Prolonged grief disorder symptomology in three African countries: A network analysis and comparison

Background
Bereavement is a globally prevalent life stressor, but in some instances, it may be followed by a persistent condition of grief and distress, codified within the 11th edition of the International Classification of Diseases (ICD-11) as prolonged grief disorder (PGD). Network analysis provides a valuable framework for understanding psychological disorders at a nuanced symptom-based level.
AimThis study novelly explores the network structure of ICD-11 PGD symptomology in a non-Western sample and assesses the replication of this across three African country sub-samples in these data.
MethodologyNetwork models were estimated using the “Inventory of Complicated Grief-Revised” in a sample of trauma-exposed individuals who experienced bereavement throughout life (N = 1,554) from three African countries (Ghana, n = 290; Kenya, n = 619; Nigeria, n = 645). These networks were statistically evaluated using the network comparison test.
ResultsIt was found that “Feelings of Loss” and “Difficulty moving on” were the most central symptoms in the combined sample network. These findings were largely consistent for the Ghana and Nigeria sub-samples, however, network structure differences were noted in the Kenya sub-sample.
ConclusionThe identified PGD network highlights particular indicators and associations across three African samples. Implications for the assessment and treatment of PGD in these cultural contexts warrant consideration.
Healthcare Professionals Perception of Mobile Phone Usage and Hand Hygiene Adhesion in Intensive Care Units

Background: The introduction of new technologies into the medical field has the duality of improvement and concerns about correct usage and cleaning. Mobile phones are used by healthcare professionals (HCPs) in the work place, and there is not an official policy about their use in health environment. Methods: We asked 60 intensive care unit (ICU) HCPs from 2 units (the burn unit and the internal medicine unit) to participate in an electronic survey about mobile phone usage and hand hygiene compliance; we also cultured the hands and mobile phones of the participants. Unfortunately, 13 HCPs did not participate. Susceptibility testing of the strains was conducted, as well as molecular testing. Results: Overall, 47 HCPs responded to the inquiry: 19% were nurses (9 of 47), 19% were resident physicians (9 of 47), 17% were nursery technicians (8 of 47), 17% were physiotherapists (8 of 47), 13% were cleaning staff (6 of 47), 11% were consultants (5 of 47), and 4% were technicians (2 of 47). Moreover, 26 of 47 participants (55%) were woman and 21 (45%) were men. From all HCP categories, 39 of 47 respondents (83%) reported that they had optimal hand hygiene compliance. However, 92% of respondents had a colonized hand and 90% had a colonized mobile phone. Also, 44 of 47 HCPs (94%) reported that the took their personal mobile phone into the workplace; 40 (85%) reported that they used it during the work day and 35 (74%) reported that they cleaned it. However, 8 HCPs (26%) reported that they had never cleaned the device. All of the HCPs understood that mobile phones can harbor bacteria, and 27 of 47 HCPs (57.45%) indicated that they use 70% alcohol to clean their mobile phones. In contrast, the first choice for hand hygiene was water and soap in 51% of HCPs (24 of 47). Also, 3 HCPs did not have any colonization in the hand culture but had healthcare-associated infection (HAI) pathogens in the mobile phone culture. Conclusions: A policy regarding mobile phone usage in the healthcare setting should be in place, and cleaning of electronic devices in hospitals should be standardized.

Background and Aims:
A relatively small proportion of patients account for a disproportionate share of healthcare utilization and cost with, on average, 1% of patients responsible for 20-25% of cost, 5% of patients for 40% and 10% for two thirds. These “high-utilizers” frequently suffer from co-morbid medical and psychiatric illnesses, but they are not well characterized in terms of diagnoses, current treatment patterns, or long-term outcomes. We sought to characterize further such patients at a large inner city acute care hospital.
Methods:We applied a validated tool, Patients At Risk for Re-hospitalization, to the entire hospital population and then performed a mixed methods (quantitative/qualitative) study of 100 patients judged to be at high risk (>67%) of re-hospitalization during the ensuing year.
Results:Of over 130,000 patients, 6,000 were identified. These individuals were overwhelmingly non-elderly adults (96% ages 18-64). Most common medical diagnoses were hypertension (49%), asthma (41%), diabetes (33%), and HIV/AIDS (32%). Schizophrenia, bipolar illness, or other psychosis was found in 48%. Over two-thirds had substance abuse diagnoses. Although 56% had made at least one emergency department visit in the past two years, only 37% had seen a primary care provider. Patient interviews revealed high rates of unstable housing, social isolation, and failure to appreciate the severity of health problems.
Conclusion:High utilizers of general health care have very high rates of serious mental illness and substance abuse. Interviews suggest need for improved medical/psychiatric coordination with community outreach. Although such interventions are resource intense, the economic and health benefits may be large.
Feasibility and Accuracy of a Wearable Biosensor Device for Vital Sign Monitoring in Septic Emergency Department Patients in Rwanda

