO and PAO and their association with bipolar disorder disease characteristics.
MethodGenome-wide association studies (GWASs) and polygenic score (PGS) analyses of AAO (n = 12 977) and PAO (n = 6773) were conducted in patients with bipolar disorder from 34 cohorts and a replication sample (n = 2237). The association of onset with disease characteristics was investigated in two of these cohorts.
ResultsEarlier AAO was associated with a higher probability of psychotic symptoms, suicidality, lower educational attainment, not living together and fewer episodes. Depressive onset correlated with suicidality and manic onset correlated with delusions and manic episodes. Systematic differences in AAO between cohorts and continents of origin were observed. This was also reflected in single-nucleotide variant-based heritability estimates, with higher heritabilities for stricter onset definitions. Increased PGS for autism spectrum disorder (β = −0.34 years, s.e. = 0.08), major depression (β = −0.34 years, s.e. = 0.08), schizophrenia (β = −0.39 years, s.e. = 0.08), and educational attainment (β = −0.31 years, s.e. = 0.08) were associated with an earlier AAO. The AAO GWAS identified one significant locus, but this finding did not replicate. Neither GWAS nor PGS analyses yielded significant associations with PAO.
ConclusionsAAO and PAO are associated with indicators of bipolar disorder severity. Individuals with an earlier onset show an increased polygenic liability for a broad spectrum of psychiatric traits. Systematic differences in AAO across cohorts, continents and phenotype definitions introduce significant heterogeneity, affecting analyses.

The first demonstration of laser action in ruby was made in 1960 by T. H. Maiman of Hughes Research Laboratories, USA. Many laboratories worldwide began the search for lasers using different materials, operating at different wavelengths. In the UK, academia, industry and the central laboratories took up the challenge from the earliest days to develop these systems for a broad range of applications. This historical review looks at the contribution the UK has made to the advancement of the technology, the development of systems and components and their exploitation over the last 60 years.

Despite many advances in recent years for patients with critical paediatric and congenital cardiac disease, significant variation in outcomes remains across hospitals. Collaborative quality improvement has enhanced the quality and value of health care across specialties, partly by determining the reasons for variation and targeting strategies to reduce it. Developing an infrastructure for collaborative quality improvement in paediatric cardiac critical care holds promise for developing benchmarks of quality, to reduce preventable mortality and morbidity, optimise the long-term health of patients with critical congenital cardiovascular disease, and reduce unnecessary resource utilisation in the cardiac intensive care unit environment. The Pediatric Cardiac Critical Care Consortium (PC4) has been modelled after successful collaborative quality improvement initiatives, and is positioned to provide the data platform necessary to realise these objectives. We describe the development of PC4 including the philosophical, organisational, and infrastructural components that will facilitate collaborative quality improvement in paediatric cardiac critical care.

The articles collected here bear witness to the continued and wide interest in England and its neighbours in the "long" thirteenth century. The volume includes papers on the high politics of the thirteenth century, international relations, the administrative and governmental structures of medieval England and aspects of the wider societal and political context of the period. A particular theme of the papers is Anglo-French political history, and especially the ways in which that relationship was reflected in the diplomatic and dynastic arrangements associated with the Treaty of Paris, the 750th anniversary of which fell during 2009, a fact celebrated in this collection of essays and the Paris conference at which the original papers were first delivered.

Objective.
Hospital-acquired Legionella pneumonia has a fatality rate of 28%, and the source is the water distribution system. Two prevention strategies have been advocated. One approach to prevention is clinical surveillance for disease without routine environmental monitoring. Another approach recommends environmental monitoring even in the absence of known cases of Legionella pneumonia. We determined the Legionella colonization status of water systems in hospitals to establish whether the results of environmental surveillance correlated with discovery of disease. None of these hospitals had previously experienced endemic hospital-acquired Legionella pneumonia.
Design.Cohort study.
Setting.Twenty US hospitals in 13 states.
Interventions.Hospitals performed clinical and environmental surveillance for Legionella from 2000 through 2002. All specimens were shipped to the Special Pathogens Laboratory at the Veterans Affairs Pittsburgh Medical Center.
Results.Legionella pneumophila and Legionella anisa were isolated from 14 (70%) of 20 hospital water systems. Of 676 environmental samples, 198 (29%) were positive for Legionella species. High-level colonization of the water system (30% or more of the distal outlets were positive for L. pneumophila) was demonstrated for 6 (43%) of the 14 hospitals with positive findings. L. pneumophila serogroup 1 was detected in 5 of these 6 hospitals, whereas 1 hospital was colonized with L. pneumophila serogroup 5. A total of 633 patients were evaluated for Legionella pneumonia from 12 (60%) of the 20 hospitals: 377 by urinary antigen testing and 577 by sputum culture. Hospital-acquired Legionella pneumonia was identified in 4 hospitals, all of which were hospitals with L. pneumophila serogroup 1 found in 30% or more of the distal outlets. No cases of disease due to other serogroups or species (L. anisa) were identified.
Conclusion.Environmental monitoring followed by clinical surveillance was successful in uncovering previously unrecognized cases of hospital-acquired Legionella pneumonia.
Neuropsychological deficits associated with driving performance in Parkinson's and Alzheimer's disease

