A new fossil site in a previously unexplored part of western Madagascar (the Beanka Protected Area) has yielded remains of many recently extinct vertebrates, including giant lemurs (Babakotia radofilai, Palaeopropithecus kelyus, Pachylemur sp., and Archaeolemur edwardsi), carnivores (Cryptoprocta spelea), the aardvark-like Plesiorycteropus sp., and giant ground cuckoos (Coua). Many of these represent considerable range extensions. Extant species that were extirpated from the region (e.g., Prolemur simus) are also present. Calibrated radiocarbon ages for 10 bones from extinct primates span the last three millennia. The largely undisturbed taphonomy of bone deposits supports the interpretation that many specimens fell in from a rock ledge above the entrance. Some primates and other mammals may have been prey items of avian predators, but human predation is also evident. Strontium isotope ratios (87Sr/86Sr) suggest that fossils were local to the area. Pottery sherds and bones of extinct and extant vertebrates with cut and chop marks indicate human activity in previous centuries. Scarcity of charcoal and human artifacts suggests only occasional visitation to the site by humans. The fossil assemblage from this site is unusual in that, while it contains many sloth lemurs, it lacks ratites, hippopotami, and crocodiles typical of nearly all other Holocene subfossil sites on Madagascar.

1. During a period of 1 year the incidence of infection with haemolytic streptococci was determined among children on admission to, and during residence in, the children's wards of University College Hospital, London.
2. The increased incidence among those in residence, 13·1% of whom acquired haemolytic streptococci during their stay, was due principally to outbreaks of infection by these organisms. By the slide agglutination method the majority of strains could be assigned to one or other of Griffith's thirty types and the presumed path of their spread studied.
3. The source of the infection in most of the outbreaks was not ascertain-able from the data collected, but in general it did not appear to be in the patients themselves.
4. With a few exceptions, isolation by cubicles was an effective measure in preventing the spread of haemolytic streptococci.
5. In general the haemolytic streptococci seemed to spread from patient to patient, though the exact mode of spread was not ascertained.
6. The clinical manifestations of infection by any one type of haemolytic streptococcus was varied, e.g. wound suppuration, sore throat, pyrexia, otitis media. Many of the infections were “latent”.
7. In addition to the haemolytic streptococcal outbreaks, there were two outbreaks of diphtheria (fifteen patients), two of Bact. sonnei dysentery (ten patients and one nurse), one of catarrhal jaundice (one patient and three nurses), six cases of intercurrent gastro-enteritis of infancy and one case of measles.
8. In all, at least 104 (or 18·9%) of the 551 patients of these wards developed intercurrent infections, manifest or latent, during their stay in hospital.

1. The incidence of cross-infection with Str. pyogenes during a period of four months was compared in two blocks occupied by scarlet-fever patients. One block was nursed by the bed isolation method and the other by the ordinary method in current use for multiple-bed wards. 2. The rules for the bed isolation blook are given. 3. The incidence of cross-infection with Str. pyogenes among patients in the bed isolation block was 20·3%, that among patients in the ordinary block 22·6%. 4. We concluded that the bed isolation nursing technique practised was not effective in reducing cross-infection with Str. pyogenes among scarlet-fever patients. 5. A further test of bed isolation nursing was made for six months, using the extra precaution of vacuum-cleaning the floors. We found that crossinfection was still occurring, but the incidence could not be estimated since the Str. pyogenes strains isolated had to be destroyed at the outbreak of war. At this time the serological typing had not been completed. 6. The importance of dust-borne and droplet-borne infection in addition to that of contact infection is discussed. The type of investigation required to decide whether cross-infection in multiple-bed scarlet-fever wards can be eliminated by the methods at our present disposal is outlined.

Biochemical and serological investigations were made upon Bact. coli O 55 B 5 strains isolated from infants with diarrhoea and vomiting. On the basis of their reactions with six fermentable substances and of their H antigen types, the strains could be differentiated into three sero-fermentative types, I, II and III, which comprised, respectively, those having H6, H2 and H7 antigens.
We wish to acknowledge our thanks and indebtedness to the following: the Chairman and Members of the Tottenham Group Hospital Management Committee, and Dr T. H. C. Benians, Dr W. I. Leslie and Dr R. Swyer, for facilities for research in the laboratory and wards of St Ann's General Hospital; Mr L. G. C. Maslen for most valuable advice and assistance; the entire laboratory staff for their willing and efficient help.

Aims and Method
We conducted a questionnaire study to establish the incidence, specialist staff availability, treatment provision and socio-demographic profile of patients with first-episode psychosis referred to all adult and child and adolescent community mental health teams in south and west London.
ResultsAll 39 teams completed the questionnaire, identifying 295 cases of first-episode psychosis (annual incidence 21/100 000/year) referred in the year 2000. Teams manage to engage most patients with first-episode psychosis. A total of 73% of cases of first-episode psychosis were on some form of Care Programme Approach. However, many teams did not have adequately trained staff to provide psychosocial interventions. Even where such staff were available, care was focused mainly on monitoring medication and risk assessment, with only half the teams providing psycho-educational programmes and only a quarter offering individual cognitive–behavioural therapy to those with first-episode psychosis.
Clinical ImplicationsEstablishing early intervention services nationwide will require significant new resources, including specialist trained staff, which could prove difficult to provide in inner-city areas. Rather than a single, uniform service model, several models of early intervention services based on locally determined need might be more realistic and appropriate, and also allow research into their relative efficacy.
