Conceptualising socially inclusive environmental policy: a just transition to Net Zero

The policy area addressing the climate crisis in the UK, ‘Net Zero’, will affect many aspects of people’s everyday life. Given that policy builds from where we are now, which for some (post austerity, and mid cost of living crisis) means in financial crisis, there is work to be done in enabling a socially inclusive Net Zero. In this article, we modify the Bristol Social Exclusion Matrix’s four forms of participation for social inclusion, drawing on the existing literature on the social risks of environmental policy, to articulate the risks of social exclusion in transition to Net Zero. This enables us to develop a ‘person-centred’ approach to understanding the risks of Net Zero, articulating the risks of exclusion, and who is likely to be affected by them. We conclude by outlining a framework for an inclusive transition, and commenting on the policy and research implications of our thinking.

Nutrition during the periconceptional period influences postnatal cardiovascular health. We determined whether in vitro embryo culture and transfer, which are manipulations of the nutritional environment during the periconceptional period, dysregulate postnatal blood pressure and blood pressure regulatory mechanisms. Embryos were either transferred to an intermediate recipient ewe (ET) or cultured in vitro in the absence (IVC) or presence of human serum (IVCHS) and a methyl donor (IVCHS+M) for 6 days. Basal blood pressure was recorded at 19–20 weeks after birth. Mean arterial pressure (MAP) and heart rate (HR) were measured before and after varying doses of phenylephrine (PE). mRNA expression of signaling molecules involved in blood pressure regulation was measured in the renal artery. Basal MAP did not differ between groups. Baroreflex sensitivity, set point, and upper plateau were also maintained in all groups after PE stimulation. Adrenergic receptors alpha-1A (αAR1A), alpha-1B (αAR1B), and angiotensin II receptor type 1 (AT1R) mRNA expression were not different from controls in the renal artery. These results suggest there is no programmed effect of ET or IVC on basal blood pressure or the baroreflex control mechanisms in adolescence, but future studies are required to determine the impact of ET and IVC on these mechanisms later in the life course when developmental programming effects may be unmasked by age.

Background
Abstract presentations of scientific information at meetings are important for broadcasting new information. Publication of these studies should be the final goal, but minimal data exist documenting publication rates, especially for paediatric sub-speciality meetings. The goal of this study was to document the manuscript publication rate for paediatric cardiac echocardiography abstracts and to determine whether there were differences between abstracts that were published versus not published.
MethodsPaediatric cardiac echocardiography abstracts presented from 2007 to 2011 at the American Society of Echocardiography Meetings were reviewed. Characteristics of the abstracts were noted. A Medline/Pubmed search was performed using keywords, first author, and senior author criteria to determine publication. Fisher’s exact tests or χ2 tests were used for analysis.
ResultsA total of 194 abstracts were reviewed. In all, 27 abstracts were oral presentations and 167 were poster presentations. A total of 124 abstracts were prospective studies and 70 were retrospective studies; 11 abstracts were basic science studies and 183 were clinical studies. Altogether, 25 abstracts dealt with three-dimensional echocardiography, 15 with fetal echocardiography, 56 with deformation analysis, 79 with standard transthoracic echocardiography, and 19 were in the other category. A total of 73 abstracts were subsequently published – with a 37.6% publication rate – 2.1±1.7 years after initial presentation. There were no significant differences in publication rates based on the above-noted variables.
ConclusionA paediatric cardiac echocardiography abstract publication rate of 37.6% is comparable to previous published publication rates for other meetings. No differences in variables analysed were noted between published versus unpublished abstracts.
A Seed Bioassay to Identify Acetyl-CoA Carboxylase Inhibitor Resistant Wild Oat (Avena fatua) Populations