Neuropsychological and motor deficits in Parkinson's disease that may contribute to driving impairment were examined in a cohort study comparing patients with Parkinson's disease (PD) to patients with Alzheimer's disease (AD) and to healthy elderly controls. Nondemented individuals with Parkinson's disease [Hoehn & Yahr (H&Y) stage I–III], patients with Alzheimer's disease [Clinical Demetia Rating scale (CDR) range 0–1], and elderly controls, who were actively driving, completed a neuropsychological battery and a standardized road test administered by a professional driving instructor. On-road driving ability was rated on number of driving errors and a global rating of safe, marginal, or unsafe. Overall, Alzheimer's patients were more impaired drivers than Parkinson's patients. Parkinson's patients distinguished themselves from other drivers by a head-turning deficiency. Drivers with neuropsychological impairment were more likely to be unsafe drivers in both disease groups compared to controls. Compared to controls, unsafe drivers with Alzheimer's disease were impaired across all neuropsychological measures except finger tapping. Driving performance in Parkinson's patients was related to disease severity (H&Y), neuropsychological measures [Rey Osterreith Complex Figure (ROCF), Trails B, Hopkins Verbal List Learning Test (HVLT)-delay], and specific motor symptoms (axial rigidity, postural instability), but not to the Unified Parkinson Disease Rating Scale (UPDRS) motor score. Multifactorial measures (ROCF, Trails B) were useful in distinguishing safe from unsafe drivers in both patient groups. (JINS, 2005, 11, 766–775.)

Summary
Introduction
Advances in landscape ecology, ecosystem management, geographic information systems, and remote sensing have led us from the stand, to the landscape, and to broader scales in natural resources planning and management. As science and management have expanded to these scales, they frequently encompass multi-ownership landscapes. The management and scientific challenges posed by multi-ownership landscapes are especially complex. Species and ecosystems do not recognize legal boundaries between ownerships (Forman, 1995; Landres et al., 1998), and the landscape dynamics of individual ownerships is controlled by a complex of economic, social, political, and biophysical forces. The aggregate ecological conditions of landscapes are controlled by the spatial pattern and dynamics of individual owners and ecological interactions among those ownerships. Solutions to problems of conservation policy and practices for multi-ownership landscapes do not lie in isolated owner-by-owner planning and management. Broader scale approaches are needed. Work in multi-ownership landscapes also reveals the need for increased integration among ecological and social sciences. In most contemporary landscapes, the dominant disturbance regimes are directly or indirectly controlled by human activities. In this chapter we will present a case study to demonstrate the importance of taking a multi-ownership view of landscapes and describe an approach we are developing to assess the effects of different forest management policies on ecological components of a province (i.e., subregion) in coastal Oregon.

Background
Studies suggest that symptoms of traumatic grief constitute a distinct syndrome worthy of diagnosis.
AimsA consensus conference aimed to develop and test a criteria set for traumatic grief.
MethodThe expert panel proposed consensus criteria for traumatic grief. Receiver operator characteristic (ROC) analyses tested the performance of the proposed criteria on 306 widowed respondents at seven months post-loss.
ResultsROC analyses indicated that three of four separation distress symptoms (e.g. yearning, searching, loneliness) had to be endorsed as at least ‘sometimes true’ and four of the final eight traumatic distress symptoms (e.g. numbness, disbelief, distrust, anger, sense of futility about the future) had to be endorsed as at least ‘mostly true’ to yield a sensitivity of 0.93 and a specificity of 0.93 for a diagnosis of traumatic grief.
ConclusionsPreliminary analyses suggest the consensus criteria for traumatic grief have satisfactory operating characteristics, and point to directions for further refinement of the criteria set.

The report on partial rescue excavations of the Collfryn enclosure between 1980–82 presents a summary of the first large-scale investigation of one of the numerous semi-defensive cropmark and earthwork enclosure sites in the upper Severn valley in mid-Wales. Earlier prehistoric activity of an ephemeral nature is represented by a scattering of Mesolithic and Late Neolithic or early Bronze Age flintwork, and by a pit containing sherds of several different Beaker vessels. The first enclosed settlement, constructed in about the 3rd century bc probably consisted of three widely-spaced concentric ditches, associated with banks of simple dump construction, having a single gated entranceway on the downhill side. It covered an area of about 2.5 ha and appears to have been of a relatively high social status, and appropriate in size for a single extended-family group. This was subsequently reduced in about the 1st century bc to a double-ditched enclosure, by the recutting of the original inner ditch and the cutting of a new ditch immediately outside it. The habitation area between the 3rd and 1st centuries bc probably focused on timber buildings in the central enclosure of about 0.4 ha, whose gradually evolving pattern appears to have comprised between 3–4 roundhouses and 4–5 four-posters at any one time. Little excavation was undertaken between the outer ditches of the first phase settlement, but these are assumed to have been used as stock enclosures. A mixed farming economy is suggested by cattle, sheep/goat and pig remains, and remains of glume wheats, barley and oats. Industries included small-scale iron and bronze-working. The Iron Age settlement was essentially aceramic, although there are significant quantities of a coarse, oxidized ceramic probably representing salt traded from production centres in the Cheshire Plain. The entranceway was remodelled in about the late 1st or early 2nd, century AD by means of a timber-lined passage linked to a new gate on the line of the inner bank. There is equivocal evidence of continued occupation within the inner enclosure continuing until at least the mid-4th century AD, possibly at a comparatively low social level, associated with domestic structures of uncertain form sited on earlier roundhouse platforms, and including some four-posters and possible six-posters. Drainage ditches were dug across parts of the site during the Medieval and post-Medieval periods, which were associated with various structures, including a corn-drying kiln inserted into the inner enclosure bank in the 15th century.