Introduction:
Low and middle-income countries (LMICs) bear a disproportionately high burden of sepsis, contributing to an estimated 90% of global sepsis-related deaths. Critical care capabilities needed for septic patients, such as continuous vital sign monitoring, are often unavailable in LMICs.
Aim:This study aimed to assess the feasibility and accuracy of using a small wireless, wearable biosensor device linked to a smartphone, and a cloud analytics platform for continuous vital sign monitoring in emergency department (ED) patients with suspected sepsis in Rwanda.
Methods:This was a prospective observational study of adult and pediatric patients (≥ 2 months) with suspected sepsis presenting to Kigali University Teaching Hospital ED. Biosensor devices were applied to patients’ chest walls and continuously recorded vital signs (including heart rate and respiratory rate) for the duration of their ED course. These vital signs were compared to intermittent, manually-collected vital signs performed by a research nurse every 6-8 hours. Pearson’s correlation coefficients were calculated over the study population to determine the correlation between the vital signs obtained from the biosensor device and those collected manually.
Results:42 patients (20 adults, 22 children) were enrolled. Mean duration of monitoring with the biosensor device was 34.4 hours. Biosensor and manual vital signs were strongly correlated for heart rate (r=0.87, p<0.001) and respiratory rate (r=0.74 p<0.001). Feasibility issues occurred in 9/42 (21%) patients, although were minor and included biosensor falling off (4.8%), technical/connectivity problems (7.1%), removal by a physician (2.4%), removal for a procedure (2.4%), and patient/parent desire to remove the device (4.8%).
Discussion:Wearable biosensor devices can be feasibly implemented and provide accurate continuous vital sign measurements in critically ill pediatric and adult patients with suspected sepsis in a resource-limited setting. Further prospective studies evaluating the impact of biosensor devices on improving clinical outcomes for septic patients are needed.
X-ray diffraction and density functional theory studies of R(Fe0.5Co0.5)O3 (R = Pr, Nd, Sm, Eu, Gd)

The structure of a series of lanthanide iron cobalt perovskite oxides, R(Fe0.5Co0.5)O3 (R = Pr, Nd, Sm, Eu, and Gd), have been investigated. The space group of these compounds was confirmed to be orthorhombic Pnma (No. 62), Z = 4. From Pr to Gd, the lattice parameter a varies from 5.466 35(13) Å to 5.507 10(13) Å, b from 7.7018(2) to 7.561 75(13) Å, c from 5.443 38(10) to 5.292 00(8) Å, and unit-cell volume V from 229.170(9) Å3 to 220.376(9) Å3, respectively. While the trend of V follows the trend of the lanthanide contraction, the lattice parameter “a” increases as the ionic radius r(R3+) decreases. X-ray diffraction (XRD) and transmission electron microscopy confirm that Fe and Co are disordered over the octahedral sites. The structure distortion of these compounds is evidenced in the tilt angles θ, ϕ, and ω, which represent rotations of an octahedron about the pseudocubic perovskite [110]p, [001]p, and [111]p axes. All three tilt angles increase across the lanthanide series (for R = Pr to R = Gd: θ increases from 12.3° to 15.2°, ϕ from 7.5° to 15.8°, and ω from 14.4° to 21.7°), indicating a greater octahedral distortion as r(R3+) decreases. The bond valence sum for the sixfold (Fe/Co) site and the eightfold R site of R(Fe0.5Co0.5)O3 reveal no significant bond strain. Density Functional Theory calculations for Pr(Fe0.5Co0.5)O3 support the disorder of Fe and Co and suggest that this compound to be a narrow band gap semiconductor. XRD patterns of the R(Fe0.5Co0.5)O3 samples were submitted to the Powder Diffraction File.

Group III nitrides are promising materials for light emitting diodes (LEDs). The occurrence of structural defects strongly affects the efficiency of these LEDs. We investigate the optical properties of basal plane stacking faults (BFSs), and the assignment of specific spectral features to distinct defect types by direct correlation of localized emission bands measured by cathodoluminescence in a scanning electron microscope with defects found in high resolution (scanning) transmission electron microscopy and electron beam induced current at identical sample spots. Thus, we are able to model the electronic structure of BSFs addressing I1, I2, and E type BSFs in GaN and AlGaN with low Al content. We find hints that BSFs in semipolar AlGaN layers cause local changes of the Al content, which strongly affects the usability of AlGaN as an electron blocking layer in nitride based LEDs.