A seed bioassay was developed and tested for the rapid identification of aryloxyphenoxypropionate (APP) and cyclohexanedione (CHD) resistance in wild oat. Two susceptible (S) genotypes, UM5 and Dumont, were treated with fenoxaprop-P and sethoxydim over a range of dosages on filter paper and agar. The former is a wild oat line and the latter a tame oat cultivar. Within 5 d, shoot and root development of both genotypes were completely inhibited by 10 μM fenoxaprop-P and 5 μM sethoxydim. These dosages were then tested to determine if they were suitable for distinguishing between resistant (R) and susceptible (S) plants. Agar medium was preferred over filter paper because of the ease of preparation and maintenance. Four known R wild oat populations were included in the tests. Those with high levels of resistance produced significantly longer coleoptiles and roots than S genotypes, but those with moderate or low levels of resistance could not be separated statistically from S biotypes based on quantitative measurements. However, after exposing the germinating, treated seeds to light for 24 to 48 h, all the R populations produced green coleoptiles and initiated a first leaf, unlike the S genotypes which did not turn green or produce any new growth. This procedure proved useful in discriminating between R and S genotypes and in ranking populations in terms of relative levels of resistance.

Tobacco Cessation in Economically Disadvantaged Dominican Republic Communities: Who are the Ex-Users?

Introduction: Tobacco use and its harm continue to increase in low and middle income countries (LMICs) globally. Smoking cessation is the most effective means of reducing morbidity and mortality from tobacco use. Increasing the prevalence of ex-users is an indicator of population cessation.
Aims: This study provides the first examination of factors associated with ex-tobacco use status in the Dominican Republic (DR), a LMIC in the Latin America and Caribbean region.
Methods: Baseline surveillance was conducted for 1,177 randomly selected households in seven economically disadvantaged DR communities (total N = 2,680 adult household members).
Results: Ex-user prevalence was 10.6% (1.0%–18.5% across communities), 14.8% were current users (9.1–20.4), and quit ratios were 41.7% (9.7%–52.7%). Among ever users, females (OR 2.02, 95% CI 1.41, 2.90), older adults (45–64: OR 1.75, 95% CI 1.12, 2.74; 65+: OR 2.09, 95% CI 1.29, 3.39), and those who could read/write (OR 1.64, 95% CI 1.08, 2.50), had health conditions (OR 1.63, 95% CI 1.11, 2.41), and lived with ex-users (OR 1.70, 95% CI 1.12, 2.58) were over 60% to two times as likely to be ex-users. Those from remote communities (OR 0.52, 95% CI 0.36, 0.74), using chewed tobacco (OR 0.14, 95% CI 0.04, 0.48) and living with tobacco users (OR 0.55, 95% CI 0.37, 0.81) were less likely to be ex-users.
Conclusions: Ex-user prevalence and quit ratios were lower than for high income countries. Implementing broad tobacco control measures, combined with clinically targeting vulnerable groups, may increase tobacco cessation to most effectively reduce this public health crisis.
Recovery-focused cognitive–behavioural therapy for recent-onset bipolar disorder: Randomised controlled pilot trial

Background
Despite evidence for the effectiveness of structured psychological therapies for bipolar disorder no psychological interventions have been specifically designed to enhance personal recovery for individuals with recent-onset bipolar disorder.
AimsA pilot study to assess the feasibility and effectiveness of a new intervention, recovery-focused cognitive–behavioural therapy (CBT), designed in collaboration with individuals with recent-onset bipolar disorder intended to improve clinical and personal recovery outcomes.
MethodA single, blind randomised controlled trial compared treatment as usual (TAU) with recovery-focused CBT plus TAU (n = 67).
ResultsRecruitment and follow-up rates within 10% of pre-planned targets to 12-month follow-up were achieved. An average of 14.15 h (s.d. = 4.21) of recovery-focused CBT were attended out of a potential maximum of 18 h. Compared with TAU, recovery-focused CBT significantly improved personal recovery up to 12-month follow-up (Bipolar Recovery Questionnaire mean score 310.87, 95% CI 75.00–546.74 (s.e. = 120.34), P = 0.010, d=0.62) and increased time to any mood relapse during up to 15 months follow-up (χ2 = 7.64, P<0.006, estimated hazard ratio (HR) = 0.38, 95% CI 0.18–0.78). ConclusionsRecovery-focused CBT seems promising with respect to feasibility and potential clinical effectiveness. Clinical- and cost-effectiveness now need to be reliably estimated in a definitive trial.

Communication and Health Protection Issues Arising from a Flooding Emergency

Objective:
To identify and describe the key communication and health protection issues surrounding a flooding emergency.
Methods:A telephone questionnaire was used to assess health and safety information sources, information recalled, and behavioral responses among residents of the town of Feilding, New Zealand following a major floodin February 2004. This was supplemented by key informant interviews and a newspaper content analysis.
Results:A total of 154 adult residents were surveyed (76% of those successfully contacted and eligible). Of the respondents, 79% recalled being provided with health and safety information from authorities. The majorsources were: radio (41%), pamphlets (23%), and newspapers (20%). The regional newspaper had extensive coverage of this flooding emergency but “health” only was mentioned in 46 (7%) of the 664 flood-related articles.
Of those who recalled receiving information from authorities, most (89%) perceived that it was adequate. Nevertheless, 18% actively sought out information (mainly about water safety and availability). In the post-flood period, 73% of all respondents boiled water; however, only 4% maintained boiling water or bottled water use for the necessary nine-week period. Some forms of emergency preparations did not change substantially from before the flood, compared to four months afterwards (e.g., having a radio with batteries increased from 62% to 68%). Perceived health concerns around the flooding were low, though 59% were concerned about the risk ofwater contamination. Actual self-reported, health consequences from the flood were rare, as only two (1%) people out of 154 reported flood-related health problems.
Conclusions:Health and safety information generally was communicated successfully and was regarded as appropriate by the population of this floodaffected town. Nevertheless, there are further improvements needed in the emergency response by both residents and authorities.

Background
Cognitive deficits are a core aspect of schizophrenia but there has been no study of cognitive function in a catchment-area-based population of patients with schizophrenia.
AimsTo assess cognitive function in a population of patients with schizophrenia, and relate it to community functioning.
MethodAll patients with schizophrenia in Nithsdale, south-west Scotland, were identified (n=182). Measures of assessment were: National Adult Reading Test (NART), Mini-Mental State Examination (MMSE), Rivermead Behavioural Memory Test (RBMT), Executive Interview (EXIT), FAS Verbal Fluency and Health of the Nation Outcome Scales (HoNOS).
ResultsWe assessed 138 patients, mean age 48 years (standard deviation (s.d.) 15). Only 14% were in-patients. The mean premorbid IQ as assessed by NART was 98 (s.d. 14); 15% of patients had significant global cognitive impairment (MMSE); 81% had impaired memory (RBMT); 25% had executive dyscontrol (EXIT); and 49% had impaired verbal fluency (FAS). Scores on the functional impairment sub-scale of HoNOS correlated with all measures of cognitive impairment.
ConclusionsCognitive dysfunction is pervasive in a community-based population of patients with schizophrenia.

Background
Recent work has reported a decline in the incidence of schizophrenia, but it is unclear if these findings reflect a true decrease in its incidence or are an artefact arising from methodological difficulties.
AimsTo take account of these methodological difficulties and report service-based incidence rates for schizophrenia in Dumfries and Galloway in south-west Scotland for 1979–98.
MethodUsing both clinical diagnoses and diagnoses generated from the Operational Checklist for Psychotic Disorders (OPCRIT) computer algorithm for ICD–10 and DSM–IV schizophrenia, we measured change in the incidence rates over time. We used indirect standardisation techniques and Poisson models to measure the rate ratio linear trend.
ResultsThere was a monotonic and statistically significant decline in clinically diagnosed schizophrenia. The summary rate ratio linear trend was 0.77. However, using OPCRIT-generated ICD–10 and DSM–IV diagnoses, there was no significant difference over time.
ConclusionsOPCRIT-generated consistent diagnoses revealed no significant fall in the incidence of schizophrenia. Changes in diagnostic practice have caused the declining rates of clinically diagnosed schizophrenia in Dumfries and Galloway.

Background. The stability of the National Adult Reading Test (NART) as a measure of pre-morbid intelligence in schizophrenia has not yet been satisfactorily established despite the widespread use of the NART in schizophrenia research.
Method. We examined NART stability in a diverse group of 45 schizophrenic patients in a prospective longitudinal study over 6·5–7·5 years.
Results. The results showed that NART performance does not decline significantly with increasing duration of schizophrenic illness and that test–retest reliability, even over 6·5–7·5 years, is extremely high.
Discussion. Our results provide the necessary evidence that the NART can be used as a stable measure of pre-morbid intelligence in schizophrenia.