In order to detect directly faint planets around other stars it is important to develop techniques to cancel the central starlight. We report progress in terms of null depth on the visible nulling experiments at JPL, essential for NASAs future space missions as well as for an upcoming sounding rocket based experiment. A level of 1 Million to 1 nulling ratio (1e-6 rejection ratio) has been obtained for the first time for visible laser light, and a level of 100000 to 1 nulling ratio has been obtained for a relatively large (5%) bandpass at 650nm. Using a combination of a fiber array and deformable mirror a null of 1e-6 would correspond to a contrast of 1e-9 at a distance of 2-3$\lambda$ in the airy disk plane, only 10x away from TPFs goal. The configuration used was a fiber fed Mach Zender type interferometer, using 2 or 3 mirrors in each two arm, placed in an enclosed air container. For broadband light we used tilted dispersion glass plates in the nuller arms and an avalanche photon diode module to cover the large dynamic range. We describe a variety of conditions that have to be met and optimized to reach very deep nulls and sub-nanometer optical path difference stability, such as optical alignment, symmetry in the two arms, mechanical stability and vibration isolation.
Calibration of Residual Speckle Pattern in a Coronagraph

Ground and space based coronagraphs have been proposed to suppress the light of the star so a planet nearby can be imaged. But even when starlight has been suppressed by $10^{10}$, the residual starlight is as bright as the planet, and must be subtracted to $2*10^{-11}$ for a 5 sigma detection of the planet. For a ground based AO coronagraph, the problem is even more severe. Typically suppression of starlight to $10^{-5}$ of the star is possible and the residual speckle pattern must not have any “bumps” that mimic a planet at $10^{-7}-10^{-8}$. This paper describes a speckle calibration approach that measures the electric field of the light after it exits the coronagraph, in order to estimate the speckle pattern in the image plane. This technique makes use of the coherence of star light or rather the incoherence of starlight to planet light, and has very significant advantages compared to other techniques.
For a space based coronagraph, an alternative approach is to rotate the telescope /coronagraph and subtract two images. The calibration interferometer described here has the advantage that the temporal stability of the system can be relaxed by several orders of magnitude. For a ground based AO coronagraph system this approach has none of the serious limitations of the techniques based on the radial expansion of the speckle pattern with wavelength and enables ground based AO coronagraphs to approach the photon limit rather than the atmospheric limit. The calibration interferometer is being built for a NASA sounding rocket experiment by BU, JPL, MIT, and GSFC (PICTURE) with a 50cm telescope and a nulling coronagraph to be launched in 2007. It is also part of a design study for an extreme AO coronagraph for the Gemini Telescope, and a conceptual study of an extreme AO coronagraph for the TMT.

Visible Nulling Coronagraphy (VNC) is the proposed method of detecting and characterizing exo-solar Jovian planets (null depth $10^{-9}$) for the proposed NASA's Extrasolar Planetary Imaging Coronagraph (EPIC) Clampin & Lyon 2004 and is an approach under evaluation for NASA's Terrestrial Planet Finder (TPF) mission. The VNC approach uses a single unobscured filled-aperture telescope and splits, via a 50:50 beamsplitter, its re-imaged pupil into two paths within a Mach-Zender interferometer. An achromatic PI phase shift is imposed onto one beam path and the two paths are laterally sheared with respect to each other. The two beams are recombined at a second 50:50 beamsplitter. The net effect is that the on axis (stellar) light is transmitted out of the bright interferometer arm while the off-axis (planetary) light is transmitted out of the nulled interferometer arm. The bright output is used for fine pointing control and coarse wavefront control. The nulled output is relayed to the science camera for science imagery and fine wavefront control. The actual transmission pattern, projected on the sky, follows a $\theta^2$ pattern for a single shear, $\theta^4$ for a double shear, with the spacing of the successive maxima proportional to the inverse of the relative lateral shear. Combinations of shears and spacecraft rolls build up the spatial frequency content of the sky transmission pattern in the same manner as imaging interferometer builds up the spatial frequency content of the image.
Neuropsychological functioning in a cohort of HIV infected women: Importance of antiretroviral therapy

We evaluated neurocognitive function in 149 HIV-seropositive and 82 seronegative women enrolled in the Women's Interagency HIV Study (WIHS), a large multi-center study of disease progression in women living with HIV/AIDS. We evaluated the prevalence of abnormal neuropsychological (NP) test findings in HIV-seropositive and seronegative women and factors associated with increased risk of abnormal NP test performance. Risk of NP impairment was no higher for HIV positive women receiving antiretroviral therapy at testing than for HIV-negative women (OR = 1.00). However, the risk of abnormal NP performance increased significantly for seropositive women not receiving antiretroviral therapy (OR = 2.43). Further, treatment status was a significant predictor of NP impairment in a multivariate analysis that included viral load (OR = 1.48) and CD4 count (OR = 1.08) which were not significant. The multivariate analyses controlled for substance use, age, education, head injury, ethnicity, estimated IQ, and psychological distress. This study emphasizes the critical association of antiretroviral therapy with the risk of neurocognitive impairment in women living with HIV/AIDS. (JINS, 2002, 8, 781–793.)
